Abstract

Eye chemical trauma was an eye trauma caused by a substance with a pH<7 (acid) and pH>7 (alkali). Chemical trauma can be caused by pesticides. In Indonesia, 78.9% of farmers had eye complaints due to pesticides exposure. The occurrence of alkali chemical trauma of eye could cause cornea neovascularization (CNV). Alkali chemical trauma caused intense inflammation resulting a vascularization of the cornea that was supposed to be avascular. The purpose of this study was to find out the effect of Isotoma longiflora leaves extract the cornea neovascularization of wistar rats chemical trauma model. This research used true experimental design method with post-test only controlled group design. The sample of this study were 30 male wistar rats (Rattus norvegicus) divided into six groups, 1 positive control group (dexamethasone 0.1%), 1 negative control group (DMSO 0,1%), and 4 treatment groups of Isotoma longiflora leaves extract (0.25 mg/ml, 0.5 mg/ml, 1 mg/ml, 2 mg/ml). The result of Shapiro-Wilk and Levene's test showed that p>0.05 that means the data was normally distributed and has the same variant. One Way ANOVA test results obtained significant differences between groups on the 7th day (p=0.001). The result of Post Hoc LSD test showed that group P3 and P4 were significantly different to negative control, P1 and P2 groups. P3 and P4 groups with 1 mg/ml and 2 mg/ml Isotoma longiflora leaves extract could inhibit cornea neovascularization.

Abstract

Abstract Perinatal deaths are babies who die at <28 days of birth. According to WHO, there are 2.7 million perinatal deaths worldwide in 2015. In 2016, the number of infant mortality at Jember is 50.19 per 1000 live births and become the second rank in Jawa Timur after Probolinggo. Infant mortality has been associated by maternal, gestational and neonatal factors. The aim of this study is to determine the risk factors of perinatal death age 0-28 days in RSD dr. Soebandi Kabupaten Jember. This research was analytic observasional with case control design doing in RSD dr. Soebandi Kabupaten Jember. The samples were 65 cases and 65 controls. Data analysis was done by Chi-square test and Fisher’s exact test with significance value < 0,05. The results showed APGAR Score (p <0.001, OR = 6.51), birth weight (p <0.001, OR = 9.66), body length (p <0.001 OR = 5.07), gestational age (p = 0.002; OR = 2.92), congenital anomalies (p = 0.013; OR = -) and maternal age (p = 0.001; OR = 4.51) were risk factors for perinatal death 0-28 days in RSD dr. Abstract

Conjunctivitis is inflammation in the conjunctiva, with clinical signs of exudation, cellular infiltration, and vascular dilatation. In adults, Staphylococcus species are the most common pathogenic bacteria resulting in bacterial conjunctivitis. Previous studies in vitro, explaining that mulberry leaf extract can inhibit the growth of S. aureus bacteria. The purpose of this study was to investigate the effect of mulberry leaf ethanol extract (Morus alba L.) on wistar mice conjunctivitis model by S. aureus. This type of research is true experimental with pretest-posttest control group design. The 24-wistar mice were adapted for 7 days. On day 8, a conjunctivitis induction was performed with 1.5 x 108 CFU / ml of S. aureus at 1 drop every 10 minutes for 1 hour (left 3 days) until conjunctivitis . On day 12 until day 19 (7 days), given mulberry leaf extract 45%, 55%, 65% and 75%. The results of TPC S. aureus logarithm on MSA medium of the 11th (pretest) day conjunctival swab and the 17th day (posttest) of rat adaptation were K (-) (5.70 and 4.07); K (+) (5.78 and 3.31); P1 (5.65 and 3.78); P2 (5.63 and 3.82); P3 (5.84 and 3.74); and P4 (5.69 and 3.63), with the posttest group there was improvement in conjunctivitis symptoms rather than the pretest group. The result of One Way Way Anova data analysis showed significant result at pretest p = 0,002 and posttest p = 0,046 (p <0,05) and paired t-test got significant result p = 0,000 (p <0,05). From this study it can be concluded that mulberry leaf extract (Morus alba L.) has the effect of decreasing the number of colonies of S. aureus bacteria and improving symptoms of conjunctivitis. Abstract

In primary open angle glaucoma, decrease of hyaluronic acid level is related to extracellular matrix deposition of trabecular meshwork. Extracellular matrix deposition will increase humor aqueous outflow resistance which in turn increase the intraocular pressure. MMP-9 as one of matrix metalloproteinase (MMP) family, has a high potency to degrade the extracellular matrix. The expression of MMP-9 can be activated by hyaluronic acid through its binding with CD44 as hyaluronic main receptor. The aim of this research is  to determine the effect of hyaluronic acid on MMP-9 expression in trabecular meshwork cell culture of patient with primary open angle glaucoma by immunocytochemical method. Trabecular meshwork cell culture were exposed to hyaluronic acid at four different concentration (0mg/ml, 1 mg/mL, 3 mg/mL, 6 mg/mL). After 24 hours incubated, we find a significant difference of MMP-9 expression between control group and group with hyaluronic acid. There was significant correlation between hyaluronic acid and MMP-9 expression. Hyaluronic acid exposure will increase MMP-9 expression in trabecular meshwork cell culture. This finding suggest that hyaluronic acid can influence the resistance of humor aqueous outflow in trabecular meshwork by up-regulating MMP-9 expression, and therefore will increase extracellular matrix degradation.Key words : hyaluronic acid, primary open angle glaucoma, MMP-9

Abstract

Intradialytic hypertension is one of the complications of chronic kidney disease patients (CKD) stadium V undergoing routine hemodialysis. There is still no definite data on incidence of intradialytic hypertension in Indonesia. Therefore, it is important to do further research about the relationship between factors of intraialytic hypertension with incident intradialytic hypertension as preventionefforts morbidity and mortality in patients CKD stadium V undergo hemodialysis. One of the factors is quick of blood (QB). The purpose of this research is to know the connection between QB with intradialytic hypertension on patients PGK stadium V at RSD dr. Soebandi Jember. This research is a type of observational analytic study with cross sectional method which is implemented in the dialysis unit at RSD dr. Soebandi Jember in October 2017. Population and sample in this research are all patients who undergo PGK stadium V routine hemodialysis in RSD dr. Soebandi Jember at a number of 82samples. The data obtained were tested normalitasnya with the Kolmogorov-Smirnov test with the results of the significance of 0.71, indicating the data is distributed normally. Pearson test generates the value of significance (p) 0.032 and strong relationship (r) 0.237 indicating that there is a meaningful relationship with the powerful relationship between weak QB with the incidence of hypertension in patients intradialisis CKD stadium V at RSD dr. Abstract

Cataract is a degenerative disease and one of the leading causes of blindness in Indonesia and in the world. Free radical formation leads to pathological reactions in the lens that can induce lipid peroxidation that produces malondialdehyde (MDA). Mulberry leaf (Morus alba L.) is one of the plants rich in antioxidants. The purpose of this study was to determine the effect and effective dose of mulberry leaf extract on MDA content of eye lens in rats cataract model. This study is true experimental with posttest only control group design. This study used 7 groups, each 5 tails. Normal control groups were not treated. The negative control group induced sodium selenite 25 μmol / kgBW and was given 0.1% HPMC eye drops. The treatment group induced sodium selenite 25 μmol / kgBB and given 0.05%, 0.1%, 0.2%, 0.3% and 0.4% mulberry leaf extract for 14 days. Measurement of MDA lens concentration are using MDA-TBA method. Data analyzed by using one way Anova test showed significant result with value p = 0,000 (p <0,05). Based on the results of this study it can be concluded that mulberry leaf extract has an effect on decreasing MDA lens in rats cataract model. Among the concentrations used, the concentrations that showed the lowest decrease in MDA concentration were 0.4%.Keywords: cataract, sodium selenite, malondialdehid, Morus alba L

Abstract

Surgical site infection (SSI) in open fracture is often caused by bacterial contamination in the management of open fracture. Because of that, one of the most important thing in handling open fracture is debridement. Prophylactic antibiotics given are Cephalosporin and Aminoglycosides. Post-debridement culture is important in predicting the incidence of infection. One of the bacteria that is often found in post-debridement culture is Klebsiella spp. which can produce ESBL to fight β-lactam class of antibiotics. The purpose of this study was to determine antibiotic sensitivity against Klebsiella spp. in the post-debridement culture of cases of open fractures in the emergency department of dr. Soebandi hospital Jember. This study uses a laboratory exploratory research design. The sample of this study was the isolate of Klebsiella spp. which amounts to 5 from post debridement culture of open fracture patients in the emergency department of dr. Soebandi hospital Jember from March to May 2019.The method used is diffusion (Kirby Baurer) by matching using the CLSI standard table to determine sensitive, intermediate, or resistant. The results of this study showed that most antibiotics had resistance to Klebsiella spp., including β-lactam antibiotics, such as Amoxicillin, Ceftriaxone, Cefixime, Penicilin, Meropenem, and Cefadroxil. Vancomycin antibiotics are still sensitive to Klebsiella spp. in all patients. Gentamicin, Ciprofloxacin, Tetracycline, and Chloramphenicol antibiotics were sensitive in 1 patient. Erythromycin intermediates antibiotics against Klebsiella spp.. The conclusion of this study is that all β-lactam group antibiotics are resistant to Klebsiella spp while the most sensitive antibiotic is Vancomycin.

Abstract

Background: The most common psychological disorder in cycloserine treatment is depression. The purpose of this study was to identify the risk factors associated with depression in MDR-TB patients receiving cycloserine therapy at dr. Soebandi Regional Hospital. Data were obtained from medical records and the Depression Anxiety Stress Scales (DASS-42) questionnaire from 89 MDR-TB patients at dr. Soebandi Regional Hospital in 2022-2024. Methods: Data analysis was performed using the Lambda and Somers’ tests, followed by multivariate analysis using ordinal logistic regression. Results: Out of a sample of 89 patients, 27 (30.3%) experienced depression, with the majority experiencing moderate-level depression (18%). The bivariate test results showed that the variables that significantly influenced the level of depression were gender (P=0.011), education (P=0.000), occupation (P=0.037), BMI (P=0.000), and comorbidities (P=0.029). Multivariate tests showed that three variables had a significant effect (P<0.05) on the level of depression: gender (P=0.047), occupation (P=0.001), and comorbidities (P=0.011). The top two dominant risk factors affecting the level of depression are occupation (OR=17.12) and comorbidities (OR=9.14). Conclusion: Occupation and comorbidities are the risk factors that most influence the level of depression in MDR-TB patients receiving cycloserine.
