Abstract

Kehamilan risiko tinggi adalah kehamilan yang harus dideteksi sejak dini karena dapat menimbulkan terjadinya bahaya atau komplikasi baik terhadap ibu ataupun janin. Kompilkasi dapat terjadi selama kehamilan, pada masa persalinan atau masa nifas. Keadaan ini dapat terjadi sebagai akibat atau komplikasi dari kehamilan atau disebabkan oleh suatu kondisi yang dimiliki ibu dari sebelum hamil. Kehamilan risiko tinggi merupakan penyebab terbanyak kematian pada ibu hamil. Oleh karena itu untuk mengurangi bahaya atau komplikasi akibat kehamilan risiko timggi, pengabdian ini bertujuan untuk membantu ibu hamil beserta penunggu pasien untuk dapat mengenali tanda-tanda kehamilan risiko tinggi sehingga angka kesakita dan kematian untuk ibu hamil dan jani dapat diturunkan. Kegiatan diawali dengan sosialisasi serta koordinasi dengan kelompok mitra, focus group discussion serta pelaksanaan kegiatan kemiteraan kepada masyarakat (PKM). Kegiatan PKM diawali dengan pre-test untuk mengukur tingkat pengetahuan ibu dan penunggu pasien, lalu penyuluhan mengenai kehamilan risiko tinggi dengan pamphlet, serta video. Pada akhir pelaksanaan PKM dilaksanakan post-test untuk mengetahui peningkatan pengetahuan mitra. Output serta outcome PKM dievaluasi dari peningkatan pengetahuan mitra, kedatangan ibu dalam perawatan ante natalcare serta penurunan komplikasi akibat kehamilan risiko tinggi. Peningkatan pengetahuan mitra ditemukan sebesar 90% dalam pengenalan gejala kehamilan risiko tinggi, dan frekuensi antenatal care. Evalausi keberhasilan PKM akan dilaksanakan dari penurunan kasus kehamilan risiko tinggi serta penurunan kesakitan dan kematian ibu hamil.

Abstract

Background: Immune Thrombocytopenia Purpura (ITP) is an autoimmune disease characterized by a low level of platelets in the blood caused by accelerated platelet destruction or disruption of the thrombopoiesis due to antibodies to platelets. Drugs with similar functions to thrombopoietins (TPO) such as eltrombopag and romiplostim have been widely used and have been shown to be quite successful in treating ITP in nonpregnant patients. This case study aims to evaluate eltrombopag therapy's role in managing the refractory ITP in pregnant women.Case Presentation: A 25-year-old patient, a housewife, presented to the obstetrics clinic at Sanjiwani Hospital for the first time at a gestational age of 13 weeks for her third pregnancy, with a history of two abortions. ITP was diagnosed in 2017 during the second abortion treatment. The platelet examination results were between 32,000-66,000/?l, with platelet count during delivery planning at 34 weeks of 41,000/?l. The patient was diagnosed with chronic refractory ITP and was considered for second-line drug administration. Intravenous Immunoglobulin Therapy (IVIG) and anti-D administration as second-line drugs were not possible due to cost. The patient was given 50 mg eltrombopag per day for a week. The platelet counts increased to 202,000/?l a week after therapy. The patient then underwent vaginal delivery with a baby weight of 2,850 gram, Apgar Score of 8-10. The baby was exclusively breastfed and subjected to clinical monitoring and laboratory tests. The patient's clinical condition was favorable and the platelets fell to 20,000/?l nineteen days after delivery.Conclusion: In this case, the patient with chronic ITP refractory to steroid therapy showed an immediate response to eltrombopag. However, the platelet level decreased two weeks after the treatment cessation. More studies are needed on the use of eltrombopag in pregnant women.

Abstract

Prelabor rupture of membranes (PROM) is a condition rupture of the membranes before the time of labor. The prevalence of PROM that occurs in Indonesia is still high. Anemia as one of the factors causing PROM. The purpose of this study to determine the relationship between anemia and the incidence of prelabor rupture of membranes at the Sanjiwani Hospital in 2020. This study used a cross-sectional and using secondary data from medical records from January 2020-December 2020. The study sample size was 96 samples obtained using the consecutive sampling method. Data analysis was carried out univariate and bivariate with chi-square test with a significance level (α) of 0.05. The data will be analyzed using SPSS. The result of this study obtained the number of prevalence of PROM in pregnant women with anemia is 26,9 with a significance level is 0,412. The result of this study is that there is no significant relationship between anemia and the incidence of prelabor rupture of membranes at the Sanjiwani Hospital in 2020. A non significant result can occur because the antenatal care (ANC) carried out by the mother during pregnancy has been good. Pregnant women are expected to carry out ANC properly to prevent anemia. Abstract

Low birth weight (LBW) is a condition when babies are born below normal weight limits 2.500 grams and is the highest cause of death in neonates. One of the risk factors for LBW is preeclampsia. Preeclampsia is a common condition in pregnant women which is characterized by blood pressure of 140/90 mmHg and proteinuria, which is 300 mg or more of protein in urine per 24 hours or 30 mg/dL (1+ on the dipstick) according to the results of the urine sample test. Preeclampsia in pregnant women can affect the condition of the fetus conceived by pregnant women from the period of pregnancy, childbirth, and postpartum. The purpose of this study was to determine the relationship between preeclampsia and LBW at the Sanjiwani Regional General Hospital, Gianyar. This study used an analytic observational method with a cross sectional study design. This study is considered to be unpaired categorical scale. Research data collection is done through secondary data in the form of medical records between 2018 - 2020. Bivariate data analysis is done to determine the relationship between variables using the chi-square test with a significance level of 0.05. The results showed that the p value = 0.000 and the PR was 7.917 (95% CI =2,569-24,395). As such, there is a significant relationship between preeclampsia and the incidence of LBW.
