Abstract

Fasting that is too long will affect the patient's condition, one of which is the patient's blood sugar levels. In this case, it is necessary to control the start time of fasting with the time of the operation that the patient will undergo. The purpose of the study was to determine Preoperative Fasting and Blood Sugar Highlighting Emergency Complications in General Anesthesia Patients at Dr. Reksodiwiryo Class III Hospital in Padang. A correlational analytical observational study was conducted using a cross-sectional approach. This observational analytical correlational study was conducted with a cross-sectional approach. The number of samples was 34 people, the sampling technique used consecutive sampling. This research instrument used an observation sheet and a blood sugar level monitoring tool (Glucometer) With the respondent criteria determined according to the inclusion and exclusion criteria. The test results showed that there was a relationship between the length of fasting and blood sugar levels highlighting emergencies in preoperative general anesthesia patients at Dr. Reksodiwiryo Class III Hospital in Padang with a p value = 0.005. It can be concluded that the duration of preoperative fasting can affect blood sugar levels in general anesthesia patients which causes patients to become hypoglycemic or hyperglycemic which triggers various emergency complications that can worsen the patient's clinical condition when undergoing general anesthesia.

Abstract

Based on data from the World Health Organization (WHO) in 2020, the prevalence of stunting in children worldwide reached 22% or 149.2 million, making it a global issue. Indonesia ranks second highest in Southeast Asia with a stunting prevalence of 31.8%, following Timor Leste at 48.8%. Adolescent girls suffering from anemia are at risk of becoming anemic pregnant women, as well as experiencing protein-energy deficiency. This increases the likelihood of giving birth to low birth weight (LBW) and stunted babies, along with complications during childbirth. The aim of this study was to examine the differences in hemoglobin levels between stunting and non-stunting adolescents. The study was conducted in 2022 at the adolescent health post of the Lubuk Buaya Padang Health Center, using non-probability sampling through consecutive sampling. This observational study employed a cross-sectional approach and involved 42 female adolescents aged 12-21 years. Quantitative data were processed manually and with computer software, and statistical tests were performed using the t-test at a significance level of α = ≤ 0.05. The results indicated that non-stunting adolescents had an average hemoglobin level of 12.8 g/dl, while stunting adolescents had an average level of 12.0 g/dl. The conclusion of this study is that there is no significant difference in hemoglobin levels between stunting and non-stunting adolescents.

Abstract

Preoperative fasting is a standard applied in surgical services to prevent aspiration, but in hospital practice, fasting often occurs that exceeds the recommendations of the American Society of Anesthesiologists (ASA) which recommends a minimum fast of 6-8 hours, the negative impact of extended fasting will affect the hemodynamic and metabolic status of patients. This study aims to analyze the impact of variations in fasting duration with hemodynamic status and blood sugar levels in patients. This study used a quantitative design with a cross-sectional approach with a sample of 252 elective surgical patients collected by purposive sampling. Data collected included demographic characteristics, ASA physical status I-II, fasting duration, and preoperative clinical parameters. with inclusion criteria: respondents aged 18-55 years, ASA physical status I-II, undergoing elective surgery while exclusion criteria were patients with metabolic disorders such as diabetes mellitus or unstable cardiovascular disorders and cytotoxic patients. Data were collected using observation sheets related to demographics, physical status, type of surgery, last meal, and post-operative complaints, as well as a Non-Invasive Blood Pressure Monitor (NIBP) to measure hemodynamics, a pulse oximeter for oxygen saturation, and a glucometer for glucose levels. The results of the one way anova test showed that variations in fasting duration did not affect hemodynamics (p > 0.05), but did affect blood sugar levels where the value (p < 0.05). It was found that these findings confirm that pre-operative fasting beyond the recommendation can affect the patient's metabolic status, so that fasting duration management according to ASA standards is important to maintain the patient's metabolic and physiological stability.
