Abstract

Background : Obesity is one of nutritional problems which often encountered, and potentially cause health disturbance due to several complications and high risk of comorbidity. Thare are several factors that may contribute as etiology and risk factors of obesity, like macrosomia.Purpose : To investigate the association between macrosomia and obesity in children who were born at Margono Soekardjo hospital Purwokerto from January to December 2010.Method : A cohort retrospective was conducted since October 2013 to April 2014. The subjects are babies with macrosomia (≥4000g) and normal weight (2500-3999g) who were born at Margono Soekardjo hospital Purwokerto from January to December 2010 and resided in Banyumas. Total sampling was conducted. Birth weight of the subjects were seen in medical record. Body mass index was calculated and classified as obesity and normal using CDC curve by sex and age (≥ 95th percentile). Data were analyzed using Chi-square and logistic regression.Results : Total sampels were 82 respondents composed of 44 boys and 38 girls, and found nineteen respondents were obese. Macrosomia associated with obesity, with p-value=0,018 and RR=2,80 (95%CI; 1,11–7,06) by chi-square test. Macrosomia, formula feeding and timing of introduction of complementary food effect to obesity, with p value < 0,05 using logistic regression.Conclusions : Macrosomic newborns were significantly associated with obesity in children who were born at Margono Soekardjo hospital Purwokerto from January to December 2010.

Abstract

Background: The first 1,000 days of life are the foundation for growth and development of children in the next period. Birth length may represent growth and development during the intrauterine life. This study aimed to explore the correlation of birth length, growth, and development in children.Methods: This was analytic observational study with a cross-sectional design. Total sampling was taken, obtaining 368 children aged 6−60 months in several Integrated Service Post (Pos Pelayanan Terpadu, Posyandu), conducted from October to November 2018. Data were gathered from the book of Maternal and Child Health (Kesehatan Ibu dan Anak, KIA), including birth length, body length/height, and nutritional status using weight-for-length/height index. Data were analyzed by Spearman and Eta test. Results: Birth length was not correlated with nutritional status (p=0.108, r=0.084), but had a good correlation with body length/height (p=0.027, r=0.115) and development based on the KIA book (F calculated > F table, Ƞ=0.245).Conclusions: There is a correlation between birth length and body length/height as well as a correlation between birth length and development based on the KIA book. It is important for the community and health workers to monitor growth and development of children, by maximizing the use of KIA book.

Abstract

Posyandu (integrated health center) as a health information center for the community is expected to be the spearhead of early detection of developmental disorders in children under five. Posyandu activities are driven by cadres. Cadres monitor the growth of children under five through anthropometric measurements: body weight, height, head circumference and development milestones. Knowing how to measure anthropometric parameters, data plotting on a growth chart in the Maternal and Child Health (Kesehatan Ibu dan Anak=KIA) book, and interpret the results are very important. Cadres with good understanding and skills will greatly assist in monitoring child growth and development. This activity is expected to increase the knowledge and skills of cadres in monitoring child development. This activity was carried out through counseling, training, case study, and discussion. Pre-tests and post-tests were conducted to evaluate this activity. The mean pre-test was 40.00 ± 11.69, with the lowest value being 10.00 and the highest value being 60.00. The mean post-test was 54.10 ± 12.08, the lowest value was 30.00 and the highest was 90.00. The Wilcoxon test found p=000, where 20.5% participants' knowledge did not change while 69.23% had better knowledge compared to before the activity. Conclusion: This training can significantly improve cadres' knowledge and skills about the use of the KIA book.

Abstract

Latar belakang. Hiperbilirubinemia merupakan masalah umum yang sering dijumpai pada bayi baru lahir. Tata laksana hiperbilirubinemia neonatal dilakukan untuk mencegah timbulnya komplikasi dan fototerapi merupakan metode yang paling sering digunakan. Fototerapi konvensional menurunkan kadar bilirubin lebih lama dibanding fototerapi intensif sehingga berpotensi menyebabkan inefisiensi. Saat penelitian ini dilakukan, RSUD Prof.dr. Margono Soekarjo (RSMS) Purwokerto hanya memiliki alat fototerapi konvensional.Tujuan. Mengevaluasi pengelolaan hiperbilirubinemia neonatal dengan fototerapi konvensional. Metode. Penelitian crossectional melibatkan 157 subjek dengan kiteria inklusi adalah hiperbilirubinemia neonatal, yang dirawat inap pada Januari–September 2018 di RSMS Purwokerto, dengan menggunakan data sekunder rekam medis. Analisis data dan statistik digunakan SPSS dan uji Wilcoxon digunakan untuk mengalisis perbedaan antara kadar bilirubin serum total (BST) pra dan pascafototerapi. Hasil. Sebanyak 157 bayi (13,08 %) dirawat dengan hiperbilirubinemia, dengan usia tersering 3 hari. Rerata lama fototerapi 60,27 jam. Analisis komparatif antara kadar BST pra dan pascafototerapi, terdapat penurunan BST yang signifikan (p <0,001) dari 17.23±5.04 mg/dL (prafototerapi) menjadi 10,18±2,02 mg/dL (pascafototerapi). Rerata lama rawat inap 4,48±4,47 hari. Kecepatan penurunan kadar bilirubin 0,12 mg/dL per jam. Kesimpulan. Fototerapi konvensional efektif menurunkan kadar BST hiperbilirubinemia neonatal, tetapi tidak efisien.

Abstract

Latar belakang. Masa setelah bayi lahir, sebelum plasenta dilahirkan, terjadi peralihan peran oksigenasi dariplasenta ke paru bayi. Selama masa tersebut, oksigenasi bayi melalui plasenta masih berlanjut, darah masihditransfusikan ke bayi (disebut transfusi plasenta). Jika peran oksigenasi plasenta dihentikan mendadakdengan penjepitan tali pusat dini, sementara paru belum berfungsi optimal, maka cerebral blood flowmenjadi tidak adekuat. Kapan penjepitan tali pusat seharusnya dilakukan, masih menjadi kontroversi danperdebatan lebih dari satu abad, namun mana yang lebih baik bagi bayi, belum mendapatkan jawabanyang memuaskan.Tujuan. Membuktikan pengaruh waktu penjepitan tali pusat setelah bayi lahir terhadap kadar hemoglobin(Hb) dan hematokrit (Ht) bayi baru lahir.Metode. Penelitian dengan posttest-only control group design, menganalisis pengaruh waktu penjepitan talipusat 45 detik (penjepitan lanjut) setelah bayi lahir terhadap kadar Hb dan Ht bayi baru lahir dibandingkandengan 15 detik (penjepitan dini). Subjek adalah 36 bayi baru lahir (19 subjek dilakukan penjepitan dini),lahir spontan di RSUP Dr. Kariadi Semarang dan rumah Bidan praktek swasta, antara Agustus 2007 -Februari 2008. Uji beda rerata kedua kelompok menggunakan independent t-test. Uji multivariat digunakanuji regresi logistik.Hasil. Kadar Hb subjek kelompok penjepitan dini (13,4-18,4)g% dan lanjut (14,5-20,1)g%. Kadar Ht bayipenjepitan 15 detik (37,6-54,7)% dan penjepitan 45 detik antara (41,6-60,6)%. Pada kelompok penjepitan15 dan 45 detik terdapat perbedaan bermakna rerata Hb subjek (16,30g±1,36) dan (17,34±1,67)g% dan Ht(47,08±4.54)g% dan (51,34±6,07)g% dengan angka signifikansi berturut-turut p=0,048 dan p=0,022.Kesimpulan. Rerata kadar Hb dan Ht kelompok penjepitan tali pusat 45 detik lebih tinggi (secara statistikbermakna) dibandingkan kelompok penjepitan 15 detik.

Abstract

ECONOMIC EMPOWERING OF THALASSEMIA PATIENTS THROUGH ONLINE ENTREPREUNERS ACTIVITIES IN BANYUMAS. Thalassemia in Banyumas is a part of portrait of Indonesia’s problems. Todays, there is not yet any national thalassemia countermeasures program so that thalassemia prevalence is getting increased year by year. Increasing life expectancy of thalassemia patients brings economical problem consequence. Social stigma and businessmen support Thalassemia patients to be powered economically in their productive age. Entrepreneurship autonomy becomes an alternative to overcome it. Online entrepreneurs has flexibility, creative space, global range and unlimited time that support it to be an optional solution for preparing thalassemia patients welcoming productive age.

Abstract

HEALTH PROMOTION FOR PREVENTING AND COMPLICATIONS MYOPIA IN STUDENTS OF SDN 2 BERKOH, PURWOKERTO. Myopia is the most common refractive disorder in school-aged children. These visual disorders need attention, because they can interfere with life and daily work, and also their intellectual development. This health promotion activity is conducted to improve knowledge and understanding about myopia and the efforts that must be done to maintain eye health. This activity was conducted on 140 students of 4th, 5th, and 6th grade of SDN 2 Berkoh Purwokerto, through counseling, training, giving module, leaflet and poster. Based on the T-test of the pre-post and post-test in each class, the p values obtained in grades 4,5 and 6 respectively p = 0.058; 0.588; and 0.074. Conclusion: this activity can increase students' knowledge of myopia, although the increase is not significant.

Abstract

LACTATION MANAGEMENT TRAINING FOR PREGNANT WOMEN AND BREASTFEEDING MOTHERS: EFFORTS TO OPTIMIZE CHILD GROWTH. The key person for making quality generation lies on mother, through breast milk. Breastfeeding, besides providing the best nutrition for optimal baby growth and development, it is also a form of love to a baby. Ironically, not all mothers realize and many mothers do not breastfeed their babies exclusively. The low level of knowledge and working mothers is often an obstacle for mothers to give exclusive breastfeeding. In Indonesia, almost 9 of 10 mothers give breast milk, but only 49.8% give exclusive breastfeeding for 6 months. Lactation management health promotion activities are carried out in accordance with WHO recommendations and the Indonesian government in promoting breastfeeding, especially increasing the achievement of the rate of early breastfeeding initiation and exclusive breastfeeding. The activities carried out using several methods: counseling, demonstration, video and discussion. We found that 54.1% of participants gave exclusive breastfeeding for 6 months, 27% for 4 months, and others 2-3 months. Wilcoxon Rank Test showed a significant difference (p = 0,000) between the score of pre-test and post-test, with a mean pre-test of 49.02 ± 19.74, and a mean post-test of 68.53 ± 20.00. Conclution: This activity increased participants' knowledge regarding lactation management.

Abstract

Thalassemia ranks top on the list of diseases caused by the monogenic mechanism, especially in hematology disorders. Currentlly, Thalassemia ranks 5th in the national health insurance coverage. Prevention is the only effective way to control the clinical, psychological and financial burden of the country. One aspect of prevention is the discovery and genetic counseling process of persons who carry the mutan gene or carier of thalassemia. Midwives as the most peripheral health workers in the national health system are very important in their role in the discovery and counseling of these people. Provision of sufficient knowledge and skills of midwives can be a reliable tool in efforts to prevent thalassemia in the community.

Abstract

Pandemic has had implications for all aspects of life in the world, including Indonesia. Under normal conditions, maternal and neonatal mortality is still a big challenge, especially during the current pandemic. Maternal and child health services are one of the services that are affected in terms of access and quality. The existence of restrictions on most routine services, including maternal and child health services, is feared to increase maternal and newborn morbidity and mortality, reduce maternal and child health, and interfere with child development. The team from FK UNSOED wants to take a role in the community to prevent an increase in infant and neonatal mortality in the midst of this pandemic situation. Through this activity, the community (especially pregnant women and mothers of toddlers) is expected to be able to strive for pregnancy, childbirth, postpartum, and child development so that they remain optimal. Activities are held through the lay webinar method and mentoring of posyandu cadres. The target audience for the public webinar is the general public and partners. The enthusiasm of the participants can be seen from the number of participants, who come from various circles and regions. The Zoom meeting was attended by 235 participants from various regions in Java and outside Java (Bali, Pontianak, etc.). Participants came from various backgrounds: 45.3% midwives, 15.4% housewives, 11.3% lecturers/teachers, 4.5% doctors, 2.4% posyandu cadres, private employees, students, etc. Meetings and direct mentoring activities in Karangnanas village were carried out with limited participants, namely representatives of posyandu cadres and village midwives. More intensive efforts are needed to empower cadres in the midst of this pandemic.
