Abstract

Background: Tooth discoloration is a prevalent issue that significantly impacts an individual's self-confidence. Fortunately, dentists offer highly effective and non-invasive teeth whitening techniques that use higher concentrations of hydrogen peroxide (HP) of around 35% to 40%. These techniques can help resolve the problem of teeth discoloration better than at-home methods designed to deliver faster results. This is particularly helpful when immediate whitening is required. Purpose: This case study aims to report cases of in-office external bleaching of discolored teeth. Case: A 29-year-old woman complained about the yellowish shade of her teeth, which made her feel less confident due to the yellowish color. Case Management: The management for discoloration of teeth was done with dental bleaching using 40% hydrogen peroxide. As a result of the treatment, the desired tooth color increased from 3M1 to 1M1 Vita 3D Master and from Opalescence no.6 to 4. Conclusion: In-office dental bleaching is a highly effective treatment option for correcting tooth discoloration that dental professionals widely use. The procedure can significantly improve the appearance of discolored teeth and increase self-confidence of the patient.

Abstract

Background: In dental treatment planning, aesthetic considerations are crucial due to the relationship between a smile and facial beauty. Crown lengthening is a procedure used to achieve aesthetic smiles, particularly in cases of subgingival caries, fractures, and short clinical crowns. Purpose: The purpose of this case report is to achieve aesthetic improvement of the central incisors through crown lengthening procedures. Case: A 31-year-old female patient presented with a complaint of a broken upper left front tooth and easily bleeding gums. Clinical examination revealed Ellis class 1 fracture at the incisal-distal of tooth 21. The patient exhibited poor oral hygiene with visible calculus on upper and lower jaw teeth. There was gingival hyperplasia, redness, bleeding tendency, and the gingival margin height of teeth 11 and 21 appeared lower than teeth 12 and 22. Radiographically, the alveolar crest was distant from the CEJ. Case Management: Treatment plan included crown lengthening on teeth 11 and 21 with composite restoration on tooth 21. Crown lengthening, a surgical procedure designed to increase the extent of supragingival tooth structure for restorative or aesthetic purposes by repositioning the gingival margin apically was performed. This procedure is indicated for teeth with subgingival caries, extensive caries shortening the tooth, fractures, and short clinical crowns. Conclusion: Crown lengthening should consider the biological width to prevent bone resorption, gingival recession, inflammation, or hypertrophy. When performed under ideal clinical conditions, crown lengthening provides satisfactory outcomes both functionally and aesthetically.

Abstract

Background: The developments in endodontics have created opportunities for patients to maintain functional teeth for longer. Surgical endodontic treatment, such as hemisection, has become a more conservative treatment than complex treatments, such as removable or fixed partial dentures or implants. Purpose: The aim of this treatment is to preserve the remaining tooth structure through a hemisection procedure and socket preservation using an alloplastic bone graft and platelet-rich fibrin (PRF). Case: A female patient presented with mastication pain and a large carious tooth in the right mandibular first molar and wanted to save the tooth. Examination showed deep caries and perforation in the bifurcation area of the tooth with loss of the distal crown. However, the mesial root could be preserved, thus hemisection was proposed. Case management: A root canal treatment was performed on the mesial root, followed by separation of the mesial and distal roots, and, finally, distal root extraction. A mixture of PRF and bone graft was used for socket preservation. The tooth was restored with a splinted zirconia crown. Conclusion: Hemisection with socket preservation using alloplastic bone graft and PRF represents a more conservative treatment option for molar teeth with extensive caries. This approach exhibits a good long-term prognosis and enhances the bone healing process.

Abstract

Background: Dental aesthetic issues may affect individual’s aesthetic, function, and psychological well-being. The functions of the stomatognathic system are supporting mastication, speaking, breathing and swallowing functions, so that structural integrity and an ideal and harmonious smile can be achieved as well. Aesthetic issues due to caries of maxillary anterior teeth requires deep analysis, consideration and appropriate treatment planning to aim a good result both functionally and aesthetically. Digital smile design (DSD) plays an important role in aesthetic dentistry where problems can easily be rectified. Purpose: This case report describes a step-by-step aesthetic treatment of a male patient with multiple caries of anterior teeth. Case: This study reports a 21-year-old man with multiple caries on his maxillary anterior teeth. Tooth number 12 with class IV caries lesion. Teeth number 11 and 22 with class IV and VI caries lesion and were endodontically treated, tooth number 21 with class III mesiodistal caries lesion. The patient’s chief complaint is that he felt less confidence with his appearance. Case Management: tooth number 21 was managed with endodontic treatment and followed by insertion of fiber post. Teeth number 11 and 22 that endodontically treated previously, were managed with prefabricated fiber post. Tooth number 12 was managed with crown. Teeth number 11, 21, 22 11, 21, 22 were managed with gingivectomy for aesthetic crown lengthening. Lithium disilicate was chosen for the crown materials Conclusion: Anterior teeth with multiple caries can be improved through endodontic-periodontal-restoration treatments to improve the function and aesthetic performance.

Abstract

Background: Vital pulp therapy is a treatment to repair and maintain pulp vitality. Calcium hydroxide is the gold standard in pulp capping materials. Calcium hydroxide has advantage antibacterial. However, calcium hydroxide has some disadvantages such form a layer liquefaction necrosis. Blood clam shells contain 98.7% calcium carbonate. Calcium carbonate (CaCO3) content becomes a good source of calcium can be used as a bone repair material. Purpose: To review effect nano CaCO3 from Anadara granosa shell induced odontoblast-like cells as a pulp capping candidate. Nanoparticles CaCO3 has advantages such as high surface area, low toxicity, biocompatibility, biodegradability, pH-sensitivity, and osteoconductivity. Reviews: Nano CaCO3 from Anadara granosa are promising to promote odontoblast-like cells formation. Conclusion: Nano CaCO3 its potential to influence odontoblast-like cells, which play a crucial role in dentin regeneration and repair.

Abstract

Background: Dental caries affects 57.6% of the Indonesian population, with Enterococcus faecalis found in 80% of deep caries cases. Conventional materials used in vital pulp therapy (VPT), such as calcium hydroxide, have limitations including transient antibacterial action and poor sealing capacity. This prompts the exploration of alternative materials like epigallocatechin-3-gallate (EGCG), a polyphenol from green tea with antimicrobial activity, and tricalcium silicate (C₃S), a bioactive cement known for dentin regeneration potential. Purpose: This review evaluates the synergistic antibacterial potential of EGCG and C₃S combinations against E. faecalis, and assesses their feasibility as an alternative in VPT. Review: EGCG disrupts bacterial membranes, inhibits biofilm formation, and suppresses virulence factors, but lacks remineralization capability. Conversely, C₃S promotes dentin repair and creates an alkaline antibacterial environment, though its efficacy diminishes over time. Their combination compensates for each other’s limitations: EGCG enhances antimicrobial potency while C₃S provides long-term alkalinity and regenerative support. Conclusion: The EGCG–C₃S combination exhibits synergistic antibacterial and regenerative effects, offering a promising alternative to current VPT materials.

Abstract

Background: Aesthetic issues on anterior maxillary teeth often require comprehensive treatment due to both functional and psychological impacts. This case report highlights the clinical approach and management for anterior maxillary teeth with crown fracture and multiple diastemas through restorative aesthetic procedures. Purpose: To describe a clinical approach and restorative aesthetic management for a 37 year-old female patient with a crown fracture and multiple diastemas on the anterior maxillary teeth, aiming to restore aesthetics and function through a carefully planned dental treatment. Case(s): A 37 year-old female patient presented to RSGMP UNAIR with a chief complaint of fractured upper anterior teeth and a history of intermittent pain. The patient also reported concern about the spacing between her upper front teeth. The clinical examination revealed involvement of teeth 13, 12, 11, 21, 22, and 23. The patient had no previous dental treatment and no systemic health issues. Case Management: A detailed clinical and radiographic examination was performed, followed by a comprehensive treatment strategy targeting tooth elements 13, 12, 11, 21, 22, and 23. The management focused on restoring tooth structure, closing the diastema, and improving smile aesthetics. Conclusion: Aesthetic and functional rehabilitation of the anterior maxillary teeth was planned and carried out through an integrative restorative approach. The case emphasizes the importance of proper diagnosis dan treatment planning in managing complex aesthetic cases.
