Abstract

Latar belakang: Dermatitis atopik (DA) merupakan keradangan kulit yang kronis, berulang, disertai rasa yang sangat gatal, kulit terasa kering, timbul pada tempat predileksi tertentu. Peningkatan potential hydrogen (pH) dan transepidermal water loss (TEWL) pada pasien DA menunjukkan gangguan fungsi sawar kulit dan dapat menyebabkan berbagai kelainan kulit berupa kulit kering di daerah lesi maupun non lesi. Tujuan: Mengetahui nilai pH dan TEWL pada DA anak di Divisi Dermatologi Anak Unit Rawat jalan (URJ) Kesehatan Kulit dan Kelamin RSUD Dr. Soetomo Surabaya. Metode: Penelitian deskriptif observasional dengan cross sectional (potong lintang) bertujuan untuk mengetahui nilai pH dan TEWL pada pasien DA anak di RSUD Dr Soetomo Surabaya, dengan besar sampel 20 pasien DA anak. Derajat penyakit diukur dengan menggunakan indeks SCORAD. Hasil: Nilai median pH di area lesi dan area non lesi adalah 6,54 dan 5,38. Nilai rerata TEWL di area lesi dan area non lesi adalah 22,51 g/m2/h dan 16,04 g/m2/h. Nilai pH dan TEWL pada area lesi lebih tinggi dibandingkan dengan area non lesi. Simpulan: Terdapat kecenderungan peningkatan nilai pH dan TEWL pada area lesi maupun non lesi sesuai dengan derajat keparahan DA.

Abstract

Latar Belakang: Dermatitis atopik (DA) adalah penyakit kompleks dengan gejala klinis berspektrum luas. Penyakit ini ditandai dengan gejala gatal yang dominan sampai dapat mengganggu kualitas tidur, dan dapat menyebabkan infeksi sekunder. Patogenesis DA meliputi gangguan sawar kulit, faktor lingkungan, agen infeksius, dan abnormalitas sistem imun. Gangguan sawar kulit menjadi faktor utama yang harus diperbaiki salah satunya dengan pemberian pelembap seawal mungkin. Tujuan: Membandingkan efikasi pelembap yang mengandung seramid, mentol, dan polidokanol dengan jeli petrolatum pada pasien DA derajat ringan. Metode: Penelitian ini merupakan penelitian eksperimental analitik dengan menggunakan metode uji klinis acak terkontrol tersamar tunggal, membandingkan terapi pelembap yang mengandung seramid, polidokanol, dan mentol dengan pelembap jeli petrolatum pada pasien DA anak dan dewasa dengan derajat keparahan ringan yang dinilai melalui indeks scoring of atopic dermatitis (SCORAD). Hasil: Penghitungan nilai SCORAD sebelum dan sesudah intervensi pada kelompok perlakuan atau kontrol memiliki perbedaan yang tidak  signifikan  dengan nilai p>0,05. Simpulan: Pemberian kedua jenis pelembap dapat menurunkan derajat keparahan SCORAD pada pasien DA.

Abstract

Background: Infantile Hemangioma (IH) is a benign vascular neoplasm and most commonly found in newborns. IH appears in the first few weeks of life and mostly resolve immediately. Early recognition of symptoms, diagnosis, and therapy has an important role in determining the prognosis of this condition. Purpose: The purpose of this study is to evaluate the clinical features of new IH patients in the Pediatric Dermatology Division of the Dermatology & Venereology Department outpatient clinic of Dr. Soetomo General Hospital Surabaya in 2008-2017. Methods: This is a retrospective descriptive study from the medical records of new IH patients over a period of 10 years (2008-2017). Results: A total of 84 new IH patients were observed in this study. The data showed that: the most age group was 0-1 year old (71.43%), female (63.1%), and no family history of a similar condition (94.05%). The most common complaint reported was the presence of bumps (59.52%). Most complaints occurred before the patients turned one year old (89.29%) and involved the facial region (58.33%). From the clinical features, the most frequent skin lesions were macules (35.36%). In addition, there was a combination of macular and nodular lesions in 28.13% of patients. The most widely used therapy is laser modality found in 55.45% of patients. Conclusion: Clinical features of IH patients, in accordance with the theory and treatment choices, dependent on the patients’ conditions and clinical manifestations of the disease.

Abstract

Background: Scabies, an infectious disease caused by Sarcoptes scabiei mites, is still one of the most common skin diseases found in developing countries, including Indonesia. The 5% permethrin cream for scabies treatment has been investigated as having 2–3 time to cure scabies. Tea Tree Oil (TTO) is one of the agents that has been proven to have acaricidal potential as an anti-inflammatory, antibacterial, and antipruritic agent. It has minimal side effects in topical use. Purpose: To evaluate the effectiveness of 5% TTO cream and 10% TTO cream in scabies treatment of pediatric patients. Methods: This was an experimental, analytical study with controlled clinical trial methods, and a double-blind, parallel design comparing 5% TTO cream (treatment 1), 10% TTO cream (treatment 2), with 5% permethrin cream (control) to children with scabies. Result: The 5% TTO cream gave a cure rate of 61.5% and was significantly different from the 5% permethrin cream groups with a p-value = 0.044 in the first week of the study. The 5% permethrin cream gave a cure rate of 15.4%, and the cure rate of 10% TTO cream was 53.8% in the first week of the study. Conclusion: There was a significant difference between the 5% TTO cream and 5% permethrin cream in the first week of the study. The highest and fastest cure rates were found in the 5% TTO cream treatment group.

Abstract

Background: Oral candidiasis is the most common clinical manifestation in a person infected with HIV which has been linked with the severity of the disease. Candida albicans responsible for most oral candidiasis. The increased resistance to the available antifungal drugs can reduce the efficacy of therapy. Therefore, it will be crucial to discover novel antifungal agents. One of them is tea tree oil (TTO) or Melaleuca alternifolia which has a potent antifungal effect to inhibit the growth of Candida sp.  Purpose: To evaluate the antifungal susceptibility of TTO 5% compared with nystatin against Candida sp. taken from HIV/AIDS patients. Methods: The study assessed the antifungal effect of TTO 5% and nystatin against 33 isolates of Candida sp. that was isolated from the oral cavity of 30 HIV/AIDS patients in Dr. Soetomo General Academic Teaching Hospital Surabaya, Indonesia. Antifungal susceptibility was evaluated by the disc diffusion method. The diameter of the inhibition zone was determined as the result of this study. Result: The most strains found in this study was Candida albicans (84.8%). The mean inhibition zone of nystatin against all strains was 23.24 mm whether it was 17.55 mm for TTO 5%. The inhibition zone of both TTO 5% and nystatin in Candida non-albicans were tended to be higher than in Candida albicans. It revealed that the inhibition zone of TTO 5% significantly lower than nystatin (P = 0.00; P < 0.05).Conclusion: Therefore, the antifungal susceptibility of nystatin was significantly higher than TTO 5% against Candida albicans and non-albicans.

Abstract

Background: Oral candidiasis is caused by the mycotic activity of Candida albicans present in the oral cavity, and it is one of the most common opportunistic infections found in patients with Human Immunodeficiency Virus (HIV)/acquired immune deficiency syndrome (AIDS). The growing resistance and side effects to common antifungal drugs have promoted herbal essential oils as antifungal agents in recent years. In this study, essential oils (EO) of Rosmarinus officinalis (Lamiaceae) were examined for in vitro antifungal activ­ity against Candida species. Purpose: To evaluate the antifungal activity of essential oils of Rosmarinus officinalis (Lamiaceae) and nystatin using the microdilution technique by determining the minimum inhibitory concentration (MIC) and the minimum fungicidal concentration (MFC) of Candida spesies. Methods: This was an experimental laboratory study with a post-test-only design conducted in Dr. Soetomo General Academic Teaching Hospital, Surabaya. Forty isolates consisted of twenty isolates of Candida albicans and twenty isolates of Candida non-albicans were collected. The isolates were tested for antifungal activity using the microdilution on 96-well plates. Result: There was a significant difference from the results of the MIC concentration of rosemary essential 100% to 6.25% microdilution method between nystatin and rosemary essential oil (p < 0.05). Conclusion: The antifungal activity of rosemary essential oil was better than nystatin that the lowest MIC value, which was 6.25%, has been obtained the microdilution method. The minimum fungicidal concentration of rosemary essential oil was 25%, while the minimum fungicidal concentration nystatin was higher than 100%.

Abstract

Introduction: Callus and clavus are skin disorders in the form of hyperkeratotic lesions that are common and can affect the patient's quality of life. Callus and clavus can occur in men or women of all ages. Treatment of callus and clavus has a low cost benefit but has received little attention and until now there is no gold standard of treatment that is considered ideal. Data on callus and clavus characteristics, common treatments, and the final outcome of callus and clavus therapy have not been widely reported so that it has an impact on the standard management of these two lesions. The aim of this study was to evaluate the description of callus and clavus cases in the form of gender, age, location lesions, management, and cure rate 1 month after therapy in the Outpatient Unit (URJ) Skin and Venereology Tumor Division and Skin Surgery (TBK) Regional General Hospital (RSUD) Dr. Soetomo, Surabaya. Methods: A cross-sectional retrospective descriptive study on 25 Electronic Medical Records (EMR) patients with callus and clavus who met the inclusion and exclusion criteria at the Dermatology and Venereology Outpatient Unit, Skin Surgery Tumor Division (TBK) RSUD Dr. Soetomo Surabaya period January 2016 - December 2020. Results: Twenty-five samples were recruited in this study consisting of 13 (52%) male patients and 12 (48%) female patients. Clavus dominated all cases in 20 (80%) cases, while callus was in 5 (20%) cases. The lower extremity was the most common site for lesions in 20 (80%) cases. Excision was the most frequently chosen method in 21 (84%) cases. Clinical recovery after 1 month after therapy was obtained in 20 (80%) cases. Conclusion: Management of callus and clavus in URJ Skin and Venereology, TBK Division, RSUD Dr. Soetomo, Surabaya showed a cure rate of 80% with the most preferred modality being excision.   Pendahuluan: Kalus dan klavus adalah kelainan kulit berupa lesi hiperkeratotik yang umum didapatkan dan dapat mempengaruhi kualitas hidup pasien. Kalus dan klavus dapat terjadi pada pria ataupun wanita di segala rentang usia. Tatalaksana kalus dan klavus memiliki cost benefit yang rendah namun kurang mendapat perhatian dan hingga saat ini belum terdapat standar emas tatalaksana yang dianggap ideal. Data karakteristik kalus dan klavus, tatalaksana yang umum dilakukan, dan hasil akhir dari terapi kalus dan klavus belum banyak dilaporkan sehingga berdampak pada standar manajemen kedua lesi tersebut. Tujuan dari penelitian ini adalah mengevaluasi gambaran kasus kalus dan klavus berupa jenis kelamin, usia, lokasi lesi, tatalaksana, dan angka kesembuhan 1 bulan paska terapi di Unit Rawat Jalan (URJ) Kulit dan Kelamin Divisi Tumor dan Bedah Kulit (TBK) Rumah Sakit Umum Daerah (RSUD) Dr. Soetomo, Surabaya. Metode: Penelitian deskriptif retrospektif potong lintang pada 25 Electronic Medical Records (EMR) pasien dengan kalus dan klavus yang memenuhi kriteria inklusi dan eksklusi di Unit Rawat Jalan Kulit dan Kelamin Divisi Tumor Bedah Kulit (TBK) RSUD Dr. Soetomo Surabaya periode Januari 2016 - Desember 2020. Hasil: Dua puluh lima sampel direkrut pada penelitian ini terdiri dari 13 (52%) pasien laki-laki dan 12 (48%) pasien perempuan. Klavus mendominasi keseluruhan kasus yaitu 20 (80%) kasus, sedangkan kalus 5 (20%) kasus. Ekstremitas bawah merupakan lokasi tersering didapatkannya lesi yaitu pada 20 (80%) kasus. Eksisi merupakan metode yang paling sering dipilih yaitu pada 21 (84%) kasus. Kesembuhan klinis setelah 1 bulan paska terapi didapatkan pada 20 (80%) kasus. Simpulan: Tatalaksana kalus dan klavus di URJ Kulit dan Kelamin Divisi TBK RSUD Dr. Soetomo, Surabaya menunjukkan angka kesembuhan sebesar 80% dengan modalitas yang paling banyak dipilih berupa eksisi.
