Abstract

Introduction: Ventilator-Associated Pneumonia (VAP) is a nosocomial pneumonia which develops after the patient received 48 hours of mechanical ventilation intubation. VAP raises various problems such as prolong length of stay and ventilator time, increase treatment costs, and increase risk of death. Appropriate and effective oral care is one of the nursing interventions that can reduce the incidence of VAP. The aim of this study was to identify procedure of oral care for prevention of VAP in critical patients. Methods: This research method is a literature review. Literature is searched through electronic database such as EBSCOhost, Scopus, Science Direct, and Proquest. The inclusion criteria of article were randomized controlled trials or randomized clinical trial designs, evaluate oral care effects on the prevention of VAP in critically ill patients, full text articles and publication years from January 2013 to May 2020. Results: Search results found as many as 38 articles, but only 9 articles were eligible to be reviewed. This review indicate that oral care has a significant effect on the prevention of VAP. The combination of toothbrush and swab is the most appropriate equipment. Chlorhexidine 2% was the most influential solution in reducing the incidence of VAP, while the frequency of oral care was most effective when adjusted to the results of the Back Oral Assessment Score (BOAS). Conclusion: This result reinforces the use of chlorhexidine in the oral care of mechanically ventilated patients. Oral care assessment needs to be done by nurses to guide in determining the frequency of oral care.

Abstract

Fifty percent of end-of-life patients spend the last time of their life in the Intensive Care Unit (ICU). End of life conditions have an impact on the psychological, physical, social, and family environment that affect the quality of life of other families. Family welfare will have an impact on care outcomes, hospitalization time, and patient stressor levels. The purpose of this study was to explore the life quality of end of life patient's family in ICU. This research used qualitative method with descriptive phenomenology approach. Sampling was done by purposive sampling technique with 9 participants. data were collected through in-depth interviews and analyzed using the Colaizzi method.  The data validity was designed to meet the principle of triangulation of researchers and theory. The results showed 7 dimensions of life quality of end of life patient's family of physical health (fatigue, sleep disorder, headache, decreased appetite), mental health (anxiety, sadness, hope of improved family health condition), spiritual health (acceptance), health social (social support), daily life (daily life adjustment), financial health (financial burden), and environment (opportunity to get information, family infrastructure).  This study concludes that end of life condition of critical patients impacts on the 7 dimensions of family life quality. Nurses should pay attention to the family life quality because it affected the condition of the patient  and family as the care service unit. Keywords: Life Quality, Family, End Of Life, Intensive Care Unit

Abstract

Background: Surgery can cause anxiety of family member because uncertainty during awaiting surgery. Nursing intervention is needed to reduce anxiety of family member during intraoperative period. Objective: To identify strategies or nursing intervention in alleviating anxiety of family member of patient during intraoperative period. Method: This is literature review. Authors search article in electronic database such as SpringerLink, Scopus, EBSCOhost, Semantic Scholar, Portal Garuda and web search engine Google Scholar. The inclusion criteria of article are full text, quantitative research, in English or Indonesian language, year of publication was 2010 – 2020, and article’s topic in accordance with the research problem area. Results: Thirty – four articles were obtained and only nine articles were eligible to be included in this review. The review shows that anxiety level of family member during intraoperative decline after being given an intervention. The nursing intervention are providing information cards, providing information and visiting hours, watching TV, audiovisual orientation, and sending text messages. Conclusion: There are several nursing interventions that can be implemented by nurses to reduce family members’ anxiety during intraoperative period. Providing information to the family is very important and beneficial during intraoperative period.  Latar Belakang: Tindakan pembedahan dapat mengakibatkan kecemasan pada keluarga pasien karena ketidakpastian selama menunggu operasi. Intervensi keperawatan diperlukan untuk menurunkan kecemasan keluarga pasien selama periode intraoperative.  Objektif: Untuk mengidentifikasi strategi atau intervensi keperawatan dalam menurunkan kecemasan keluarga pasien selama periode intraoperative. Metode: Studi ini merupakan literature review. Peneliti melakukan penelusuran artikel secara elektronik melalui SpringerLink, Scopus, EBSCOhost, Semantic Scholar, Portal Garuda dan mesin pencari Google Scholar. Pemilihan artikel ditetapkan berdasarkan kriteria inklusi, yaitu full teks, design penelitian kuantitatif, berbahasa Inggris atau Indonesia, tahun publikasi 2010 – 2020, dan artikel sesuai dengan topik masalah penelitian. Hasil: Sebanyak 34 artikel didapatkan dan 9 artikel yang memenuhi kriteria inklusi. Hasil review menunjukkan rata – rata tingkat kecemasan keluarga pasien mengalami penurunan setelah mendapatkan intervensi keperawatan. Intervensi keperawatan yang dapat menurunkan kecemasan keluarga pasien selama menunggu operasi meliputi pemberian kartu informasi, pemberian informasi dan kunjungan, menonton TV, orientasi audiovisual, dan pengiriman pesan teks. Kesimpulan: Perawat dapat memberikan beberapa bentuk intervensi keperawatan untuk menurunkan kecemasan keluarga pasien selama periode intraoperative. Pemberian informasi kepada keluarga sangat penting dan bermanfaat selama fase intraoperative.

Abstract

Background: The prevalence and burden of stroke are still high, especially in low and middle-income countries. Stroke affects the economy and physics related to the Quality of Life (QoL). Various QoL-associated factors, including sociodemographic, functional outcome, emotional, and cognitive function, were studied. However, there was very limited information about the QoL-associated factors among stroke survivors in Semarang, Indonesia.Purpose: This study aimed to identify the QoL-associated factors among stroke survivors in Semarang, Indonesia.Methods: This cross-sectional study was conducted in outpatient services and involved 57 ischemic stroke survivors using convenience sampling. Demographic and clinical information were obtained using medical records and questionnaires. The questionnaires were the Short Version of Specific Stroke Quality of Life (SS-QoL) to measure QoL, GRID-HAMD 17 to measure Post Stroke Depression (PSD), Mini-Mental Status Examination (MMSE) to measure cognitive impairment, Barthel–Index (BI) to measure functional outcome, Hamilton Anxiety Rating Scale (HAM-A) to measure anxiety, and Multidimensional Scale of Perceived Social Support (MSPSS) to measure social support. Linear regression was conducted in the model performance of QoL-associated factors.Results: The stroke duration was 155(18) days, and 50.9% of participants were males. The linear regression showed that age (-.164 95% CI -.412 - .084), marital status (3.937 95% CI 1.010 - 6.864), functional outcome (.127 95% CI .013 - .241), PSD (-1.090 95% CI -2.144 - -.036), cognitive function (.308 95% CI -.482 - 1.098) and anxiety (-.408 95% CI -1.125 - .268) were QoL-predictors (p<.001, adjusted R2=52.1). It is assumed that age, marital status, functional outcome, PSD, cognition, and anxiety significantly predict the QoL among ischemic stroke survivors.Conclusion: The QoL-associated factors were age, marital status, functional outcome, PSD, cognition, and anxiety. These associated factors of QoL should be considered as elements in formulating nursing interventions that aim to improve the good QoL among stroke survivors.

Abstract

Pasien di Intensive Care Unit (ICU) memberikan dampak kecemasan dan depresi pada pasien dan keluarga. Keluarga tidak dapat menemani selama pasien di ICU salah satu faktor pemicu, disisi lain keluarga mempunyai peran untuk merawat pasien. Partisipasi keluarga dalam perawatan memberikan dukungan psikologis dan emosional kepada pasien. Penelitian ini bertujuan untuk mengetahui persepsi keluarga terkait partisipasi dalam perawatan di ICU. Penelitian studi kualitatif ini dilaksankan dengan cara indepth interview dengan 5 subjek anggota keluarga terdekat yang menunggu pasien selama menjalani perawatan di ICU. Tema penelitian ini adalah keluarga membutuhkan kedekatan dengan pasien sehingga mampu membantu untuk memenuhi kebutuhan pasien. Hasil penenilitian ini adalah keluarga untuk selalu menjalin hubungan yang baik dengan petugas. Penelitian ini bermanfaat bagi keluarga untuk selalu menjalin hubungan yang baik dengan petugas sehingga dapat berpartisipasi dalam perawatan. Kata kunci: ICU, keluarga, partisipasi, persepsi Abstract Family Perception Of Family Participation Caring For Patients In ICU Room: Qualitative Study. Patients in the Intensive Care Unit (ICU) have an impact on anxiety and depression in patients and families. Families cannot accompany as long as the patient in the ICU is one of the trigger factors, on the other hand the family has a role to care for the patient. Family participation in care provides psychological and emotional support to patients. This study aims to determine family perceptions regarding participation in ICU care. This qualitative study was conducted by way of in-depth interviews with 5 closest family member participants who waited for patients during ICU care. The theme of this research is that families need closeness to patients so they can help meet patients' needs. The results of this research are families to always establish good relationships with officers. Abstract

Background: Death can occur in any situation at any time, and each hospital setting has different difficulties in providing high-quality end-of-life care. The hospital's Islamic culture might offer new experiences to nurses when providing EOLC and be related to the challenges they face. This study identified the difficulties nurses encounter when providing end-of-life care in an Islamic-based hospital. Methods: A descriptive cross-sectional survey was applied to this study. A questionnaire that was adapted from the Nursing Survey Questionnaire Regarding End-of-Life Care on Medical-Surgical Units was used to collect the data. The translation and back-translation processes were carried out in the Indonesian version. All questionnaire items were declared valid with a validity value range of 0.820 to 0.950 (r table = 0.312), and the reliability test results obtained an alpha coefficient value of 0.977 (very reliable). The consecutive sampling method was applied. The data were analyzed using univariate analysis. At an Islamic hospital in Semarang, 97 surgical and medical nurses in total participated in the survey. Results: The findings indicate that the three main obstacles to nurses delivering end-of-life care are knowledge gaps (x = 2.53), a lack of education or training (x = 2.34), and health professionals’ avoidance of dying patients (x = 2.30). Other major impediments include a lack of family acceptance and uncooperative family attitudes. Conclusion: Nurses and family factors are the biggest obstacles for nurses in caring for dying patients.

Abstract

Unstable and critical conditions of Covid-19 patients require intensive care in the Intensive Care Unit (ICU) Covid-19 isolation room. The patients require intensive actions and continuous monitoring resulting in an increase in the workload of ICU nurses. ICU nurses should also wear third level Personal Protective Equipment (PPE) while dealing with Covid-19 patients. The number of patients and nurses who died has become one of the stressors for ICU nurses. ICU nurses also feel anxious and afraid of being infected, feel worried about the effectiveness of PPE, and the risk of transmission to family members at home. This condition leads to stress, depression and affects the quality of service. Therefore, this study aims to know the psychological respons of the critical care nurses in caring Covid-19 patients. This study used descriptive survey with the sample size of 181 respondents. The respondents were members of the Central Java-Indonesian Critical Care Nurses Association recruited by some inclusion and exclusion criteria. Depression, Anxiety, and Stress Scale 42 (DASS 42) was used as an instrument to collect the data. The data were analyzed in frequency and percentage. The results showed that the majority of respondents did not experience anxiety (58%), stress (82.3%), and depression (72.9%) although there were still severe and very severe anxiety levels (11.6%), a severe stress level (2.8%), and a severe depression level (1.7%) experienced by the respondents respectively. Nurses who are anxious, stressed or depressed should obtain serious attention from the nurses, the hospital and the government. Therefore, the psychological management of nurses can be improved to prevent unwanted problems such as ongoing depression or burnout syndrome.

Abstract

Pendahuluan: Bantuan Hidup Dasar (BHD) merupakan upaya penyelamatan yang pertama kali dilakukan pada korban gawat darurat seperti henti jantung dan henti nafas. Mahasiswa keperawatan merupakan calon tenaga kesehatan yang berpotensi menjadi penolong pada situasi gawat darurat.Tujuan: Penelitian   ini bertujuan untuk mengetahui gambaran kesadaran, pengetahuan, dan sikap mahasiswa keperawatan terkait BHD. Metode: Penelitian deskriptif kuantitatif dengan metode survei. Sebanyak 295 mahasiswa keperawatan Universitas Diponegoro menjadi responden dengan menggunakan teknik purposive sampling. Pengambilan data menggunakan kuesioner Kesadaran, Pengetahuan, dan Sikap BHD secara online dan data dianalisis secara univariat.Hasil: Penelitian ini menunjukkan bahwa sebagian besar mahasiswa memiliki kesadaran BHD yang baik. Namun, pengetahuan dan sikap tentang BHD mereka tidak jauh berbeda antara yang baik dan kurang baik. Kesalahan terbanyakyaitu pada bagian rasio, kedalaman pijat jantung dan ventilasi, dan letak defibrillator. Mahasiswa cenderung lebih ingin memberikan pertolongan kepada orang yang dikenal daripada orang yang tidak dikenal. Kesimpulan: Berdasarkan hasil penelitian diharapkan mahasiswa dapat meningkatkan kompetensi BHD khususnya pada aspek pengetahuan dan sikap, serta dibutuhkan metode pempelajaran yang tepat seperti skenario dan simulasi untukmeningkatkan kompetensi BHD mahasiswa keperawatan.

Abstract

Introduction: Sleep disturbance is one of the complaints in criticaly illl patients and have an impact on the patient's physical and psychological condition. Research on the effect of massage therapy on sleep quality has been widely studied with various techniques and outcomes. This study aims to review the evidence regarding the location, equipment, dose and timing of massage that can improve sleep quality in critically ill patients.Methods: This research method is a literature review. Literature is searched through electronic database such as EBSCOhost, Scopus, Science Direct, ProQuest, Clinical Key, SpringerLink and Portal Garuda. The keywords used are massage, massages, reflexology,”pemijatan”, “pijat”, sleep, sleep disturbance, sleep deprivation, sleep apnea, sleep quality, quality of sleep, critical area, critical patients, intensive care unit, and ICU. The inclusion criteria of article was  an experimental research design that tested the effect of massage on improving sleep quality, full text articles and published from January 2011 to May 2021. Data were analyzed in a narrative manner and grouped according to the searched theme, namely location/area, equipment, dose and timing of massage.Results: Search results found 107 articles, but only 9 articles were eligible to be reviewed. The review indicate that massage has a significant effect on improving sleep quality. The combination of hand and valerian or lavender essential oil is the most appropriate way. Feet are the most numerous and effective area. Massage dose is once a day with repetitions for two consecutive days with a duration of 10-20 minutes and it carried out before bedtime.Suggestion: This result recommend the application of massage to improve the sleep quality of critically ill patients. However, massages which given to critically ill patients must be carried out by certified nurses or expert in massage.
