Abstract

Introduction Anemia is a frequent complication in Chronic Kidney Disease (CKD), especially in End-State Renal Disease (ESRD) undergoing hemodialysis (HD). Reticulocyte Hemoglobin (Ret.He) represents the average value of Hemoglobin (Hb) distribution in reticulocytes and the availability of iron in the process of erythropoiesis. Hepcidin is a hormone that plays a role in the regulation and metabolism of iron. Hepcidin correlation and Ret.He levels in ESRD patients undergoing regular HD is not known clearly. The aim of study is to determine the correlation of hepcidin and Ret.He in ESRD patients undergoing regular HD.Methods This is a cross sectional study that were conducted at H.Adam Malik Hospital, December 2015-March 2016. Subjects were ESRD outpatients undergoing regular HD in Hemodialysis Unit of H. Adam Malik Hospital over 3 months with a frequency of two times per week and signed the informed consent. Spearman correlation test was used for statistical analysisResults The median levels of hepcidin (0.3 ng/ml) and Ret.He (33.6 pg), were found in 31 subjects. There is a weak correlation but insignificant of hepcidin and Ret.He (r = 0.135, p = 0.467).Conclusion There was no correlation of hepcidin and Ret.He in ESRD patients undergoing regular HD.Keywords : Anemia, Hepcidin in End-Stage Renal Disease, Reticulocyte Hemoglobin, ,Regular hemodialysis, iron status

Abstract

Background: Study showed that 95% women had serum level of 25(OH)D lower than normal and women have riskier lifestyle than men, this is based on women’s lifestyle, such as avoiding sun rays, sunblock application, low vitamin D intake, and low physical activity. As result of the deficiency, there will be increasing number of metabolic disorders such as obesity and diabetes mellitus type 2.Objectives: To discover lifestyle, knowledge, attitude, and action of women aged 20-50 years old with vitamin D deficiency-insufficiency regarding risk of metabolic disorder caused by vitamin D deficiency.Method: Cross-sectional study engaging 98 women participants with vitamin D deficiency-insufficiency. Lifestyle including avoiding sunlight, low physical activity, and sunblock application were assessed. The scoring was to assess knowledge, attitude and action of women aged 20-50 years old with vitamin D deficiency-insufficiency regarding risk of metabolic disorder in Medan and then the result was analyzed.Result: The study engaged 98 women aged 20-50 years old with vitamin D deficiency-insufficiency, showed vitamin D level with mean 19.66±5.69 ng/ml. There were 95% participants categorized into vitamin D deficiency-insufficiency and 5% participants categorized into vitamin D suficiency. All subjects were found to be low on sun rays exposure, high incidence sunblock applicaton, low vitamin D intake, and low physical activities. According to knowledge about risk of metabolic disorder, there were 42.3% participants with sufficient knowledge and 57.7% with deficient knowledge, and regarding attitude, 48.52% had negative attitude and 51.5% had positive attitude, regarding action, 24.7% was sufficient and 75.3% was deficient. According to the analysis, there was no significant association between action and knowledge, attitude and knowledge, but there was significant association between action and knowledge based on education (p=0.01).Conclusion: Lifestyle, sufficient knowledge, and action could be the caused of low vitamin D level. There was significant association between action and knowledge based on education, but didn’t show any associations. Based on this result, knowledge needs to be improved in order to improve attitude and action with counseling or books that are easy to be understood about the importance of vitamin D

Abstract

Diabetes melitus (DM) memiliki efek jangka panjang terhadap sistem genitourinari yang menjadi faktor predisposisi terjadinya infeksi saluran kemih (ISK) pada pasien wanita dan umumnya bersifat asimtomatik. Bakteri gram negatif merupakan mikroorganisme penyebab tersering yang dapat membentuk biofilm sehingga sering menyebabkan resistensi antibiotik. Penelitian ini bertujuan untuk membuktikan hubungan antara pembentukan biofilm bakteri gram negatif dengan resistensi antibiotik pada wanita diabetes melitus tipe 2 (DMT2). Menggunakan desain penelitian cross sectional dengan teknik total sampling. Jumlah sampel penelitian adalah 45 orang wanita DMT2 anggota Program Pengendalian Penyakit Kronis (PROLANIS). Pada semua responden dilakukan kultur urin porsi tengah, uji sensitivitas dan pemeriksaan biofilm terhadap isolat bakteri yang teridentifikasi.  Hasil kultur urin menunjukkan bakteriuria signifikan 14 (31%) responden. Identifikasi koloni menunjukkan bakteri penyebab ISK antara lain Escherichia coli (35,7%), Klebsiella pneumoniae (35,7%), Enterobacter sp (21,5%) dan Citrobacter sp (7,1%). Uji sensitivitas dijumpai 8 (57%) isolat resisten terhadap antibiotik, yaitu 2 (14,2%) isolat terhadap ciprofloksasin dan 6 (42,8%) isolat terhadap TMP-SMX. Bakteri gram negatif yang diisolasi 100% mampu membentuk biofilm dengan kategori weak. Berdasarkan analisis bivariat dengan uji Rank Spearman correlation dapat disimpulkan bahwa terdapatnya hubungan yang sangat lemah antara pembentukan biofilm oleh bakteri gram negatif dengan resistensi antibiotik ciprofloksasin dan TMP-SMX.

Abstract

Background: LDH has proved to be a significant predictor for prognosis and follow-up in COVID-19 patients. Several studies report elevated LDH in hospitalized COVID-19 patients to be associated with a higher risk of death. Increased LDH levels can result from any tissue damage due to SARS-CoV-2 infection. The study aims to describe the relationship between LDH levels with COVID-19 19 degree of severity. Methods: This study is a cross-sectional study of 55 patients who confirmed positive by RT-PCR SARS-CoV-2 at H. Adam Malik General Hospital Medan (November 2021 – Maret 2022). Examination of the LDH level was carried out by the Chemiluminescent Immunoassays method (Abbott Architect). COVID-19 degree of severity was assessed based on the diagnostic criteria for COVID-19 cases in Indonesia. Results: Among the 55 COVID-19 patients in this study, the average age found was 45.73, with men 54.5%. Moderate, severe, and critical patients were found in 63.6%; 29.1%; and 7.3% respectively. The mean LDH level at admission was 278 U/L, higher in the severe (333.19 U/L) and critical (465.75 U/L) groups compared to the moderate (231.31 U/L) groups. Increased LDH ≥ 256 U/L proved to be related to COVID-19 degree of severity upon hospital admission (CI 95%; p = 0.001). Conclusions: LDH levels that increased ≥ 256 U/L were significantly related to COVID-19 degree of severity during hospital admission.

Abstract

Acute Coronary Syndrome (ACS) is a leading cause of death worldwide, with platelet activation playing a crucial role in the formation and evolution of atherothrombosis. Platelet indices, such as Mean Platelet Volume (MPV) and Immature Platelet Fraction (IPF), play a significant role as indicators of platelet activation. Troponin-I is a specific protein for the myocardium and serves as an ideal biomarker, providing crucial insights into the extent of myocardial damage. This study aims to identify the relationship between MPV and IPF with Troponin-I levels in patients with ACS. This research was a cross-sectional study involving 52 ACS patients undergoing treatment at H. Adam Malik Hospital, Medan, from February 2023 to May 2023. Blood samples were taken upon admission and examinations for MPV, IPF, and Troponin-I were conducted. The data were analyzed using SPSS software, and the Spearman test was employed. A p-value less than 0.05 was considered statistically significant. No correlation was found between MPV and Troponin-I, with a p-value of 0.251. Similarly, there was no correlation between IPF and Troponin-I, with a p-value of 0.090. However, a strong correlation was observed between MPV and IPF, with a p-value < 0.001 and an r-value of 0.840. The findings indicate insignificant results between MPV and IPF values in relation to Troponin-I levels in ACS patients.

Abstract

Background: HbA1c is used as a parameter for diagnosis and monitoring of diabetes mellitus (DM). Apart from that, HbA1c, red blood cells also experience changes which result in changes in erythrocyte structure and hemodynamic characteristics. The parameter that changes in this case is red cell distribution width (RDW). This study aimed to analyze correlation between HbA1c levels and RDW in type 2 DM patients. Subjects and Method: This a cross sectional study conducted at H. Adam Malik General Central Hospital, Medan, North Sumatera, Indonesia, from December 2022-April 2023. Total sample of 75 type 2 DM patients was selected consecutively. The dependent variable was RDW. The independent variable was HbA1c. Data of RDW and HbA1c were collected from complete blood count test. Data were analyzed using the Spearman correlation. Results: HbA1c was negatively associated with RDW, but it was statistically non-significant (r= -0.08; p= 0.508). Conclusion: HbA1c is negatively associated with RDW, but it is statistically non-significant.   Keywords: diabetes melitus, HbA1c, red cell distribution width
