Abstract

Manajemen nyeri post-operative menjadi salah satu fokus utama dalam perawatan pasien COVID-19. Minimnya jumlah publikasi tentang pilihan terapi post-operative untuk pasien COVID-19 di Indonesia membuat masih dibutuhkannya informasi mengenai pilihan obat-obatan atau teknik manajemen nyeri post-operative untuk pasien COVID-19. Manajemen nyeri post-operative pada pasien COVID-19 positif atau terduga pasien COVID-19 perlu mempertimbangkan ha-hal sebagai berikut.

Abstract

Latar belakang: Nyeri kronik pasca bedah mayor merupakan komplikasi bermakna pada sebagian besar pasien.  Faktor sosiodemografi menjadi salah satu perhatian dalam prevalensi kejadian nyeri kronik pasca bedah mayor dan belum ada data mengenai angka kejadian nyeri kronik dan faktor yang mempengaruhinya di Indonesia, khususnya di Malang. Penelitian ini bertujuan untuk mengetahui hubungan faktor sosiodemografi seperti usia, jenis kelamin, pendidikan, dan tingkat pendapatan terhadap angka kejadian nyeri pasca bedah.Metode: Penelitian ini tergolong penelitian epidemiologi analitik dengan metode cross sectional pada 123 pasien yang menjalani operasi elektif bedah mayor di RS Dr. Saiful Anwar Malang periode Juli-Desember 2018. Penelitian dilaksanakan dengan metode wawancara pada responden. Variabel penelitian ini yaitu usia, jenis kelamin, tingkat pendidikan dan  tingkat pendapatan. Data yang diperoleh dianalisa dengan menggunakan uji korelasi Spearman pada SPSS 25.0.Hasil: Sebanyak 66 subjek tidak mengalami nyeri kronik dan 57 subjek mengalami nyeri kronik pasca bedah mayor. Tingkat pendidikan memiliki hubungan yang signifikan dengan dengan angka kejadian nyeri kronik (p=0,038). Akan tetapi hubungan yang terjadi bersifat lemah  (koefisien korelasi Spearman =0,187). Faktor sosiodemografi di antaranya usia, jenis kelamin dan pendapatan tidak berhubungan dengan angka kejadian nyeri kronik pasca bedah mayor (p>0,05).Kesimpulan: Faktor tingkat pendidikan memiliki hubungan yang bersifat lemah dengan angka kejadian nyeri kronik pasca operasi bedah mayor. Akan tetapi, faktor sosiodemografi seperti usia, jenis kelamin, dan pendapatan tidak berhubungan dengan angka kejadian nyeri kronik pasca operasi bedah mayor.

Abstract

Pain is one of the most common complain encountered in outpatient clinic. Inadequate pain management has been widely reported. As an anesthesiologist and pain physician, we perform a social service of pain management for patients unresponsive to conservative treatment. The social service  was held from April - June 2022. We enrolled thirty-two patients from other physician reference and Primary Health Care in several districts around Malang city to come to Brawijaya University Hospital with the age range of 30-80 years old. The pain interventions given to the participants mainly include prolotherapy, pulsed radiofrequency (PRF), and others. The pain measurement of the patients were assessed with Numerical Rating Scale (NRS), pre and post-intervention. The social service is performed on 32 patients (male 34,37%, female 65.62%), majority aged 60 years od and older (34.37%) with low back pain become the major complaint (43.75%). The majority of patients receive prolotherapy (68%). The mean NRS decrease from 6.19 ± 0.18 (before treatment) to 2.69 ± 0.10 (after treatment). After this social service, most patients (96.875%) have obtained better pain control after given pain interventions.

Abstract

Background: The prevalence of femoral neck fractures is high in geriatric patients with a high mortality rate. Many geriatric patients have comorbidities and find it difficult to tolerate general or neuraxial anesthesia during hip hemiarthroplasty. A more safe technique in lower extremities using peripheral nerve blocks is preferred. The combination of sciatic nerve blocks and psoas compartments can supply adequate anesthesia for hip surgery thus reducing mortality. Femoral nerve blocks decrease the incidence of complications than psoas compartment blocks.Case: A 88-year-old female patient, 70 kg, with distal femoral subtrochanteric fracture accompanied by dislocation, hypertension emergency, hyperplasia heart disease (HHD), and heart failure stage B Fc II were undergoing hemiarthroplasty with regional anesthesia sciatic nerve block and femoral block. After surgery, a block was performed using regional anesthesia sub-arachnoid block bupivacaine 0,5% 7mg + fentanyl 25 µg + Morphin 0,1 µg, post-operative pain with ultrasound, the patient received 0.375% naropin and 50 mg trilac total volume 20 cm3. Then femoral block was done with 0.375% and 50 mg trilac total volume 20 cm3.  Patients were observed for pain scale during hospitalization, time of mobilization, and length of stay. Post-operative hemodynamic was stable, the pain scale using VAS assessment was 0-1 during hospitalization without additional opioid analgesia, active mobilization began on the 2nd day, and the length of stay was 3 days.Conclusion: The combination of a femoral and sciatic nerve block to the proximal part of the skin incision can supply adequate pain compliance for hip hemiarthroplasty.

Abstract

Background: Sensory nerve damage is the earliest leprosy sign which can lead to disability. Previous studies showed that autologous platelet-rich plasma (PRP) perineural blind injection can stimulate leprosy sensory nerve regeneration. Our study provided a safer and more accurate PRP agent delivery method through ultrasound-guided injection and was the first to compare PRP versus standard neuropathy treatment, the neurotropic vitamin. This study aimed to determine the effectiveness of combination therapy of ultrasound-guided PRP perineural injection and oral vitamin B complex compared to single oral vitamin B complex in sensory peripheral neuropathy of posterior tibial nerve in leprosy patients.

Abstract

Motivation plays a critical role in enabling palliative volunteers to provide effective psychosocial support to breast cancer patients. The negative psychological problems patients face often pose challenges for volunteers, as each patient requires a personal approach. Thus, good communication skills are essential, and training in effective communication can enhance volunteers' motivation to optimize psychosocial assistance. This study aimed to measure the effect of effective communication training on the motivation of palliative volunteers in supporting breast cancer patients in Batu City, Indonesia. This research employed a pre-experimental design with a pre-and post-test approach. A total of 45 palliative volunteers were recruited for this study. The training, which included role-playing exercises and group discussions, was conducted over a period of five weeks. Data were collected using the Volunteer Functions Inventory (VFI) questionnaire and analyzed using SPSS with the Paired-Samples t-Test. The results showed a significant increase in volunteer motivation after the training. Statistical tests confirmed a significant improvement in motivation scores after the training. This study highlights the positive impact of effective communication training on increasing volunteer motivation. It underscores the importance of nurses collaborating with volunteers to address the psychosocial needs of breast cancer patients through empathetic listening and effective communication.
