Abstract

Background: Adverse event studies of favipiravir use in treating COVID-19 have been ongoing since it was established as a treatment option. A better understanding of the side effects of favipiravir from recent studies is important in developing and assessing the recognition of effective treatments for COVID-19. Method: This was a systematic review based on studies and case reports on favipiravir monotherapy in COVID-19. Access to the included studies was gained via PubMed, SCOPUS, Science Direct, SpringerLink, and MedRxiv. Results: Twelve studies consisting of eight studies and four case reports were reviewed. The most common side effects were diarrhea, elevated liver enzyme levels, and hyperuricemia. None of which were significantly different from the comparison. Currently, various adverse event were reported in case reports such as drug fever, acute generalized exanthematous pustulosis (AGEP), and transient increase in viral load. The side effects would mostly be subsided after the treatment was discontinued. Conclusion: The use of favipiravir to treat COVID-19 caused dose-related side effects such as diarrhea, changes in liver enzymes, and increased level of uric acid. There were no serious side effectscompared to other antiviral drugs. To improve the efficacy and safety of COVID-19 therapy, it is important to prepare an incidence report of antiviral adverse events in special populations such as children, pregnant women, and  patients with organ dysfunction.

Abstract

Domain swapping menjadi salah satu mekanisme terbentuknya struktur multimer.

Abstract

The global monkeypox (MPOX) outbreak, which has resulted in more than 100,000 confirmed cases across over 120 countries, underscores the urgent need for effective therapeutic strategies. MPOX, caused by a virus belonging to the Orthopoxvirus genus, poses significant clinical and public health challenges, particularly in regions with limited healthcare capacity. Although the World Health Organization declared MPOX a Public Health Emergency of International Concern in 2022, no specific antiviral treatment has yet been formally approved. While vaccination remains essential for long-term prevention, its impact is constrained by limited availability and unequal global distribution. Consequently, drug repurposing has emerged as a practical, rapid, and cost-efficient strategy to address immediate therapeutic demands during active outbreaks. This narrative review evaluates key repurposed antiviral agents including tecovirimat, brincidofovir, and cidofovir were originally developed for other Orthopoxvirus infections. A structured literature search using PubMed, ScienceDirect, and Google Scholar (2014–2024) identified studies providing clinically relevant evidence regarding their therapeutic roles in MPOX. Tecovirimat demonstrated consistent potential to improve viral clearance, shorten symptom duration, and reduce severe manifestations. Brincidofovir exhibited a more favorable safety profile, albeit with limited clinical data, whereas cidofovir showed antiviral activity but remains restricted by its nephrotoxicity. Collectively, current evidence suggests that drug repurposing offers a timely and resource-efficient option for managing MPOX and mitigating disease burden while vaccine accessibility continues to vary globally. When integrated with targeted public health interventions, repurposed drugs may serve as a crucial interim strategy to reduce transmission and enhance outbreak response capacity.

Abstract

Research on the safety of remdesivir in Indonesian subjects for COVID-19 therapy is still limited and has yielded different results. None of these studies have used the Naranjo algorithm to analyze the causality of remdesivir safety. This study aimed to evaluate the safety of remdesivir in patients with severe COVID-19. Data were collected using medical records of patients with severe COVID-19 at Cengkareng Regional Hospital, Indonesia. The study was conducted retrospectively with a cohort study design. Subjects consisted of the remdesivir group (n=80) and the non-remdesivir group (n=80). Adverse drug reactions (ADRs) were observed, and causality analysis of ADRs was performed using the Naranjo algorithm. The study found 10 ADRs experienced by patients during treatment. ADRs were more common in the remdesivir group than in the non-remdesivir group (58.8% vs 53.8%; p=0.633). Significant events with a high proportion in the remdesivir group were increased ALT (p=0.037) and lymphopenia (p=0.01). Hepatotoxic reactions associated with remdesivir use were associated with P-glycoprotein (P-gp) inhibitor interactions. Decreased lymphocyte counts were closely related to the clinical condition of the study subjects, namely patients with severe COVID-19, and this condition was associated with a poor prognosis. Increased blood triglycerides, lymphopenia, and decreased estimated glomerular filtration rate (eGFR) were categorized as probable. Meanwhile, nausea, melena, QT interval prolongation, increased ALT, increased AST, hyperuricemia, and neutrophilia were categorized as quite possible. The results of this study can be concluded that remdesivir causes liver dysfunction manifested by increased ALT values and decreased lymphocyte counts, which worsen the condition of patients with severe COVID-19.

Abstract

Candida glabrata is one of the leading causes of non-albicans Candida infections with an increasing prevalence worldwide. This species is associated with high mortality rates and a marked tendency to develop resistance to antifungal agents, particularly azoles such as fluconazole. A comprehensive understanding of its biological characteristics, pathogenic mechanisms, and resistance patterns is essential for improving diagnosis and therapeutic strategies. The genetic factor that plays an important role in this resistance mechanism is the ERG11 gene, which encodes the lanosterol 14-α-demethylase enzyme, the main target of fluconazole in the ergosterol biosynthesis pathway. This review was conducted based on an analysis of recent literature focusing on the biology, epidemiology, pathogenesis, antifungal resistance mechanisms, susceptibility testing methods, and molecular identification of Candida glabrata. Special emphasis was given to the role of the ERG11 gene in fluconazole resistance and the application of molecular tools such as ITS and D1/D2 rDNA sequencing for species-level identification. The findings indicate that Candida glabrata exhibits several virulence factors, including adhesins, biofilm formation, cell wall modifications, and iron acquisition strategies. Antifungal resistance is mediated through mutations in the ERG11 gene, overexpression of efflux pumps (CgCDR1, CgCDR2, CgSNQ2), alterations in sterol biosynthesis pathways, and mitochondrial dysfunction (petite mutants). Susceptibility testing methods following CLSI/EUCAST standards, as well as automated systems such as VITEK 2, have shown reliable and rapid results. Candida glabrata is an emerging pathogen with rising prevalence and complex antifungal resistance mechanisms. Accurate species identification and molecular analysis of the ERG11 gene are crucial for understanding local resistance patterns and supporting the development of more effective therapeutic strategies.
