Abstract

Integrated care models (ICMs) are increasingly promoted as a strategy to improve chronic disease management, but evidence of their effectiveness in resource-constrained settings like Surabaya, Indonesia, is limited. This study aimed to evaluate the impact of an ICM on chronic disease management outcomes in underserved communities in Surabaya. A mixed-methods study design was employed, combining a quantitative quasi-experimental component with qualitative data collection. The quantitative component compared pre- and post-intervention data on key clinical indicators (blood pressure control, HbA1c levels, medication adherence) and healthcare utilization (hospital admissions, emergency room visits) for patients with hypertension, diabetes, and cardiovascular disease enrolled in an ICM program (n= 250) versus a control group receiving standard care (n= 250). Data was analyzed using descriptive statistics, t-tests, and chi-square tests. The qualitative component involved semi-structured interviews with patients (n=30) and healthcare providers (n=15) participating in the ICM to explore their experiences and perceptions of the program's impact. Thematic analysis was used to analyze the qualitative data. The quantitative analysis revealed statistically significant improvements in several clinical indicators for the ICM group compared to the control group. Data showed a mean reduction in systolic blood pressure of 8 mmHg (p<0.001) and a decrease in HbA1c levels of 0.7% (p<0.01) in the ICM group. Hospital admissions related to chronic disease complications were also lower in the ICM group (p<0.05). Qualitative findings highlighted improved patient self-management, enhanced provider coordination, and increased patient satisfaction with the ICM. Barriers to implementation included resource constraints, logistical challenges, and the need for ongoing provider training. In conclusion, this study provides evidence that ICMs can improve chronic disease management outcomes in underserved communities in Surabaya, Indonesia. The findings support the scaling up of ICMs in similar settings, with careful consideration of resource allocation, provider training, and community engagement.

Abstract

The utilization of bricks made of styrofoam is expectedly able to be a soundproof for noise control and as a preventive action to reduce the steadily increasing prevalence of hearing loss. This study aimed to assess the use of sound absorption material in which styrofoam was utilized to reduce the noise exposure. In this study, fine aggregates (sand and styrofoam) were made with a mixture of cement with a composition of 1:4 and 1:6, also the addition of polystyrene waste with a percentage of 0%, 20%, 40%, 60%, and 80%. Determination of acoustical property of the mixture was done by testing the sound absorption coefficient (α) using Four Microphones Impedance Tube (ISO 140-3). The results showed that the highest value of absorption coefficient was at a frequency of 800 Hz with an additional 80% styrofoam for the composition of 1:4 at 0.4100 dB and at a frequency of 800 Hz with an additional 40% styrofoam for the composition of 1:6 at 0.5870 dB.

Abstract

Frequent use of digital devices among office workers contributes to a high risk of Digital Eye Strain (DES), with a reported prevalence of 69.0%, which can adversely affect visual performance, sleep quality, and work productivity. This study aimed to analyze the association between individual, device-related, and environmental factors with DES among office workers using a cross-sectional design involving 66 respondents. Data were analyzed using Chi-square and Mann–Whitney tests. The prevalence of DES was 69.7%, with common symptoms including itchy eyes, eye discomfort, and light sensitivity. Key individual factors significantly associated with DES were unsafe digital device use behavior (OR = 11.25; 95% CI: 1.169–30.280), lack of 20-20-20 rest practice (OR = 10.00; 95% CI: 2.895–30.542), and refractive error (OR = 8.20; 95% CI: 2.287–23.399). Among device-related factors, improper computer screen position (OR = 4.263; 95% CI: 1.323–13.736) was identified as a significant predictor. Environmental factors such as inadequate lighting (OR = 5.455; 95% CI: 1.595–18.657) also showed a strong association with DES. In conclusion, DES among office workers is predominantly influenced by behavioral factors, particularly unsafe digital device use and inadequate eye rest practices, which demonstrated the strongest associations. Preventive strategies should prioritize behavioral interventions, ergonomic improvements, and optimization of workplace environmental conditions.

Abstract

The main cause of work accidents due to falls from heights was unsafe behavior. In the Jakarta LRT Project, there were still unsafe behaviors performed by workers at heights such as not use complete PPE, not hooking FBH (Full Body Harness) to lifelines or scaffolding, not follow safety procedures, and smoke while working at heights. This research aimed to describe the unsafe behavior and determinants in the workers at heights in the Jakarta LRT Project Phase 1B Velodrome-Manggarai Zone 2 in 2025. This research used quantitative methods with a cross-sectional design and was conducted from January to May 2025. The sample was workers at heights using random sampling techniques and obtained 73 respondents. Data analysis used descriptive analysis. The research results showed that 72 workers (98.6%) behaved unsafely with the highest percentage of unsafe behavior being 71% of workers had never reported work conditions and hazard and risk findings to supervisors. The description of determinants of unsafe behavior showed that most workers had poor knowledge, did not comply with safety procedures, had never received occupational health and safety training, had poor supervision, rarely received rewards, and rarely received punishment. HSE personnel need to educate workers about the dangers and safe working practices when working at heights and increase supervision to ensure compliance with safety procedures. Management needs to re-conduct training on working at heights and safe use of scaffolding for workers and increase the number of award recipients and strengthen the punishment for workers.
