Abstract

The use of technology-based light therapies such as intense pulsed light and heat energy(IPL) provides an alternative therapy for patients with acne. However, clinical evidenceis required to evaluate the efficacy and safety of the IPL. This study aimed to evaluate theefficacy of IPL compared to benzoyl peroxide (BP) as standard therapies in patients withacne vulgaris. The study was conducted with randomized controlled trial parallel designinvolving patients with mild and moderate acne vulgaris. Acne severity was determined bythe method of Combined Acne Severity Classification (CASC). Statistical analysis usingrepeated measurement analysis of variance was conducted to assess the reduction inlesions and number of P. acnes in each group followed by independent t-test to compareof both groups. A p value <0.05 was considered significant. Sixty-two patients withmild and moderate acne vulgaris were enrolled in this study and treated with IPL (32patients) and with BP gel 2.5% (30 patients). Two patients from the IPL were droppedout. All subjects showed improvement in acne lesions. Reduction of the number ofnon-inflammatory lesions at IPL therapy group was not significantly different than theBP gel 2.5% at week 2 (p=0.705) and 4 (p=0.186). Reduction in the numberof inflammatory lesions in the IPL treatment group was not significantly different than BPgel 2.5% at week 2 (p=0.604) but significantly higher at week 4 (p=0.003). Thereduction of P. acnes colonization in the IPL group was significantly higher than BP gel2.5% group at week 2 (p=0.000) and 4 (p=0.000). In conclusion, the efficacy of IPLin the reduction of the number of inflammatory lesions and the P. acnes colonization isbetter than BP on patients with acne vulgaris.

Abstract

BACKGROUND: Psoriasis is an autoimmune disease involving genetic-environmental factors. Human Leukocyte Antigen (HLA)-Cw6 allele is the main genetic risk factor of psoriasis, but its prevalence varies widely. HLA-Cw6 allele correlates with psoriasis clinical type and treatment responses. Narrowband ultraviolet B (NB-UVB) and methotrexate (MTX) are effective psoriasis treatments but their association with HLA-Cw6 allele has not been identified. The study aims to determine the association between HLA-Cw6 allele expression, NB-UVB, and MTX treatment responses in Javanese-Indonesian psoriasis subjects.METHODS: Ninety Javanese-Indonesian psoriasis subjects were recruited in this study, 45 subjects were treated using NB UVB, while the other 45 subjects were treated using MTX, respectively, for 12 weeks. The psoriasis diagnosis and treatment responses were evaluated by dermatologists using Psoriasis Area Severity Index (PASI) score. The HLA-Cw6 allele was examined using the single-specific-primer polymerase chain reaction method. Fisher's Exact and Chi-square tests were employed, where p-value<0.05 was considered of significant association.RESULTS: The HLA-Cw6 allele positivity was identified in 23 psoriasis subjects (25.56%), while the other 67 subjects expressed HLA-Cw6 allele negative (74.44%). Female with HLA-Cw6 allele positivity who did not have comorbid disease show good response to NB-UVB than MTX. Meanwhile, subjects who were treated with MTX showed no association between therapeutic response and HLA-Cw6. HLA-Cw6 status was not correlated with the onset of psoriasis, family history, and comorbid diseases in all subjects.CONCLUSION: HLA-Cw6 allele expression is more associated with good NB-UVB treatment response than with MTX treatment response in female Javanese-Indonesian psoriasis subjects without comorbid disease.KEYWORDS: HLA-Cw6, narrowband ultraviolet B, methotrexate, Javanese, psoriasis

Abstract

Discoid lupus erythematosus is the most common forms of chronic cutaneous lupus erythematosus. It is characterized by clinical manifestations of erythematous macules, papules, or plaques with a coin-like shape and the face is the most common predilection site. Clinical features often resemble granuloma faciale. This case report aimed to distinguish discoid lesions on the face based on the histopathological examination. A 71-year-old male with a few reddish lumps appeared on his face since three months ago. Physical examination showed multiple discrete erythematous plaques with overlying squamous. Hematoxylin and eosin staining on the epidermis demonstrated basket weave type orthokeratosis, basal vacuolar cell degeneration, epidermal atrophy with flat rete ridges, and follicular plugging while in the dermis obtained inflammatory cell infiltrates, especially in periadnexal areas. Histopathological features of DLE are hyperkeratosis, pilosebasea gland atrophy, follicular plugging, basal membrane thickening, and cellular infiltrate in periadnexa or perivascular areas more visible than in other types of CLE. In DLE, no subepidermal gren zone and eosinophil infiltrate were found, like histological features of granuloma faciale. Histopathological examination can be used to establish a diagnosis for discoid lesions on the face, although serology examination remains as the gold standart. Keywords:  Discoid lupus erythematosus; Granuloma faciale; Histopathology

Abstract

Latar belakang: Morbus Hansen (MH) merupakan infeksi kronik akibat Mycobacterium leprae yang mengenai terutama kulit dan saraf. Reaksi reversal ditandai dengan bercak eritematosa keunguan, bercak lama menjadi lebih tampak serta muncul bercak baru. Risiko silent neuropati dapat terjadi pada reaksi reversal sehingga diperlukan deteksi kerusakan saraf dini. Pemeriksaan histopatologi sebagai salah satu metode pemeriksaan yang diperlukan untuk mendukung diagnosis dan tipe MH secara tepat.Tujuan: Penulisan makalah ini bertujuan untuk melaporkan satu kasus reaksi reversal pada MH tipe borderline tuberkuloid berdasarkan tinjauan histopatologi Kasus: Seorang wanita, berusia 55 tahun mengeluhkan bercak kemerahan yang mati rasa pada regio fasialis, trunkus anterior et posterior, ekstrimitas inferior. Pasien memeriksakan ke dokter spesialis kulit dan mendapat terapi multi drug therapy (MDT) pausibasiler (PB) selama 3 bulan. Bercak kemudian menjadi lebih tampak dan bertambah disertai pengelupasan kulit, lecet, pembengkakan kedua tungkai. Pemeriksaan basil tahan asam (BTA) tidak didapatkan BTA. Pemeriksaan histopatologi pada lapisan epidermis tampak ortokeratosis tipe lamelar, atrofi epidermis dan pendataran rete ridge. Pada dermis tampak edema, granuloma dengan batas tidak tegas, histiosit epiteloid dan sel raksasa berinti banyak tipe langhans, tidak ada “gren zone”. Pada pengecatan FF tidak didapatkan BTA.Diskusi: Reaksi pada MH dapat terjadi sebelum, saat dan setelah pengobatan MDT ditandai dengan peningkatan inflamasi dan peninggian pada lesi sebelumnya. Keterlambatan dalam diagnosis dan tatalaksana pada MH dengan reaksi dapat menyebabkan kerusakan saraf yang berimplikasi pada disabilitas dan deformitas yang mempengaruhi kualitas hidup pasien.

Abstract

Background: Primary cutaneous diffuse large B-cell lymphoma, leg type (PCDLBCL-LT), is a rare and aggressive lymphoma with poor prognostic. It is difficult to diagnose PCDLBCL-LT at an early stage due to its nonspecific manifestations that overlap with other lymphomas. A histopathological examination can be performed to establish the diagnosis. Case Presentation: A 74-year-old female presented with itchy red, swollen patches accompanied by a burning sensation on both her legs. Clinical examination revealed a hard ping pong ball-sized lump palpable on her left inner thigh. Haematoxylin eosin staining demonstrated the proliferation of diffused neoplastic cells, homogenous cells with atypical nuclei, and mitoses. Immunohistochemical examination revealed positive for CD 45 as well as CD 20 and negative for CD3. These findings were consistent with a diagnosis of PCDLBCL-LT. Conclusion: The presented case demonstrates that skin lesions in patients with diagnosed PCDLBCL-LT may have a variable clinical presentation. Histologically is characterized by a diffuse dermal infiltrate mostly consisting of activated B-cell (centroblast) and activated lymphocyte (immunoblast), with high mitotic activity and a minimal T-reactive component. Immunohistochemical analysis is necessary for establishing the diagnosis of PCDLBCL-LT which shows positive markers for CD 45, CD 20, CD79a, Bcl-2, MUM-1, FOX-P1, CD10, IgM, CD 138, Ki-67, CD 30 and MYC.

Abstract

Nail psoriasis can affect the patient's quality of life, so to evaluate the quality of life in patients with skin diseases in general, the Dermatology Quality of Life Index (DLQI) can be used. The questionnaire was unable to distinguish the impact on quality of life caused by nail abnormalities or skin lesions, so a special scoring system was needed to evaluate the quality of life for nail psoriasis patients with the Nail Psoriasis Quality of Life Scale 10 (NPQ10). To find the relationship between the severity of nail posirais based on Nail Psoriasis Severity Index (NAPSI) score with DLQI and NPQ10. The total respondents were 20 people with a diagnosis of nail psoriasis who came to the Dermatovenereology Outpatient Clinic of Dr. Moewardi General Hospital, Surakarta from April-June 2020. We took photography and dermoscopy of fingernails and toes and then calculated the NAPSI score. All respondents filled out the DLQI and NPQ10 questionnaires. Pearson's (r) correlation test was used to find the relationship between the mean and standard deviations of NAPSI and DLQI and NPQ10. We also tested the correlation between DLQI and NPQ10. The data significance value was declared significant if p <0.05. The results of the correlation test between NAPSI scores with DLQI and NPQ10 showed a statistical relationship, where the correlation value between NAPSI and DLQI was slightly more significant than NPQ10 (r = 0.948: r = 0.877; p <0.05). The mean value of DLQI and NPQ10 in women was higher than men (14.12 ± 8.21: 11.5 ± 8.29 and 11.5 ± 3.58: 8.16 ± 4.52) with a correlation test value of DLQI and NPQ10 is r = 0.888, p <0.05. We found that the association between NAPSI correlates more significantly with DLQI than NPQ10. This may be due to some questions that do not reflect the daily activities of our study subjects, but NPQ10 score in women is higher than men where the quality of life for nail psoriasis is also influenced by gender.

Abstract

Background: The COVID-19 pandemic causes health workers to use level 2 and level 3 personal protective equipment. Hand washing and the use of latex gloves are important in limiting the spread of COVID-19, but they can potentially induce hand eczema and skin pH disturbance if used too frequently. Purpose: Determine the correlation between the frequency of handwashing and duration of using hand sanitizer and moisturizer as well as skin pH level on the incidence of hand eczema in nurses in the COVID-19 treatment room at RSUD Dr. Moewardi Surakarta. Methods: This cross-sectional study was conducted in the COVID-19 treatment room at RSUD Dr. Moewardi Surakarta with 90 nurses as subjects. A validated questionnaire carried out the frequency of handwashing, hand sanitizer, moisturizer, duration of using latex gloves, and hand eczema complaints. The subject's pH assessment was conducted after 4-5 hours in the COVID-19 treatment room. Data were analyzed bivariately with a Spearman rank correlation test. Result: There was a significant correlation with the incidence of hand eczema with the frequency of handwashing with soap (r=0.305 and p=0.003) and the duration of using latex gloves (r=0.328 and p=0.002) with a weak category correlation (r=0.200-0.399). The frequency of using hand sanitizer, moisturizer, and skin pH did not correlate with the incidence of hand eczema. Conclusion: Frequency of handwashing and the duration of using latex gloves correlated with the incidence of hand eczema, meanwhile frequency of using hand sanitizer, using moisturizer, and skin pH didn't correlate with the incidence of hand eczema.

Abstract

Cutaneous T-cell lymphomas (CTCLs) are clinically heterogeneous T-cell lymphomas that arise in the skin and are characterized by their clinical and pathological features. The pathogenesis of CTCL is not fully understood. The incidence of CTCL increases significantly with age, with a median age at diagnosis at about 55 years of age and a four-fold increase in incidence appreciated in patients over 70. A 53 year old woman presented with complaints of reddish spots and lumps on her left forearm and back which spread to her chest, stomach and legs. On examination, a lump the size of a golf ball was found on the left forearm and back as well as reddish spots that felt itchy and painful. Histopathological examination with HE staining showed proliferation of lymphoid follicles without a germinal center and monotonous cells of small to medium size partially surrounding the tubular glands in the dermis layer. Immunohistochemical examination revealed positive for CD 45 as well as CD 3 and negative for CD 20. A case of CTCL has been reported in a 53-year-old woman. Histopathological examination and immunohistochemistry are important to diagnose this disease.

Abstract

Sucralate is an aluminum salt from sucrose octasulfate that is known for its anti-ulcer activity, mucosal protection, and anti-mucositis potential. Recently, sucralfat has been used topically for the healing of various epithelial wounds, including ulcers, inflammatory dermatitis, mucositis, and burns. This systematic review aimed to evaluate the effectiveness of sucralfat as a topical therapy in reducing itching (pruritus) and improving the skin barrier. The analysis method used is Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA). A literature search was conducted on studies from 2004 to 2024 using keywords such as "sucralfate", "pruritus", "dermatitis", and "skin barrier" on PubMed, ProQuest, Science Direct, and Scopus databases. Inclusion criteria include topic relevance, research design, human subjects, and United Kingdom-speaking studies. Of the 141 articles found, 7 articles met the inclusion criteria. These studies involved a total of 605 subjects from different countries and used clinical trial methods and randomized controlled trials. The results showed that topical sucralfat was effective in reducing itching and improving the skin barrier in various skin conditions such as diaper dermatitis, chronic ulcers, and postoperative wounds. Sukralfat shows great potential in wound healing and skin barrier repair through the mechanism of protective layer formation, increased expression of epidermal growth factors, and anti-inflammatory properties. This effect indirectly helps reduce pruritus which often occurs due to damage to the skin barrier. Topical succulthate is effective in reducing itching and repairing the skin barrier, making it a promising therapy for a variety of inflammatory and ulcerative skin conditions.

Abstract

Background: The elderly population has been growing rapidly in recent decades. Research has shown that the prevalence of itching (pruritus) increases with age, with 20.8% of individuals aged 60-69 years experiencing it, 22.9% for those aged 70-79 years, and 26% for those aged 75 years and above. Assessing the skin's barrier function by measuring transepidermal water loss (TEWL) using a tool called a Tewameter is important for objectively evaluating dry skin, which is the most common cause of itching in the elderly. It's important to understand how itching affects the quality of life of the elderly, particularly in nursing homes. Objective: This study aims to determine how itching affects the quality of life of the elderly in nursing homes in Surakarta City, especially at PMI Peduli and Aisyiyah Nursing Homes. Methods: This research used an observational, cross-sectional design. Data was collected through interviews using the 5D itch scale questionnaire, and the quality of life was measured using the DLQI (Dermatology Life Quality Index) questionnaire. TEWL levels were also measured using a Tewameter. Results: 29 subjects participated in the interviews, with 69% being women and 31% being men. Most subjects were aged 60-69 years (41%), and the most common comorbidity was hypertension (41%). The 5D Itch Scale questionnaire revealed that itching affected the subjects for less than 6 hours daily (66%), and during the last 2 weeks, itching did not significantly impact the subjects' activities, including sleep, hobbies, housework, and work (51%-69%). The most common location of itching was the back (14%). The DLQI data showed that itching had a severe impact on the subjects (65.5%), followed by a moderate impact (24.1%), and a very severe impact (10.3%). TEWL data indicated that all subjects had an increase of >10 g/h/m2. Conclusion: The study found that itching had a mild impact on the lives and activities of the subjects in general, lasting for less than 6 hours a day. The statistical results showed a positive and significant relationship between the intensity of itching and the quality of life of the elderly with pruritus.
