Abstract

Dengue Hemorrhagic Fever (DHF) remains a public health issue in Pekalongan City, particularly among elementary school children. Mosquito Nest Eradication (PSN) is a key strategy in DHF control; however, cases are still being reported. This indicates challenges in PSN implementation and factors affecting its success. Objective to analyze the implementation of PSN in elementary schools and identify factors contributing to the persistence of DHF cases among elementary school children in Pekalongan City. A qualitative study with a case study approach was conducted, involving 20 participants. The primary informants included representatives from the Health Office, Education Office, school principals, teachers, and school health officers. Triangulation informants consisted of jumantik (mosquito monitoring) cadres, parents, and students. Data were collected through in-depth interviews, observations, and documentation.The data analysis followed a thematic analysis approach. First, all interviews were transcribed verbatim, and field notes from observations were reviewed. The data were then coded systematically to identify patterns and emerging themes. Codes were grouped into broader categories, and themes were refined through an iterative process. Triangulation was conducted by comparing data from different sources to enhance credibility and validity. Finally, findings were interpreted in relation to the study objectives, supported by direct quotes from participants to provide depth and context. PSN implementation in elementary schools in Pekalongan City has been carried out through activities such as draining water containers, covering potential mosquito breeding sites, and monitoring by health officers and school jumantik cadres. However, several challenges remain, including low community awareness of environmental cleanliness, lack of regular supervision in schools, and environmental and climatic factors that support mosquito breeding. Additionally, limited facilities and human resources in schools pose challenges to optimizing the PSN program. The implementation of PSN in elementary schools in Pekalongan City has been conducted but remains suboptimal, leading to the continued occurrence of DHF cases. Increasing community awareness, strengthening routine supervision in schools, and cross-sectoral support are needed to ensure the sustainability of the PSN program and reduce DHF incidence.

Abstract

Medical waste refers to the waste generated from medical activities within healthcare facilities, including hospitals. Other studies indicate that 15-25% of medical waste is toxic, infectious, or radioactive, while 75-85% is non-hazardous. Despite the smaller volume of medical waste compared to other waste types, its improper management can pose significant health and environmental risks. This research aims to investigate the control of environmental health risks in the handling of infectious medical waste at hospitals using the FMEA method. This study employed an observational research design using a cross-sectional approach. A mixed-methods approach was applied, combining both quantitative and qualitative analyses to assess environmental health risk management related to medical waste processing based on priority or critical risks at RSUP Persahabatan. The target population included individuals involved in both the handling and treatment stages of medical waste using an incinerator at the facility. The sample selection for the FMEA analysis employed purposive sampling with a quota sampling technique.The study utilized an environmental risk analysis framework based on the Failure Mode and Effect Analysis (FMEA) method. The critical Risk Priority Number (RPN) analysis revealed the following three primary environmental health risks: (1) The risk of contracting environmental health-based infectious diseases through the transmission of pathogenic microorganisms or hazardous materials, associated with 12 failure modes in the medical waste handling process. (2) The risk of ambient air (work environment) contamination, associated with 5 failure modes in the medical waste handling process. (3) The risk of contamination of facilities and rooms where medical waste is generated, associated with 4 failure modes related to the operation of the incinerator during the medical waste treatment process. Environmental health risks with a high potential to arise from failure modes during the medical waste management stage at RSUP Persahabatan, as indicated by critical Risk Priority Number (RPN) scores.

Abstract

Domestic wastewater discharge into rivers remains a persistent environmental and public health challenge in rapidly urbanizing regions of developing countries such as China. Although communal wastewater treatment systems have been increasingly implemented, empirical studies integrating community willingness to pay (WTP), behavioral determinants, and sustainability planning are limited. This study analyzes community willingness to pay for communal wastewater treatment services and formulates sustainability-oriented strategies for the Sewon Communal Wastewater Treatment Plant (WWTP) in the Special Region of Yogyakarta, Indonesia. A mixed-methods approach combined a survey of 100 households with interviews and a document analysis. WTP was estimated using the contingent valuation method with a payment card format, while behavioral determinants were examined using covariance-based Structural Equation Modelling (SEM) with AMOS. Strategic formulation was conducted using SWOT analysis supported by IFAS and EFAS matrices based on validated SEM results. The results indicate a positive willingness to pay, with a mean of IDR 7,114 and a median of IDR 7,500. The SEM revealed that supporting institutional factors significantly influenced the WTP (X4 = 0.664; p < 0.01), whereas environmental perception and inhibiting factors were not statistically significant.
