Abstract

BACKGROUNDMicroalbuminuria is the earliest evidence of diabetic nephropathy and a major predictor of end stage renal disease (ESRD). The objective of the study was to determine the influence of several risk factors on the presence of microalbuminuria in type 2 diabetics.METHODSThis observational cross-sectional study was done on 73 patients with type 2 diabetes, who attended the Prolanis program in Primary Health Care from May to November 2018. Detailed medical histories including duration of diabetes and relevant clinical examinations including fasting blood sugar (FBS), post-prandial blood sugar (PPBS), HbA1c, serum creatinine, blood urea and urinary microalbumin were recorded for each patient. A multiple regression analysis was used to analyze the data. The analysis was assessed at 5% level of significance.RESULTSMean age of study population was 51.89 ± 6.78 years with female preponderance (51.1%). Mean FBS, PPBS, HbA1c, duration of diabetes, systolic blood pressure, microalbuminuria and serum creatinine was 182.51 ± 74.63 mg/dL, 186.25 ± 26.72 mg/dL, 8.8 ± 1.83%, 9.37 ± 5.96 years, 118.44 ± 4.13 mmHg, 30.32 ± 3.2 mg/day and 1.33 ± 0.64 mg/dL respectively. Duration of diabetes and HbA1c were positively correlated with microalbuminuria (â=0.052; Beta =0.367; p<0.001 and â=0.058; Beta=0.363; p<0.001) respectively.CONCLUSIONSDuration of diabetes was the most important risk factor of microalbuminuria in type 2 diabetes patients. Therefore microalbuminuria can predict diabetic nephropathy earlier, as a warning to prevent further worsening of diabetic complications.

Abstract

The golden period of children’s growth occurred in the first 1000 days and every disruption in this period would damage the number of cells and myelination that could not be achieved in the next period. Head circumference (HC) at birth was correlated with brain size to measure the brain’s growth and development. This study aimed to find the correlation between HC at birth and children’s growth and development. The research method used was cross-sectional. A total sample of 233 children at 6-60 months old in Posyandu Bina Kasih I-XI Rempoah Village, Baturaden District, was taken using total sampling. Research data used secondary data from previous study and were analyzed by Eta and spearman correlation tests. Results show 180 children who have normal nutritional status and 221 children with development based on their age. Median HC at birth value is 33 cm belongs in the normal category. Spearman test shows HC at birth is not correlated with nutritional status (p=0.412). Eta test shows HC at birth was statiscally significant correlated with development based on KIA book albeit with a very weak level of correlation(F calculated > F table; ETA=0.196). In conclusion, Head circumference at birth is correlated with development based on the KIA book, but does not correlated with nutritional status and head circumference.

Abstract

Background: The first 1,000 days of life are the foundation for growth and development of children in the next period. Birth length may represent growth and development during the intrauterine life. This study aimed to explore the correlation of birth length, growth, and development in children.Methods: This was analytic observational study with a cross-sectional design. Total sampling was taken, obtaining 368 children aged 6−60 months in several Integrated Service Post (Pos Pelayanan Terpadu, Posyandu), conducted from October to November 2018. Data were gathered from the book of Maternal and Child Health (Kesehatan Ibu dan Anak, KIA), including birth length, body length/height, and nutritional status using weight-for-length/height index. Data were analyzed by Spearman and Eta test. Results: Birth length was not correlated with nutritional status (p=0.108, r=0.084), but had a good correlation with body length/height (p=0.027, r=0.115) and development based on the KIA book (F calculated > F table, Ƞ=0.245).Conclusions: There is a correlation between birth length and body length/height as well as a correlation between birth length and development based on the KIA book. It is important for the community and health workers to monitor growth and development of children, by maximizing the use of KIA book.

Abstract

Carbon tetrachloride (CCl₄) is a chemical that can cause damage to liver cells. One of the natural ingredients developed to reduce liver damage due to chemicals and infections is Chlorella sp. The research aimed to explore the hepatoprotective effect of the antioxidant superoxide dismutase (SOD) by administering Sun Chlorella in Rattus norvegicus CCl₄ induced rats. The study design used a post-test with a control group design with a completely randomized design trial on 30 male rats of Wistar strains, aged 2–3 months, 200–250 grams in weight. The research was conducted in November 2016–January 2017 in Purwokerto. Rats were divided into five groups and treated for four weeks as follows: K1 group was given aquades; K2 was given CCl₄; K3, K4, K5 were given CCl₄ and Sun Chlorella 3.6 mg/200 gBW, 7.2 mg/200 gBW, and 14.4 mg/200 gBW respectively, by the gastric probe. Statistical analysis with correlation test and one way ANOVA multivariate test showed that Sun Chlorella 7.2 and 14.4 mg/200 gBW significantly increased SOD levels in rats induced CCl₄ (p=0.004, p=0.009). SOD rates were significantly strong associated with aspartate aminotransferase/AST (r=−0.685, p=0.000) and alanine aminotransferase/ALT (r=−0.659, p=0.000). The conclusion is Sun Chlorella increases SOD levels in CCl₄-induced rats. Increased SOD levels may decrease AST and ALT levels. EFEK HEPATOPROTEKTIF SUN CHLORELLA SEBAGAI ANTIOKSIDAN PADA RATTUS NORVEGICUS YANG DIINDUKSI KARBON TETRAKLORIDAKarbon tetraklorida (CCl₄) merupakan bahan kimia yang dapat menyebabkan kerusakan sel hati. Berbagai bahan alami telah dikembangkan untuk mengurangi kerusakan hati baik akibat bahan kimia maupun infeksi, salah satunya adalah Chlorella sp. Penelitian ini bertujuan melihat efek hepatoprotektif  dengan pemberian Sun Chlorella pada tikus Rattus norvegicus yang diinduksi CCl₄. Desain penelitian menggunakan post-test with a control group dengan rancangan percobaan rancangan acak lengkap pada 30 tikus jantan galur Wistar, usia 2–3 bulan, dan berat 200–250 gram. Penelitian ini dilakukan di Purwokerto pada periode November 2016–Januari 2017. Tikus dibagi menjadi lima kelompok dan mendapatkan perlakuan per oral dengan sonde lambung selama 4 minggu sebagai berikut: kelompok K1 sebagi kontrol negatif diberikan aquades; kelompok control positif K2 diberikan CCl₄; kelompok perlakuan K3, K4, dan K5 diberikan CCl4 dan Sun Chlorella 3,6 mg/200 gBB; 7,2 mg/200 gBB; 14,4 mg/200 gBB tikus berurutan. Uji statistik dengan menggunakan uji korelasi dan ANOVA satu arah menunjukkan bahwa pemberian Sun Chlorella 7,2 mg dan 14,4 mg meningkatkan kadar SOD pada tikus yang diinduksi CCl₄ secara bermakna (p=0,004; p=0,009). Kadar SOD berhubungan kuat dengan kadar aspartat aminotransferase/AST (r=−0,685; p=0,000) dan alanin aminotransferase/ALT (r=−0,659; p=0,000). Kesimpulan penelitian ini adalah pemberian Sun Chlorella meningkatkan kadar SOD pada tikus yang diinduksi CCl₄. Peningkatan kadar SOD menyebabkan penurunan kadar AST dan ALT.

Abstract

Purwoceng (Pimpinella pruatjan Molk) is an original Indonesian herb which is known to have an aphrodisiac effect. The active compounds in Purwoceng potentially have hepatotoxic and nephrotoxic effects.This study was aimed to analyze the effect of subchronic administration of Purwoceng roots ethanol extract to Wistar Strain of Rattus norvegicus rats. The method of this subchronic toxicity study was an experimental post test only with control group design. Forty male Rattus norvegicus were randomly divided into four groups and get 28-days treatment. Group A as control received aquadest and 1% CMC (carboxymethyl cellulose), group B, C, and D were given Purwoceng roots ethanol extract of 42, 84, and 168 mg/KgBW/day and 1% CMC. Parameters tested were the levels of urea, creatinine, SGPT (serum glutamic pyruvic transaminase), SGOT(serum oxaloacetic pyruvic transaminase), kidney and hepar histopatology.The results showed a statistically significant for the liver histopathological in group B, creatinine, urea, and kidney histopathology  in group C. Our study concluded that subchronic administration of Purwoceng (Pimpinella pruatjan Molk) roots ethanol extract could induce hepatotoxicity at the 42 mg/KgBW/day dose level and nephrotoxicity at the 84 mg/KgBW/day dose level.

Abstract

Sepsis is a life-threatening organ dysfunction induced by the body's response to infection and is a significant cause of critical illness and death in hospitals. Bacteria are the most common pathogens that cause sepsis, and their ability to form biofilms increases their resistance to antibiotics. As a result of the failure of antibiotic administration therapy, the severity and pain of sepsis worsen. The study used a descriptive research design to determine the antibiotic resistance pattern of biofilm-producing bacteria from clinical isolates of sepsis patients. Using the BacT/Blood Culture System Alert, all patients suspected of sepsis in the intensive care unit of Prof. Dr. Margono Soekarjo General Hospital Purwokerto were examined for blood cultures between March and July 2018. These were then identified and tested for antibiotic resistance with the Vitek 2 Compact. Biofilm formation was detected utilizing the microtiter plate assay method, and the data were analyzed using a frequency distribution table. The results obtained 12 bacterial isolates, with Escherichia coli (41.67%), Staphylococcus haemolyticus (33.33%), Klebsiella pneumoniae ssp pneumoniae, Enterobacter cloacae complex, and Acinetobacter baumannii complex (8.33%) as the most common bacteria. All gram-negative bacteria (more than 80%) were resistant to ampicillin, cefoxitin, ceftazidime, ceftriaxone, aztreonam, and trimethoprim but were sensitive to meropenem (100%). Gram-positive bacteria were resistant to cefoxitin, benzylpenicillin, oxacillin, ciprofloxacin, erythromycin, and clindamycin (100% each). However, they were sensitive to tigecycline, nitrofuran, quinupristin, linezolid, vancomycin, and tetracycline (100% each). Gram-negative bacteria formed 50% biofilms, and 50% did not, whereas gram-positive bacteria produced 100% biofilms. In conclusion, bacteria clinical isolates of septic patients from Prof. Dr. Margono Soekarjo General Hospital Purwokerto are multiresistant to more than six types of antibiotics and produce weak to moderate biofilms, which can promote antibiotic resistance.

Abstract

Tujuan: Untuk mengetahui hubungan antara rasio neutrofil-limfosit terhadap kejadian metastasis intraperitoneal pada pasien karsinoma musinosum ovari di RSUD Prof Dr. Margono SoekarjoMetode: Penelitian analitik observasional dengan desain cross-sectional dilakukan pada 43 pasien karsinoma musinosum ovari. Data diambil melalui rekam medis pemeriksaan laboratorium berupa neutrofil dan limfosit pre-operasi pertama tahun 2017-2020. Teknik pengambilan data menggunakan consecutive sampling. Data dianalisis secara statistik dengan uji chi-squareHasil: Penelitian menunjukkan bahwa dari 43 pasien, didapatkan rerata usia pasien 51 tahun, mayoritas stadium IIIB (25%). Rasio Neutrofil Limfosit katagori rendah ≤ 4,1 (30%), Rasio Neutrofil Limfosit  katagori tinggi >4,1 (70%) serta rerata Rasio Neutrofil Limfosit 10.74. Nilai Rasio Neutrofil Limfosit pada pasien yang mengalami metastasis cenderung lebih tinggi daripada pasien yang tidak mengalami metastasis. Lokasi metastasis karsinoma musinosum sebagian besar pada rongga pelvis. Analisis hubungan antara Rasio Neutrofil Limfosit dengan metastasis intraperitoneal secara statistik menunjukan (CI:0.22-9.12,p = 0,000; p ≤ 0,05).Kesimpulan: Terdapat hubungan antara rasio neutrofil-limfosit terhadap kejadian metastasis intraperitoneal pada karsinoma musinosum ovari.Correlation between Neutrophil Lymphocytes Ratio With Metastases Intraperitoneal Of Mucinous Ovary Cancer in Hospital Prof Dr Margono Soekarjo 2017-2020Abstract Objective: Purpose to determine the correlation between Neutrophils-Lymphocytes Ratio (NLR) with the metastases intraperitoneal of mucinous ovarian cancer patients in RSUD Prof Dr. Margono Soekarjo.Method: This study uses observational analytic method with cross-sectional design. The subjects of this study were 43 patients with mucinous ovary cancer which data neutrophil and lymphocytes pre one surgery. The data source comes from secondary data taken from the medical record installation in the period 2017-2020 using consecutive sampling technique.Results: This study showed that from 43 patients , the mean were aged 51 years. The majority were stage IIIB (25%). Neutrophil Lymphocyte Ratio low category ≤4.1 (30%), Neutrophil Lymphocyte Ratio high category >4.1 (70%) and the mean Neutrophil Lymphocyte Ratio 10.74. The location of mucinous carcinoma metastases is mostly in the pelvic cavity. Analysis of the correlation between NLR with metastases intraperitoneal showed that statistically (CI:0.22-9.12,p = 0,000; p ≤ 0,05).Conclusion: There is correlation between Neutrophils-Lymphocytes Ratio (NLR) with the metastases intraperitoneal of mucinous ovarian cancer.Key words: carsinoma mucinous ovary cancer,neutrophil-lymphocyte ratio (NLR),metastases intraperitoneal

Abstract

The association between the risk factors for male infertility including smoking, obesity, male age, and leukocyte count with sperm analysis, still shows mixed results. This study aims to determine the association between smoking, obesity, male age, and leukocytes count with sperm quality (sperm concentration, sperm motility, and sperm morphology) of infertility patients in Purwokerto. This study is an observational study with a cross-sectional design conducted in the medical records section of the Bunda Arif Hospital Purwokerto. The sample was taken by total sampling. The bivariate test of smoking and obesity behaviour variables with the results of sperm quality using the Chi-Square test and Fisher's exact test. Male age variables used the Kruskal-Wallis test and the Spearman correlation test. Variable leukocytes count using the Spearman test. The results showed no association between male age, obesity and smoking behaviour with sperm quality, sperm concentration, sperm motility, and sperm morphology (p>0.05). There is a significant association (p<0,05) between leukocyte semen count and spermatozoa concentration, sperm motility, and morphology of spermatozoa. It is concluded that there was a significant association between leukocyte count and sperm quality in infertility patients in Purwokerto. Research needs to be continued by examining the relationship between leukocytopenia and sperm DNA damage by looking at sperm DNA fragmentation.

Abstract

Hypertension is often found in patients with type 2 diabetes. Albumin Creatinine Ratio (ACR) is used to detect early symptoms of kidney disease in patients with type 2 diabetes. The aim of this study is to determine the correlation between ACR and hypertension, as well as its relationship with other risk factors in type diabetes patients 2. Data were collected from 112 diabetic patients at PROLANIS FKTP Banyumas to see ACR and blood pressure. The results showed that poor glycemic control could be one of the risk factors. It can be concluded that detecting kidney dysfunction through ACR and starting treatment as early as possible can avoid the worst possibilities in type 2 diabetes patients.

Abstract

Abstract: Sleep deprivation-induced stress decreases the quality of spermatozoa due to the increase of glucocorticoid levels through the hypothalamic-pituitary-adrenal axis. Sleep recovery (SR) restores the effect of stress due to sleep deprivation by decreasing lipid peroxides and other free radicals and increasing glutathione antioxidants. This study aims to determine the spermatozoa quality in male albino rats (Rattus norvegicus) after the stress induction by diverse sleep deprivation protocols. This study is experimental research with posttest-only control group design in 30 male albino rats divided into five groups consisting of group I for negative control (no induction of sleep deprivation), group II for paradoxical sleep deprivation (PSD), group III for total sleep deprivation (TSD), group IV for PSD followed by SR, and group V for TSD followed by SR. PSD and TSD were performed by depriving rats sleep for 20 and 24 hours/day, respectively, for five days. SR was done for the next five days after PSD or TSD. Shapiro-Wilk test was used for normality of data distribution, and Levene's test was used for variance homogeneity. Comparative hypothesis was then analyzed with One Way Analysis of Variance (ANOVA) test, and significance between groups was analyzed with Tukey's Post-Hoc test. Group III (TSD) demonstrated the lowest mean of spermatozoa count (41,53±2,29), spermatozoa morphology (13,00±2,91), spermatozoa viability (31,00±7,39), and percentage of motility rate (37±8,58%) among all groups. In conclusion, total sleep deprivation (TSD) decreases the quality of spermatozoa in male albino rats (Rattus norvegicus).
