Abstract

ABSTRAK Pendahuluan: Psoriasis vulgaris merupakan inflamasi kulit kronis residif yang diperantarai oleh sistem imun dan berhubungan dengan beberapa komorbiditas seperti aterosklerosis, penyakit kardiovaskular, gangguan fungsi ginjal, sindrom metabolik, dan diabetes melitus. Terdapat hubungan antara respon imun Th-17 pada gangguan fungsi ginjal terutama pasien dengan riwayat infeksi ginjal dengan psoriasis vulgaris. Tujuan: Mengetahui hubungan kadar ureum dan kreatinin terhadap tingkat keparahan psoriasis vulgaris. Metode: Penelitian analitik observasional dengan rancangan penelitian cross-sectional yang dilakukan di RSUD Dr. Moewardi Surakarta dari bulan Maret-Mei 2022. Pemilihan sampel dilakukan dengan teknik incidental sampling dengan subjek penelitian diambil dari pasien psoriasis vulgaris yang berkunjung ke Poli Kulit-Kelamin sesuai dengan kriteria inklusi dan eksklusi. Hasil: Subjek penelitian mendapatkan hasil usia pasien rata-rata 46.95±15.85 tahun dengan median 46.00 (16.00- 70.00) tahun. Rerata kadar ureum dan kreatinin masing-masing adalah 20.30±7.83 dan 0.79±0.21. Kadar kreatinin memiliki hubungan yang bermakna terhadap skor PASI (r=0.53; p=0.016) dan DLQI (r=0.57; p=0.009), kadar ureum tidak memiliki hubungan yang signifikan baik dengan skor PASI maupun DLQI (r=0.114; p=0.663 dan r=0.037; p=0.876). Kesimpulan: Kadar kreatinin memiliki hubungan yang bermakna terhadap tingkat keparahan psoriasis berdasarkan skor PASI dan DLQI. Kata kunci: DLQI, kreatinin, PASI, psoriasis, ureum

Abstract

Latar Belakang: Akne vulgaris (AV) adalah penyakit inflamasi kulit kronik yang mengenai unit pilosebasea, mengakibatkan kelainan non-inflamasi berupa komedo dan tanda inflamasi. Mikroba kulit seperti Propionibacterium acnes (P. acnes) dan Malassezia spp. berperan dalam patogenesis AV. Laporan kasus ini mengenai efek terapi probiotik topikal terhadap perbaikan lesi AV. Kasus: Tiga pasien AV diberi terapi tabir surya (SPF 30) dan probiotik topikal selama 4 minggu tanpa terapi standar AV. Pemeriksaan swab dilakukan pada semua pasien untuk melihat jenis dan jumlah mikrobiom. Hasilnya tidak ditemukan kolonisasi P. acnes tetapi terjadi penurunan jumlah dan derajat keparahan lesi pada semua pasien. Terapi yang sama dilanjutkan selama 4 minggu pada satu pasien. Kolonisasi P. acnes hanya ditemukan pada awal terapi. Evaluasi klinis yaitu penurunan jumlah lesi AV dan kulit wajah menjadi lebih halus dan lembap. Ringkasan: Probiotik topikal memberikan hasil baik pada kasus AV. Introduction: Acne vulgaris (AV) is chronic inflammatory skin disease affecting pilosebaceous unit, resulting in non-inflammatory disorders in the form of comedones and signs of inflammation. Skin microbes such as Propionibacterium acnes (P. acnes) and Malassezia spp. play role in the AV pathogenesis. This paper reports the effects of topical probiotic on AV lesions improvement. Case: Three AV patients were given sunscreen (SPF 30) and topical probiotics for 4 weeks without standard AV therapy. Swab examination showed no P. acnes colonization but the amount and severity of lesions in all patients were decreased. One patient then continued the same therapy for 4 weeks; P. acnes colonization was only found at the beginning of therapy. Clinical evaluation resulted in decreasing lesions and smoother and moisturized facial skin. Summary: Topical probiotic gives good results in AV.

Abstract

Metotreksat merupakan pilihan terapi psoriasis yang efektif. Laporan pemberian MTX parenteral pada psoriasis vulgaris masih jarang. Kami melaporkan 8 kasus psoriasis vulgaris dengan terapi injeksi MTX dosis inisial 10 mg/minggu dan dinaikkan menjadi 15 mg/minggu. Dalam evaluasi 8-12 minggu, terdapat perbaikan klinis yang dievaluasi menggunakan Psoriasis Activity and Severity Index (PASI), luas Body Surface Area (BSA), dan Dermatology Life Quality Index (DLQI). Bioavailabilitas MTX parenteral lebih baik dibandingkan per oral. Tidak ditemukan efek samping selama pemberian injeksi MTX.   Methotrexate (MTX) is the commonly prescribed for psoriasis. Reports on parenteral MTX for psoriasis in Indonesia are still scarce. We reported eight psoriasis patients treated with intramuscular MTX 10 mg/week increased to 15 mg/week. The clinical improvement was remarkable demonstrated with Body Surface Area (BSA), Psoriasis Activity and Severity Index (PASI), and Dermatology Life Quality Index (DLQI) scores. Bioavailability of parenteral MTX is better than oral MTX. No adverse effect was found and the clinical improvement was good.

Abstract

Background: Atopic diathesis, a hereditary predisposition to allergic disorders such as atopic dermatitis, asthma, and allergic rhinitis, is more strongly transmitted maternally. Maternal atopy may influence neonatal skin barrier development, but evidence on its effect on neonatal skin pH and hydration is limited. Methods: This cross-sectional study was conducted at Dr. Moewardi General Hospital, Surakarta, Indonesia, between March–April 2025. Neonates (28–41 weeks gestation), <24 hours old, and delivered by cesarean section were included. Skin pH was measured on the volar forearm and axilla using a calibrated pH meter, and hydration (water and oil content) was assessed with a Skin Tester. Maternal atopy diathesis was classified by the Erlangen Atopy Score (EAS). Results: Twenty-nine neonates were enrolled, comprising 12 with and 17 without maternal atopy. No significant association was found between maternal atopy and neonatal hydration (water: p = 0.460; oil: p = 0.997) or skin pH (p = 0.876). Conclusion: Maternal atopy diathesis was not associated with neonatal skin pH or hydration in the first 24 hours of life. Early neonatal skin physiology appears to be influenced more by intrinsic maturation than maternal atopic status. Longitudinal studies are needed to assess potential delayed effects.

Abstract

Acne vulgaris (AV) is an inflammatory skin disorder with multifactorial pathogenesis involving genetics, hormones, and immune dysregulation. Interleukin-21 (IL-21), a pleiotropic cytokine mainly secreted by T follicular helper and Th17 cells, has been implicated in various inflammatory skin diseases. However, its role in AV remains unclear. This cross-sectional study included 46 patients with AV, classified into mild (n=19), moderate (n=16), and severe (n=11) groups. Serum IL-21 levels were measured using an enzyme-linked immunosorbent assay (ELISA). Baseline characteristics were compared across groups, and interobserver reliability of AV severity grading was evaluated using Cohen’s kappa test. One-way ANOVA was applied to assess differences in IL-21 levels among severity groups, with a significance level set at p<0.05. The mean serum IL-21 level was highest in the severe AV group (303.53 pg/mL), followed by the moderate (223.7 pg/mL) and mild (213.2 pg/mL) groups. Although there was a trend toward increasing IL-21 levels with higher AV severity, statistical analysis revealed no significant difference among the three groups (p=0.130). Serum IL-21 levels were elevated in AV patients compared with previously reported healthy controls but did not differ significantly across severity levels. These findings suggest that IL-21 may play a role in the pathophysiology of AV, though it is not a reliable marker of clinical severity. Further case-control and tissue-based studies are needed to clarify its role.

Abstract

Syphilis remains a significant global health burden, particularly among high-risk populations such as men who have sex with men (MSM). A major challenge in management is the serofast state, where patients fail to achieve serological cure despite adequate treatment. The immunological mechanisms driving this, especially in the context of HIV co-infection, remain unclear. This study aims to evaluate clinical and immunological factors associated with serofast status versus serological cure. A retrospective cohort study was conducted on 40 patients diagnosed with syphilis (early latent, late latent, or secondary). Data on demographics, HIV status, CD4/CD8 counts, and pre- and post-treatment VDRL titers were analyzed. Bivariate and multivariate logistic regression analyses were performed to identify predictors of the serofast outcome. The cohort was predominantly males (90.0%) and unmarried (80.0%). HIV co-infection was identified as the sole independent predictor of the serofast state (aOR = 10.94; 95% CI: 1.72-69.65; p=0.011). While post-treatment VDRL titers significantly decreased in the total cohort, the serofast group retained significantly higher antibody levels compared to the serological cure group (p<0.001). However, no statistically significant differences were found in CD4 counts, CD8 counts, or CD4/CD8 ratios between the two groups. HIV co-infection is a strong predictor of the serofast state in syphilis patients. The lack of significant differences in quantitative cellular immune markers suggests that the persistence of antibodies may be driven by functional immune dysregulation rather than absolute T-cell depletion. These findings emphasize the need for vigilant serological monitoring in HIV-syphilis co-infected individuals.

Abstract

Acne vulgaris is a chronic inflammatory disorder of the pilosebaceous unit associated with Cutibacterium acnes colonization and activation of the Toll-like receptor–NF-?B pathway, which upregulates production of inflammatory mediators such as interleukin-6 (IL-6). Bromelain, a proteolytic enzyme derived from pineapple (Ananas comosus), is known to possess anti-inflammatory properties; however, experimental evidence regarding its effect on inflammatory cytokines in acne remains limited. This study aimed to evaluate the effect of oral bromelain administration on IL-6 levels in the tissue of acne vulgaris model rats. This in vivo laboratory experimental study employed a posttest-only control group design. Fifteen male Sprague-Dawley rats induced with C. acnes were divided into three groups: normal control (K1), negative control (K2), and treatment group (P) receiving oral bromelain at 88 mg/kgBW/day for 21 days. Tissue IL-6 levels were measured by enzyme-linked immunosorbent assay (ELISA). Data were analyzed using one-way ANOVA followed by a post hoc least significant difference (LSD) test. C. acnes induction significantly elevated tissue IL-6 levels compared to the normal control (p<0.001). One-way ANOVA revealed statistically significant differences in IL-6 levels between groups (p<0.001). The lowest IL-6 levels were observed in K1 (36.50±0.43) and the highest in K2 (94.01±0.57). The treatment group (P; 47.46±0.83) demonstrated significantly lower IL-6 levels than K2 and approximated K1 values. The post hoc LSD test confirmed statistically significant differences across all pairwise group comparisons (p<0.001). Oral bromelain administration effectively reduced IL-6 levels in the tissue of acne vulgaris model rats, demonstrating significant anti-inflammatory activity. These findings suggest that bromelain has potential as an adjunct therapy for inflammatory control in acne vulgaris.

Abstract

Background: Acne vulgaris (AV) is a chronic inflammatory disorder of the pilosebaceous unit that significantly impacts patients' quality of life. The inflammatory process involves immune responses partially regulated by IL-24, suggesting its potential role in AV pathophysiology and clinical severity. However, data on serum IL-24 levels based on AV severity remain limited, particularly in Indonesia. This study aims to evaluate the relationship between serum IL-24 levels and acne vulgaris severity to further elucidate its molecular mechanisms. Methods: This observational analytical study with a cross-sectional design was conducted at Dr. Moewardi General Hospital, Surakarta, Indonesia, from November 2025 to January 2026, involving healthy individuals and acne vulgaris patients. AV severity was assessed using the Lehmann classification, and serum IL-24 levels were measured by ELISA. Results: A total of 60 subjects were involved, consisting of 15 healthy controls, 15 patients with mild AV, 15 with moderate AV, and 15 with severe AV. Pearson correlation analysis showed no significant relationship between serum IL-24 levels and AV severity (p = 0.186). However, there was a trend towards increasing serum IL-24 levels along with acne severity. In the healthy control group, IL-24 levels were lower than in the AV group. Conclusions: Serum IL-24 levels are not associated with acne vulgaris severity. This is likely due to its effect on local keratinocyte proliferation. Further research is needed to elucidate the pathophysiology using histopathological examination of skin tissue from patients with AV

Abstract

Background: Second-degree burns result in significant tissue damage and require optimal management to prevent complications and delayed healing. Silver sulfadiazine (SSD) is widely used as a standard topical therapy for burn wounds. Topical sucralfate has cytoprotective properties and has been suggested to enhance wound healing, making it a promising adjuvant therapy in wound management. Objective: To compare the reduction in second-degree burn wound size following treatment with 10% sucralfate cream, 7% sucralfate cream, and silver sulfadiazine in a rat model Methods: This experimental study employed a post-test only control group design using 48 Sprague Dawley rats with standardized second-degree burn injuries. The subjects were randomly allocated into four groups: Negative control with no treatment (NC), positive control with silver sulfadiazine (PC), treatment group with 7% sucralfate cream (T1), treatment group with 10% sucralfate cream (T2). Wound size was evaluated on days 1, 3, 7, and 14 using digital photographic analysis. Results: A statistically significant difference in wound size reduction was observed among the groups on day 14 (p < 0.001). The 10% sucralfate group (T2) demonstrated the greatest wound reduction, with a mean wound diameter of 44.62 ± 21.43 mm, which was comparable to the silver sulfadiazine group (48.89 ± 10.41 mm). Both groups showed significantly smaller wound sizes compared to the 7% sucralfate group (281.35 ± 99.33 mm) and the negative control group (936.48 ± 159.71 mm). Conclusion: The second-degree burn wound size in the 10% sucralfate group was smaller compared to the 7% sucralfate and silver sulfadiazine groups.
