Abstract

Background: In Indonesia, every year more than 15,000 cases of cervical cancer detected and approximately 8000 cases of which were fatal. The high prevalence of cervical cancer deaths in Indonesia may be caused by delays in diagnosis so that when detected, the disease has reached an advanced stage. Early detection of cervical cancer by Pap Smear is suitable method applied in developing countries because in addition to easy, cheap, effective, non-invasive. But in practice, these methods are still having problems such as doubt of the importance of examinations, lack of knowledge, and the fear of pain during the examination. The objective of this research is to know  the correlation between knowledge level of women in  Reproductive Age Couples (RAC) concerning Pap Smear with Pap Smear Test services utilization. Methods: The study design was analytic observational with cross sectional approach. The sample of this research was women in  Reproductive Age Couples (RAC) in Gajahan Health Center, Surakarta. The number of samples were 90 people and were teken by simple random sampling tehnique. The people who was chosen as the sample are given the questionnaire, then they answered the question in the questionnaire. After the data collected, then the data was analyzed using Chi-Square test. (χ2). Results: Percentage of women in Reproductive Age Couples who use VIA test services is 37.8%. Statistical tests using Chi-Square test performed on the variables which  were knowledge level concerning Pap Smear with Pap Smear Test services utilization. Statistical test result value of the test statistic (χ2) was 9.137 with a significance (p) of 0.003. P-value < 0.1 means that the confidence level of 90% or 10% significance level significant correlation both variables. Conclusions: There was a correlation between knowledge level of women in  Reproductive Age Couples (RAC) concerning Pap Smear with Pap Smear Test services utilization. Women with higher knowledge concerning Pap Smear, lean to utilize Pap Smear test  services. Abstract

Orbital metastases of cervical cancer cell are very rare. There is only one case reported in Dr. Moewardi Public Hospital Surakarta between 1999 and 2017. A 45-year-old woman was admitted with the upper right orbital lesion. She had not received a routine gynecologic examination for more than 10 years. She had normal chest X-ray. Abdominal ultrasonography revealed a heteroechoic lesion poor defined edge in cervix utery expands to corpus utery and rectum. Bilateral uropathy obstruction occured. Bone survey demonstrated osteolytic bone metastasis in the right supra orbital bone and a compression fracture of the twelfth thoracal spine. Head and orbital Computed Tomography (CT) scan, which revealed multiple metastatic lesions in the right orbital expanding to extra cranial, was performed. Our diagnosis was cervical cancer FIGO IV-B stage with distant metastasis thus chemoradiation was given. The aim of this report is to give more references regarding this complicated clinical condition.  ABSTRAKKanker serviks metastasis ke orbita sangat jarang terjadi. Hanya satu kasus yang dilaporkan di Rumah Sakit Umum Daerah Dr. Moewardi Surakarta antara tahun 1999 - 2017. Seorang wanita usia 45 tahun dirawat oleh karena lesi di orbita kanan atas sejak 2 bulan yang lalu. Pasien tersebut belum pernah mendapatkan pemeriksaan kandungan rutin selama lebih dari 10 tahun. Foto Chest X Ray normal. Ultrasonografi abdomen tampak lesi heteroekoik batas tak tegas pada serviks uterus meluas ke korpus uterus dan rektum. Terjadi obstruksi uropati kanan dan kiri. Bone survei memperlihatkan gambaran metastasis tipe litik pada tulang supra orbita kanan dan fraktur kompresi pada tulang belakang toraks ke dua belas. Computed Tomography (CT) Kepala dan Orbita memperlihatkan beberapa lesi metastasis pada supra orbita kanan yang meluas ke tulang tengkorak. Diagnosis kami adalah kanker serviks tahap FIGO IV-B dengan metastasis jauh dan diberikan therapi kemoradiasi. Tujuan penulisan ini adalah untuk memberikan lebih banyak referensi data tentang kondisi klinis yang rumit ini.

Abstract

Background: Uterine cervical cancer is one of the main causes of female death related to cancer. Neoadjuvant chemotherapy aims to reduce tumor mass to allow radical surgery. HIF-1α is thought to have a key role in the development of cancer and the main target for chemoprevention.Objective: This study aims to prove the effect of neoadjuvant chemotherapy on HIF-1α expression in uterine cervical cancer as an assessment parameter for chemotherapy response. Subjects and methods: The study was carried out in the Department of Obstetrics and Gynecology and the Pathology Department of Anatomy Dr. Moewardi - Faculty of Medicine, Sebelas Maret University, Surakarta. Thirty (30) cervical cancer samples that met the inclusion and exclusion criteria were examined for HIF-1α expression before and after giving 3 times neoadjuvant chemotherapy. Examination using immunohistochemical methods. Data analysis using t-test.Results: Mean HIF-1α expression before administration of neoadjuvant chemotherapy 5.10± 1.174 cell/field, after administration of neoadjuvant chemotherapy 4.00±1.174 cell/field with p=0.001.Conclusion: Neoadjuvant chemotherapy has an effect on reducing HIF-1α expression in uterine cervical cancer.Keywords: neoadjuvant chemotherapy, HIF-1α, uterine cervical cancerCorrespondence: Sri Sulistyowati. Abstract

Background: Uterine cervical carcinoma is the most gynecological malignancy in women worldwide. The most common type of cervical carcinoma histology is squamous cell carcinoma and has a better prognosis than adenocarcinoma. Neoadjuvant chemotherapy is the management of early-stage cervical carcinoma before radical hysterectomy is performed. In addition to clinical-pathological factors, biomolecular markers are now being developed and used as parameters for evaluating chemotherapy responses. Vascular Endothelial Growth Factor-C is a VEGF derivative that plays a role in the process of vasculogenesis, limphangiogenesis, and the formation of new lymphatic channels which play an important role in metastasis. This study aimed to examine the effect of Paxus-Carboplatin chemotherapy in decreasing VEGF-C in squamous cell cervical carcinoma tissue.Subjects and Method: This was a quasi experiment study conducted at Department of Obstetrics and Gynecology and Pathology Anatomy Laboratory of Dr. Moewardi Hospital/ Faculty of Medicine, Universitas Sebelas Maret. 25 tissue samples of squamous cell stage IB2-IIA2 cervical carcinoma immunohistochemistry VEGF-C expression was carried out before and after Paxus-Carboplatin chemotherapy was given. The data were analyzed by Wilcoxon test.Results: VEGF-C expression in squamous cell cervical carcinoma tissue after chemotherapy was lower (mean = 4.20) than before chemotherapy (mean = 6.16) and it was statistically significant (p<0.001).Conclusion: Neoadjuvant chemotherapy decreases VEGF-C in squamous cell cervical carcinoma tissue.Correspondence: Uswatun Khasanah Kartikasari. Abstract

ABSTRAKLatar Belakang: Pemahaman akseptor KB (Keluarga Berencana) tentang KB IUD dan implan masih rendah, hal ini dikarenakan minimnya pengetahuan dan adanya persepsi negatif mengenai kontrasepsi IUD dan implan di masyarakat. Perlu adanya upaya peningkatan pemahaman akseptor KB untuk meningkatkan kualitas dalam pelayanan KB untuk mencapai kesehatan individu yang optimal dan menurunkan angka fertilitas melalui peningkatan pemakaian alat kontrasepsi.Tujuan: Mengetahui pengaruh perlakuan penyuluhan terhadap peningkatan pemahaman dan kesiapan akseptor KB terhadap efek samping alat kontrasepsi di Klinik Solo Peduli.Metode: Pengabdi memberikan penyuluhan kepada akseptor KB mengenai KB IUD dan implan. Kemudian, Akseptor KB dibagikan kuesioner meliputi pertanyaan tentang efek samping yang dirasakan, serta langkah yang dapat dilakukan ketika efek samping timbul.Hasil: Dari 60 responden, didapatkan mayoritas tingkat pengetahuan efek samping alat kontrasepsi akseptor KB sebanyak 65% adalah “baik”, 35% adalah “kurang”, dan terdapat 18 (30%) orang yang mengalami efek samping minimal, diantaranya: nyeri, ketidaknyamanan hubungan seksual, kenaikan berat badan, jerawat, siklus mentruasi berubah setelah pemasangan alat kontrasepsi.Kesimpulan: Terdapat peningkatan pengetahuan efek samping alat kontrasepsi setelah dilakukan penyuluhan dan efek samping yang relatif minimal.Kata Kunci: Akseptor KB, IUD, Implan, Pelayanan KB. ABSTRACTBackground: The understanding of family planning acceptors about IUD and implant contraception is still low, this is due to the lack of knowledge and negative perceptions about IUD contraception and implants in the community. There needs to be an effort to increase understanding of family planning acceptors to improve the quality of family planning services to achieve optimal individual health and to reduce fertility rates through increased use of contraceptives.Objective: To determine the effect of counseling treatment on increasing understanding and readiness of family planning acceptors on the side effects of contraceptives at the Solo Peduli Clinic.Methods: The service provider provides counseling to family planning acceptors about IUD and implant family planning. Then, family planning acceptors were distributed questionnaires covering questions about side effects that were felt, as well as steps that could be taken when side effects arose.Results: Of the 60 respondents, it was found that the majority of the level of knowledge about the side effects of contraceptives acceptors of family planning as much as 65% were "good", 35% were "less", and there were 18 (30%) people who experienced minimal side effects, including: pain, discomfort of sexual intercourse, weight gain, acne, menstrual cycles changed after the insertion of contraceptives.Conclusion: There is an increase in knowledge of the side effects of contraceptives after counseling and the relatively minimal side effects.Keywords: family planning acceptors, IUD, implants, family planning services.

Abstract

Background: Assisted reproductive technology (ART) programs have proven to be a solution to help infertility couples to conceive. However, most couples face a financial crisis related to the relatively expensive cost of ART treatment. This service aims to educate about ART's meaning, direction, and procedures. Thus, couples want children to be able to understand the ART program at an affordable cost (low-cost) and undergo the ART program without any concerns about costs.Method: Seminar participants be given socialization in person and online through zoom software by the service provider. It is in two stages, in-person and online, so participants who cannot attend in person can follow this service program. In addition, participants are given a pre-test and post-test questions to assess their understanding of the ART program at an affordable cost.Result: The pre-test and post-test results were analyzed using Wilcoxon, which showed that after providing education about the ART program, the post-test results were significantly better than the pre-test results. Down value: 5.5%, fixed value: 15.2%, value rise: 79.3% with significance p<0.001.Conclusion: There are significant differences in the pre-test and post-test scores, which means that the participants understood the explanation the servicer gave.

Abstract

Background: The development of screening methods for infertility patients during the Coronavirus-2019 (COVID-19) pandemic provides new challenges in the adaptation of children's want programs in the era of the COVID-19pandemic. Several screening methods have been developed but various factors affect the awareness of couples to want to do COVID-19 screening. This study aimed to analyze biopsychosocial factors associated with awareness of COVID-19 screening in infertile couples. Subjects and Method: A cross-sectional study conducted at The Sekar Polyclinic of Dr. Moewardi Regional General Hospital during the COVID-19 pandemic. A number of 60 infertile couple of reproductive age who underwent the ART program was selected by random sampling. The dependent variable was awareness for COVID-19 screening. The independent variables were anxiety, knowledge, age, and length of infertility. The data were obtained from medical record and questionnaire and analyzed by a multiple logistic regression. Results: Anxiety (OR= 26; 95% CI; p<0.001) and above-average knowledge levels (OR=16; 95% CI; p<0.001) affected awareness for COVID-19 screening. While the age and length of infertility do not affect awareness for COVID-19 screening (p >0.05). Conclusion: Anxiety and above-average knowledge levels affect awareness for COVID-19 screening.
