Abstract

Abstract The Community Partnership Program (PkM) entitled “Empowering Community Health Volunteers in Assisting the Screening and Monitoring of the Ankle-Brachial Index (ABI) among Patients with Diabetes Mellitus” was implemented in Pandansari Village, Poncokusumo Subdistrict, Malang Regency. This program aimed to enhance the capacity of community health volunteers in the early detection of complications related to Peripheral Artery Disease (PAD) among patients with Diabetes Mellitus (DM). The prevalence of DM in Pandansari Village is relatively high; however, community awareness regarding the early detection of PAD complications remains low. Although community health volunteers are actively involved in integrated health services (posyandu and posbindu), they lack specific skills in assisting with ABI screening and monitoring. The partner-related problems included the high number of DM cases, limited community knowledge regarding early PAD detection, and insufficient knowledge, skills, and structured mentoring systems among volunteers for conducting ABI screening and monitoring. The program was conducted through health education on DM and its complications, technical training on ABI measurement, and capacity building in systematic documentation of examination results. Evaluation using pretest and posttest demonstrated a 45.45% increase in volunteers’ knowledge (from a mean score of 55.00 to 80.00) and a 42.22% improvement in practical skills (from 56.25 to 80.00). These findings indicate that the program was effective in improving volunteers’ competencies as educators and facilitators in preventing DM complications at the community level. Keywords: diabetes mellitus; community health volunteers; peripheral artery disease; screening and monitoring; Ankle-Brachial Index (ABI).

Abstract

Death is the final process of human life. In the course of a typical human lifespan, old age represents the final stage before death. The elderly’s perception of death varies significantly among individuals and is influenced by various factors, including previous experiences of loss, closeness to significant others, and living arrangements—whether residing with family or in a nursing home. This study aims to explore and analyze the perceptions of older adults regarding death at the Social Welfare Institution (LKS) LU Pangesti Lawang. Death is a topic often avoided in conversation, yet it is essential to understand, particularly among elderly individuals living in institutional settings. As a group that is inherently closer to the end of life, the elderly's perception of death may greatly affect their psychological well-being. This research employed a qualitative method using a phenomenological approach to understand the subjective experiences of older adults in interpreting death. The participants consisted of seven elderly individuals residing in LKS LU Pangesti Lawang, selected through purposive sampling. Data were collected through in-depth interviews and observation, then analyzed using thematic analysis to identify the main themes related to their perceptions of death. The findings revealed six main themes that address the specific objectives of the study: Emotional unrest, End of life, Changes in perspective, Hopes for life after death, Frailty, Death as destiny Each of these six themes comprises several sub-themes, which were derived from categories and key phrases in participants’ responses to the research questions.

Abstract

Peripheral artery disease (PAD) is one of the chronic complications that frequently occurs in patients with diabetes mellitus (DM). PAD can be detected early using the non-invasive Ankle-Brachial Index (ABI) method. An abnormal ABI value may indicate impaired peripheral blood flow and serves as an important indicator of the risk of amputation and cardiovascular morbidity. The aim of this study was to analyze the factors affecting ABI values in patients with DM, namely age, sex, Body Mass Index (BMI), duration of DM, smoking status, physical activity, foot care status, history of DM complications, adherence to DM therapy, and blood glucose levels. This research employed a quantitative descriptive non-experimental approach with a cross-sectional design. The sample was obtained through accidental sampling, resulting in 106 respondents. Data were analyzed using the Spearman’s rho correlation test. The findings revealed that factors significantly associated with ABI values included duration of DM (p = 0.000), smoking status (p = 0.029), foot care status (p = 0.000), history of DM complications (p = 0.000), adherence to DM therapy (p = 0.000), and blood glucose levels (p = 0.000). The results of this study are expected to encourage respondents, particularly DM patients, to improve their overall adherence to diabetes therapy.
