Abstract

People nowadays with different characteristics are very selective when choosing medical services. Hospitals as service providers are expected to have a good brand image which is the patient's perception of a brand. It is built by trust in care and knowledge about the hospital. The hospital's brand image makes patients recognize and differentiate them from others. Patient selection of health care facilities is a complex process. The brand image expected to had a direct effect on hospital selection. This article explains the influence of hospital brand image on patients' decisions to choose a hospital. The method used is a literature review study and sources from online academic media such as Mendeley. Studies related to the field of health services and searched for using valid keywords. We found 48 articles, and after evaluation, eight selected articles were included in this study. From various studies, we found that brand image positively impacts patients' decisions in choosing a hospital. A hospital's brand image will influence patient perceptions of service quality. The social aspect and marketing communication strategy have proven to be excellent induction processes for establishing a brand image in the minds of consumers.

Abstract

Antibiotic resistance is an increasing global health challenge, especially in patients with advanced chronic kidney disease (CKD). This study aims to investigate cefixime resistance in patients with stage 4-5 CKD at Harapan Keluarga Hospital with a retrospective cohort design using medical record data. Chi-squared test analysis showed a significant association between cefixime resistance and advanced CKD (χ² = 34.714, p = 0.003). The results showed a higher prevalence of cefixime resistance in male patients (60%), antibiotic use of more than 14 days (65%), and hospitalization of more than five days (80%). The mechanism of resistance involves beta-lactamase production and changes in the gut microbiota due to dysbiosis. Factors such as irrational antibiotic use, length of hospital stay, and catheter use contributed to higher resistance. This study highlights the need for evidence-based antibiotic stewardship strategies to reduce resistance, especially in vulnerable populations such as patients with advanced CKD. Scientific implications include the development of more effective infection management strategies, close monitoring of antibiotic use, dose adjustment according to CKD severity, and education of health care workers to increase awareness of antibiotic resistance. This study contributes to efforts to prevent antibiotic resistance in the advanced CKD population.

Abstract

Hypertension and chronic kidney disease (CKD) are interrelated global health challenges with rising prevalence, particularly in low-resource settings such as Indonesia. This retrospective cohort study aimed to investigate the association between hypertension stage 2 and CKD among 65 patients treated at Harapan Keluarga Hospital between 2023 and 2024. Data were obtained through medical record reviews, and statistical analysis was conducted using Chi-square testing and odds ratio (OR) calculations. A significant association was found between stage 2 hypertension and CKD (p = 0.001), although the OR value of 0.888 (95% CI: 0.821–0.920) suggests a negative correlation, potentially influenced by variations in treatment adherence, demographic characteristics, or other confounding factors. Differences in BMI and blood pressure between male and female patients were also observed, highlighting the role of sex-specific physiological and lifestyle variables in disease progression. The findings emphasize the importance of tailored clinical management strategies for hypertensive patients to prevent CKD progression, particularly in healthcare settings with limited diagnostic and therapeutic resources.

Abstract

Chronic Kidney Disease (CKD) is a gradually progressing condition that frequently remains asymptomatic in it’s early stages, making it an increasingly significant public health concern, particularly among the elderly. Early awareness and preventive behavior are crucial to mitigate it’s impact, yet many elderly individuals lack sufficient knowledge about disease. This study aims to examine the relationship between the knowledge level of elderly individuals and the incidence of CKD in the Puskesmas Taliwang area in 2024. A cross-sectional analytical study was conducted involving 30 elderly participants selected through simple random sampling. Data on knowledge level were collected using a structured was performed using the Chi-square test. The result showed a significant association was found between knowledge level and CKD incidence P-value=0.000 and OR value is between 0,900 and 1,020, indicating that lower knowledge correlates with a higher risk of CKD. There is a significant relation between the knowledge level of elderly individuals and the incidence of CKD in the Puskesmas Taliwang area. Strengthening educational outreach and awareness programs is essential to reduce CKD risk among the elderly.

Abstract

Chronic Lymphocytic Leukemia (CLL) is a slowly progressing hematological cancer characterized by the accumulation of abnormal B lymphocytes in the blood, bone marrow, and lymphatic tissue, commonly affecting the elderly. This review aims to comprehensively review the epidemiology, molecular basis of the disease, clinical symptoms, diagnosis, and therapeutic advances of CLL, emphasizing the importance of shifting treatment approaches towards more precision therapy. A literature search was conducted through PubMed, Scopus, and Web of Science using the keywords "Chronic Lymphocytic Leukemia", "epidemiology", "molecular basis of the disease", and "targeted therapy", with publication criteria between 2018–2025, full text, and topic relevance. Analysis was conducted using a narrative review and thematic synthesis approach to identify epidemiological trends, therapeutic effectiveness, and clinical implications. The results showed that the incidence of CLL is higher in Western countries compared to Asia, with a higher prevalence in elderly men. The main genetic factors that play a role include del(13q), del(17p), and TP53 mutations. Over the past decade, targeted therapies such as second-generation Bruton's tyrosine kinase inhibitors (acalabrutinib, zanubrutinib) and the venetoclax-obinutuzumab combination have been shown to improve remission rates with fewer side effects compared to chemoimmunotherapy. Minimal Residual Disease (MRD) status has become an important marker for determining when therapy can be stopped early. In conclusion, this review underscores the crucial role of biomarkers and MRD in modern CLL management. Future developments include the integration of personalized therapy approaches and translational research to develop more effective and safe treatment strategies.
