Abstract

The suitability of CABG compared to PCI for patients with coronary artery disease (CAD) remains a controversial issue. Despite growing evidence supporting the clinical efficacy of these revascularization strategies, there is little evidence regarding their long-term cost-effectiveness. The aim of this study was to critically evaluate the literature regarding the cost-effectiveness of CABG compared to PCI and assess the quality of the available economic evidence.The methods is a systematic review was conducted using three databases: PubMed, Scopus and Google Scholar. Three studies were retrieved then compared the economic evaluation of CABG vs PCI measures The result is the improvement (ICER) reported across studies varied widely by perspective and timeframe. ICER calculation was reported to be favorable and cost effective for CABG. The conclusions is CABG is more cost-effective than PCI in cases of coronary artery disease. The evidence supporting this cost-effectiveness will continue to evolve and further evaluation over a period of 10 years or more is needed considering societal perspectives.

Abstract

There have been a growing number of patients demanding endodontic treatment to save their teeth. This study aimed to determine the pattern of demand and treatment need for root canal treatments at a tertiary healthcare facility in Jambi, Indonesia and update the dental literature with the demand and pattern of root canal treatments as seen in Jambi. This was a retrospective study. Data from 10.429 patients treated at conservative dentitistry clinic, at top two tertiary referral hospital in Jambi. Data was taken from January through December 2022 were reviewed. Data were analyzed using IBM SPSS Version 26. The association between categorical variables was assessed using the Chi-square test. Significance level was set at p<0.05. There were 5.940 patients age 15 to 70 years (Mean ± SD 34,60 ± 13,06) of which 3.564 (60.2%) were female and 2.376 (40%) male received root canal treatment. More female patients demanded for root canal treatment but treatment need was greater in female patients and increased with age. Demand for root canal treatment was more prevalent in those < 40 years of age. Central incisors were the most frequently root filled teeth in the maxilla and this was statistically significant (p=0.002). Prevalence of teeth diagnosed Pulpitis Irreversible were the most frequently endodontic treatment. The finding in this study corroborate those of several other previous studies.

Abstract

Sejak ditemukannya sinar-X pada tahun 1895, pelayanan radiologi terus berkembang dengan munculnya radiografi digital pada tahun 1979. Penelitian ini mengevaluasi efektivitas penggunaan radiologi digital dan PACS untuk meningkatkan aksesibilitas, mengurangi biaya, dan meningkatkan kualitas perawatan pasien di Rumah Sakit X. Penelitian ini menggunakan data envelope analysis (DEA) untuk mengukur efisiensi pelayanan radiologi berbasis film dan layanan radiologi berbasis digital yang dilakukan di suatu rumah sakit, serta mengevaluasi kondisi aktual fasilitas radiologi di RS X (radiologi berbasis film) dan hipotetikal (radiologi berbasis digital). Nilai efisiensi digunakan untuk menentukan efisiensi. Studi DEA terhadap layanan radiologi di Rumah Sakit X membandingkan efisiensi radiologi berbasis film (DMU 1) dan radiologi digital (DMU 2). Dengan asumsi output layanan sama, model berbasis digital mengasumsikan perubahan input. Analisis menggunakan model CCR-DEA menunjukkan bahwa radiologi berbasis digital memiliki efisiensi paling tinggi (skor 1,0), sedangkan radiologi berbasis film memiliki nilai efisiensi sebesar 10,1%. Meskipun DMU 1 memiliki peringkat efisiensi yang rendah, analisis menunjukkan adanya potensi perbaikan dan DMU 2 dinilai efisien secara optimal. Berdasarkan penelitian tersebut, DEA dapat digunakan untuk meningkatkan efisiensi pelayanan radiologi  dengan  teknologi tersebut

Abstract

Latar Belakang: SIMRS adalah alat informasi yang kuat yang dapat membantu manajer rumah sakit untuk memperbaiki proses pengambilan keputusan dan untuk meningkatkan fungsi positif rumah sakit. Keberhasilan implementasi SIMRS tergantung pada kepuasan pengguna, kebutuhan akan persepsi sistem, kepercayaan dan rasa kepemilikan pengguna terhadap sistem, serta partisipasi pengguna dalam pengembangan sistem.Tujuan: Penelitian ini bertujuan untuk Menganalisis Faktor yang Mempengaruhi Penerimaan Sistem Informasi Manajemen Kesehatan Rumah Sakit.Metode: Scoping Review menggunakan database seperti Pubmed, Wiley Online Library, dan ScienceDirect. Hasil pencarian yang memenuhi kriteria kemudian di analisis menggunakan PRISMA Flowchart, ekstraksi data, dan Mapping Tema.Hasil:  Hasil penelitian dari 148 artikel yang terkait dengan judul dan abstrak, 7 artikel memenuhi kriteria inklusi dan eksklusi. Ditemukan tiga faktor yaitu faktor manusia, faktor teknologi, dan faktor organisasi.Kesimpulan: Menyederhanakan penggunaan sistem melalui pendidikan bagi pengguna dan menyediakan komprehensif dan pedoman khusus yang sesuai dengan spesialisasi atau departemen pengguna, termasuk kebutuhan kerja pengguna dalam kemampuan penerimaan  SIMRS. Melibatkan pengguna dalam pengembangan, implementasi, dan pendidikan langkah-langkah perangkat lunak SIMRS sangat penting untuk ditingkatkan ke tingkat yang ideal, meningkatkan kepuasan pengguna, dan meningkatkan penerimaan sistem ke tingkat yang optimal.

Abstract

Healthy Indonesia Program with Family Approach (PIS PK) is a way to expand Puskesmas reach and access to health services through family home visit Overcoming non-communicable diseases (NCD) is a national priority in achieving Healthy Indonesia because the prevalence of NCD continues to increase. Managing NCD specifically hypertension, Diabetes Mellitus and mental disorders are among the indicators of healthy families in achieving PIS PK.This research uses the qualitative method through in-depth interviews and related document. Analysis of readiness of financing for hypertension, Diabetes Mellitus and mental is calculated using costing methode of SPM. The results indicated health spending for PIS PK activities were utilized for socialization, education and data collection. Referring to SPM costing calculation, the City of Depok was able to implement SPM for hypertension, Diabetes Mellitus and Mental Disorder with using only 1.38% of the total APBD (non-salary). Depok City is not ready in implementing PIS PK, specifically in terms of commitment, human resources, and budget. Though already familiar with PIS PK, Depok City states due to its limited resources, PIS PK implementation depends on the availability of funding from Non-Physical DAK disbursement that tends to have a lengthy lag time between the proposal process and the realization of the disbursement. The financing of PIS PK requires a more optimal planning process and allocation of human resources as needed.

Abstract

One of the foremost government policies implemented in achieving Universal Health Coverage for the Indonesian population is the provision of financial assistance through contribution of Social Health Insurance for 92.4 million targeted poor and near poor households. This segment of the population is referred to as Penerima Bantuan Iuran (PBI) and represents about 35% of the total population. This study reveals the government’s progress in protecting the health of this sub-population. The data is derived from the 2016 National Social Economic Survey. The results indicate that, at the national level, the government’s health protection program has reached the majority of poor and near poor household (51%), especially those residing in Eastern part of the country (58%). Together with local government’s support, 59.5% of poor and near poor households (66.4% in Eastern region) have been insured. While improvements in coverage should still be top policy agenda, this achievement of the government deserves more appreciation. We show that misallocation of PBI to wealthy households is only small (3%), yet often broadcasted with much hype to create agitation

Abstract

Diabetes mellitus is an epidemic disease that seriously threatens global health. In addition to the high levels of morbidity and mortality, it also causes many countries around the world to spend a significant amount of money on healthcare. This study aims to calculate the direct medical costs and influencing factors of type 2 diabetes mellitus outpatient care. The study was conducted from January to February 2015 at RSUD dr. Abdul Aziz Singkawang in West Kalimantan. The study design used quantitative analysis with secondary data retrospectively collected from 2013, with a sample size of 200. The results of the multivariate analysis showed significant differences in the direct medical costs of type 2 diabetes mellitus outpatient care based on the type of care, disease duration, and complications. Health promotion and prevention interventions for type 2 diabetes mellitus should be increased to achieve effective and efficient care costs and to improve patients' quality of life.

Abstract

Budget management is a form of accountability for managing cash budgets distributed by the government to hospital agencies so that the budget provided by the central government can be used effectively, efficiently and on target. To examine and explore budget management at hospitals in Indonesia during the COVID-19 pandemic in relation to the budget for the procurement of Personal Protective Equipment (PPE) and infrastructure in hospitals as an effort to overcome the COVID-19 pandemic. PPE (Personal Protective Equipment) is one of the crucial instruments that must be used by all medical personnel in carrying out their duties, this triggers an increase in the PPE procurement budget. Literature Review method by reviewing relevant previous research. Budget management in hospitals has not been categorized as effective and efficient so that more mature planning and budget management are needed for hospitals to manage. As we know that the allocation of funds made to several hospitals in Indonesia was realized by purchasing tools and PPE equipment to add facilities and infrastructure used in tackling the COVID-19.

Abstract

Rumah sakit harus dapat terus meningkatkan kualitas pelayanannya. Transparansi harga dapat menjadi salah satu strategi pemasaran yang bisa dilakukan oleh layanan Kesehatan untuk tetap bertahan dalam pasar. Transparansi harga merupakan salah satu metode untuk dapat menginformasikan harga sebuah produk atau layanan sehingga pelanggan dapat secara efektif menentukan keputusan dalam membeli. Metode yang digunakan dalam penulisan ini menggunakan Litterature review dengan menggunakan pencarian ditiga database online yaitu Pubmed, Proquest, dan Google Scoolar. Kata kunci yang dipakai adalah “price transparency” dan hospital, didapatkan sebanyak 110 artikel dan setelah dilakukan penelaahan lebih lanjut sesuai dengan kriteria yang telah ditentukan didapatkan 7 artikel akhir. Penerapan transparansi harga pada layanan kesehatan di rumah sakit dapat mendorong menurunnya variasi harga dan kesenjangan tarif pada layanan kesehatan. Hal tersebut dapat memicu standarisasi harga di setiap rumah sakit.

Abstract

Background: The COVID-19 pandemic had resulted in an unprecedented strain on healthcare syste­ms. Hospitals are faced with the unexpected influx of patients, increasing need for protection equ­ip­ment and infection control procedures, increased workload and operational hours, and the depleting revenues due to closure of elective services. This study aims to further analyze the financial str­a­tegies used by hospitals to ensure its operations and to overcome financial challenges presented by the COVID-19 pandemic.Subjects and Method: A systematic review has been identified using four databases: MEDLINE (Pub­Med), Google Scholar, SCOPUS, and MedSci. This search in literature was published articles from January 2020 to June 2021 using PRISMA guidelines. The keywords used “COVID-19” OR “SAR­S-CoV-2” AND (ii) "hospital" OR "hospital cost" OR "cost" AND (iii) "strategy" OR "financial stra­tegy" OR "economic" OR "economic analysis".Results: 6 studies were included in our final analysis. We found that effective financial strategies to co­ver for reduced revenues and increasing cost of care include: (1) Ensuring incentives from external ag­e­ncies, including the government; (2) Creating new budgets or modifying existing budgets to cover for COVID-19-related cost of care; and (3) Use of contactless health system such as telehealth. Speci­fically for modifying existing budgets, studies have found that implementing proper health protocol and infection control measures is the most effective, as it has been reported to yield a return of invest­ments.Conclusion: Government funds and relief aids remain the most important source of financial suppo­rt for hospitals in caring for COVID-19 patients. Without sufficient financial support, hospitals have to get creative in adding additional service fees or finding innovative solutions such as telehealth to cover the dwindling revenues from their closed service units.Keywords: COVID-19, hospital cost, financial strategiesCorrespondence: Suci Kirana Yulius.
