Abstract

Environmental sanitation encompasses housing, sewage disposal, and clean water supply, all crucial to health. A healthy environment sustains the ecosystem, while a poor one leads to sanitation issues and disease spread. The STBM program in Indonesia aims to empower communities to reduce open defecation, though challenges in improving sanitation access persist. This study explores the factors influencing open defecation and provides recommendations for enhancing sanitation and environmental health. The research, conducted in Kampung Dalam Village, Bilah Hulu Sub-district, Labuhan Batu Regency, North Sumatra, began in January 2023. An initial survey found that 96 households lacked healthy latrines. The study used a quantitative cross-sectional design with a sample of 50 households, determined using the Lameshow formula and selected through Simple Random Sampling (SRS). Independent variables included knowledge, attitude, economic status, and facilities (healthy restrooms), while the dependent variable was open defecation behavior. Data collection involved questionnaires and direct interviews with respondents, analyzed using univariate and bivariate methods and the Chi-Square test with a 95% confidence level. The Chi-Square test results showed significant relationships between open defecation behavior and knowledge (p=0.019), attitude (p=0.001), economic status (p=0.009), and facilities (p=0.002) with p-values ≤ 0.05. The study concluded that while knowledge and attitude are linked to open defecation behavior, they do not always directly influence it, suggesting other factors like environment and facilities also play a role. Lower economic status was more often associated with open defecation, and better facilities and access contributed to reduced open defecation behavior.

Abstract

Background: Controlling hypertension through pharmacological treatment is effective but can potentially lead to long-term side effects. Thus, more patients with hypertension have turned to herbal plants for treatment. The objective of this study was to examine the effectiveness of soursop leaf tea in reducing blood pressure in patients with hypertension. Methods: This study employed a quasi-experimental approach with a non-randomized pretest-posttest control group design, involving 30 participants in the intervention group and 30 participants in the control group. Data was analyzed using the Wilcoxon test to assess the disparity in mean blood pressure levels before and after consumption of soursop leaf tea. The Mann-Whitney test was used to examine the distinction in mean blood pressure between the intervention group and the control group. Results: In the intervention and control groups, most respondents were aged 56–65 years (60% and 66.7%), male (70% and 56.7%) and had suffered from hypertension for more than 5 years (70% and 56.7%). There was a significant difference in systolic blood pressure (p<0.001) and diastolic blood pressure (p<0.001) before and after the administration of soursop leaf tea. The mean systolic and diastolic blood pressure in the intervention group decreased significantly compared to the control group (p<0.001).Conclusions: Consumption of soursop leaf tea can reduce systolic and diastolic blood pressure, suggesting that this local herb has a potential effect on individuals diagnosed with hypertension.

Abstract

Job burnout is a common psychological condition experienced by nurses due to high work pressure, heavy workload, and continuous emotional demands. Two important factors believed to influence job burnout are self-efficacy and social support. This study aims to analyze the influence of self-efficacy and social support on the level of job burnout in nurses. This study used a quantitative method with a cross-sectional approach. A sample of 100 nurses was taken using a purposive sampling technique. The research instrument was a structured questionnaire that measured the level of self-efficacy, social support, and job burnout. Data analysis was performed using univariate, bivariate (Chi-Square test), and multivariate (logistic regression). The results showed that self-efficacy had a significant effect on job burnout (p = 0.019), while social support showed no significant effect (p = 0.397). Most respondents had high self-efficacy (60%) and experienced moderate to high levels of job burnout. Self-efficacy is a protective factor against job burnout in nurses, while social support did not significantly influence this study. Therefore, improving self-efficacy needs to be a focus of interventions to reduce job burnout levels in hospital settings.

Abstract

Abstract Service quality is a key determinant of patient satisfaction and hospital performance. In psychiatric settings, satisfaction depends not only on clinical competence but also on empathy, communication, and responsiveness. This study aimed to analyze the relationship between service quality and patient satisfaction among psychiatric outpatients at Aek Kanopan Regional General Hospital, North Sumatra. This quantitative descriptive–analytic study used a cross-sectional design. A total of 150 outpatients were selected using accidental sampling. Data were collected using a SERVQUAL questionnaire with five dimensions—tangibility, reliability, responsiveness, assurance, and empathy—rated on a five-point Likert scale. Validity and reliability tests yielded Cronbach’s α = 0.977. Data analysis employed descriptive statistics, Pearson correlation, and simple linear regression with a significance level of p < 0.05. The mean perception score (17.05) was lower than the mean expectation score (18.48), producing an average negative GAP of –1.43, indicating unmet patient expectations. The largest gap occurred in reliability (–3.26), followed by tangibility (–2.56), responsiveness (–1.31), and empathy (–0.34), while assurance showed a small positive GAP (+0.11). Correlation tests revealed that responsiveness and empathy were positively and significantly associated with patient satisfaction (r = 0.027 and r = 0.001, respectively; p < 0.05). Overall, psychiatric outpatients at RSUD Aek Kanopan were dissatisfied with most dimensions of service quality. Enhancing reliability, responsiveness, and empathy, while maintaining strong assurance performance, is essential to improving patient satisfaction and the overall quality of mental health services.
