Abstract

This research was conducted at UD. SI in order to control the quality of plastic rack products by applying the lean six sigma method and fault tree analysis. This method is often applied when a problem occurs because it is effective as a data processing method and analyzes a major failure and finds the cause of a defect in the production process in controlling product quality and improving quality in the company. This research uses Lean Six Sigma and Fault Tree Analysis methods. This data retrieval is done by using primary data and secondary data. Primary data was obtained through observation, interviews with employees and staff in the production department, while secondary data was obtained from the company when conducting research. From the results of the study that for perforated shelves, the error rate achieved can be seen in the pareto diagram where there are three types, namely perforated shelves, crinkled shelves and thin shelves where the causative factors of the product can be seen in the fault tree analysis method, namely humans and machines as well as recommendation for improvement by monitoring and checking as well as operators at the time of production as well as conducting strict product quality checks which are expected to minimize repeated repetitions.

Abstract

Real-time and accurate risk management data is critical in making informed corporate decision-making. An Enterprise Risk Management (ERM) application enhances data access speed and accuracy, supporting strategic actions. Developed with an integrated risk and technology management approach, this application aids in effective risk management in operational power plants facing complex, high-risk challenges. It offers a structured framework for identifying, assessing, and managing risks, ultimately enhancing operational efficiency, effectiveness, and safety. This research examined the impact of system quality and readiness to change on the perceived usefulness, perceived ease of use, and behavioral  intentions toward ERM applications. Data were gathered from 300 risk management professionals through purposive sampling. Then, the data were analyzed with PLS-SEM using a quantitative survey-based approach. The finding reveals that system quality and readiness to change significantly influence perceived usefulness and ease of use, which subsequently affect behavioral intentions. Interestingly, perceived usefulness does not directly impact behavioral intentions, emphasizing the critical roles of system quality and user readiness in ERM adoption. These finding underscores organizations' need to enhance the technical aspects of ERM applications and implement effective change management strategies to improve user engagement and adoption rates. The originality of this research resides in its integrated approach, merging system quality and readiness to change within the Technology, Acceptance, Model (TAM) framework, offering a deeper understanding of the factors driving ERM application acceptance. This research provides valuable insights for standardized frameworks, financial incentives, and tailored training programs to boost technology adoption.

Abstract

Kata Kunci : Perencanaan, Material Requirement Planning, Period Order Quantity, Economy OrderQuantity, Lot for lot.

Abstract

PT. XYZ is a foreign capital investment companies are located in one area of town of Sidoarjo which has more than 7000 employees in it and of course want to have a high quality product . PT footwear products. XYZ is the world quality products that promote the quality and comfort of the wearer. Problems that occur in the production process of PT . XYZ of 7 -injection engine that has the most number of engine defect is 5.6 and 7. Quality products at heavily influence the shoe production to consumers. If damaged, then the product can not be distributed to consumers .Therefore , the quality of the shoes must be maintained for the successful marketing of the product. From our previous improvements made in reducing the defect in the production process , but the result is less than optimal , so do research that aims to make improvements again with another method . The method used in this research is to apply the methods of Statistical Processing Control (SPC) and Root Cause Analysis (RCA). The results of the check sheet analysis and Pareto diagram can be seen the frequency of product defects (defects) over roughing most occurred in March 2016 as many as 345 shoes (62%) of the total production of 489 products. Then the second frequency in February 2016 as many as 214 shoes (38%) of the total production of 357 products. And from the image Control Chart P can be concluded that the data are in a state of uncontrolled. Because of all these 50 data, there is one data point that is outside the control limits (out of control), on the 4th point the data has a value of the proportion of 1, beyond the limits Upper Control Limit (UCL) is equal to 0.938. During the dots located within the boundaries of a controller, a process is under controlled conditions, and no action required. However, one point that lies outside the control limit is interpreted as the fact that the quality control in the production process (injection) PT. XYZ uncontrolled or still experiencing irregularities. Whereas the causes of the failure of a critical process in the production process are humans, machines, methods and materials.

Abstract

XYZ is a company engaged in assembling agricultural machinery, namely four-wheeled tractors. The components of the four-wheeled tractor itself consist of the front wheels, rear wheels, engine, engine cover, chassis/frame, steering floor, steering seat, fenders, pedals and roof. For parts that will be assembled, the painting process is carried out within the company itself. Therefore, during the painting process, several problems arise, namely differences in color, paint thickness and paint strength. The painting process at this company has not yet reached zero defects, because several defects were still found in the fenders, floor (steering floor), and engine cover. Situations like this can result in increased production costs and decreased product quality, which ultimately reduces profitability. The aim of this research is to find out the main factors that cause defects in painting parts and make appropriate suggestions/improvements to reduce and prevent defects in the painting process. The method used is the Six Sigma method and the FMEA (Failure Mode Effect and Analysis) method. Six Sigma in technical methods is oriented towards a statistical approach to calculating product defects. The goal is to reduce process variance by eliminating defects that interfere with customer satisfaction. FMEA is a technical analysis which, if carried out correctly and at the right time, will provide great value in helping the decision making process. The research results show that the average DPMO value for tractor product production on weekdays from January 1 2023 to April 30 2023 is 107618.46 with an average sigma value of 2.74. The recommendation for improvement from the FMEA method is that the cause of the defect with the highest RPN value is painting that does not comply with the SOP of 576. The proposed improvement for the cause of this defect is supervision of painting employees.
