Abstract

Background: Hypertension is often found in children. It could cause serious complications and added risk of hypertension in adulthood. Hypertension in children is likely secondary type and some of them might be severe. The aim of this study was to determine the demographic and clinical characteristics of hypertension in children admitted to Dr. Hasan Sadikin General Hospital Bandung in January to December 2014.Methods: The study design used was a quantitative, descriptive cross-sectional study. Data were obtained from medical records retrospectively of inpatient children from the period January to December 2014. Data were collected from the period  September to November 2015 based on age and gender as a demographic characteristics and stage of hypertension, etiology of hypertension, and emerge of hypertensioncomplication as a clinical characteristics.Results: Ninety(1.9%)children with hypertension were found from 4,681 of total inpatient children consisting of 58 males and 32 females with median of age 8 (0−13) years old.The main stage of hypertension was hypertension stage 2 with 55 cases and nephrotic syndrome as the most common causes with 43 cases. The complications were present in hypertension stage 2 with 3 cases and hypertensive crisis with 2 cases.Conclusions: Children with hypertension are most commonly males and median of age was 8 years old. Hypertension is characterized by hypertension stage 2 and nephrotic syndrome as the most common etiology. Complications of hypertension appear in severe hypertension.

Abstract

  Background: Reduced handgrip strength is an aging process that influences the activities of daily living among elderly people. Handgrip strength varies greatly with age, gender, anthropometric measurement and nutritional status. This study aimed to determine the profile of handgrip muscle strength, physical characteristic, nutritional assessment and activities of daily living among elderly Methods: This study was conducted at Karitas nursing home, Bandung on September 2012. This is a descriptive study by collecting primary data from 28 elderly people. Data were collected in 2 days within 2 weeks. The first data collection included details of anthropometric measurement and handgrip measurement. The second data collection included face to face interview using standardized questionnaire for mini nutritional assessment and Barthel index.Result: The mean age of the respondents was 74.14 ± 8.5189 years. The average reading on handgrip muscle strength was 30.84 ± 12.0175 lbs for right hand and 27.77 ± 11.3778 lbs for left hand. For anthropometric measurement, most of the respondents had normal weight body mass index status (39.3%). Nutritional status of respondents evaluated by mini nutritional assessment showed that most of the respondents were at risk of malnutrition (64.3%). The activities of daily living assessment based Barthel Index showed that half of the respondents were dependent in activities of daily living (50%).Conclusions: From this  study, it can be concluded that the grip strength of all the respondents are weaker. Majority have normal body weight, malnutrition risk, and half of the subjects are depend on others in doing their daily activities.Keywords: Activities of daily living, anthropometry, elderly, handgrip strength, nutritional status DOI: 10.15850/amj.v2n1.423

Abstract

Pendahuluan Tenaga kesehatan berperan dalam meningkatkan derajat kesehatan masyarakat dan berhak mendapatkan jaminan kesehatan dan keselamatan kerja. Penelitian sebelumnya melaporkan kecelakaan kerja yang sering terjadi di rumah sakit adalah tertusuk jarum, teriris pisau, terluka dan terpercik cairan tubuh. Kejadian tersebut banyak yang belum terlaporkan karena padatnya jadwal pelayanan kesehatan dan rendahnya persepsi terhadap risiko terpapar infeksi. Rendahnya persepsi dan tingginya risiko kejadian kecelakaan kerja menjadi tujuan penelitian ini dilakukan untuk mengetahui hubungan antara persepsi tenaga kesehatan terhadap penggunaan alat pelindung diri dengan kejadian kecelakaan akibat kerja. Metode Penelitian dilakukan dengan metode analitik dan desain potong lintang yang melibatkan 102 tenaga kesehatan yang dipilih dengan teknik simple random sampling di salah satu rumah sakit di Kota Bandung pada bulan Oktober hingga November 2016. Variabel yang diteliti meliputi persepsi terhadap penggunaan alat pelindung diri dan kejadian kecelakaan akibat kerja. Data diambil melalui instrumen kuesioner kemudian dilakukan analisis data dengan menggunakan uji Chi-square dan Uji Spearman Correlation. Hasil Tidak terdapat hubungan yang signifikan antara variabel yang diteliti dengan nilai r= -0,085 dan p= 0,395. Pembahasan Tidak ada hubungan antara persepsi tenaga kesehatan terhadap penggunaan alat pelindung diri dengan kejadian kecelakaan akibat kerja. Terdapat faktor-faktor lain yang dapat memengaruhi kepatuhan dalam penggunaan alat pelindung diri dan kejadian kecelakaan kerja.Kata kunci: APD, KAK, Persepsi, Tenaga kesehatan

Abstract

Aerobic physical activity is known to induce skeletal muscle adaptation. Some genes, including MyoD, are known to have a major role in the process of muscle adaptation. Several studies has stated that the expression of MyoD increases during the aerobic activity process; however, the studies were carried out in a single bout. Studies on the effects of acute phase exercise (<2 weeks) are still rare, especially those regarding the difference effect of acute training phase at different times. This study was performed in 2018 at the Central Laboratory and Animal Physiology Laboratory, Faculty of Medicine, Universitas Padjadjaran. The aim of this study was to determine the different effects of MyoD gene expression in a two-week acute aerobic physical activity. The effects were measured in the skeletal muscle in day 3, 7, and 14 of the activity. Twenty four male rats (rattus novergicus) were divided into 4 groups. Those rats in the treatment groups were run on an animal treadmill (P1=3 days, P2=7 days, and P3=14 days). On the last day of ran, rats were sacrificed and the soleus and gastrocnemius muscles were dissected. The expression of the MyoD gene in both muscles was then amplified using reversed PCR and detected by agarose gel electrophoresis. This study showed that there were differencesin MyoD gene expression levels in both muscles and that the level increased with days of treatment, although it was statistically insignificant. This study concludes that the MyoD gene expression level is not significantly affected by an acute aerobic physical activity in certain periods (3, 7 and 14 days).Key words: Acute, Aerobic physical activity, gastrocnemius, MyoD, rats, soleus Perbedaan Pengaruh Aktivitas Aerobik Akut Intensitas Sedang terhadap Ekspresi Gen MyoD pada Otot Rangka Tikus Galur WistarAktivitas fisik aerobik diketahui menginduksi adaptasi otot rangka. Beberapa gen diketahui memiliki peran utama dalam proses adaptasi otot termasuk MyoD. Dalam beberapa penelitian diketahui bahwa ekspresi MyoD meningkat selama proses aktivitas aerobik, tetapi penelitian dilakukan dalam pertarungan tunggal. Penelitian tentang efek latihan fase akut (<2 minggu) masih belum banyak dilakukan, terutama mengenai perbedaan efek fase latihan akut pada periode tertentu (3, 7, dan 14 hari). Penelitian ini dilakukan pada tahun 2018 di Laboratorium Sentral dan Laboratorium Fisiologi Hewan Fakultas Kedokteran Universitas Padjadjaran. Tujuan penelitian ini adalah mengetahui perbedaan efek ekspresi gen MyoD dalam aktivitas fisik aerobik akut pada berbagai hari (3, 7, 14 hari) pada otot rangka. 24 tikus jantan Rattus novergicus dibagi menjadi 4 kelompok. Kelompok perlakuan dijalankan dengan treadmill hewan (P1=3 hari, P2=7 hari, dan P3=14 hari). Pada hari terakhir, tikus dikorbankan dengan soleus dan otot gastroknemius dibedah. Ekspresi gen MyoD pada kedua otot kemudian diamplifikasi menggunakan PCR terbalik dan dideteksi dengan elektroforesis gel agarosa. Penelitian ini menunjukkan perbedaan level ekspresi gen MyoD pada kedua otot, hasil ini menunjukkan bahwa ada peningkatan level perawatan per hari, walaupun secara statistik tidak signifikan. Penelitian ini menyimpulkan bahwa pada otot gastrocnemius tidak terlihat efek yang berbeda dari durasi aktivitas fisik aerobik terhadap tingkat ekspresi gen MyoD, hanya otot soleus menunjukkan perubahan dan peningkatan ekspresi konsisten terlihat pada hari ke 14 (3, 7, dan 14 hari).Kata kunci: Aktivitas fisik aerobik, akut, gastroknemius, MyoD, soleus, tikus

Abstract

ABSTRAK Sarkopenia adalah penurunan massa dan fungsi otot rangka yang disebabkan oleh penuaan. Sarkopenia dapat meningkatkan morbiditas dan mortalitas pada lansia. Penyebab dan patofisiologi sarkopenia bersifat kompleks dan belum diketahui secara pasti hingga saat ini. Autophagy diketahui mempunyai peran penting untuk proses eliminasi dan daur ulang protein dan organel yang rusak sehingga sementara pada penuaan proses akumulasi berlebihan organel dan protein yang rusak sebagai konsekuensi metabolisme merupakan salah satu penyebab terjadinya penyakit karena penuaan. Kondisi autophagy basal yang baik mempunyai efek protektif terhadap pemeliharaan massa otot dan stimulasi sel satelit untuk proses regenerasi otot. Sementara di sisi lain, inhibisi autophagy menyebabkan penurunan massa otot dan berhubungan dengan atrofi serat otot pada proses penuaan. Inhibisi autophagy diperlukan di awal fase regenerasi myogenesis otot sarkopenia, namun kemudian diduga melalui peran FoXO,autophagy diaktivasi untuk mengeliminasi sel yang rusak dan biogenesis mitokondria. Keseimbangan dinamis dari autophagy merupakan salah satu faktor kunci peranautophagy yang berperan positif pada pencegahan sarkopenia dan bahkan prosesmyogenesis sebagai tatalaksana sarkopenia.

Abstract

Uji Kompetensi Mahasiswa Program Profesi Dokter (UKMPPD) adalah uji kompetensi yang bersifat high-stake exam di tataran nasional sebagai salah satu upaya penjaminan mutu pendidikan tinggi bidang kesehatan sesuai dengan amanah UU No.12/2012. Kompetensi akan dicapai mahasiswa setelah melalui proses pembelajaran terstruktur berbasis kompetensi. Kurikulum, proses pembelajaran dan evaluasi pembelajaran yang sesuai dan berbasis kompetensi sejak tahap sarjana akan menjadi alur yang jelas untuk kelulusan mahasiswa pada UKMPPD. Penelitian ini dilakukan bertujuan untuk menganalisis apakah IPK tahap Sarjana dan skor MDE dapat menjadi prediktor kelulusan UKMPPD. Penelitian ini menggunakan rancangan deskriptif analitik dengan populasi adalah mahasiswa Fakultas Kedokteran Universitas Padjadjaran yang mengikuti UKMPPD periode Februari 2015 sampai dengan Agustus 2016 sebanyak 524 peserta. Analisis data menggunakan analisis uji korelasi (Pearson Correlation).Hasil penelitian diperoleh korelasi antara antara nilai IPK tahap Sarjana dengan skor CBT UKMPPD begitu juga dengan rata-rata skor MDE tahap Sarjana dengan skor CBT UKMPPD.Simpulan.Hal ini menunjukkan bahwa proses pembelajaran dan metode evaluasi pembelajaran berkaitan erat dengan kelulusan mahasiswa pada UKMPPD sebagai refleksi kompetensi mahasiswa. Hasil ini menunjukkan bahwa IPK dan rata-rata nilai MDE tahap sarjana dapat menjadi prediktor skor kelulusan CBT UKMPPD. Hal ini juga menunjukkan bahwa kelulusan mahasiswa pada UKMPPD merupakan suatu proses berkelanjutan dari tahap sarjana dan profesi. Abstract

Exercise-induced hearth muscle adaptation is important for physiological process after exercise. This adaptation will ensure basal mitochondrial homeostasis and as a part of the mitochondria quality control. This process is reflected by equal level of biogenesis stimulation and as well as the selective degradation of old and undesirable mitochondria through fusion or fission cycle and Mitophagy. There is limited information about genetic regulation stimulated by training in cardiomyocytes. We believe there is a specific myokines or protein release in the serum and initiate cardiac muscle adaptation process. In the present study, twelve male wistar rats were appointed to two group: sedentary control and aerobic-intensity (AE, 15m/minute). Rats were trained for running with specific protocol as follows: 30 minutes/day with a 5 times/week interval for 8 weeks. On the last day, serum form control and exercise groups were taken via retro-orbital sinus. Then, 3.105  H9C2 cells (Rat cardiomyocytes cell line)  were cultured and incubated by this serum for 24 hours. After treatment, cell were extracted using trisure for RNA purification and continue with reverse transcriptase PCR. Our data showed that expression of the Pgc-1α, Mfn1, Mfn2, Opa1, Drp1, Pink, and Parkin genes were altered and modulated. Specifically, Mfn1, Mfn2, and Opa1 gene expression levels significantly increased. Interestingly, we did not find significant modulation for  Pgc-1α, Drp1, Pink, and Parkin. Taken together, serum of exercise rats might be contained with myokines or specific protein which was released during training and it altered mitochondrial genes expression in cardiomyocytes culture cell. We believe that myokines release in the serum had a contribution in cardiacmyocyte adaptation.

Abstract

Background: Hepatotoxicity induced by the combination of isoniazid (INH) and rifampicin (RIF) in the treatment of tuberculosis (TB) remains a major concern. Oxidative stress has a role in mechanism of hepatotoxicity. Curcuma longa L. has been widely used as a traditional medicine and has shown antioxidant activity. This study aimed to provide evidence of Curcuma longa L as protection against oxidative stress induced by isoniazid and rifampicin therapy.Methods: This was an experimental study on male Wistar rats weighing 150-200 grams, aged 8-10 weeks which were divided into a negative control group (K0), a positive control group with INH + RIF (K1), a treatment group with a dose of 2.2 gr/kg/day Curcuma longa L powder (K2), and treatment group with INH+RIF and additional 2.2 gr/kg/day turmeric rhizome powder (K2+). SGOT and SGPT were measured from blood plasma on the 28th day; then hepatic tissue was obtained to measure MDA levels and observed histologically. Statistical analysis was done using ANOVA and continued with Duncan procedure using SPSS ver. 27.Results: SGOT, SGPT, the highest average MDA level in the liver, and the highest mean necrotic cell count in the positive control group showed a significant difference (p<0.05). The treatment group had a smaller average number of necrotic cells than the positive control group with a significant difference (p <0.05).Conclusion: Curcuma longa L powder has been shown to prevent elevation in SGOT, SGPT, MDA of liver tissue and hepatocyte necrosis, indicating its potential in protecting the liver from oxidative stress.

Abstract

Objective: To identify the characteristics of endometrioma recurrence cases. Method: This study was a descriptive retrospective, using secondary data taken from all medical records of Clinic Aster and Medical Records Installation in Dr. Hasan Sadikin General Hospital, Bandung, from 1st January 2018 – 31st December 2022. Data were presented in tables.Results: Of the 37 endometrioma recurrences, most of the patients are 20 to 35 in age. None of them has a parity history after the first surgical procedure. Both previous and recent cases are dominantly unilateral endometriomas. The history of postoperative medication is higher (54,1%). Obstetricians/Gynecologists appear to perform the most surgical procedure (67,6%). Mass is the main clinical manifestation of the recurrence. While menstrual and BMI profiles appear to be normal.Conclusions: The characteristic of the recurrence of endometrioma is related parity status after the first surgery, history of the previous medical treatment, and previous form of endometrioma are related to the current characteristic of endometrioma (age, lesion form, clinical manifestation, and recurrence interval).Karakteristik Pasien Endometrioma RekurenAbstrakTujuan: Mengetahui karakteristik dari pasien endometrioma rekuren.Metode: Penelitian ini merupakan penelitian deskripsi retrospektif dengan menggunakan data sekunder yang diambil dari seluruh rekam medis pasien kista endometriosis rekuren di Klinik Aster dan Instalasi Rekam Medis di Rumah Sakit Umum Pusat Dr. Hasan Sadikin Bandung dengan periode 1 Januari 2018 – 31 Desember 2022. Kemudian, data-data disajikan dalam bentuk tabel.                                                                                . Hasil: Dari 37 kasus e ndometrioma rekuren, sebagian besar pasien berusia 20 hingga 35 tahun. Seluruh pasien tidak mempunyai riwayat paritas setelah prosedur pembedahan sebelumnya. Baik kasus-kasus kista endometriosis sebelumnya maupun yang kambuh, keduanya dominan dalam bentuk unilateral. Riwayat pengobatan pasca operasi lebih tinggi (54,1%). Dokter Spesialis Obstetri/Ginekologi tampak melakukan tindakan pembedahan terbanyak (67,6%). Massa adalah manifestasi klinis utama dari kekambuhan. Selain itu, profil menstruasi dan BMI tampak normal.Kesimpulan: Karakteristik pada endometrioma rekuren berkaitan dengan status paritas setelah operasi pertama, riwayat pengobatan medis sebelumnya, dan bentuk endometrioma sebelumnya berhubungan dengan karakteristik endometrioma saat ini (usia, bentuk lesi, manifestasi klinis, dan rentang rekurensi)Kata kunci: karakteristik, rekuren, endometrioma

Abstract

The Rebozo technique is a non-pharmacological method that is effective in supporting the delivery process. This research aims to explore the benefits, implementation challenges, and role of education in the application of this technique. The study was conducted through a scoping review of 19 selected articles and analyzed thematically based on certain inclusion criteria from various international databases. The results showed that the Rebozo technique improved maternal comfort, reduced the need for medical intervention, and supported a positive childbirth experience. The main obstacles include a lack of training and limited facilities. Structured education and training for health workers, as well as couple education to support mothers during childbirth, are important elements in the optimization of this technique. Research recommendations include practice-based training for health workers and strengthening the role of couples through educational programs to support the implementation of the Rebozo technique in physiological childbirth services.
