Abstract

Background :  Innovative topical psoriasis therapy continues to be developed, Spatholobus littoralis Hassk or Bajakah has antipsoriatic activity so can be used as a topical herbal medicine in reducing the severity of psoriasis. In silico is a computational experiment which is analogous to biological experiments in vivo and in vitro. Objective : To evaluate content of Spatholobus littoralis Hassk using in silico analysis in the treatment of psoriasis. Methods : The active compound Spatholobus littoralis Hassk extracted from the knapsack database. The simplified molecular input line entry system (SMILE) format was taken from the pubchem database. Prediction in antioxidants, antiinflammatory, antipruritic and immunosuppressive was done using a pass server. The molecular mechanism of active compounds in human body was taken from search tool for interacting chemicals (STITCH) which was predicted experimentally, then analyzed computationally. Further pathway analysis using cytoscape software. Results : There are 14 active compounds in Spatholobus littoralis Hassk have potential as antioxidants, anti-inflammatory, antipruritic and immunosuppressive are predicted to have ability test computationally tested activity but laboratory tests have not been proven or have little potential. The highest bioactivity potential of Spatholobus littoralis Hassk is antioxidants where the most important role is dihydrokaemferol with an average probable to be active (Pa) value of 0.691, the compound has ability to computationally test but in laboratory tests it has not been proven or has a small potential. Conclusions : Spatholobus littoralis Hassk is a good choice for the treatment of psoriasis.  Latar belakang : Terapi psoriasis topikal inovatif terus dikembangkangkan, Spatholobus littoralis Hassk atau bajakah memiliki aktivitas antipsoriatik sehingga dapat digunakan sebagai obat herbal topikal dalam mengurangi keparahan psoriasis. In silico merupakan percobaan komputasi yang analog dengan percobaan biologis secara in vivo dan in vitro. Tujuan : Untuk mengevaluasi kandungan Spatholobus littoralis Hassk menggunakan analisis in silico pada pengobatan psoriasis. Metode : Senyawa aktif Spatholobus littoralis Hassk yang diekstraksi dari database knapsack. Format simplified molecular input line entry system (SMILE) diambil dari basis data pubchem. Prediksi dalam antioksidan, antiinflamasi, antipruritus dan immunosupresor dilakukan menggunakan pass server. Mekanisme molekuler senyawa aktif dalam tubuh manusia diambil dari search tool for interacting chemicals (STITCH) yang diprediksi secara eksperimental, kemudian dianalisis secara komputasional. Analisis pathway lebih lanjut menggunakan perangkat lunak cytoscape. Hasil : Terdapat 14 senyawa aktif pada Spatholobus littoralis Hassk yang memilik potensi sebagai antioksidan, antiinflamasi, antipruritus dan immunosupresor diprediksi memiliki kemampuan pada aktivitas yang diuji secara komputasional, namun secara uji laboratorium belum terbukti atau memiliki potensi kecil. Potensi bioaktivitas Spatholobus littoralis Hassk tertinggi adalah antioksidan dimana yang paling berperan adalah dihydrokaemferol dengan ata-rata nilai probable to be active (Pa) 0,691 dimana senyawa tersebut secara komputasional memiliki kemampuan pada aktivitas yang diuji namun secara uji laboratorium belum terbukti atau memiliki potensi kecil. Kesimpulan : Spatholobus littoralis Hassk merupakan pilihan yang baik untuk terapi psoriasis karena memiliki efek antioksidan, antiinflamasi, antipruritus dan immunosupresor serta menguntungkan dari segi ketersediaan serta keamanan.

Abstract

Melasma is a hyperpigmentation disorder that is often found, is chronic. Hydroquinone is considered the standard therapeutic standard of melasma, has limitations and side effects vary. L-ascorbic acid acts as a reactive oxygen species (ROS) scavenger and has the ability to bind copper ions in the tyrosinase workplace. Glutation has the function of depigmentation of the skin that inhibits melanogenesis by suppressing the activity of tyrosinease enzymes. The purpose of this study was to prove and analyze the effectiveness of serum combination therapy L-ascorbic acid 10% and glutation 2% in lowering MASI score compared to serum hydroquinone 4% in melasma patients. Clinical experimental research method, pre and posttest double blind randomized controlled trial with the study subjects as many as 36 melasma patients. In group I was given a combination of serum L-ascorbic acid 10% and glutation 2%, while group II got serum hydroquinone 4%. The effectiveness of serum is assessed with an MASI score. The analyses used are t-paired tests, Wilcoxon tests and Difference-in-Differences (DID) analyses. Statistical tests are considered meaningful if p<0.05. The results of the study in both groups had a significant decrease in MASI score (p<0,001), but the decrease did not differ significantly between the two groups. The decrease in MASI scores between the two groups made no significant difference in week 4 (p=0.535) and 8th (p=0.303). The conclusion of this study was a combination of serum L-ascorbic acid 10% and glutation 2% lowered MASI score, but not proven more effective in lowering MASI score than serum hydroquinone 4% in melasma patients.

Abstract

Melasma is a hypermelanosis disorder that arises typical of areas of the skin that are often exposed to sunlight. Melasma therapy is relatively difficult because there is often chronicity in melasma. Until now therapy with Kligman formula is still an option, but kligman formula has some side effects, especially for long-term use. Cysteamine works by inhibiting the enzyme tyrosinase. Traneksamat acid works by inhibiting the activity of plasmin in keratinocytes known as stimulators of tyrosinase activity The purpose of the study proved and analyzed the effectiveness of cysteamine cream layering therapy 5% and tranexilic acid 3% compared to kligman formula modification in melasma sufferers to improve MASI score. Experimental research method, pre and post control group design double-blind randomied controlled trial with 28 samples of melasma patients namely group A using layering cysteamine cream 5% and traneksamat acid 3% compared to the group of patients melasma i.e. group A using layering cysteamine cream 5% and traneksamat acid 3% compared to the group of patients. Assessment and measurement of the effectiveness of therapy is carried out during weeks 0, 2, 4 and 8, with an assessment of MASI score. The analysis used is the t-pair test and the Wilcoxon test. Statistical tests are considered meaningful if p<0.05. The results of the study decreased the difference in MASI score in group A obtained in week 4 and week 8, while group B in week 8. The difference in the difference in MASI score change between group A and group B in weeks 4 and 8 indicates a significant difference (p=0.025 and p=0.003). Conclusion of 5% layering cysteamine therapy and 3% tranex acid indicates a decrease in the difference in MASI score earlier than modified Kligman cream therapy.

Abstract

Metotreksat merupakan pilihan terapi psoriasis yang efektif. Laporan pemberian MTX parenteral pada psoriasis vulgaris masih jarang. Kami melaporkan 8 kasus psoriasis vulgaris dengan terapi injeksi MTX dosis inisial 10mg/minggu dan dinaikkan menjadi 15mg/minggu. Dalam evaluasi 8-12 minggu, terdapat perbaikan klinis yang dievaluasi menggunakan Psoriasis Activity and Severity Index (PASI), luas Body Surface Area (BSA) dan Dermatology Life Quality Index (DLQI). Bioavailabilitas MTX parenteral lebih baik dibandingkan peroral. Tidak ditemukan efek samping selama pemberian injeksi MTX.Methotrexate (MTX) is the commonly prescribed for psoriasis. Reports on parenteral MTX for psoriasis in Indonesia are still scarce. We reported eight psoriasis patients treated with intramuscular MTX 10mg/week increased to 15mg/week. The clinical improvement was remarkable demonstrated with Body Surface Area (BSA), Psoriasis Activity and Severity Index (PASI) and Dermatology Life Quality Index (DLQI) scores. Bioavailability of parenteral MTX is better than oral MTX. No adverse effect was found and the clinical improvement was good.

Abstract

Latar Belakang: Akne vulgaris (AV) adalah penyakit inflamasi kulit kronik yang mengenai unit pilosebasea, mengakibatkan kelainan non-inflamasi berupa komedo dan tanda inflamasi. Mikroba kulit seperti Propionibacterium acnes (P. acnes) dan Malassezia spp. berperan dalam patogenesis AV. Laporan kasus ini mengenai efek terapi probiotik topikal terhadap perbaikan lesi AV. Kasus: Tiga pasien AV diberi terapi tabir surya (SPF 30) dan probiotik topikal selama 4 minggu tanpa terapi standar AV. Pemeriksaan swab dilakukan pada semua pasien untuk melihat jenis dan jumlah mikrobiom. Hasilnya tidak ditemukan kolonisasi P. acnes tetapi terjadi penurunan jumlah dan derajat keparahan lesi pada semua pasien. Terapi yang sama dilanjutkan selama 4 minggu pada satu pasien. Kolonisasi P. acnes hanya ditemukan pada awal terapi. Evaluasi klinis yaitu penurunan jumlah lesi AV dan kulit wajah menjadi lebih halus dan lembap. Abstract

Vitiligo is an idiopathic skin depigmentation disorder with clinical symptoms of macules and depigmented patches on the skin. Vitiligo is a multifactorial disorder associated with genetic and non-genetic. There are two types of vitiligo: non-segmental and segmental vitiligo. The type of vitiligo can be determined based on physical examination and affects the disease course, prognosis, and therapy response. Vitiligo management is still a challenge on its own because the etiology is not clear, and many factors must be considered in the vitiligo treatment. Vitiligo management greatly varies depending on the activity and the disease expansion. The goals of vitiligo treatment include stopping disease progression, increasing repigmentation, and preventing recurrence. Vitiligo therapy options vary widely, but none have been effective in treating vitiligo with a high recurrence rate. One of the vitiligo treatment options is systemic corticosteroids given in pulse doses bringing the effect of decreasing the vitiligo progression and minimizing side effects. This literature review aims to determine the use of pulse dose corticosteroid therapy in vitiligo.

Abstract

Nail psoriasis can affect the patient's quality of life, so to evaluate the quality of life in patients with skin diseases in general, the Dermatology Quality of Life Index (DLQI) can be used. The questionnaire was unable to distinguish the impact on quality of life caused by nail abnormalities or skin lesions, so a special scoring system was needed to evaluate the quality of life for nail psoriasis patients with the Nail Psoriasis Quality of Life Scale 10 (NPQ10). To find the relationship between the severity of nail posirais based on Nail Psoriasis Severity Index (NAPSI) score with DLQI and NPQ10. The total respondents were 20 people with a diagnosis of nail psoriasis who came to the Dermatovenereology Outpatient Clinic of Dr. Moewardi General Hospital, Surakarta from April-June 2020. We took photography and dermoscopy of fingernails and toes and then calculated the NAPSI score. All respondents filled out the DLQI and NPQ10 questionnaires. Pearson's (r) correlation test was used to find the relationship between the mean and standard deviations of NAPSI and DLQI and NPQ10. We also tested the correlation between DLQI and NPQ10. The data significance value was declared significant if p <0.05. The results of the correlation test between NAPSI scores with DLQI and NPQ10 showed a statistical relationship, where the correlation value between NAPSI and DLQI was slightly more significant than NPQ10 (r = 0.948: r = 0.877; p <0.05). The mean value of DLQI and NPQ10 in women was higher than men (14.12 ± 8.21: 11.5 ± 8.29 and 11.5 ± 3.58: 8.16 ± 4.52) with a correlation test value of DLQI and NPQ10 is r = 0.888, p <0.05. We found that the association between NAPSI correlates more significantly with DLQI than NPQ10. This may be due to some questions that do not reflect the daily activities of our study subjects, but NPQ10 score in women is higher than men where the quality of life for nail psoriasis is also influenced by gender.

Abstract

Manifestasi klinis infeksi kulit yang disebabkan oleh bakteri piogenik salah satunya S. aureus terutama pada kondisi imunokompromais seperti infeksi HIV/AIDS. Kebersihan personal mempengaruhi peningkatan risiko transmisi S.aureus melalui kontak dari individu satu ke yang lainnya, sehingga dapat meningkatkan risiko infeksi. Penelitian ini bertujuan untuk mengetahui hubungan status HIV dan kebersihan personal dengan jumlah koloni S.aureus pada anak-anak yang tinggal di lingkungan panti asuhan. Penelitian ini merupakan studi analitik korelasi dengan rancangan cross sectional dilakukan pada dua rumah panti asuhan di kota Surakarta, Jawa Tengah, Indonesia. Penelitian dilakukan pada total 32 orang subjek anak dengan rentang usia 3-10 tahun yang terbagi menjadi dua kelompok HIV negatif dan HIV positif. Kriteria eksklusi subjek adalah riwayat kinis atopi, ada lesi kulit pada tempat pengambilan sampel dan memiliki nilai CD4+<500 sel/ mm3, kemudian kebersihan personal subjek didata. Uji normalitas dilakukan, kemudian dilanjutkan uji non parametrik dengan uji korelasi Eta. Nilai r ditentukan untuk melihat kekuatan hubungan kedua variabel dan signifikan apabila didapatkan p<0,005. Berdasarkan analisa diketahui bahwa status HIV tidak menunjukan hubungan yang signifikan dengan jumlah koloni bakteri S. aureus (x10-2) CFU (r=0,297; p=0,098). Kebersihan personal memiliki hubungan signifikan dengan jumlah bakteri S. aureus antara lain frekuensi mandi (r=0,500; p=0,004), penggunaan sabun saat mandi (r=0,480; p=0,005), berbagi gelas (r=0,392; p=0,026), berbagi handuk (r=0,570; p=0,001), memakai alas kaki saat di rumah (r=0,355; p=0,046). Kebersihan personal dan kontak alat rumah tangga secara signifikan berhubungan dengan banyaknya jumlah koloni S.aureus di kulit anak pada kedua kelompok subjek HIV positif maupun negatif.
