Abstract

ABSTRAKHipertensi hingga kini masih menjadi masalah kesehatan di dunia. Terapi yang diberikan kepada pasien hipertensi biasanya dilakukan dalam jangka panjang. Pengobatan hipertensi yang dilakukan sesuai dengan kondisi pasien. Penelitian ini bertujuan untuk mengetahui gambaran pola penggunaan obat antihipertensi pada pasien rawat inap di RSUD Panembahan Senopati. Penelitian dirancang dengan metode deskriptif crosssectional. Subyek penelitian ialah pasien hipertensi rawat inap yang memenuhi kriteria inklusi. Objek pada penelitian ini adalah rekam medis pasien rawat inap hipertensi periode bulan Oktober 2016- Juli 2017. Hasil penelitian menunjukkan bahwa dari 53 pasien yang memenuhi kriteria inklusi, sebanyak 38 pasein (71,8 %) menjalani pengobatan antihipertensi secara politerapi, sedangkan hanya 15 pasien (28,2 %) menjalani pengobatan monoterapi. Pengobatan monoterapi yang paling banyak dipakai golongan diuretik yaitu Furosemid sebanyak 6 pasien (11,3%), sedangkan penggunakan obat politerapi yang paling banyak dipakai yaitu Valsatan+ Amlodipin sebanyak 13 pasien ( 24,5%). Secara keseluruhan, golongan obat yang paling banyak digunakan adalah golongan Calcium Chanel Blocker dengan jenis terbanyak adalah Amlodipin, dilanjutkan golongan Diuretik yaitu Furosemid, dan golongan ARB yaitu Valsartan. Penelitian ini disimpulkan bahwa penggunaan obat sebagian besar politerapi dan secara umum penggunaan obat antihipertensi yang dipakai terdiri dari 6 golongan obat yaitu diuretik, ACE inhibitor, Angiotensin Receptor Blocker, ẞ- blocker. Kata kunci: Pola Penggunaan Obat; Antihipertensi; Rawat Inap. ABSTRACTHypertension is a health problem in the world. Therapy given to hypertensive patients is usually done in the long run. Hypertension treatment is carried out according to the patient's condition. This study aims to determine the pattern of the use of antihypertensive drugs in inpatients at Panembahan Senopati Hospital. The study was designed using descriptive cross sectional method. The subjects of the study were hypertensive inpatients who met the inclusion criteria. The object of this study was the medical record of hypertensive inpatients for the period October 2016-July 2017. The results showed that of 53 patients, 38 patients (71.8%) used antihypertensive treatment by polytherapy, whereas only 15 patients (28.2%) used monotherapy treatment. The most used monotherapy treatment of diuretics is Furosemide with 6 patients (11.3%), while the most used polytherapy drugs are Valsatan + Amlodipin with 13 patients (24.5%). Overall, the most used group of drugs is the Calcium Chanel Blocker with the most types, Amlodipine, followed by the Diuretic group, Furosemide, and the ARB group, Valsartan. This study concluded that the use of drugs is mostly polytherapy and in general the use of antihypertensive drugs used consists of 6 classes of drugs namely diuretics, ACE inhibitors, Angiotensin Receptor Blockers, ẞ-blockers. Keywords: Drug Use Patterns; Anti-hypertension; Inpatient.

Abstract

Tuberkulosis dan Diabetes adalah kondisi penyakit yang saling berkaitan sehingga diperlukan terapi agresif untuk mengatasinya. Adanya Drug Related Problems (DRPs) yang terjadi dapat mempengaruhi outcome pengobatan pasien Tuberkulosis-Diabetes Mellitus (TB-DM). Penelitian bertujuan untuk menganalisis hubungan terjadinya DRPs dan outcome pengobatan pasien TB-DM. Penelitian menggunakan rancangan kohort retrospektif. Pengambilan sampel dilakukan menggunakan consecutive sampling  melalui catatan rekam medik pasien TB-DM pada 9 Puskesmas di Kota Malang. Sejumlah 100 kasus pasien TB-DM yang menjalani pengobatan  tahun 2017 hingga 2020 yang memenuhi kriteria inklusi selanjutnya  dibagi menjadi dua kelompok, yaitu kelompok yang mengalami DRPs dan kelompok pasien yang tidak mengalami DRPs sejumlah masing-masing 48 pasien dan 52 pasien. Jenis DRPs mengikuti klasifikasi Cipolle 2004. Outcome pengobatan dinilai berdasarkan hasil tes BTA (Bakteri Tahan Asam) pada akhir masa pengobatan bulan kedua. Analisis data menggunakan statistik Chis-quare dilanjutkan analisis multivariat untuk menganalisis adanya variabel perancu yang diprediksikan dapat mempengaruhi outcome pengobatan pada penelitian ini. Hasil penelitian menunjukkan terdapat hubungan antara kejadian DRPs dan outcome pengobatan pasien (p-value <0,05). Pasien dengan DRPs 5,41 kali lebih beresiko mengalami kegagalan terapi dibandingkan dengan pasien yang tidak mengalami DRPs (RR 5,417; 95% CI, 1,994-14,713). Keterlibatan farmasi klinis sangat diperlukan untuk mencegah dan menyelesaikan masalah terkait obat serta monitoring pengobatan pada pasien TB-DM yang ada di Puskesmas sehingga luaran pengobatan menjadi optimal

Abstract

The availability of drugs in healthcare facilities will ensure that people get the medicines they need. To ensure the availability of drugs in healthcare facilities, good drug management must be carried out. The drug management cycle includes the stages of selection, planning, procurement, storage, and distribution. This systematic review aims to identify and characterize research methods, guidelines, indicators, and delivery of results in the evaluation of drug and vaccine management in District/City Health Offices. This systematic review uses the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology. Search for articles using Google Scholar, limiting the management of drugs and vaccines carried out at District/City Health Offices which are published from 2017 to 2022. Articles that meet the criteria are evaluated for methods, guidelines, and data processing, and the management stages are evaluated. Article quality was assessed using a modified Downs and Black checklist. Search results using the Google Scholar database obtained 1435 articles. After checking the research location and the year the article was published, 1426 articles did not meet the inclusion criteria. Of the nine articles than one article entered the exclusion criteria because it was a literature review. The results of a systematic review show that qualitative methods are the most frequently used in evaluating drug management, but qualitative methods also have many weaknesses to cover these weaknesses, mixed method research can be carried out. Quantitative research obtained the results of the percentage of drug management accuracy against indicators and qualitative research was carried out to find problems through content analysis. It is hoped that combining these methods will result in more precise drug management evaluation results so that suggestions for improvement will be more targeted.

Abstract

Hyperlipidemia is a non-communicable disease (NCD) caused by several factors, such as a person's socioeconomic status, culture, customs, habits, and lifestyle. Through user interaction on social media, we can discover the model anti-hyperlipidemia by extracting information, complaints, suggestions, and calls for help about the treatment, which will play a role as an intervention to reduce hyperlipidemia in Indonesia. This study aimed to identify factors influencing perceptions of hyperlipidemia drugs and resulting sentiment on the social media platform Twitter. This study used user-uploaded tweet data to compare perceptions of hyperlipidemia drugs in 2020 and keywords for hyperlipidemia terms and medicine. Tweets related to anti-hyperlipidemia were extracted by issuing tweets containing advertisements, news, re-tweet, and content outside of health. The tweet data obtained was then carried out through content analysis, including point of view, theme, and sentiment analysis, to identify whether the resulting tweets are positive, neutral, or negative using the Support Vector Machine (SVM) method. We identified 1572 hyperlipidemia-related tweets and 153 specific tweets describing hyperlipidemia medications. Tweets about anti-hyperlipidemia showed 99 tweets from the first-person perspective, 23 from the second-person perspective, 22 from healthcare professionals, and nine unidentifiable (other). Sixty-three tweets talked about the benefits of lipid-lowering drugs, 17 complaint tweets, 49 suggestion tweets, 17 question tweets, and two side effect tweets. Assessing public perceptions and sentiment toward hyperlipidemia treatment can be used to develop strategies to increase treatment adherence, improve treatment outcomes, and target health promotion efforts.
