Abstract

HIGHLIGHTS 1. Acquired uterine arteriovenous malformation (AVM) is an uncommon sequela of cesarean section, warranting consideration in instances of persistent uterine bleeding in the puerperium.2. Embolization represents a viable and well-established treatment modality for AVM, providing a safe and efficacious intervention that serves as an alternative, less invasive modality for patients desiring fertility preservation.   ABSTRACT Objectives: To demonstrate that embolization is a viable and well-established treatment for acquired arteriovenous malformations (AVMs), offering a safe and effective, less invasive option for patients seeking to preserve fertility.Case Report: A 20-year-old female experiencing recurrent massive bleeding with a history of previous caesarean section and wound infection resuturing. The transvaginal color Doppler ultrasound showed a hypervascularity in the uterus around the surgical lesion, suggesting a uterine AVM and its confirmed by angiography. The patient underwent a series of uterine artery embolization interventions, which proved to be a successful therapeutic approach. As a result of this treatment modality, not only was the patient's condition effectively resolved, but her fertility was also preserved. Conclusion: Acquired uterine arteriovenous malformation (AVM) is an uncommon sequela of cesarean section, warranting consideration in instances of persistent uterine bleeding in the puerperium. Embolization represents a viable and well-established treatment modality for AVM, providing a safe and efficacious intervention that serves as an alternative, less invasive modality for patients desiring fertility preservation.

Abstract

This study reviews a range of non-hormonal pharmacological interventions used to treat vasomotor symptoms in postmenopausal women, including SSRIs, SNRIs, gabapentin, and neurokinin-3 receptor (NK3R) antagonists. Methods used included a literature review of multiple clinical trials and meta-analyses published over the past decade. Results showed that all interventions were effective in reducing the frequency and severity of hot flashes, with efficacy rates ranging from 36 to 72%. In addition, the side effect profile was generally mild to moderate, including nausea, sleep disturbance, and headache. NK3R antagonists, such as fezolinetant, have shown great promise with a rapid onset of action and comparable efficacy to hormone therapy, but longer-term studies are needed to assess safety. This study emphasizes the importance of an individualized approach to therapy selection and the need for monitoring liver function and potential side effects. Conclusions suggest that these non-hormonal interventions are a promising alternative for postmenopausal women for whom hormone therapy is contraindicated.

Abstract

Objective: analyzed the relationship between NGF mRNA expression and pain levels and severity of endometriosis. Methods: This research involved 30 endometriosis patients undergoing surgery from November 2024 to April 2025. NGF mRNA expression was measured in blood plasma and ectopic tissue. Pain history was assessed retrospectively, and disease stage determined intraoperatively. Results: No significant relationship was found between NGF mRNA expression and pain levels or severity of endometriosis in either plasma or lesion tissue. Based on the Livak method analysis, the average value of relative expression of NGF gene mRNA increased 6.943-fold in blood plasma and 17.319-fold in endometriosis lesions of women with endometriosis. In the Mann Whitney test, there was a significant difference in the level of NGF gene mRNA expression between blood plasma and with p < 0.0001. NGF expression may not always correlate with disease severity because it is influenced by many factors. Conclusion: No significant correlation was found between NGF mRNA expression and endometriosis severity or pain intensity, but expression levels differed significantly between plasma and lesion tissue.

Abstract

Objective: To assess the correlation between vitamin D levels, dysmenorrhea intensity measured by the visual analogue scale (VAS), and the stage of endometriosis determined by the American Society of Reproductive Medicine (ASRM) grading score. Methods: A cross-sectional study was conducted involving 37 women diagnosed with suspected endometriosis who met the inclusion and exclusion criteria. The aim was to determine the correlation between vitamin D levels, dysmenorrhea VAS scores, and the ASRM endometriosis stage at RSUP Dr. Mohammad Hoesin Palembang from November 2021 to April 2022. Bivariate analysis was employed to assess correlation, utilizing Pearson's correlation test and the Spearman Rank correlation test as an alternative method. Results: There was a significant positive correlation between vitamin D levels and the VAS score for dysmenorrhea (r = 0.678; p = 0.000) and a very strong positive correlation between vitamin D levels and the degree of endometriosis (r = 0.774; p = 0.000) based on Spearman Rho's correlation test. Conclusion: There is a significant relationship between vitamin D levels with the VAS score of dysmenorrhea and the degree of endometriosis ASRM. Abstract

Introduction: Increasing amount of research indicates that women with PCOS are more likely to have a shortage in vitamin D with abnormally high anti-Müllerian hormone (AMH) levels. This study aims to assess the correlation between vitamin D and Anti-Mullerian Hormone (AMH) levels in infertile women with polycystic ovarian syndrome (PCOS) and obesity. Method: This observational cross-sectional study examined all obese women with PCOS who sought infertility treatment at Dr. Mohammad Hoesin Hospital Palembang from August 2021 to November 2021. PCOS diagnosis was determined using the Rotterdam criteria. All research subjects underwent a physical examination and transvaginal ultrasound to evaluate body mass index (BMI) and polycystic ovary morphology. Blood samples were collected to evaluate their testosterone, AMH, and vitamin D levels. Statistical analysis was performed using the Spearman correlation test with IBM® SPSS Statistics version 24. Results: Thirty infertile women with PCOS and obesity were included in the study, with average age of 31.17±4.48 years and average BMI of 33.45±2.67 kg/m2. The majority of the subjects had polycystic ovary morphology (86.7%) and oligomenorrhea (76.7%). No correlation between BMI and vitamin D as well as AMH levels was found in infertile women with PCOS and obesity. The Spearman test showed moderate negative correlation (p = 0.011. r = -0.458) between vitamin D and AMH levels in infertile women with PCOS and obesity. Conclusion: There is a moderate negative correlation between vitamin D and AMH levels among infertile women with PCOS and obesity. Further research is required to comprehend the role of vitamin D in female fertility.
