Abstract

Stunting merupakan masalah kesehatan anak yang tidak hanya berdampak pada perkembangan fisik anak tetapi juga kognitif, belajar hingga peningkatan risiko kejadian penyakit degeneratif saat dewasa. Deteksi dini stunting merupakan langkah pencegahan stunting yang dapat dilakukan dengan pemantauan pertumbuhan anak oleh ibu dan kader. Oleh sebab itu Kader dan ibu harus mempunyai pengetahuan cukup tentang deteksi dini stunting. Kegiatan pengabdian masyarakat ini bertujuan mengetahui pengaruh penyuluhan singkat tentang deteksi dini Balita stunting dengan melihat kurva KMS dan HAZ pada kader dan Ibu di Kelurahan Muktiharjo Lor, Semarang. Kegiatan intervensi dilakukan dengan ceramah singkat menggunakan peraga gambar dan tanya jawab, diikuti oleh 20 peserta (11 kader dan 9 ibu). Peraga gambar sederhana yang digunakan adalah kurva pertumbuhan KMS (kode A) dan HAZ (kode B), masing-masing diberikan kurva pertumbuhan normal (1), kurang (2) dan buruk (3). Peraga gambar sederhana juga digunakan untuk pre-test dan post-test. Hasil pengukuran pengetahuan sebelum intervensi (pre-test) menunjukkan hanya 30% responden mampu menjawab dengan benar. Dan hasil post-test menunjukkan 75% peserta mampu merepresentasikan grafik KMS dan HAZ dengan benar. Artinya penyuluhan dengan media peraga gambar sederhana mampu meningkatkan pengetahuan peserta terkait deteksi dini stunting Balita. Kegiatan penyuluhan untuk meningkatkan pengetahuan Ibu dan Kader perlu dilakukan secara berkala untuk memaksimalkan peran mereka dalam pencegahan stunting pada Balita.Stunting is a child health problem that not only affects children's physical development but also cognitive, learning and increases the risk of degenerative diseases as adults. Early detection of stunting is a step to prevent stunting in children that can be done by monitoring the growth of children by mothers and health cadres. Therefore cadres and mothers must have sufficient knowledge about early detection of stunting. The purpose of this community service activity is to determine the effect of brief counseling on early detection of stunting toddlers by looking at the picture of the KMS and HAZ curves for cadres and mothers in Muktiharjo Lor Village, Semarang. The intervention activity was carried out with a short lecture using a picture display and a question and answer session, followed by 20 participants (11 cadres and 9 mothers). The simple image display used is the growth curve of KMS (code A) and HAZ (code B), which are given normal (1), less (2) and poor (3) growth curves, respectively. Simple image display was also used for pre-test and post-test. The results of the knowledge measurement before the intervention (pre-test) showed that only 30% of respondents were able to answer correctly. And the post-test results showed 75% of the participants were able to represent the KMS and HAZ charts correctly. This means that counseling using simple picture display media is able to increase participants' knowledge regarding early detection of toddler stunting. Extension activities to increase knowledge of mothers and cadres need to be carried out regularly to maximize their role in preventing stunting in toddlers.

Abstract

Background: Stunting toddlers in Indonesia was still a concern of the government. The Indonesian Ministry of Health noted that the 2020 stunting rate was 27.7% to 24.4% in 2021, while the government's target in 2024 was 14%. Objectives: The purpose of the study was to prove the relationship between Zinc intake and the average height growth of toddlers, in addition to proving the relationship between Zinc intake and stunting.  Methods: A cross-sectional study was conducted on 54 toddlers with a history of births at Sultan Agung Hospital and Bangetayu Health Center in August-November 2017. The average body length growth was the rate of body length increase per month for the first three years of life (cm/month). Data on Zinc intake (mg/day) were obtained from food recall with a food model and analysis of Zinc content in food using the Nutrisurvey application. Test the hypothesis statistically using Spearman, Chi-square, and Fisher's Exact.  Results: Zinc intake was not correlated with mean height growth (p=0.231 r=0.166). Stunting was significantly associated with Zinc intake <3 mg/day, p=0.001. There was a relationship between nutritional status WAZ-score and stunting at three years, p = 0.001. There was a negative correlation between birth length and average height growth. Conclusions: Zinc intake was not associated with the rate of growth in body length in the first three years. Daily Zinc intake was associated with stunting in toddlers.

Abstract

Latar belakang. Air susu ibu (ASI) berperan penting dalam sistem kekebalan innate bayi baru lahir. Kolostrum telah diketahui mengandung antibodi terbesar yaitu sIgA yang berasal dari Entero-broncho-Mammary Pathway. Sekretori IgA kolostrum merupakan imunomodulator perkembangan sistem imun bayi baru lahir. Antibodi pada ASI sering dihubungkan dengan perlindungan bayi terhadap infeksi sampai usia 6 bulan. Masih perlu diketahui peranan sIgA kolostrum terhadap kejadian infeksi Balita.Tujuan. Membuktikan hubungan antara kadar sIgA kolostrum dengan frekuensi infeksi umum dan infeksi saluran pernapasan akut (ISPA) pertahun pada Balita. Mengetahui faktor lain yang berhubungan dengan infeksi Balita secara umum.Metode. Studi prospektif pada 53 Balita usia 3 tahun dengan riwayat kehamilan cukup bulan, lahir sehat beserta ibunya. Pengumpulan ASI pada hari ke 2-3 postpartum, sIgA diukur dengan teknik ELISA. Data didapat dari kuesioner dengan kunjungan rumah atau pertelepon. Analisis statistik menggunakan uji Mann-Whitney, chi-square dan Fisher.Hasil. Rerata frekuensi infeksi umum kelompok riwayat kadar sIgA kolostrum rendah (<120mg/dL) lebih sering dibanding kelompok kontrol (5,9 vs 4,2, p=0,03), demikian juga frekuensi ISPA lebih sering dibanding kontrol (5,2 vs 3,8, p=0,041). Terdapat hubungan antara kepadatan hunian dengan sering infeksi, p=0,01.Kesimpulan. Balita dengan riwayat sIgA kolostrum ibu rendah (<120mg/dL) lebih sering mengalami infeksi saluran pernapasan akut dan infeksi secara umum dibanding kontrol. Kepadatan hunian berhubungan dengan riwayat sering infeksi.

Abstract

Background Secretory IgA (sIgA) content of breastmilk in the first postpartum month is a reflection of the pregnant woman's immune response to environmental antigen exposure. The role of secretory IgA in breastmilk is to protect and support the development of the neonatal immune response in early life. Objective To examine possible factors associated with sIgA levels in breastmilk and colostrum, including environmental exposure, food consumed, maternal history of atopy, and the appearance of allergic symptoms in infants. As a secondary objective, we determined the association between infant factors (IgE, exposure to cigarette smoke) and maternal factors (sIgA, maternal allergies) with infant allergies. Methods This prospective cohort study of 80 postpartum mothers and their infants was conducted at Sultan Agung Islamic Hospital, Semarang. We collected maternal colostrum on the second or third postpartum day and mature milk between the 22nd to 25th postpartum day. Exposure factors to mothers and infants in the final trimester of pregnancy up to one month postpartum was recorded through a questionnaire and home visits. The infant’s IgE level was measured at 4 months of age. Results Mean colostrum sIgA was 108.9 (SD 16.5) mg/dL (95%CI 97.9 to 121.1 mg/dL) and mean mature milk sIgA was 94.1 (SD 23.9) mg/dL (95%CI 89.1 to 99.2 mg/dL). Mean colostrum sIgA levels were higher in mothers exposed to cigarette smoke [119.1 (SD 1.7) vs. 92.9 (SD 1.5) mg/dL; P=0.026] and frequent infections [128.2 (SD 1.7) vs. 95.9 (SD 1.6) mg/dL; P=0.007] compared to that in unexposed mothers. Mean colostrum sIgA was also higher in mothers with atopic allergy than in those without (136.8 mg/dL vs. 99.3 mg/dL; p=0.017) and in mothers of infants with IgE levels >29 IU/ml than in mothers of infants with IgE levels <29 IU/mL (136.8 vs. 101.2 mg/mg/dL; P=0.045). Elevated colostrum sIgA (>136.8 mg/dL) was not associated with allergies in the infants (P=0.269). Conclusions Maternal atopic allergy and frequent infections are associated with increased colostrum sIgA levels. Breastmilk sIgA levels are not associated with allergies in the infant. Maternal exposure to antigens may stimulate the production of specific breastmilk sIgA.

Abstract

Borax is a hepatotoxic substance and induces oxidative stress in hepatocytes. It may trigger the expression of pro-inflammatory cytokines like Interleukin-1 and apoptotic signals like Caspase-3. Extract of the seaweed Eucheuma cottonii contains carotenoids and flavonoids which suppress oxidative stress in hepatocyte. This study was conducted to determine the effect of E. cottonii extract on borax-induced Interleukin-1 and Caspase-3 expression in rat hepatocyte cells. This research employs a posttest-control group design. Twenty Wistar male rats were separated into four groups: control group (K1) received no intervention, K2 received Na-CMC 0.5% for 17 days and Borax 40 mg/200 g on the fourth day every hour for 14 days, P1 and P2groups  were administered E. cottonii extracts at doses of 600 mg/200 g and 1200 mg/200 g for 17 days, respectively, followed byborax induction at a dose of 40 mg/200 g on the fourth day with a one-hour interval for 14 days. Interleukin-1 (IL-1) levels were measured with ELISA, while caspase-3 expression was measured with PCR. For the statistical analysis, Kruskal Wallis and Mann Whitney were used. The level of IL-1 in P2 was significantly lower than in K2 (p<0.05). Caspase-3 expression was significantly lower in P1 and P2 than in K2 (p<0.05). IL-1 level and caspase-3 expression of hepatocytes are affected by administration of E. cottonii extract. The extract has the ability to prevent inflammation and hepatocyte apoptosis.<img Abstract

The prevalence of stunting in Indonesia still exceeds WHO recommended standards. Stunting increases the risk of inhibition of brain growth. Head circumference is one of the parameters of brain growth. The study aimed to prove the mean head circumference growth difference between the stunted and non-stunted groups in the first 3 years of life. Another goal was to prove the relationship between zinc intake and zinc levels in toddlers on head circumference. The cohort study involved 50 subjects who were observed from birth to the age of 3 years and were born at Sultan Agung Islamic Hospital and Bangetayu Health Center Semarang City. The mean head circumference growth in the stunting group was lower than in the non-stunting group (4.1 v.s 4.6), p=0.043. The mean head circumference of stunted toddlers was smaller than that of non-stunted toddlers (44.9 vs. 47.2 ), p=0.000. There was no relationship between daily zinc intake and zinc levels of toddlers with stunting, p0.05. The growth of the head circumference of stunted toddlers was lower than in the non-stunted group, as well as the head circumference was smaller than in the non-stunted group.

Abstract

Background To determine the effect of Apium graveolens L (Celery) extract in the TNF-α and caspase-3 gene expression on the lead poisoning-induced renal injury rats’ model. Methods This study is experimental research with post test only control group design. Twenty male Wistar rats were randomly allocated into four groups (n=5). The control group was orally administrated with distillate water. The second group received lead acetate 200mg/kg body weight orally for 14 days, the third group were orally administered lead acetate 200 mg lead acetate/kg body weight and vitamin E 50IU/kg body weight for 14 days. The fourth group was administrated with leas acetate like second group and celery extract 300mg/kg body weight for 14 days. The TNF-a and caspase-3 gene expression was analyses under qRT-PCR. Results The phytochemical analysis of APE indicated the presence of alkaloids, flavonoids, tannins, saponins, and steroids. Leads acetate increased the TNF-α (3.87±0.09) and caspase-3 (7.95±0.23) gene expression. The celery extract was significantly decrease in the TNF-α (3.13±0.34) and caspase-3 (2.48±1.23) gene expression. Conclusion: In conclusion, the presence of celery extract inhibited the renal injury-induced lead acetate toxicity by inhibition of TNF-α proinflammation protein and caspase-3 proapoptosis protein

Abstract

Pada 2023 masih terdapat 18,8% balita stunting dan pneumonia sebanyak 2,24% di Kota Semarang. Deteksi stunting dapat dicegah dengan deteksi kurva pertumbuhan KMS 2020. Infeksi saluran pernapasan akut yang berulang adalah penyakit yang harus dicegah karena berdampak pada pertumbuhan anak. IDAI perwil kota Semarang dalam rangka hari anak nasional dan ulang tahun IDAI ke 70 bekerjasama dengan Pemerintah kota Semarang mengadakan kegiatan Pengmas Pediatric Social Responsibility (PSR). Tujuan kegiatan ini adalah meningkatkan pemahaman tenaga kesahatan di UPTD Puskesmas Genuk terkait edukasi makanan sapihan guna mencegah stunting, deteksi dini stunting, dan pencegahan pneumonia. Kegiatan pengabdian masyarakat ini dilakukan dengan teknik Ceramah menggunakan media presentasi dan sesi tanya jawab pemateri untuk menilai pemahaman peserta. Penyampaian materi dilakukan selama 60 menit, kemudian dilanjutkan dengan sesi tanya jawab selama 30 menit. Penilaian pemahaman peserta secara subyektif dengan menilai proses pembelajaran tercapai jika ada diskusi interaktif dua arah antara narasumber dan peserta. Hasilnya, peserta mampu melakukan komunikasi dua arah (aktif bertanya dan menjawab) dengan baik mengenai deteksi dini stunting dari kurva pertumbuhan, pencegahan stunting dengan MPASI tepat berbasis pada Protein-lemak Hewani, deteksi dini Pnemonia dan pencegahan penularan pneumonia balita dengan menjaga etika batuk, pemakaian masker, imunisasi dan hindari asap rokok. Kesimpulan: Kegiatan ceramah ini berhasil meningkatkan pemahaman tenaga Kesehatan terkait Stunting-MPASI-Pnemonia, ini dinilai dari terciptanya suasana diskusi intraktif dua arah.In 2023, there still be 18.8% stunted toddlers and 2.24% pneumonia in Semarang City. IDAI Perwil Semarang City in the context of National Children's Day and IDAI's 70th anniversary, in collaboration with the Semarang City Government, held Pediatric Social Responsibility (PSR) community service activities. The aim of this activity was to increase the understanding of health workers at the Genuk Community Health Center UPTD regarding weaning food education to prevent stunting, early detection and prevention of stunting. This community service activity was carried out using a lecture technique using presentation media and a question-and-answer session for the presenters to assess participants' understanding. The material was delivered for 60 minutes, then followed by a 30-minute question and answer session. Subjectively assessing participants' understanding by assessing the learning process is achieved if there is a two-way interactive discussion between the resource person and the participants.The result shows that the participants were able to carry out good two-way communication (actively asking and answering) regarding early detection of stunting from the growth curve, prevention of stunting with appropriate weaning food based on animal protein and fat, early detection of pneumonia and prevention of transmission of pneumonia to toddlers by maintaining cough etiquette, wearing masks, immunizations and avoiding cigarette smoke. Conclusion: This lecture activity was successful in increasing the understanding of Health workers regarding Stunting-MPASI-Pnemonia, this was assessed by creating an atmosphere of interactive, two-way discussion.

Abstract

Continuous exposure to ultraviolet B (UVB) radiation can induce photoaging and accelerate premature skin aging. Turmeric (Curcuma longa) is a medicinal plant that contains curcumin, an active compound known for its anti-inflammatory properties and its role in accelerating wound healing. This study aimed to evaluate the effect of turmeric ethanol extract on TNF-α levels and wound lesion areas in Wistar rats exposed to UVB radiation. A post-test-only control group design was employed, involving 24 Wistar rats divided into four groups: a healthy control group (K1), a negative control group (K2), and two treatment groups receiving oral turmeric ethanol extract at doses of 100 mg/kg BW (K3) and 200 mg/kg BW (K4) for 14 days, with concurrent UVB exposure. TNF-α levels were assessed using one-way ANOVA followed by Tukey's post hoc test, while wound lesion areas were analyzed using the Kruskal–Wallis test followed by the Mann–Whitney U test. The results showed that the highest TNF-α levels were observed in the healthy control group (129.14 ng/L), although no statistically significant differences were found between groups (p = 0.538). In contrast, a significant difference was observed in wound lesion areas (p < 0.001), with the largest lesions occurring in the negative control group. In conclusion, oral administration of turmeric ethanol extract did not significantly affect TNF-α levels but was effective in reducing the area of wound lesions caused by UVB exposure.
