Abstract

Background: The treatment of severe periodontitis must consider immune responses and local conditions, including the aesthetical aspects. The tooth loss in the anterior area can lead to a psychological issue in some patients, and the daily use of dexamethasone contributes greatly to the severity of aesthetical aspects. Periodontal treatments must be cautious of these aesthetical effects. Purpose: To report the aesthetical consideration in patient management of severe periodontitis aggravated by oral dexamethasone. Case: The 44-year-old female patient reported having tooth mobility in the upper right and left central incisor and lower right and left posterior. Due to the condition, the central anterior needed to be extracted. The patient had seafood allergies and consumed oral dexamethasone periodically to prevent allergic reactions for two years. Case management: The initial periodontal therapy was designed prior to the tooth extraction, socket preservation, and immediate denture on teeth 11 and 21. A metal frame combined with an acrylic denture was designed to support the tooth splint and replace the teeth on the mandible. The patient was treated with 20 mg of sub-antimicrobial-dose doxycycline twice a day for three months, and vitamin E was prescribed once a day. Since dexamethasone may contribute to immune response and osteoclastogenesis, dexamethasone was replaced by cetirizine. Conclusion: The treatment of severe chronic periodontitis must consider immune responses, local conditions, and aesthetical aspects. In this case, the use of dexamethasone might worsen the periodontal breakdown. However, the periodontal treatment, use of host modulation therapy, and replacement of dexamethasone with cetirizine are expected to improve these conditions.

Abstract

Background: Electromagnetic fields are forces associated with moving electric charges and have electrical, magnetic components and contain electromagnetic energy, one of which is radio frequency (RF) energy which is commonly used in telecommunications. Cell phones are one of the RF electromagnetic radiation devices that can emit 90-2450 MHz waves and are often placed near the head. The human body works like an electromagnetic field in that each cell has its own electrical circuit characteristics. As the number of electromagnetic radiation devices in the environment increases, the electromagnetic balance in the human body may be disturbed by the magnetic waves produced by cell phones. Electromagnetic radiation is known to have the ability to induce oxidative stress, which is characterized by the accumulation of reactive oxygen species (ROS) in tissues. The accumulation of ROS in the body leads to osteoblast cell death. Osteoblasts are needed for mineralization of the extracellular matrix during bone growth. Therefore, bone growth is not optimal and can caused malocclusion. Purpose: This study aims to evaluate the relationship between electromagnetic radiation and osteocalcin and bone alkaline phosphatase (BALP) serum levels. Methods: Experimental laboratory research with a pre- and post-control group design approach was carried out on 12 Rattus norvegicus Wistar strain. Osteocalcin and BALP serum levels were calculated before and after treatment. This study used the t-test as a comparative study (p<0.05). Results: There are significant differences in osteocalcin and BALP values between the treatment groups before and after treatment. Conclusion: Cell phone radiations (electromagnetic field exposure) reduce osteocalcin and BALP serum levels.

Abstract

Background: Cerebral palsy described as a spectrum of movement and posture disorders which result from pathological injury to the developing fetal or infant brain. Many conditions associated with cerebral palsy require surgery. Purpose: This article presents case report of a 15-year old, autistic male patient with cerebral palsy who required urgent management of multiple gangren radix, multiple pulpitis and gingivitis. Case Management: General anaesthesia was chosen for the purpose of behaviour management. The dental procedure were restoration with flow able composite of 7 teeth, extraction of 15 gangren radix, scaling of maxilla and mandibular region, also preventive treatment with topical aplication of fluoride. Discussion: Principles to the care of a such patient including early recognition, parental support, multi-disciplinary planning of procedure requiring general anesthesia, continuity of anesthesia care, and clear guidelines about the perioperative management of uncooperative children. Dental surgery under general anaesthesia was done, the patient was received restoration with flow able composite of 7 teeth, extraction of 15 gangren radix, scaling calculus at maxilla and mandibular region, also preventive treatment with topical aplication of fluoride. Conclusion: The case report showed that dental treatment should be done in patient with cerebral palsy and autism comprehensively.

Abstract

Background: Cleft lip and palate is one of most common craniofacial anomalies. It is a congenital defect and can be resulted from genetic mutation, environment and multifactor causes. The problems of cleft lip and palate on baby complicate as poor feeding ability can lead to growth and development disturbances. Purpose: This article described the use of PSO in a 4-day old male neonate with unilateral cleft lip and palate and a 3- year old male child with soft and hard palate cleft, both with chief complaint of feeding difficulty. Case Management: Custom tray was used to deliver the alginate impression. The appliance was both fabricated using self-cured acrylic and with / without lip strapping. Discussion: Pre-surgical orthopedic appliances are most frequently provided by the pediatric dentist on cleft palate teams at most hospital-based programs. Conclusion: The PSO appliance could approximate the segments of the cleft maxilla by allowing spontaneous growth of palatal shelves prior to the reconstructive and definitive surgery.

Abstract

Background: Incidence of dental avulsion is 1-11% of all dental injuries to the permanent dentition, with the maxillary central incisor being the most frequently involved tooth. The age group of 7-10 years appears to be most affected. If the avulsed teeth are not treated immediately, they can have a significant negative impact on the child such as functional, aesthetic and psychological disorder. Purpose: The aims of this case reports was to discuss the important role of storage medium that can influence the success of replantation treatments of avulsion teeth. Case Management: The first phase of treatment was an extra oral root canal treatment and were repositioned into the socket. Clinical and radiographic control was carried out periodically for 2 months. Discussion: Delayed replantation has a poor long-term prognosis. The periodontal ligament will be necrotic and not expected to heal. The goal in delayed replantation is to restore the tooth to the dentition for esthetic, functional, and psychological reasons and to maintain alveolar contour. Conclusion: This case reports showed the important role of storage medium that can influence the success of replantation treatments of avulsion teeth.

Abstract

Background: Ankyloglossia or tongue tie is a congenital condition that results when the inferior lingual fraenulum is too short and attached to the tip of the tongue, limiting its normal movements.  It causes restricted tongue mobility which in turn causes feeding difficulties and speech problems. Purpose : The article was aimed to report the treatment of an  eight years old boy  who came with the chief complaint of difficulty in moving his tongue freely which causes speech difficulties. Case Management : The patient was treated for a lingual frenectomy procedure under local anaesthesia using one haemostat method. The haemostats were used to delimit the area to be excised as well as to guide the incisions. One haemostat used to clamp the upper aspect of the fraenulum may be helpful to guide the incision close to the ventral surface of the tongue. After the release of the tongue, care must be taken not to injury the submandibular ducts when making the second incision at the lower aspect of the fraenulum. After 1 to 2 weeks the incision was completely healed. Discussion: Ankyloglossia occurs due to failure in cellular degeneration leading to longer anchorage between tongue and floor of the mouth. Surgical intervention for treating ankyloglossia includes conventional technique with hemostats, electrocautery and laser. In this case, patient was undertaken surgical intervention using one hemostat method. Conclusion: The ankyloglossia case showed that the frenectomy gave good healing with no post operative complications in 8 years old boy.

Abstract

Background: Vitamin D deficiency in children is one of the problems most often discussed and received attention from around the world today, especially during the Corona virus disease-19 (Covid-19) pandemic. Vitamin D3 increases the production of vitamin K2 protein and activates a protein involved in bone metabolism. Vitamin K2 increases bone formation by stimulating osteoblast differentiation, regulating mineralization of the extracellular matrix, regulating bone marker gene expression, and inhibiting osteoclastogenesis. Purpose: to analyzed the effect of sun exposure, Ultraviolet-B (UV-B) and vitamin K2 supplementation on fibroblast-like cell as bone formation marker in Wistar rats (Rattus novergicus). Methods: twenty-four samples divided into 4 groups namely control group, UV-B group, vitamin K2 group, combination of UV-B and vitamin K2 group. After 21 days, extraction of lower incisors was done to examine fibroblast-like cell number "‹"‹after treatment. After being decalcified, specimens underwent histological evaluation using Haemotoxylin and Eosin staining to observe the fibroblast-like cell number. Result: Data analysis of fibroblast like cells number expression using one way analysis of variance (ANOVA) test showed a significant difference between sample groups (p<0.05). Conclusion: The combination of UV-B exposure and vitamin K2 administration group increased of fibroblast-like cells in Wistar rats (R. novergicus).

Abstract

Children with autism spectrum disorder (ASD) have special characteristics and problems, including oral health problems. The Covid19  pandemic condition requires adjustment in many ways. The information about the efforts of parents, especially mothers in maintaining the oral health of their ASD children isneeded so that assistance can be provided efficiently and effectively. Purpose: to determine the mother's efforts in maintaining the dental health of children with ASD during the COVID-19 pandemic. Method: A questionnaire was given to 109 mothers who have ASD children from the Mutiara Hati Autism Foundation in Surabaya City and Mojokerto City, then the data were analyzed using descriptive analysis. Results: It was found that the majority of mothers did not bring their children to the dentist during pandemic, so the problem of dental caries had not been treated optimally. Mothers' efforts in maintaining the dental health of their ASD children during the COVID-19 pandemic were; always accompanying the child to brush his teeth, and providing painkillers when the child has a toothache. The mothers did not know that there was an online dental consultation so they did not take advantage of it. Conclusion: The mother's efforts in maintaining the dental health of ASD children during the COVID-19 pandemic more limited, due to lack of knowledge in the prevention/treatment of dental disease; and concerns about virus transmission during dental treatment; and the lack of socialization of the use of online media for consultation with dentists.

Abstract

Background: Caries is a multifactorial disease that often occurs in the oral cavity and can be caused by Streptococcus mutans. Various ways have been done to reduce the number of Streptococcus mutans, one of them is by exposing them to a material agent such as silver-ion water. Silver-ion water has been recognized as an effective material for inhibiting several pathogenic bacteria, such as Staphylococcus aureus and Escherichia coli. Thus, there is a possibility that silver-ion water can be used to reduce the number of bacteria that cause dental caries, such as Streptococcus mutans. Purpose: To determine the effect of silver ion water on the growth of Streptococcus mutans. Methods: This research is an experimental laboratory study (in vitro) with a control group post-test-only design. Streptococcus mutans were exposed to 0-15 ppm silver-ion water on Brain Heart Infusion Broth (BHIB) media, then Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) were observed. Results: Silver-ion water with a concentration of 12.5 ppm was determined as MIC, and silver-ion water with a concentration of 15 ppm as MBC. Conclusion: Silver ion water is adequate as an antibacterial material in inhibiting (12.5 ppm) and killing (15 ppm) Streptococcus mutans.

Abstract

Latar Belakang : Traumatic dental injuries atau TDIs yaitu segala bentuk trauma fisik pada gigi, struktur pendukung, atau jaringan sekitar akibat terkena benturan atau kecelakaan yang terjadi secara tiba-tiba.  TDIs  selain berdampak pada domain oral, juga dapat  mempengaruhi kesehatan anak secara keseluruhan dan berpotensi menyebabkan masalah pertumbuhan dan dikaitkan dengan dampak lebih besar pada Oral Health Related Quality of Life (OHRQoL). Manajemen efektif TDIs membutuhkan pemahaman komprehensif tentang tindakan pertolongan pertama yang tepat. Sekolah merupakan tempat prevalensi TDIs terbesar dan guru terlibat dalam penanganan pertolongan pertama di sekolah. Tujuan : Mengevaluasi tingkat pengetahuan dasar dan meningkatkan pengetahuan Guru Taman Kanak-kanak Kecamatan Mulyorejo, Surabaya melalui kegiatan pengabdian kepada masyarakat. Metode : Sebanyak 44 orang guru dari total 20 sekolah TK yang telah menyatakan kesediaan, mengisi pre-test terkait TDIs. Setelah diberikan edukasi serta pelatihan oleh tenaga medis profesional dokter gigi, kemudian tingkat pengetahuan dievaluasi kembali melalui post-test. Hasil :  Nilai rata-rata pre-test adalah 37,93 dan nilai rata-rata post-test meningkat menjadi 71,29 . Berdasarkan hasil uji Wilcoxon didapatkan nilai sig. sebesar 0,000, (p<0,05). Simpulan : Pendidikan dan pelatihan signifikan meningkatkan pengetahuan mengenai TDIs di kalangan guru TK, yang akan bermanfaat bagi anak-anak.
