Abstract

Background: The raising number of patient safety incident is one of the major hospital problems. Reporting patient safety incident is the best method to improve the patient safety.Objective of the Research: To evaluate the realization of the patient safety incident reporting program. Methods: This research used mixed method that combined quantitative and qualitative approach. The subjects of this research were nurses, doctors and the other health officers. The samples of this research were 195 health officers by using proportionate stratified random sampling and purposive sampling technique. Analyzes test using percentage and thematic analyzes. Results: There are 194 incidents report recorded in 2017, 48 % involving the unexpected incident, 28% nearmiss, 22% not injured and 2% sentinel. Based on the patient safety culture survey, there are only 3 of 12 patients safety culture dimension that already meet the standards, there are feedback and the communication about the error (75%), learning organization and upgrading process (79%) and good teamwork in the hospital (85%). While patient safety incident reporting culture is still below the specified standard. This research olso has identified successfully the barriers of the patient safety incident report, they are lack of the knowledge, blamming and punishment culture, lack of peer support, lack of leadership support, not reported the small incident, lack of time, lack of form and incident reporting is still considered as the duty of nurses. Conclusions: The patient safety incident reporting program has worked, but it has not been agood culture. This is caused by some barriers factor.

Abstract

Child exploitation in Indonesia, especially by parents in the practice of begging, is a serious problem that violates children's human rights. This research analyzes law enforcement against criminal acts of child exploitation based on Law No. 23/2002 on Child Protection, which prohibits economic exploitation and provides strict sanctions. Despite the legal provisions, implementation and law enforcement still face significant challenges, including economic factors, low parental education, and social norms that consider begging as normal. This research uses a normative juridical method with a statutory and case approach to evaluate the application of the law and provide recommendations. The results show that the lack of legal awareness and rehabilitation support for victims hinders the effectiveness of protection. Therefore, collaboration between the government, law enforcement officials, and the community is needed to raise awareness about children's rights, strengthen law enforcement, and provide sustainable social interventions. With concrete steps, it is hoped that the protection of children from exploitation can be effectively realized.

Abstract

This article examines the synergy between Islamic theological values and local wisdom as a basis for resistance to environmental exploitation in Mandailing Natal. The focus of the study lies on the application of faith principles, such as the concepts of khalifah, justice, and ihsan, and their implementation through the traditional practice of lubuk larangan. The literature study method was used to collect and analyze secondary data from scientific articles, journals, and national and local news sources. The results of the study show that these Islamic theological values have been deeply rooted in the culture of the Mandailing Natal community, encouraging a critical attitude towards illegal gold mining practices that damage the ecosystem. The lubuk larangan tradition also acts as a mechanism for social control and environmental conservation, by implementing periodic restrictions on extractive activities to provide space for natural regeneration. The findings indicate that the integration of Islamic theology and local wisdom results in a more integrated and ethical natural resource management paradigm. This research recommends strengthening the synergy of faith values and local traditions in the formulation of public policies and increasing community participation in environmental monitoring. This integrative approach can be used as a strategic model to overcome the challenges of environmental exploitation amid increasingly complex global dynamics.

Abstract

Sexual violence against children is a serious crime and requires appropriate and effective sanctions. One form of punishment applied in Indonesia is chemical castration, as stipulated in Law Number 17 of 2016 and Government Regulation Number 70 of 2020. Although it aims to provide a deterrent effect, this punishment has drawn pros and cons, especially regarding its effectiveness and compatibility with the principles of maqashid sharia. Maqashid sharia emphasises the protection of five main aspects: soul, mind, offspring, property, and religion. This study aims to analyse the application of chemical castration sanctions within the framework of maqashid sharia, as well as assess its impact on the rehabilitation of perpetrators and the prevention of paedophilia crimes. A qualitative method was used with a literature study approach collected from various legal sources and maqashid sharia theory.  The results of the analysis show that although this sanction is in line with the principles of preventing harm and protecting future generations, its implementation still draws pros and cons. The main criticism states that chemical castration may violate human rights and Islamic principles of justice as a whole, mainly due to the significant psychological and social impact on the offender, as well as its debatable effectiveness as a temporary solution.  The study concluded that although chemical castration has the potential to be a preventive measure, there is still a need for in-depth studies and the development of alternative sanctions that emphasise justice for both parties and a proportional rehabilitative approach to overcome trauma and the negative impact of sexual crimes against children.

Abstract

The phenomenon of increasing cases of online gambling (judol) and drug abuse among the community, especially the younger generation, poses a serious threat to the future of the nation. This community service program aims to identify the root causes of the problem and raise public the people of Huta Tinggi Village of the social, economic, and psychological impacts of both issues. Through an educational approach, field observations, and interactive dialogues, this initiative targets vulnerable groups such as teenagers, students, and parents. The results of the initiative indicate that low digital literacy, weak family supervision, and a lack of productive spaces are key factors driving high levels of community involvement in online gambling and drug abuse. Additionally, easy access to the internet and unrestricted social interactions accelerate their spread. The persuasive education provided has proven effective in fostering new awareness within the community. The initiative also involves local religious leaders and youth to strengthen the sustainability of positive messages. It is hoped that these efforts will serve as the first step toward building family- and community-based social resilience. Thus, addressing the dangers of gambling and drugs is not merely reactive but also preventive and sustainable.

Abstract

This study aims to analyze the role of the gula merah (traditional brown sugar) industry in promoting the economic empowerment of the community of Huta Tinggi Village, Puncak Sorik Merapi District, Mandailing Natal Regency. Conducted by KKN students of Group 33, the research used a qualitative approach: field observation, in-depth interviews with business actors, and documentation of the production process. The main findings indicate that the gula merah industry significantly increases household incomes, creates local employment, and strengthens an economy grounded in traditional wisdom. The still-traditional production process functions as a cultural identity and presents an opportunity to increase added value through packaging innovation, quality improvement, certification, and digital marketing. Identified challenges include limited equipment, low quality standards, minimal promotion, and restricted market access. Practical recommendations include technical training and quality management for producers, investment in simple equipment, assistance with packaging and branding, guidance on digital marketing, and facilitation of market access and microfinance by local government. Partnerships with educational institutions and the private sector are also advised for product research, halal and quality certification, and research to support product diversification. In conclusion, the Huta Tinggi gula merah industry is not only economically valuable but also strategic for rural community empowerment; implementing these recommendations through immediate actions, such as training, financing access, and digital promotion, combined with regular monitoring and evaluation can sustainably strengthen local economic resilience and engage younger generations.
