Abstract

Visum et Repertum (VeR) merupakan salah satu dari lima alat bukti yang sah dalam pembuktian hukum. Pembuatan VeR sampai sekarang belum mempunyai standardisasi yang jelas, sehingga dibutuhkan adanya evaluasi pembuatan VeR. Evaluasi pembuatan VeR berdasarkan dari masalah-masalah yang ditemukan polisi dalam permintaan pembuatan VeR yang kemudian digunakan sebagai saran perbaikan pembuatan VeR terutama pembuatan VeR klinik. Penelitian ini bertujuan untuk mengetahui harapan polisi di Kota Bandung terhadap pembuatan VeR klinik. Penelitian ini menggunakan desain deskriptif kualitatif dengan cara wawancara mendalam kepada enam orang polisi yang telah memenuhi kriteria inklusi dan eksklusi di Kepolisian Wilayah Kota Besar (Polwiltabes) Kota Bandung sejak bulan Agustus sampai Desember 2016. Hasil penelitian ini menunjukkan bahwa polisi mengalami kesulitan dalam memahami bahasa yang digunakan dalam visum, dokter kurang responsif dalam pembuatan visum, dan tidak terdapat standar waktu untuk memproses pembuatan visum. Temuan lainnya adalah  terdapat penolakan visum dari pihak rumah sakit karena korban sudah mendapatkan pengobatan di tempat lain dan keterlambatan surat izin serta tidak jelasnya alur pendanaan permintaan visum. Berdasarkan hasil tersebut, polisi berharap dokter dan instansi kesehatan dapat memperbaiki masalah yang ditemukan. Perbaikan tersebut berdasarkan standar operasional prosedur (SOP), panduan World Health Organization (WHO) dan hukum yang berlaku di Indonesia.Kata kunci: dokter, harapan polisi, visum

Abstract

   Dalam Rancangan Peraturan Pemerintah (RPP) terkait pendidikan Dokter Layanan Primer (DLP), terdapat konsep program Rekognisi Pembelajaran Lampau (RPL) bagi dokter yang telah berpraktik lebih dari lima tahun untuk melalui Pendidikan Masa Transisi selama enam bulan. Hal ini menimbulkan pertanyaan mengenai proses supervisi terhadap peserta didik dalam program singkat ini. Di luar negeri, proses supervisi untuk peserta program spesialis kedokteran mulai dikembangkan dengan program bernama Entrustable Professional Activities (EPAs). Penelitian ini merupakan penelitian yuridis normatif yang bersifat deskriptif berdasarkan analisis data secara kualitatif. Hasil dari penelitian ini adalah deskripsi tentang proses kredensial Mahasiswa Program DLP Masa Transisi di wahana pendidikan, deskripsi asas-asas perlindungan hukum terkait Mahasiswa Program DLP Masa Transisi, dan jawaban bahwa asas perlindungan hukum akan terpenuhi dengan integrasi EPAs ke dalam proses kredensial antara Mahasiswa Program DLP dengan wahana pendidikan. Diskusi penelitian ini adalah tentang gambaran proses kredensial Peserta Program Pendidikan Dokter Spesialis berdasarkan aturan yang telah ada, jika diterapkan bagi Mahasiswa Program DLP Masa Transisi di wahana pendidikan yang memakai EPAs dalam sistem supervisinya, gambaran asas perlindungan hukum yang terkait dengan Mahasiswa Program DLP Masa Transisi, serta menjawab apakah integrasi EPAs dalam proses kredensial Mahasiswa Program DLP Masa Transisi akan memenuhi asas perlindungan hukum bagi mereka.Kata kunci: perlindungan hukum, Entrusted Professional Activities, Program Dokter Layanan Primer Masa Transisi

Abstract

Fibrosarkoma hanya terjadi 1–3% dari seluruh keganasan jaringan lunak. Hingga saat ini etiologi fibrosarkoma belum diketahui dengan pasti. Beberapa faktor dapat menjadi penyebab patogenesis fibrosarkoma antara lain radiasi, terpapar zat kimia tertentu, serta infeksi human herpes virus 8 (HHV8) dan Epstein-Barr virus (EBV). Penelitian terkini menunjukkan bahwa banyak sarkoma terkait dengan mutasi genetik. Penelitian ini bertujuan melihat profil ekspresi mRNA gen Krüppel-like Factor4, Murine Double Minute2, dan c-Myc pada fibrosarkoma menggunakan teknik real time PCR kuantitatif (quantitative real time PCR, qRT-PCR). Analisis data menggunakan metode kuantititatif relatif 2-ΔΔCt. Penelitian ini menggunakan 10 sampel kasus fibrosarkoma yang ditemukan di Kota Jambi dari tahun 2011–2015. Hasil ΔCt (+SD) MDM2, KLF-4, dan c-Myc disusun dari nilai yang terkecil hingga tertinggi adalah 1,85±2,14; 2,06±3,86; 2,9±2,66 secara berurutan. Dibanding dengan level ekspresi dengan GAPDH sebagai housekeeping gene, gen MDM2 dan KLF-4 relatif menurun dua kali lipat, sedangkan gen c-Myc relatif menurun lebih dari tiga kali lipat. Simpulan, penelitian ini menunjukkan bahwa pada kasus fibrosarkoma, gen c-Myc disupresi lebih kuat dibanding dengan gen MDM2 dan KLF-4.STUDIES ON MRNA GENE EXPRESSION OF MDM2, KLF4,AND C-MYC IN FIBROSARCOMAFibrosarcoma is a rare soft tissue sarcoma, reported only 1–3% of all soft tissue sarcomas. Like any other soft-tissue sarcomas the definitive caused has not yet understood. Recognized causes include exposure to ionizing radiation, various physical and chemical factors, infection with human herpes virus (HHV8) and Epstein-Barr virus (EBV). Current research indicates many sarcomas are associated with genetic mutations. In this study, we investigated profile of mRNA gene expression KLF4, MDM2, and c-Myc of RNA in fibrosarcoma cases. The genes expression was examined using quantitative real time PCR (qRT-PCR) and we analyzed the relative gene expression using the 2-ΔΔCt method. Ten samples of fibrosarcoma cases found in Jambi city from 2011 to 2015 were used. The three targeting genes were placed in the order from lowest to highest base on their ΔCt values compared to internal control genes using GAPDH genes. The results are as follows: MDM2 1.85±2.14, KLF-4 2.06±3.86, and c-Myc 2.9±2.66 respectively. A relative quantification by normalized target gene relative to GAPDH, describes the changes in expression of three genes. The status of MDM2 and KLF-4 were relatively decreased expression by 2 fold, and the states of c-Myc were relatively decreased by more than 3 fold. This suggest that in fibrosarcoma the c-Myc gene are suppressed stonger than those MDM2 and KLF-4 genes.

Abstract

Academic Health System (AHS) has been developed in many countries to strengthen the relationship between medical education and health professions with the health care system, which is essential to improve health outcome. Indonesia has chosen to establish AHS in several provinces, each with the autonomy to develop the system in accord with regional needs. Health cluster faculties in Universitas Padjadjaran, with its two main teaching hospitals, the West Java provincial government strived to develop AHS to overcome health services problem and medical education in the province that has enormous geographical and demographic challenges. The strategy used focuses on two things: distributed medical education (DME) and the development of a more effective referral system. The goals are dividing the province into seven regionals, upscaling one local hospital in each to become a regional referral hospital, expanding learning opportunities for medical students, and endorsing research to strengthen the primary healthcare services. Activities were carried out through the distribution of medical students and residents to local hospitals and primary healthcare facilities along with the education of local medical professionals as supervisors. Grants were provided for research that focus on quality primary healthcare, construction of data portal for patient management referral systems, telemedicine, and tele-education. The challenges faced are mainly related to the different mindset between institutions that have different work cultures and the wide variance of situations between regions. It is therefore recommended to build a more straightforward AHS system with addition of sub-networks, besides continue to maintain close communication and policy development.

Abstract

This article aims to determine the criterion of legal capacity of a patient in a contract for medical treatment and the legal consequences of a contract for medical treatment made by a minor. This article uses a normative juridical research method with a law approach and the data sources used are obtained from literature studies and document studies, and study results showed that there is a similar criterion for determining whether a patient is competent or not in giving consent for medical treatment between Indonesian law and Dutch law that is based on a legally fixed age. However, based on Dutch law, the age of legal capacity to consent for medical treatment differs from the age of legal majority. Meanwhile, English law uses mixed approaches to determine a minor capacity in giving consent for medical treatment that is based on the age and competence of a minor. Furthermore, according to Indonesian Law, a contract for medical treatment made by minors is voidable, whereas according to Dutch Law and also English Law any contract for medical treatment made by minors is legally binding.

Abstract

Background: The decision-making process for withdrawing life-sustaining treatment (LST) in patients with severe brain injury can potentially become a problem between doctor and family because of different opinions. Objective: Explore the family consideration in withdrawing LST in patients with severe brain injury. Methods: This qualitative descriptive study with phenomenological approach conducted using semi-structured in-depth interview. Subjects were family members of coma patients due to severe brain injury and were admitted to the Intensive Care Unit in Hasan Sadikin General Hospital during January 2019 - December 2020. Results: The results of the five-participant interview showed that the considerations for withdrawing LST include: (1) the patient's condition, (2) family psychology, (3) resources, and (4) family beliefs. Conclusion: Doctors need to understand the family's considerations in terminating LST to bridge the inner conflicts that occur in the patient's family.

Abstract

Background: Ethics helps a person in taking the right attitudes and actions in life. By studying professional ethics and legislation, students are expected to be able to master factual knowledge about ethics and statutory law in midwifery care. There is concern with the hybrid learning model in the future, because the topic of professional ethics in hybrid learning can be difficult because the subjects are philosophical, legal and affective which are generally quite abstract. Purpose: this study was to analyze the effect of the e- Module on professional ethics and legislation on midwifery student course scores. Methods: This is quantitative research using a crossover design. The groups used in this research were 2 groups. The two groups were given the same material using different methods. The sampling technique is total sampling. Result: There was a significant difference between course scores before and after using the e-module (p<0.05) and there was a significant difference between the average course scores in the e-module group and the control group (p<0.05). However, there is no significant relationship between course grades and students' perceptions of e-modules (p>0.05). Results: The existence of  the professional  ethics  and  legislation  e-module increased student course grades, and there was no strong significant relationship between course grades and perceptions

Abstract

Based on data from the Global Status Report on Road Safety published by the World Health Organization in 2018 and 2023, traffic accidents are one of the factors causing death in the world. One of the influencing factors is alcohol because it can interfere with concentration while driving. Various countries have tried to reduce the number of traffic accidents by checking drivers’ alcohol levels. Indonesia is one of the countries that has not implemented the test routinely, while the number of traffic accidents is still relatively high. For example, traffic accident victims in Lampung Province have increased between 2021 and 2022, and Central Lampung Regency ranks 2nd highest in traffic accidents in Lampung Province. Efforts to check alcohol levels require the involvement of police agencies and hospitals, including police officers and doctors. This research describes the knowledge, attitudes, and behaviour of police officers and doctors in Central Lampung Regency regarding alcohol level tests in drivers who have experienced traffic accidents. The research used an explanatory sequential mixed method design. Quantitative data regarding knowledge and attitudes were collected through questionnaires (n=77, 36 doctors, 41 police officers). The convenience sampling approach was used as the sampling method, then analyzed using SPSS ver.27. Data analysis was carried out using univariate, bivariate (Spearman test, Mann-Whitney test, Chi-square and Fisher exact tests), and multivariate logistic regression tests. Qualitative data to explain the knowledge, attitudes, and behaviour were collected through interviews through purposive sampling. The results showed that there were significant differences in knowledge, attitudes and behaviour between police and doctors (p<0,001); there is a weak correlation between knowledge and attitudes and behaviour of police and doctors (r=0,399). Age, length of service, gender, education, and occupation significantly correlate with the knowledge, attitudes and behaviour of police and doctors (p<0,05). The age factor has the most dominant relationship with knowledge (OR 3,4; p=0,02) in police and doctors. Qualitative data explained that young doctors have better knowledge, as well as more positive attitudes and behaviour towards alcohol level tests, compared to that of the police and older doctors. It can be concluded that there are significant differences between police and doctors in knowledge, attitudes and behaviour towards alcohol level tests of drivers involved in traffic accidents. It is suggested that the alcohol testing policy for drivers needs to be preceded by education for police and doctors using a different approach.

Abstract

The COVID-19 pandemic has created an innovative space for technology to become a solution, which, if implemented effectively, can help reduce the negative impact of the pandemic on society. For example, contact tracing, which if done manually, requires more resources and takes a lot of time, even though the spread of COVID-19 is happening very quickly, so a faster task force response is also needed. This research aims to determine public perceptions of confidentiality issues that may arise from implementing COVID-19 contact tracing, as well as public attitudes towards implementing COVID-19 contact tracing. This research will use a quantitative study method using a cross-sectional method. The researchers used a phenomenological theoretical approach to find out how people view contact tracing. The results of this research show that the majority of people in Bandung City who were respondents to this research have a positive attitude towards contact tracing and support the implementation of contact tracing. Most of the people in Bandung City who were respondents to this research had a good perception of contact tracing and felt that their privacy would not be disturbed by contact tracing. So the government needs to ensure that the public can easily access the contact tracing method used. Health officials need to explain contact tracing in a way that is easy for the public to understand so that the public understands the purpose of implementing contact tracing. And the public should also seek the latest information about health and obey the recommendations of the government and health officials.
