Abstract

Introduction: Chronic health conditions caused by genetic, physiological, and environmental factors are common among individuals with Non-Communicable Diseases (NCDs). Hypertension is one of the most prevalent chronic NCDs. If left untreated, it may lead to serious complications such as heart disease, stroke, and kidney failure. Effective management strategies are crucial to control blood pressure and prevent further health risks. Objective: This study aimed to evaluate the effectiveness of the Chronic Disease Management Program (Prolanis) as a healthcare initiative to control blood pressure in patients with hypertension. Methods: A quantitative one-group pre-test and post-test design was employed. A total of 25 participants diagnosed with hypertension were selected using purposive sampling based on predefined inclusion criteria. Blood pressure was measured before and after participation in the Prolanis program. The data were analyzed using the paired sample t-test. Results: The analysis showed a statistically significant reduction in mean systolic blood pressure from 154 mmHg to 136 mmHg after the intervention. The paired sample t-test yielded a p-value of 0.001 (p < 0.05), indicating that the observed reduction was significant. Conclusion: The findings suggest that the Prolanis program is effective in controlling blood pressure among hypertensive patients. Integrating structured chronic disease management programs like Prolanis into routine healthcare services can contribute to improved patient outcomes and reduced risk of complications associated with hypertension.

Abstract

Introduction: Cardiac arrest is a medical emergency that requires immediate intervention by medical personnel or trained individuals. High mortality rates are often due to the lack of skills among bystanders in providing timely assistance. Objective: This community service aimed to enhance the knowledge and skills of both teachers and students in administering Basic Life Support (BLS) to increase survival chances during emergencies. Method: The activity was conducted at SMP Inspirasi, involving 7 teachers and 42 students. The methods used included interactive lectures, demonstrations, and hands-on practice. It began with a pre-test questionnaire to assess participants’ baseline knowledge of BLS. This was followed by an educational session using presentation slides and group discussions covering BLS concepts and procedures. Trained nurses performed live demonstrations using mannequins. Participants were then divided into small groups for supervised practice. The activity concluded with a post-test and a skills assessment to evaluate improvements in both knowledge and performance. Result: Prior to training, only 2 teachers (28.5%) and 3 students (7.1%) had knowledge of BLS, while 5 teachers (71.5%) and 39 students (92.9%) did not. After the training, all participants (100%) showed significant improvement in both understanding and skill execution. Conclusion: The BLS training was effective in improving school community readiness for emergencies and contributed to the development of community health independence

Abstract

Diabetic foot ulcers are one of the most common and severe complications in patients with Diabetes Mellitus, often resulting in infections and amputations. Optimal wound care is crucial to accelerate healing and prevent further complications. a herbal ointment containing tea tree oil (Melaleuca alternifolia), vitamin E, and honey, has demonstrated antimicrobial, anti-inflammatory, and wound-healing properties. This case study aims to evaluate the effectiveness of topical Tribee ointment application using moist dressings in promoting wound healing in a postoperative diabetic ulcer amputation patient with a high risk of infection. This case study applied a nursing care process including assessment, diagnosis, intervention, implementation, and evaluation over four sessions with two-day intervals at the patient’s home. The nursing diagnosis was established using the Indonesian Nursing Diagnosis Standards (SDKI), with interventions guided by the Indonesian Nursing Intervention Standards (SIKI), and outcomes assessed through the Indonesian Nursing Outcome Standards (SLKI). The SOAPIER format was used for documentation. The patient was diagnosed with a high risk of infection (D.0142) related to post-amputation wounds. After receiving wound care with topical Tribee ointment and moist dressings for four sessions (30 minutes each), the patient's wound condition improved significantly, as measured by the Bates-Jensen Wound Status Continuum, with a score decrease from 35 to 28. Topical Tribee ointment combined with moist dressings is effective in enhancing wound healing in patients with diabetic foot ulcers, particularly those at high risk of infection.

Abstract

Introduction: : Diabetes Mellitus (DM) is a non-communicable disease that continues to show an increase globally, including in Indonesia. According to WHO data in 2016, as many as 422 million adults in the world have diabetes. This disease is caused by insulin deficiency, either relatively or absolutely, and often causes complications such as peripheral neuropathy which causes decreased sensation in the feet. One of the steps to prevent these complications is through foot exercises. Foot exercises are special physical exercises for diabetics that aim to prevent wounds, improve blood flow, strengthen small muscles in the feet, and prevent deformities due to nerve disorders and muscle weakness. Objective: This case study aims to implement foot exercises in patients with impaired physical mobility with diabetes mellitus. Method: This research uses a descriptive method in the form of a case study with a nursing approach which includes assessment, nursing diagnosis, nursing care plan, nursing implementation, and nursing evaluation. Data collection was done by interview, observation, physical examination, and documentation. Result: After the assessment, it is known that Mrs.D obtained a nursing diagnosis of impaired physical mobility associated with decreased muscle strength with diagnosis number D.0054, after being given implementation in the form of foot exercises for 3 days with a duration of 5-7 minutes a day, the nursing problem of impaired physical mobility of the client is partially resolved which is characterized by increased muscle strength with muscle strength values on the right lower extremity 4 and the left 5. Conclusion: The implementation of foot exercises has proven to be useful for reducing movement limitations and increasing muscle strength in all extremities, the benefits of this foot exercise will be maximized if done gradually and periodically.
