Abstract

Japan has the world's largest elderly population, with a family culture that entrusts elderly care to nursing homes, or the so-called Roujin Home. In 2020, there were around 11,575 Roujin Houses in Japan. A common problem that arises in patients with auditory hallucinations is sensory perception disorders. The purpose of this study is to reveal the impact of therapy or reprimands in the healing process of auditory hallucinations in elderly patients. This research was conducted on June 20-22, 2023 at Iryou Houjin Aiwa Deikea, using observation instruments and interviews with patients and therapists. The study focused on one patient with auditory hallucinations. The results showed that the intervention given for 3 days did not provide significant progress, due to a lack of support and motivation from the family, which caused the patient to be reluctant to follow the reprimanding therapy. Limitations of this study include limited duration of intervention and lack of emotion supports from the family. These findings provide important insights into the importance of family support in improve the effectiveness of auditory hallucination therapy.

Abstract

Compliance with taking medication in schizophrenic patients is not only the responsibility of health workers, but requires cooperation and support from the family. The family has an important role as a health service in the smallest business unit for the implementation of treatment for people with mental disorders. This research was conducted with the aim of knowing the relationship between family support and the level of adherence to taking medication in schizophrenic patients in the working area of the Twins Health Center II. This type of quantitative research with correlational and cross sectional descriptive methods as the approach. The research sample was family members of schizophrenic patients in the working area of the Kembaran II Public Health Center as many as 43 respondents using the consecutive sampling technique. The tool used to obtain data is a questionnaire sheet. Data analysis using spearman rank. This study shows that family support in schizophrenic patients is more dominant in less family support (54.5%). The level of adherence to taking medication in schizophrenic patients mostly had a low level of adherence to taking medication (61.4%). There is a relationship between family support and the level of adherence to taking medication in schizophrenic patients in the working area of the Kembaran II Public Health Center with a p value of 0.003. Families are expected to be able to provide other information and knowledge regarding the level of adherence to taking medication. Abstrak Kepatuhan minum obat pada pasien skizofrenia tidak hanya menjadi tanggung jawab tenaga kesehatan saja, namun perlu kerja sama dan dukungan dari keluarga. Keluarga mempunyai peran yang penting sebagai pelayanan kesehatan di dalam unit terkecil usaha untuk pelaksanaan pengobatan pada orang dengan gangguan jiwa.. Penelitian ini dilakukan dengan tujuan untuk mengetahui hubungan dukungan keluarga dengan tingkat kepatuhan minum obat pada pasien skizofrenia di wilayah kerja Puskesmas Kembaran II. Jenis penelitian kuantitatif dengan metode deskriptif korelasional dan cross sectional sebagai pendekatannya. Sampel penelitian adalah anggota keluarga pasien skizofrenia yang ada di wilayah kerja Puskesmas Kembaran II sebanyak 43 responden menggunakan teknik consecutive sampling. Alat yang digunakan untuk mendapatkan data yaitu lembar kuesioner. Analisis data menggunakan spearman rank. Penelitian ini menunjukkan dukungan keluarga pada pasien skizofrenia lebih dominan pada dukungan keluarga kurang (54,5%). Tingkat kepatuhan minum obat pasien skizofrenia sebagian besar dengan tingkat kepatuhan minum obat rendah (61,4%). Terdapat hubungan dukungan keluarga dengan tingkat kepatuhan minum obat pada pasien skizofrenia di wilayah kerja Puskesmas Kembaran II dengan p value 0,003.. keluarga diharapkan mampu memberikan informasi dan pengetahuan lainnya mengenai tingkat kepatuhan minum obat.

Abstract

Introduction: The increase in life expectancy among the elderly has directly contributed to the rise in the global elderly population. According to global data, the number of elderly individuals is projected to reach 1.4 billion by 2030. This demographic growth is often accompanied by various health problems associated with aging, such as the decline in vital organ function and the emergence of degenerative diseases. One of the most common degenerative diseases among the elderly is hypertension. Hypertension is widely known as a “silent killer” because it can lead to serious complications, including stroke and heart disease, if not managed properly. A common complaint experienced by individuals with hypertension is headache, particularly in the posterior region, which significantly affects the patient’s quality of life. Management of hypertension can be carried out through pharmacological therapy with antihypertensive drugs as well as non-pharmacological interventions. One of the non-pharmacological approaches increasingly being applied is isometric handgrip exercise, which involves static muscle contractions performed at light to moderate intensity. Method:This study aimed to evaluate the effect of isometric handgrip exercise on pain reduction in patients with hypertension. The research design employed a descriptive case study involving one individual with hypertension who experienced pain complaints. The study was conducted on September 16–18, 2024, at Roujin Home X. The instruments used included observation, interview, and the provision of isometric handgrip exercise therapy. Pain measurement was carried out using a numerical rating scale before and after the intervention. Results:The three-day intervention demonstrated a significant reduction in pain levels. On the first day, the patient reported a pain score of 6, which decreased to 4 on the second day, and further declined to 2 on the third day after the intervention. Conclusion:The findings of this study indicate that isometric handgrip exercise is an effective non-pharmacological method for alleviating acute pain in patients with hypertension. This simple intervention has the potential to be applied as a complementary therapy in elderly care facilities to improve the quality of life of individuals living with hypertension.
