Abstract

Medication adherence is the key to controlling blood pressure in hypertensive patients to minimize the risk of complications. The research aims to assess level of medication adherence and identify influencing factors. The quasi-experimental research method used purposive sampling to create a control group and 2 intervention groups (pill box and medication chart) with each group 30 patients. Prospective data collection with pre-post design (week-1 and week-4) as well as observing clinical data on blood pressure, medication adherence. Questionnaire of medication non-adherence determinants was taken at week 4. The questionnaire instrument was validated with Cronbach Alpha reliability >0.06. The results of the statistical analysis were significant (p-value <0.05) with increasing the mean score of medication adherence in the pill box was 1.2 and the medication chart was 0.7. The reduction in systolic blood pressure in the intervention group was significant (p-value <0.05) by more than 2 mmHg. A total of 35.6% of patients were reluctant to ask health workers for medication information and 45.6% needed someone to remind them to take medication. In conclusion, increasing medication adherence and reducing blood pressure can be achieved with intervention and requires social support from family and health-care professional.

Abstract

TB (Tuberculosis) patients in Indonesia are the 2nd highest in the world after India. The large number of TB patients in Indonesia is due to the lack of success in TB treatment due to patient non-compliance with medication regularly. Factors that affect non-compliance are low knowledge of TB patients and lack of family support. This study aims to determine the level of family knowledge and support, medication adherence, and determine the relationship between the level of knowledge and family support to medication adherence in TB patients at the Kroya Health Center, Cilacap Regency in 2024. This study uses a non-experimental method with cross-sectional data collection conducted in April-May 2024. The study population is all TB patients totaling 130 respondents who underwent treatment at the Kroya Health Center, Cilacap Regency from November 2023 to May 2024, and the research sample was 56 respondents. The conclusion of this study is that the knowledge level of TB respondents is the highest at 37.5% and 55.4%, and the adherence to taking medication at the Kroya Health Center is 30.4% and 57.1%. In this study, the highest TB patients were at a moderate level of knowledge (55.4%); family support is not supportive (96.4%) and medication adherence is low (30.4%) to moderate (57.1%). Testing with the chi-square method there was a relationship between the level of knowledge and medication adherence in TB patients (p-value 0.000), but there was no relationship between family support and medication adherence (p-value 0.268) in patients at the Kroya Health Center, Cilacap Regency with a r value of 0.541. The limitation of this study is that there is no follow-up on the respondents such as monitoring TB drugs so that the existing data is only data taken when filling out the questionnaire.

Abstract

Medication adherence is a crucial factor in the successful management of type 2 diabetes mellitus (T2DM), particularly among older adults who are more vulnerable to treatment failure due to age-related physiological decline and multiple comorbidities. This study aimed to determine the level of medication adherence and analyze the factors associated with medication adherence behavior among older adults with type 2 diabetes mellitus at Kembaran 1 Primary Health Center. This quantitative study employed a cross-sectional design involving 62 older adults with type 2 diabetes mellitus selected using a total sampling technique. Medication adherence was assessed using the Medication Adherence Rating Scale-10 (MARS-10), while the associations between variables were analyzed using the Chi-square test. The results showed that 64.5% of the respondents had a high level of medication adherence, whereas 35.5% demonstrated low adherence. Bivariate analysis revealed that age (p = 0.008), educational level (p = 0.021), employment status (p = 0.013), body mass index (BMI) (p = 0.015), and type of antidiabetic therapy (p = 0.001) were significantly associated with medication adherence behavior. In contrast, sex (p = 0.675), duration of diabetes mellitus (p = 0.510), and the presence of comorbidities (p = 0.519) were not significantly associated with medication adherence. These findings indicate that medication adherence among older adults with type 2 diabetes mellitus is influenced by sociodemographic factors, clinical characteristics, and treatment-related factors. Identifying these determinants may serve as a basis for developing intervention strategies to improve medication adherence and optimize therapeutic outcomes.

Abstract

Pendahuluan: Infeksi saluran pernapasan akut (ISPA) merupakan salah satu penyebab utama rawat inap dan penggunaan antibiotik di seluruh dunia serta berkontribusi terhadap meningkatnya resistensi antimikroba. Pemantauan penggunaan antibiotik sangat penting untuk mendukung penggunaan antibiotik yang rasional dan memperkuat program antimicrobial stewardship. Tujuan: Penelitian ini bertujuan untuk mengevaluasi kuantitas dan pola penggunaan antibiotik pada pasien rawat inap dengan ISPA dengan metode Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD) dan Drug Utilization 90% (DU90%). Metode: Penelitian deskriptif retrospektif ini menggunakan data rekam medis pasien dewasa yang didiagnosis ISPA (kode ICD-10 J00–J06 dan J20–J22) di RSUD Prof. Dr. Margono Soekarjo, Indonesia, selama periode 2021–2025. Penggunaan antibiotik dihitung sebagai DDD/100 patient-days berdasarkan metodologi WHO ATC/DDD, sedangkan pola peresepan dianalisis menggunakan metode DU90%. Hasil: Total 237 pasien memenuhi kriteria inklusi dengan total konsumsi antibiotik sebesar 502,49 DDD/100 patient-days dengan penggunaan tahunan berkisar antara 81,13–120,51 DDD/100 patient-days. Antibiotik yang paling banyak digunakan adalah sefiksim, seftriakson, azitromisin, levofloksasin, dan seftazidim. Analisis DU90% menunjukkan bahwa tujuh jenis antibiotik menyumbang sekitar 90% dari total penggunaan, yang didominasi oleh antibiotik kategori Watch menurut klasifikasi WHO AWaRe. Kesimpulan: Penggunaan antibiotik pada pasien rawat inap dengan ISPA didominasi oleh antibiotik spektrum luas kategori Watch. Surveilans penggunaan antibiotik secara berkelanjutan menggunakan metode ATC/DDD dan DU90% diperlukan untuk mendukung program antimicrobial stewardship serta mengoptimalkan penggunaan antibiotik di rumah sakit.

Abstract

Background: Human Papillomavirus (HPV) is the primary cause of cervical cancer, making HPV vaccination a key strategy for cervical cancer prevention. As social media has become an increasingly important source of health information, evaluating the quality and credibility of HPV vaccine-related content on digital platforms is essential. Objective: This study aimed to analyze the characteristics, quality, credibility, understandability, and educational value of HPV vaccine-related videos on TikTok and YouTube Shorts in Indonesia. Method: A cross-sectional content analysis was conducted on 200 publicly accessible videos, comprising 100 TikTok videos and 100 YouTube Shorts videos identified using the keyword “HPV vaccine.” Video characteristics, uploader categories, content topics, and presentation styles were analyzed descriptively, while information quality was assessed using the Journal of the American Medical Association (JAMA) benchmark criteria, Global Quality Score (GQS), modified DISCERN (mDISCERN), and the Patient Education Materials Assessment Tool (PEMAT). Result: TikTok videos generated substantially higher audience engagement than YouTube Shorts, whereas YouTube Shorts demonstrated greater information credibility and reliability based on higher JAMA and mDISCERN scores. Vaccination targets, vaccination schedules, and vaccine types were the most frequently discussed topics, while vaccine mechanisms, side effects, and misinformation were addressed less frequently. Both platforms demonstrated high educational quality, with a mean GQS score of 4.43 and PEMAT-T scores exceeding 98%, indicating excellent understandability and actionability. However, source attribution and scientific referencing remained limited across both platforms. Conclusion: TikTok and YouTube Shorts have considerable potential as effective channels for HPV vaccine communication in Indonesia. Strengthening evidence-based communication practices and improving information transparency are essential to enhance public trust, increase vaccine confidence, and support cervical cancer prevention efforts.

Abstract

Mangosteen (Garcinia mangostana L.) is known to contain various bioactive compounds such as xanthones, flavonoids, and tannins which have great potential as herbal medicine, although its safety aspect still requires further research. This study aims to determine the LD₅₀ value and assess the effect of administering ethanol extract of mangosteen leaves on liver function of male white rats of the Wistar strain using SGOT and SGPT enzyme parameters. Acute toxicity tests were conducted based on the Acute Toxic Class (OECD 423) method by administering the extract orally to two dose groups, namely 300 mg/kgBW and 2000 mg/kgBW, as well as a control group using 1% Na-CMC. The test results showed that at a dose of 300 mg/kgBW no mortality was found in the experimental animals (0%), while at a dose of 2000 mg/kgBW mortality was recorded at 60% (2 out of 3 test animals). The LD₅₀ value cannot be determined precisely due to limited dose variation, but it can be estimated to be in the range of 300–2000 mg/kgBW, in accordance with OECD guidelines that allow reporting of LD₅₀ values ​​in interval form if data are incomplete. Furthermore, liver function tests showed an increase in liver enzyme levels in the 2000 mg/kgBW dose group, where SGOT increased from 42.25 U/L to 74.25 U/L, while SGPT increased from 35.25 U/L to 52.25 U/L, although these values ​​are still within the normal physiological range. Thus, mangosteen leaf extract shows potential toxicity at high doses, so its use as a herbal medicine still requires further study.
