Abstract

Regulations concerning grants and social assistance, particularly at the Surabaya City level, still lack legal certainty. The regulations contained in Surabaya Mayor Regulation No. 25 of 2016 and its amendments are inconsistent with higher-level regulations, one of which is Minister of Home Affairs Regulation No. 77 of 2020. In this situation, harmonization of regulations is needed to provide legal certainty that is fair. Using a normative juridical method, with a regulatory and conceptual approach. The findings of this study indicate the need for regulatory harmonization from a philosophical, sociological, and juridical perspective. Grants and social assistance are highly relevant to the state's goal of improving the welfare of its people, so access to them must be based on clear regulations. This will ultimately provide equitable benefits to the community. In addition, harmonization is directed at material regulatory changes. Several aspects of the regulations include the target recipients, use, and duration. Furthermore, it needs to be reconstructed, especially with regard to the duties or authorities of the administrators. One of the findings is the strengthening of the Regional Work Unit in managing Social Assistance.

Abstract

Setiap orang yang melakukan tindak kejahatan, pasti menerima konsekuensi hukum berupa pemidanaan sesuai dengan peraturan perundang-undangan. Salah satu bentuk pemidaan adalah dipenjara dalam kurun waktu tertentu. Meskipun demikian, terdapat peluang untuk dilakukannya bebas bersyarat atas vonis pidana penjara. Namun, pemberian bebas bersyarat tersebut masih terdapat masalah hukum berupa konflik norma, yaitu antara Permenkumham No. 3 Tahun 2018 dan Perubahannya dengan Permenkumham No. 32 Tahun 2020 dan Perubahannya. Persoalan krusial adalah pada adanya syarat mengetahui lurah atau kepala desa dalam surat jaminan untuk bebas bersyarat. Dilakukan penelitian hukum normatif, untuk mengkaji persoalan tersebut. Didukung dengan pendekatan peraturan perundang-undangan dan pendekatan konseptual. Setiap orang yang telah divonis hukuman penjara, masih sangat dimungkinkan untuk mendapatkan bebas bersayarat. Pertimbangannya adalah mengurangi kepadatan Lapas (overcrowding), menghemat pos anggaran Narapidana, dan sebagai bentuk pembinaan kepada Narapidana agar tidak mengulang kejahatannya. Syarat mengetahui lurah atau kepala desa dalam surat jaminan untuk pembebasan bersyarat, hanyalah merupakan aspek formil. Hal itu justru berdampak pada akses keadilan yang bersifat birokratis. Idealnya perlu ada penguatan dari Permenkumham No. 32 Tahun 2020 dan Perubahannya sebagai bagian dari terobosan hukum yang lebih berkeadilan dan berkepastian hukum.

Abstract

Highlights: • Sodium hydroxide causing platelet rich fibrin stimulates limbal stem cell proliferation in chemical trauma. • Limbal stem cell niche and influences limbal stemness was repaired by Platelet Rich Fibrin. Abstract: Chemical injuries of the eye produce extensive damage to the ocular surface and limbal stem cells, resulting in permanent unilateral or bilateral visual impairment. Alkali injuries occur more frequently than acid injuries. Platelets are a rich source of potential wound healing, promoting polypeptide growth factors. This study aimed to investigate the effect of platelet-rich fibrin (PRF) lysates on limbal stem cell proliferation, which was exposed to sodium hydroxide that resembled limbal stem cell deficiency due to chemical trauma. Confluent rabbit (Oryctolagus cuniculus) limbal stem cells wounded using 20µL of 0.00625 M sodium hydroxide (pH 13) were treated with platelet-rich fibrin lysates (PRF) (0, 5, and 10%). PRF lysates were prepared from peripheral rabbit blood according to Choukroun's method without using anticoagulant and foreign factors for platelet activation. The proliferation of limbal stem cells was measured by a 3-(4,5-dimethylthiazol-2-yl)-2.50 diphenyl tetrazolium bromide (MTT) colorimetric assay at 24, 48, and 72 hours after exposure to sodium hydroxide. Proliferation significantly increased limbal stem cells with PRF lysates 5% (p<0.01) and 10% (p<0.01) group compared with the control (PRF 0%). There was no significant difference between PRF lysates of 5% and 10% (p>0.01). The highest proliferation of limbal stem cells was found in the PRF lysates 5% group after 48 hours (100.24%). PRF stimulated limbal stem cell proliferation in chemical trauma caused by the sodium hydroxide model. PRF repaired the limbal stem cell niche and influenced the limbal stemness. The present findings warrant further research on PRF as a novel alternative treatment for limbal stem cell deficiency.

Abstract

Patients with advanced heart failure (NYHA FC III and IV heart failure) had positive cardiac troponin levels in previous cohort studies. In heart failure, cardiac troponin T (cTnT) is a biomarker that is sensitive to myocardial damage, especially myocardial necrosis. However, there is still little information regarding changes in cTnT levels during standard therapy. This prospective observational study is aimed at evaluating changes in cTnT levels before and after theadministration of standard therapy and evaluating symptom improvement before and after the administration of standard therapy in patients with severe heart failure. Measurement of cTnT levels and symptom improvement parameters before treatment was carried out on the first day of the inpatient and measurement after therapy was carried out on the last day of the inpatient. Sampling was done by consecutive sampling and found 30 patients in the inpatient room of the SMF Cardiovascular Disease, Dr. Soetomo Hospital, Surabaya during the months of May-July 2017. The results of the study obtained the average cTnT levels before therapy 33.48 + 31.88 pg/ml and the average cTnT levels after therapy 46.32 + 52.68 pg/ml. Based on the statistical difference test with the Wilcoxon sign-ranked test, there was no significant change in cTnT levels (p = 0.318). On the parameter of clinical symptom improvement, there was a significant decrease in pulse, respiratory rate, blood pressure, and mean arterial pressure before and after administration of therapy (p <0.05). There was no change in troponin T levels before and after the administration of therapy meant there was no worsening of myocardial necrosis.

Abstract

This study aimed to analyze the differences in Hsp 70 serum levels in patients with primary angle-closure compared to patients with open-angle glaucoma at Dr. Soetomo General Academic Hospital, Surabaya, East Java. This cross-sectional study was conducted on 41 subjects divided into 2 groups consisting of 21 patients with primary angle-closure glaucoma and 20 patients with open-angle glaucoma. Anterior segment examination was assessed by slit-lamp biomicroscope examination. Intraocular pressure on both eyes was measured by Perkin's tonometry. Posterior segment examination was performed with 78 D lens and anterior chamber angle was assessed by goniolens. The serum level of Hsp 70 was assessed using a human Hsp 70 sandwich ELISA kit. The data were statistically analyzed with the Mann-Whitney test. The mean serum levels of Hsp 70 in the primary angle-closure group was 5.55  4.75 ng/ml, while the open-angle group was 3.62  2.39 ng/ml (p = 0.134). There was no difference in Hsp 70 serum levels between the two groups, although the trend obtained serum levels of Hsp 70 patients with primary angle-closure was higher compared to patients with open-angle.
