Abstract

Pencampuran sediaan parenteral (iv admixture) yang sudah dilaksanakan secara umum di rumah sakit mempunyai kemungkinan terjadinya kegagalan baik berupa inkompatibilitas obat maupun gangguan stabilitas obat. Penelitian ini bertujuan mengetahui angka inkompatibilitas obat dalam pencampuran sediaan parenteral di RSUD Prof. Dr. Margono Soekarjo Purwokerto yang mengalami inkompatibilitas fisika yang teramati secara organoleptis. Penelitian ini merupakan penelitian deskriptif prospektif. Data yang telah dikumpulkan lalu dianalisis secara deskriptif. Dari 667 pencampuran sediaan parenteral di bangsal bedah saraf RSUD Prof. Dr. Margono Soekarjo selama bulan Februari 2010, dapat disimpulkan angka inkompatibilitas potensial sebesar 0,45% dan inkompatibilitas aktual sebesar 2,55%. Inkompatibilitas aktual yang terjadi berupa kristal 0,17%, endapan 0,17%, dan kabut sementara 2,04% pada pencampuran fenitoin dengan NaCl atau ringer laktat.Kata kunci: Bedah saraf, inkompatibilitas, iv admixtureParenteral Admixture Compatibility in Neurosurgery Ward in Prof. Dr. Margono Soekarjo Regional Public HospitalParenteral admixtures (intravenous admixtures) have been done commonly in hospitals. However, it has a possibility of failures, like incompatibilities and changes in drug stabilities. The aim of this study was to determine the rate of drug incompatibilities in mixing parenteral preparations in neurosurgery ward in Prof. Dr. Margono Soekarjo Regional Public Hospital which undergo physical incompatibility  observed in organoleptic. This study was a prospective descriptive research for one month period. Data were collected and analyzed descriptively. The results showed that from 667 parenteral admixtures in neurosurgery ward in Prof Dr Margono Soekarjo Hospital in February 2010, there were 0.45% potential incompatibility and 2.55% actual incompatibility happened. Actual incompatibility shown as crystal 0.17%, sediment 0.17%, and 2.04% was non-permanent haze in phenytoin and sodium chloride or ringerlactate admixtures.Key words: Incompatibility, iv admixture, neurosurgery

Abstract

Objective: To know profile of pregnant women with tuberculosis at PPTI Baladewa polyclinic Jakarta, to evaluate impacts of tuberculosis medicinal treatment on pregnancy and to know the obstacles of pregnant women to look for tuberculosis treatment. Methods: Subjects were all pregnant women with tuberculosis and received tuberculosis treatment at polyclinic of PPTI Baladewa from January 2006 - December 2007. We reviewed medical record and did home visitation to fill questioners. Results: All subjects were in reproductive age and most of all were low socio-economic group. Pulmonary tuberculosis was found in 88.2% cases and Extrapulmonary tuberculosis in 11.8% cases. We found 29.4% relapse cases and 11.8% multi-drug resistance. Sputum examination with a smear positive for acid fast-bacillus was found in 52.9% cases. There was abnormality for all chest x-rays. The third most frequently symptoms were cough more than two weeks (88.2%), weight loss (88.2%) and anorexia (82.4%). Average time for the patients to look for treatment was 50.71 ± 31.2 days. The patients need more time because most of the symptoms are also present in pregnancy, the patients know absolutely nothing about TB and healthcare provider didn’t examine carefully. Treatment for the patients was given immediately without considering the gestational age. Conclusion: No harmful effect was observed for neonates even if the drugs were given in the first trimester of pregnancy. Abstract

Permasalahan kurangnya pengetahuan tentang penggunaan obat pada ibu hamil dan menyusui masih ditemui di  masyarakat. Edukasi perlu dilakukan untuk meningkatkan pengetahuan kader posyandu tentang penggunaan obat pada ibu hamil dan menyusui. Edukasi dilakukan dengan metode modul, ceramah dan diskusi. Instrumen penelitian berupa kuesioner yang dibagikan kepada responden sebelum dan sesudah edukasi untuk mengevaluasi tingkat pengetahuan peserta. Kuesioner berisi 10 item pertanyaan tertutup terkait pengetahuan penggunaan obat pada ibu hamil dan menyusui. Data dianalisis dengan uji statistik paired t-test. Hasil menunjukkan bahwa rata-rata nilai pengetahuan kader meningkat 25,73% dari rata-rata nilai pengetahuan awal setelah dilakukan edukasi. Setelah dilakukan edukasi ada peningkatan tingkat pengetahuan yang signifikan. Hal ini menunjukkan bahwa edukasi dengan metode modul, ceramah dan diskusi mampu meningkatkan pengetahuan kader Posyandu.

Abstract

Latar Belakang: Penggunaan antibiotik harus sesuai dengan regimen yang telah ditentukan dokter atau apoteker untuk mencegah resistensi. Pasien setelah masa rawat inap berpotensi pada masalah kepatuhan dalam penggunaan antibiotik. Penelitian ini bertujuan untuk mengeksplorasi faktor-faktor pendukung kepatuhan pasien infeksi dalam meminum antibiotik cefixime setelah masa rawat inap di Rumah Sakit Prof. Dr. Margono Soekarjo. Metode: Penelitian dilakukan menggunakan metode non-experimental berdasarkan pada pendekatan kualitatif fenomenologis dengan cara wawancara mendalam (indepth interview). Wawancara dilakukan di rumah informan pada hari ke-7 setelah keluar dari rumah sakit dan triangulasi waktu (wawancara kedua) dilakukan satu minggu setelah wawancara pertama. Hasil wawancara dianalisis secara deskriptif dengan proses berfikir induktif. Hasil dan Kesimpulan: Hasil penelitian menunjukkan bahwa faktor-faktor pendukung kepatuhan pasien infeksi dalam meminum antibiotik cefixime setelah masa rawat inap adalah keinginan untuk sembuh, informasi bahwa antibiotik harus dihabiskan, takut terjadi kekambuhan, diingatkan anggota keluarga, bentuk sediaan, kondisi kesehatan yang memburuk dan mengikuti saran dokter. Selain itu, didapatkan hasil bahwa pasien patuh meminum dan menghabiskan antibiotik sesuai aturan pakai, etiket membantu pasien dalam mengingat jam minum obat, pasien mendapatkan informasi antibiotik harus dihabiskan dari tenaga kesehatan dan pasien mendapatkan informasi manfaat obat dari internet.

Abstract

Pencampuran sediaan parenteral memiliki beberapa kekurangan, salah satunya adalah kemungkinan terjadinya inkompatibilitas farmasetika, farmakokinetika ataupunfarmakodinamika. Pelaksanaan pencampuran sediaan parenteral selama ini masihdilaksanakan oleh perawat yang menangani pasien secara langsung, sehingga perlu adanya edukasi terhadap perawat mengenai pencampuran sediaan parenteral. Penelitian ini menggunakan desain cross sectional melalui pengumpulan data pretest-postest design dengan menggunakan kuesioner. Analisis data dilakukan dengan metode Wilcoxon Sign Rank Test. Hasil penelitian ini menunjukkan bahwa edukasi apoteker kepada perawat Rumah Sakit Tentara (RST) Wijaya Kusuma memiliki pengaruh terhadap peningkatan sikap dan peningkatan pengetahuan tentang pencampuran sediaan parenteral (T=5 < 27-144, CI 99%). Parameter pengetahuan yang meningkat adalah tentang inkompatibilitas obat, pemahaman bahwa tidak semua obat injeksi bisa dicampurkan dengan infus, dan pencampuran sediaan parenteral harus memperhatikan masalah pH, sedangkan semua parameter sikap mengalami peningkatan.

Abstract

ARNI (Angiotensin Receptor-Neprilysin Inhibitor) is a new therapy for heart failure patients that have been approved since 2016, which is a combination of sacubitril (Neprilisin Inhibitor) and valsartan (ARB). The aim of this study was to review the comparison of the effectiveness sacubitril/valsartan and enalapril in heart failure patients through a literature review method. Database used were Cochrane and PubMed. Keywords search based on PICO (heart failure, sacubitril/valsartan or LCZ696, enalapril, and efficacy or effectiveness). The inclusion criteria were articles in English, available in full text and free access, published in 2010-2020, and RCT. The exclusion criteria were review articles. The searching was conducted from 10 July to 23 July 2020 and 20 December 2020. From this searching, researcher obtained 160 articles, after the process of selection and eligibility, 16 articles were included in the inclusion criteria as primary articles, and 2 articles were included as secondary articles. Studies showed that sacubitril/valsartan reduces the incidence of readmission and has a better QoL score than enalapril, and also decreases the risk of death. According to primary research of PARADIGM-HF and 15 advanced analysis article, it’s known that the usage of sacubitril/valsartan is more beneficial than enalapril in heart failure patients.

Abstract

AbstractBackground: Pharmacists are health workers who play an important role in dealing with COVID-19 pandemic problems. This study explored the hospital pharmacist's perception of their roles and behaviours and the influence factors during the COVID-19 pandemic period.Material and Methods: The research was conducted using phenomenological qualitative methods through in-depth online interviews using an online meetings platform. Informants were taken until the data was saturated. Credibility tests by member checking, dependability, and confirmability tests were performed to ensure data validity. The results of the interviews then were analyzed by thematic analysis.Results: Eight informants were included with 5 emerging themes revealed from this study. The theme consists of the pharmacist's high commitment to his important role as a team member of the healthcare provider in providing the best service by adapting their services, ensuring rational therapy for COVID-19 patients, and maintaining the availability of medicines, medical devices, and consumable medical materials. Furthermore, pharmacists described some innovation strategies, like drug delivery for outpatients and telepharmacy services. The supporting factors for pharmacists in their services were interprofessional collaboration between pharmacists and other healthcare providers. Support from colleagues, hospital management, government, donations and technology were also revealed as positive factors. The barrier factors included uncertain pandemic conditions, pharmacists who were exposed to the virus and the mental health condition of healthcare professionals during the pandemic.Conclusion: Pharmacists have a strong commitment to playing a crucial part in the healthcare team in offering the most treatment possible and providing management work. They created some innovative initiatives to tackle the COVID-19 burden. Supportive and barrier factors that affect pharmacist performance during pandemics were also disclosed and examined in this paper.Keywords:

Abstract

This study explores the complexity of leadership succession and regeneration in a private Christian school in West Jakarta and possible ways to navigate the challenges, as the school has experienced a high turnover rate in its leadership in the past five years. Using a qualitative case study approach, this research collected the data through in-depth interviews, observations, and document analysis. Principals as well as heads of schools participated and the research results revealed a discrepancy between the educational philosophy of Christian schools and the practices of leadership succession and regeneration, lacking a specific system or strategy. Moreover, there is no formal succession and inadequate spiritual mentoring for the school leader candidates. Therefore, considering that succession is also a spiritual mandate, this study offers a contextual and values-based pathway for leadership regeneration, which includes early-stage talent mapping, structured leadership development, spiritual mentorship, and alignment with the school’s Christian philosophy to ensure the continuity of biblically grounded leadership.

Abstract

Background: Anemia in adolescents is a public health problem in Indonesia.  Based on the 2018 Basic Health Research (Riskesdas) report, Based on the 2018 Basic Health Research (Riskesdas) report, 32% of teenagers aged 15-24 years experienced anemia. The majority of teenagers who experience anemia are in the age range 10 – 18 years. Adolescent girls experiencing ongoing anemia can increase morbidity during pregnancy and childbirth. Level of knowledge, diet, nutritional status and menstrual cycle are factors that are said to influence anemia. Method: This activity was carried out in Desember 2023 and was attended by 30 young women aged 10-19 years in the Nagrak and Ciangsana district. The activity consists of several stages, namely interviews about the menstrual cycle, checking hemoglobin levels, checking nutritional status, and education about body mass index. Results: From this activity, it was found that 53.3% of teenagers had a normal body mass index, 96.7% had a normal waist/hip ratio, 50% of teenagers had irregular menstrual cycles, 80% had dysmenorrhea. Meanwhile, regarding the prevalence of anemia, 50% of adolescent girls have mild anemia and 13.3% have moderate anemia. Conclusion: The prevalence of anemia in adolescent girls in Nagrak and Ciangsana subdistricts is still high (63.3%), as are adolescents who experience irregular menstrual cycles and dysmenorrhea.

Abstract

Anencephaly is a congenital anomaly of the central nervous system, caused by a disruption in brain and skull development of a fetus, which includes the disruption of the cerebral hemispheres, cerebellum, spinal cord, and pyramidal tract. This is a lethal condition that most fetuses born with this condition cannot survive after being delivered. The prevalence of this condition is 1 in 1000 deliveries. Early detection through prenatal ultrasound testing is essential to make a correct and precise plan for managing the pregnancy and delivery process, thus avoiding potential maternal complications. We reported a 33-year-old pregnant woman who was diagnosed with having anencephalic fetus at 24 weeks of pregnancy. The patient has been informed about this condition and offered a termination, but she refused to do so and is willing to continue the pregnancy. Induction of labor was planned at 40 weeks of pregnancy, but the process was impaired, so a cesarean section was performed to save the mother. The baby was delivered with anencephaly, as shown by the ultrasound result, and died 2 days after being delivered. The mother recovered well and was given education about the importance of folic acid intake for the next pregnancy. Conclusion of this case was labor management of anencephalic fetus was planned according to medical indication, with a special individual approach due to the obstetric condition. Yet, the delivery method should be based more on the mother's condition rather than the baby's, including the maternal emotional factor.
