Abstract

BackgroundThalassemia, a monogenic genetic disease of red blood cells, is spread widely throughout the world. Glutathione S transferase (GST) enzymes have an antioxidant role in detoxification processes of toxic substances This study aimed to determine the role of the genetic modifier genes GSTT1 and GSTM1, and the catalase (CAT) gene in clinical degrees of hemoglobin (Hb)E/? thalassemia. MethodsSixty HbE/? Thalassemia patients were examined to determine their clinical pictures. Clinical score was based on age when thalassemia symptoms appeared, time of diagnosis, time of first blood transfusion, pre-transfusion hemoglobin concentration, frequency of transfusions, and enlargement of spleen. Ferritin concentration was also obtained from medical records. Gene polymorphisms of GSTT1, GSTM1, and CAT were measured using PCR and PCR-RFLP methods. Clinical scores were categorized into mild (0-3.5), moderate (4-7), and severe (7.5-10) degrees, while ferritin level was expressed in mg/dL. One way Anova was used to analyze the data. ResultsThe clinical appearance showed that severe, moderate, and mild degrees accounted for 42%, 45%, and 13%, respectively. The majority had a high ferritin level of more than 5000 mg/dL (67%). GSTT1 null, GSTM1 null, and CAT minor allele genotypes were 21.7%, 33.3%, and 12.1%, respectively. GSTT1, GSTM1, and CAT genotypes had no impact on the severity of thalassemia patients (p=0.091, p=0.082, and p=0.141, respectively).ConclusionGSTT1, GSTM1, CAT gene polymorphisms tend to be a minor aspect of severity of clinical outcome for HbE/â thalassemia patients and should be not considered a routine laboratory check.

Abstract

Talasemia menempati kelainan genetik yang paling umum di seluruh dunia dengan prevalensi karier talasemia di Indonesia adalah sekitar 3-10%. Banyumas merupakan salah satu wilayah yang menyumbang angka prevalensi penderita talasemia yang cukup besar. Salah satu perangkat dalam program pencegahan terpadu adalah memastikan diagnosa molekuler pada tahap skrining sesuai dengan mutasi lokal. Tujuan dari penelitian adalah untuk mengetahui nilai uji diagnostik indeks darah dan karakterisitik mutasi talasemia β pada pendonor darah yang dicurigai karier talasemia. Subjek penelitian adalah 183 pendonor darah rutin pada PMI Banyumas. Skrining awal menggunakan indeks darah MCV, MCH, dan Hemoglobin elektroforesis. Karakteristik molekuler dilakukan dengan teknik PCR-RFLP dan teknik ARMS.  Hasil penelitian menunjukkan bahwa MCV memiliki nilai sensitivitas 81,3%, dan spesifisitas 95,8% sedangkan MCH menunjukkan sensitivitas sebesar 80% dengan spesifisitas 97,5%. Mutasi IVS-1 nt 5 (G>C) merupakan mutasi tersering disusul dengan HBE (codon 26) dan IVS1 nt 1(G>T).Kata Kunci: Identifikasi molekuler, indeks darah, karier talasemia

Abstract

The golden period of children’s growth occurred in the first 1000 days and every disruption in this period would damage the number of cells and myelination that could not be achieved in the next period. Head circumference (HC) at birth was correlated with brain size to measure the brain’s growth and development. This study aimed to find the correlation between HC at birth and children’s growth and development. The research method used was cross-sectional. A total sample of 233 children at 6-60 months old in Posyandu Bina Kasih I-XI Rempoah Village, Baturaden District, was taken using total sampling. Research data used secondary data from previous study and were analyzed by Eta and spearman correlation tests. Results show 180 children who have normal nutritional status and 221 children with development based on their age. Median HC at birth value is 33 cm belongs in the normal category. Spearman test shows HC at birth is not correlated with nutritional status (p=0.412). Eta test shows HC at birth was statiscally significant correlated with development based on KIA book albeit with a very weak level of correlation(F calculated > F table; ETA=0.196). In conclusion, Head circumference at birth is correlated with development based on the KIA book, but does not correlated with nutritional status and head circumference.

Abstract

Background: Anemia is a condition where the number of red blood cells or hemoglobin is less than normal. Lack of hemoglobin can be affected by nutritional status. Poor nutritional status can lead to reduced hemoglobin levels resulting in anemia. Objective: To identify the relationship between nutritional status and hemoglobin (Hb) levels in junior high school adolescent girls at Purwokerto Timur 2 Health Center. Methodology: Observational analytic research with a cross sectional approach. The sample in this study was 278 seventh grade junior high school students who were taken by total sampling method. Data taken in the form of hemoglobin levels and nutritional status values determined by Body Mass Index, both data recorded in medical records. Results: There is a median result of 18.13 for nutritional status with a minimum value of 10.84 and a maximum value of 41.20. While the median results for hemoglobin levels were 11.20 with a minimum value of 4.70 and a maximum value of 18.10. The number of Body Mass Index underweight majority had a value (53.2%), while for anemia the majority were in moderate anemia (36.7%). Conclusion: There is no relationship between nutritional status and hemoglobin levels in junior high school students in the working area of Puskesmas Purwokerto Timur. Latar Belakang: Anemia merupakan kondisi jumlah sel darah merah atau hemoglobin kurang dari normal. Kurangnya kadar hemoglobin dapat dipengaruhi oleh status gizi. Status gizi yang buruk dapat menyebabkan berkurangnya kadar hemoglobin hingga terjadi anemia. Tujuan: Mengidentifikasikan hubungan status gizi dengan kadar hemoglobin (Hb) pada Siswi Putri SMP di Puskesmas Purwokerto Timur 2. Metodologi: Penelitian analitik observasional dengan pendekatan cross sectional. Sampel pada penelitian ini sebanyak 278 siswi kelas VII SMP yang diambil dengan metode total sampling. Data yang diambil berupa nilai kadar hemoglobin dan nilai status gizi yang ditentukan oleh Indeks Massa Tubuh, kedua data tersebut tercatat dalam rekam medis. Hasil: Terdapat hasil median 18.13 untuk status gizi dengan nilai minimum 10.84 dan nilai maximum 41.20. Sedangkan hasil median untuk kadar hemoglobin 11.20 dengan nilai minimum 4.70 dan nilai maximum 18.10. Jumlah Indeks Massa Tubuh underweight mayoritas memiliki nilai (53.2%), sedangkan untuk anemia mayoritas masuk dalam anemia sedang (36.7%). Kesimpulan: Tidak ada hubungan antara status gizi dengan kadar hemoglobin pada siswi SMP di wilayah kerja Puskesmas Purwokerto Timur 2.

Abstract

Good leadership helps maximize the use of resources to achieve organizational goals. Data shows that this comes from young people who are enrolling in colleges in the Banyumas area. The local government, from the village to the region, plans a way so that local arts and culture are not easily influenced by other cultures. This method is used by the local government to encourage young people at the college level to improve community activities and college student activity programs. When the organization looks less professional, this problem arises. This is caused by a number of main factors, including problems related to the vision and mission, especially in managing the organization itself. Organizations founded by young people can face various unique problems due to the dynamics and unique characteristics of young members. Based on the problems found in the previous situation analysis, it is necessary to provide counseling and guidance to young students. This is especially true for students who are members of groups and communities in the college environment. This community organization focuses on the context of arts and culture in the Banyumas area, Central Java. Solutions must be used to address problems to improve leadership in organizations managed by young people. The first thing we do is provide an important understanding of leadership in organizations, then we go into the content, namely about arts and culture in the Banyumas region of Central Java.

Abstract

Background: Maternal health during pregnancy is crucial for neonatal outcomes, particularly regarding iron status. This study investigates the impact of maternal weight gain and inflammatory markers on neonatal iron deficiency in a cohort of mothers and their newborns. Methods: A cross-sectional study was conducted involving spontaneously delivered infants from single, full-term pregnancies, with Apgar scores ≥7 at the first minute and normal birth weights (≥2,500 to <4,000 grams). Maternal weight gain was categorized as "appropriate" or "inappropriate" based on the 2009 Institute of Medicine (IOM) recommendations. Maternal blood samples were collected during the third trimester, and infant iron status was assessed through hematological parameters and serum iron levels. Statistical analyses included the Mann-Whitney test and independent t-tests, with significance set at p < 0.05. Results: A total of 59.5% of mothers did not achieve appropriate weight gain, and 57.1% exhibited positive CRP levels, indicating inflammation. Infants born to mothers with appropriate weight gain had significantly better hematological parameters, including higher erythrocyte counts, hemoglobin, and hematocrit levels (p < 0.05). Conversely, infants of mothers with positive CRP levels demonstrated lower erythrocyte counts and hemoglobin levels, indicating a potential negative impact of maternal inflammation on iron transfer (p < 0.05). No significant differences in neonatal iron status were observed between infants born to anemic and non-anemic mothers. Discussion: The findings underscore the importance of monitoring maternal nutritional status and inflammatory markers during pregnancy. Adequate weight gain according to IOM guidelines is associated with improved neonatal iron status, while maternal inflammation negatively impacts iron transfer to the fetus. These results highlight the need for targeted interventions, including nutritional education and management of inflammatory conditions, to enhance maternal and neonatal health outcomes. Conclusion: This study contributes to the understanding of the relationships between maternal weight gain, inflammatory markers, and neonatal iron status. By addressing these factors, healthcare providers can improve outcomes for mothers and their newborns, ultimately promoting healthier populations.

Abstract

Varicella Zoster Virus (VZV) can trigger various complications, one of which is Immune Thrombocytopenic Purpura (ITP), although this occurrence is rare. ITP is a bleeding disorder caused by excessive platelet destruction due to autoantibodies. This case report presents an 8-year-old female child who developed varicella accompanied by severe thrombocytopenia. Initially, the patient experienced prodromal symptoms including fever, followed by the appearance of vesicular skin lesions that spread centrifugally. Laboratory tests revealed thrombocytopenia, supporting the diagnosis of ITP. The management included antiviral therapy with acyclovir, platelet transfusion, and corticosteroid therapy to reduce inflammation. Although ITP triggered by varicella is rare, prompt and appropriate management is essential to prevent life-threatening complications.

Abstract

Background: Neonatal sepsis remains a leading cause of morbidity and mortality in newborns, particularly among preterm infants whose immature organ systems and immune defenses increase their vulnerability to infection. Evidence on the association between prematurity and neonatal sepsis remains variable across settings, highlighting the need for context-specific research. Objective: To examine the association between prematurity and the occurrence of neonatal sepsis at a tertiary referral hospital in Indonesia. Methods: A hospital-based analytic observational study with an unmatched case–control design was conducted at Margono Soekarjo Regional Hospital, Central Java, Indonesia, from June to November 2024. Medical records of neonates admitted in 2023 were reviewed. Cases were neonates diagnosed with sepsis, while controls had no sepsis diagnosis. A total of 136 neonates were selected using simple random sampling, with 68 cases and 68 controls. Data were analyzed using Chi-square tests and odds ratios (OR) with 95% confidence intervals (CI). Results: Preterm birth was more frequent among cases (64.7%) compared to controls (32.4%). The majority of sepsis cases were moderate-to-late preterm (41.2%) and predominantly late-onset (86.8%). Bivariable analysis demonstrated a significant association between prematurity and neonatal sepsis (χ² = 14.248, p < 0.001), with an OR of 3.833 (95% CI: 1.883–7.805), indicating that preterm neonates had nearly four times higher risk of developing sepsis than term neonates. Conclusion: Prematurity is a significant risk factor for neonatal sepsis, particularly late-onset cases, in this tertiary care setting. These findings highlight the urgent need for targeted infection prevention strategies and enhanced clinical monitoring for preterm infants in neonatal intensive care units.

Abstract

Peningkatan jumlah lansia di Indonesia membawa tantangan dalam pemeliharaan kesehatan saraf, khususnya terkait gangguan tidur, neuropati, dan penurunan fungsi kognitif. Pengabdian masyarakat ini bertujuan meningkatkan kesehatan saraf lansia melalui edukasi gangguan tidur, edukasi neuropati, dan penerapan senam otak di Posyandu Lansia Istiqomah Purwokerto. Kegiatan dilaksanakan pada 27 Juli di pelataran Masjid Fastabiqul Khaerat, Berkoh, melibatkan penyuluhan interaktif, sesi tanya jawab, serta praktik senam otak yang dipandu oleh dr. Untung Gunarto, Sp.S. Metode partisipatif digunakan untuk memfasilitasi transfer pengetahuan dan keterampilan secara langsung. Hasil menunjukkan lansia mampu mengingat kembali materi penyuluhan, memahami cara pencegahan gangguan tidur dan neuropati, serta mempraktikkan senam otak secara mandiri. Kader posyandu juga menunjukkan kesiapan menjadi penggerak kesehatan di komunitas. Kegiatan ini berpotensi direplikasi sebagai model pembinaan kesehatan lansia yang efektif dan berkelanjutan.
