Abstract

Mesenchymal stem cell (MSC) mempunyai kemampuan immunoregulasi dan regenerasi melalui supresi pelepasan mediator proinflamasidan peningkatan molekul proliferasi terutama vascular endothelial growth factor (VEGF) dan platelet-derived growth factor (PDGF). Mesenchymal stem cell yang diaktivasi TNF-? dengan dosis tertentu mampu meningkatkan sekresi VEGF dan PDGF, namun dosis optimal TNF-? yang mampu memaksimalkan ekspresi molekul tersebut belum diketahui secara pasti. Variasi dosis TNF-? digunakan pada penelitian ini dengan tujuan mengetahui dosis optimal, rendah, dan tinggi TNF-? dalam memaksimalkan ekspresi VEGF dan PDGF. Penelitian ini mengunakan post-test only control group design dengan 5 kelompok penelitian, terdiri atas satu kelompok kontrol (K) dan 4 kelompok perlakuan (P) (TNF-?= 5, 10, 40, 80 ng/mL) yang diinduksikan pada MSC dengan inkubasi 24 jam, kemudian kadar PDGF dan VEGF diukur dengan metode ELISA. Penelitian ini dilakukan antara bulan September?November 2017 di Laboratorium Stem Cell and Cancer Research (SCCR), Fakultas Kedokteran, Universitas Islam Sultan Agung, Semarang. Hasil penelitian menunjukkan peningkatan kadar PDGF dan VEGF secara signifikan (p<0,05) dimulai dari dosis TNF-? 5 ng/mL, optimal padadosis 10 ng/mL dan mulai terjadi penurunan pada dosis 40 ng/mL. Induksi TNF-? pada MSC mampu memaksimalkan kadar VEGF dan PDGF pada dosis 10 ng/mL.Kata kunci: MSC, PDGF, TNF-?, VEGF Effect of TNF-? Serial Doses Inducition on Increasing VEGF dan PDGF in Mesenchymal Stem Cells Mesenchymal Stem Cells (MSCs) have immunoregulation and regeneration capabilities through suppression of proinflammatory mediator release and increase of proliferative molecules, particularly the vascular endothelial growth factor (VEGF) and platelet-derived growth factor (PDGF) TNF-? activated MSC in a certain dose has the ability to increase VEGF and PDGF levels; however, the exact optimum dose of TNF-? to optimize the levels of VEGF and PDGF is unclear. In this study, TNF-? dose variations were used to determine the optimum, low, and high doses of TNF-? in optimizing VEGF and PDGF expression. This was a post-test only control group study with five study groups consisting of one control (K) and four treatment groups(P). The treatment groups were treated with 5, 10, 40 and 80 ng/mL of TNF-? for 24 hours. PDGF and VEGF levels were measured using ELISA. This study was conducted between September?November 2017 at the Stem Cell and Cancer Research Laboratory (SCCR), Faculty of Medicine, Sultan Agung Islamic University, Semarang. The results show significant increased in PDGF and VEGF levels (p<0.05) starting from TNF-? 5 ng/mL as the initiation dose to 10 ng/mL as the optimum dose and reduction was seen starting from 40 ng/mL dose. TNF-induced MSCs have the ability to increase the VEGF and PDGF levels with an optimum dose of 10 ng/mL.Key words: MSC, PDGF, TNF-?, VEGF

Abstract

BackgroundPeritoneal adhesions (PAs) are generally described as fibrous bands between intra-abdominal organs following an abdominal surgical operation. The definitive treatments of PAs are currently ineffective yet. Hypoxia-mesenchymal stem cells (H-MSCs) have a higher capability to survive at the site of injury than normoxia-MSCs (N-MSCs) to repair injured tissue without fibrosis. This study aimed to analyze the effect of H-MSCs in controlling formation of PAs by reducing TGF-β level in a rat model. Methods A study of post-test only control group design was conducted, involving eighteen PA rat models weighing 250 ± 25 g that were randomly assigned into 3 groups, comprising control group (C), and groups T1 and T2 receiving H-MSC treatment at doses of 3 x 106 and 1.5 x 106, respectively. To induce H-MSCs, MSCs were incubated in hypoxic conditions at 5% O2 and 37oC for 24 hours. Expression level of TGF-β was analyzed by enzyme-linked immunosorbent assay (ELISA) at 450 nm and adhesion formation was described macroscopically. The Kruskal-Wallis variance analysis was used to analyze significant differences among the groups.ResultsThe results of this study showed that H-MSCs in group T1 inhibited TGF-β expression significantly on day 8 (p&lt;0.001) and day 14 (p&lt;0.05). Moreover, there was almost no adhesion apparent following H-MSC administration in group T1. ConclusionsBased on this study, we conclude that H-MSCs may attenuate PA formation following inhibition of TGF-β expression in the PA rat model.

Abstract

English has been integrated into medical curriculum in higher education in countries where English is not the official language of instruction. For medical students—non-English department students—English has been taught to meet specified academic and professional needs of learners in so-called English for specific purposes (ESP). To ensure that English program is relevant to the learner’s needs, a need analysis is required. This study aimed to investigate the English needs of the first-year medical students taking a compulsory program of English for academic purposes at Faculty of Medicine, Sultan Agung Islamic University. The data were collected using a questionnaire to assess the medical students’ purpose of learning English, the importance of learning English, language learning needs of major language skills (reading, writing, listening, speaking) and their preference of assessment type. The data were descriptively analyzed. Forty-five students consisting of 67 % female and 33% male completed the questionnaire. Most students (76%) used English when studying. All students agreed on the importance of English. The most important sub skill included reading technical article in medicine, listening to audio and listening oral presentation, giving spoken presentation, writing medical prescription. Individual achievement was the most preferred type of assessment. The medical students agreed on the importance of English specific purposes. The interpretation of findings will be useful for the design of English for specific purposes in the study setting.

Abstract

From 2011 to 2014, there were six cases of suspected medical malpractice in the Central Java Region; four clinical cases were resolved by an ethics board and two by a public court, so that it serves as a reference for clinical decision-making for physicians, as many ethical problems in addition to legal problems exist in medicine. Doctors, as Indonesian citizens, are obligated to implement the norms and laws that apply in Indonesia. However, as professionals, they are also obligated to implement the norms that apply in medical professional organisations (Indonesian Doctors Association/IDI), including the Indonesian Doctors Association (IDI), the Code of Medical Ethics (KODEKI), and interprofessional ethics. So that the actions of physicians can be accounted for from both a legal and an ethical standpoint of the medical profession. Every clinical case a physician encounters has inextricable ethical and medical dimensions, necessitating a prudent method for making clinical decisions. The challenge is to comprehend the value hierarchy, beginning with norms, morals, principles, standards, and rules. Therefore, when making clinical decisions, physicians are truly guided by the order of applicable values, one of which is Jonsen-4-box Siegler’s method. The culture of the Indonesian people includes a 4-box method as an alternative. Schematics can be made for the order of values and ways of making clinical decisions based on wise ethical decisions in resolving ethical dilemmas and medical disputes.

Abstract

Background: Gastric disorder often generates clinical symptoms such as gastritis or peptic ulcer. Bee honey has high nutrient has been shown to have a protective effect against drugs destroying stomach. The purpose of this study was the difference effect of honey in various concentrations on the gastrohistopathological image in male white Wistar rat induced with indomethacin. Design and Methods: This was an experimental research using control group post test design. 24 male white rats Wistar were randomly divided into 6 groups of 4 mice each. Group I served as the control group (standar and aquades), group II: 3.78 mg of indometachin, group III: 3.78 indomethacin and 3.6 ml honey (25%,) group IV: 3.78 indomethacin 3.6 ml honey (50%,) group V: 3.78 mg indomethacin 3.78 mg and 3.6 ml (75%(), group VI: 3.78 indomethacin 3,78 mg and honey 3.6 ml 100%. Treatment given by sonde once a day for 15 days. the gastrohistopathological image was evaluated for degree of gastritis and degree of peptic ulcer. The data were tested with test Kruskal-Wallis followed by Mann-Whitney test. Results: Kruskal-Wallis test showed at least significant differences in the degree of gastritis and peptic ulcer between between the two groups (p <0.05). Mann-Whitney test howed that not all groups compared to control showed a significant different in the degrees of gastritis and peptic ulcer. Conclusion: The study suggested that honey have effect on the gastrohistopathological image of indomethacin-induced stomach (Sains Medika, 3(1):41-47).

Abstract

Visum et repertum (VER) is part of forensic and medico-legal science. VER is a written report by a sworn doctor and has the authority of what he/shehas seen and and found in the evidence presented and conclude the evaluation for the trial process in the investigation to decide the suspect, for the basis ofprosecution, prove that an accused person is guilty, at the level of investigation, the request of VER is the authority of investigator of POLRI (The IndonesianNational Police). VER is prepared by a doctor with a special expertise under oath or medical professional and legal responsibilities and liabilities. Therehas been dualism concerning to the evidence of expert witness. First, an expert witness provides a sworn testimony in the trial as it is explicitly mentionedin article 184 of KUHAP (Code of Criminal Procedure). Second, VER is part of legal means of proof of expert testimony given by a legal medical doctoras stated in 184 KUHAP. The expert testimony in form of VER can be legal means of proof which can be used before the court. It is, however, not alwaysnecessary for prosecutor to use it for the adjudicating criminal case. However, criminal case related to human body, it was found that the prosecutorsmake the judgment based on the VER. The autopsy data of RS Bhayangkara in January to October 2012 showed that there were more male victim (58)compared to female victim (51) .The most common cause of death was persecution (91 ). The most common wound was bruise.

Abstract

In general highly corrosive acids or bases injury can occur due to deliberate or unintentional, such as workplace-related accidence, vocational negligence, unintentional child corrosive poisonings. Acids are considered to be corrosive when it has a high concentration, are irritants at moderate concentrations, and stimulant at low concentrations. This is a report of a strong acid poisioning case of a 60 year-old- man found dead in the middle of a rice field in Demak in September 2015. The victim was one of the farmers protesting the illegal dredging project. At the crime scene, a hoe and a bottle made of “plastic” (used fertilizer container, classified as moderate acid) lying beside the deceased, giving an impression that he commited suicide by consuming the liquid. External examination revealed: burn marks are red brown or black, dry and hard on the eyelids, nostrils, lips, tongue, neck and chest. The results of the examination showed: 1. Mucosal irritation, with a bright red and brownish red, accompanied by ulceration. It was also found irritation in tongue, larynx and edemous glottis and esophagus. 2. Pseudomembranous inflammation of the trachea and bronchi which resulted in superficial epithelial damage and necrosis affect submucosal layer. 3. Brain edema due to anoxia. The forensic examination confirms that the cause of death was homicide.

Abstract

A Cigarette is one of the free oxygen radicals that interfere with a free radical’s balance and antioxidant in the body. It involves oxidative stress. Oxidative stress involves a failure system that triggers an inflammation response in endothelial class blood vessels that have pinned exiting the inflammation reaction in the form of cytokines, especially TNF-α. It functioned to know the effect of awarding avocado juice to MDA and TNF-α. This research used the experimental laboratory method that approached a post-test-only control group design. The subject of this research used 20 Wister rats, which kind of inclusion. It is shared in 4 sections as random services. Such as K1, K2, K3 and K4. K1 has given a standard weft. It has been done without a smoking display. Meanwhile, K2 has given a standard weft that has used a smoking display for 14 days. K3 and K4 have been given avocado juice doses. It was as much as 2,7g/200g weight/day and 54g/200g weight/day. On the fifth day, the specimen has taken the blood. It could know the results of MDA and Tnf-a. The data analysis used the normality test by Shapiro Wilk, homogeneity, Levene, and one-way ANOVA test. The result of this research has the highest rate. The highest rate was K2. It could be proven for the One-Way Anova MDA and Tnf rate test. It showed a significant difference between categories, which is p=0,000 scores. The conclusion of this research has been giving avocado juice with 2,7g/200g weight/day and 5,4g/200g weight/day. Then, it could reduce the MDA and Tnf-a rate on the Wistar rat. It has been since the smoking display.

Abstract

Latar belakang: Angka kasus COVID-19 hingga saat ini terus mengalami peningkatan yang cukup signifikan di Indonesia. Masyarakat pun terus dihimbau untuk tetap berada di dalam rumah untuk memutus rantai penyebaran COVID-19. Agar tetap aman saat harus pergi keluar rumah, Kementerian Kesehatan membuat sebuah protokol kesehatan sebagai solusi, salah satunya melalui gerakan Perilaku Hidup Bersih dan Sehat (PHBS). Lansia di panti wredha Harapan Ibu sebagai salah satu kelompok rentan COVID-19 yang perlu mendapatkan penguatan edukasi akan protokol kesehatan. Tujuan: Meningkatkan pengetahuan mengenai protokol kesehatan agar masyarakat lansia di lokasi kegiatan tetap dapat beraktivitas secara aman dan tidak membahayakan keamanan atau kesehatan orang lain. Metode: Metode yang digunakan adalah penyuluhan protokol kesehatan berupa PHBS kepada ibu-ibu di Panti Wredha Harapan Ibu Ngaliyan, tanya jawab, dan konsultasi kesehatan disertai pengecekan laboratorium sederhana (gula, kolesterol, dan asam urat). Hasil: Terdapat peningkatan pengetahuan dari 27 % peserta menjadi 90 % peserta yang dapat menjawab pertanyaan kuesioner dengan benar dan dapat mempraktekkan dengan benar teknik cuci tangan. Kesimpulan: Penyuluhan protokol kesehatan mampu meningkatkan pengetahuan lansia untuk mencegah penularan COVID-19dan meningkatkan derajat kesehatan lansia. Kata Kunci: COVID-19, lansia, panti wredha, PHBS, tanggap pandemi ___________________________________________________________________________________________ Abstract Background: Until now, the number of COVID-19 cases continues to experience a significant increase in Indonesia. The public continues to be advised to stay indoors to break the chain of transmission of COVID-19. To stay safe when you have to leave the house, the Ministry of Health has developed a health protocol as a solution, one of which is through the Clean and Healthy Behavior Movement (PHBS). The elderly in the Harapan Ibu nursing home is one of the vulnerable groups for COVID-19 who need to receive strengthening education on health protocols. Objective: The purpose of this activity is to increase knowledge and change health behavior according to health protocols so that the elderly at the activity location can still carry out their activities safely and not endanger the safety or health of others. Method: The method used was counseling on health protocols in the form of PHBS for mothers at the Ngaliyan Nursing Home, questions and answers, and health consultations accompanied by simple laboratory tests (sugar, cholesterol, and uric acid). Result: There was an increase in knowledge from 27% of participants to 90% of participants who were able to answer the questionnaire questions correctly and were able to properly practice hand washing techniques. Conclusion: Counseling is able to increase the knowledge of the elderly in terms of PHBS to prevent transmission of COVID-19 and improve the health status of the elderly. Abstract

The aims of this research is to prove the effect of bay leaf extract gel on IL-10 and IL-6 gene expression in traumatic ulcers. The research method using In vivo experimental study with a randomized posttest-only control group design. The total sample was 24 male Wistar rats, divided into four groups. Group K1 (given base gel), group K2 (given Kenalog in Orabase), group K3 (10% bay leaf extract gel), and group K4 (15% bay leaf extract gel) were given treatment for 5 days. IL-6 and IL-10 levels were tested using the One-Way Anova test, followed by the Tamhane Post Hoc test. This study revealed that there was a difference in IL-6 levels in the K4 group compared to the K1 group sig. (p = 0.007) and a significant difference in IL-10 levels in the K4 group compared to the K1 group. (p = 0.003) after being given topical treatment with bay leaf extract gel. The conclusion of this research is treatment with 15% bay leaf gel extract (Syzygium polyanthum) was proven to significantly increase IL-10 gene expression and decrease IL-6 gene expression in Wistar rats model of traumatic ulcers.
