Abstract

Depression usually occurs in the elderly with primary hypertension. In people with hypertension, depression causes a decrease in health status, quality of life and social role function. Hypertension has become a public health problem that has continued to increase in the last 20 years. Data from the World Health Organization (WHO) in 2015, around 1.13 billion people worldwide have hypertension (Pradono, 2020). In Indonesia, there were 63,309,620 cases of hypertension and 427,218 deaths due to hypertension. The prevalence of hypertension in South Sumatra Province is 35.8%, with 1432 cases in 2019 to 3981 cases and 4631 cases in 2020. Data on elderly people with hypertension and depression in 2024 in the Ariodillah Health Center work area is still high, which is around 70% of the total elderly (Puskesmas Ariodillah, 2024). Combination interventions of psychotherapy and pharmacotherapy in individuals, families and groups can help reduce symptoms of depression, especially in the elderly. This is reinforced by research (Tobing & Novianti, 2019) that the intervention group with supportive group therapy experienced a significant decrease in hypertensive stress levels. The solutions to the problems offered in this community service activity are in the form of; Coordination of the implementation of activities including preparation of the place of implementation, time and number of participants who can attend the activity. Preparation of supportive therapy modules, leaflets and blood pressure measuring devices; Implementation; Activities in the form of supportive therapy training are carried out in 4 sessions, namely; The first session, carried out with the aim of exploring the problems of the elderly and identifying the support systems that the elderly have. The second session, the use of the support system identified in session 1 owned by the elderly, monitoring its use and obstacles. In the second session of this supportive therapy, the servant and the elderly discuss the support system that exists within the elderly to solve problems and how to use it. The third session, using the support system that exists outside the elderly. This session identifies the support system that exists outside the elderly and teaches how to use it. The fourth session, evaluates how the elderly use the support system and the obstacles in using the support system. In this session, the elderly are asked to convey all the sources of support they have and convey the obstacles or barriers they face. At the end of the session, the servant evaluates and follows up with the elderly on the internal and external sources of support that they have and have practiced. Termination is the final stage of each session of this activity. The activities carried out are to evaluate by asking the elderly how they feel after gaining knowledge and skills to overcome emotional mental disorders. Based on the target achievements and results of the activities, it can be concluded that after the intervention was given, the results of the activities showed that; a) 39% of the elderly still experienced hypertension with moderate depression and 61% of the elderly were in a normal state (no longer depressed); b) Non-pharmacological therapy (Group Supportive Therapy) is used as routine therapy for the elderly at the Elderly Posyandu in the Ariodillah Health Center working area; c) 100% of the elderly have been educated and accompanied regarding the use of supportive group therapy and are required to apply it as daily therapy at home. The follow-up to this activity is that health cadres will accompany all heads of families in stages to apply supportive group therapy in their respective homes and motivate the elderly to routinely participate in activities at the Elderly Posyandu.

Abstract

ABSTRACT Chronic pain in older adults with Rheumatoid Arthritis (RA) is a significant health problem, often leading to impairments in daily activities, anxiety, and a decreased quality of life. This condition necessitates accessible and effective non-pharmacological interventions to address pain in the older adult population. This case study aims to implement and analyze the results of applying Virtual Reality Guided Imagery (SI-VIGUR) therapy as a nursing intervention to reduce chronic pain in older adult patients with Rheumatoid Arthritis at Tresna Werda Nursing Home, Lubuklinggau City in 2025. This case study involved two older adult individuals (Ny. S and Ny. A) suffering from Rheumatoid Arthritis with complaints of chronic pain. The SI-VIGUR intervention was implemented for 3 consecutive days for each subject. Evaluation was conducted daily using pain level indicators from SLKI (Standard for Nursing Outcome Indicators, Indonesia) on a 1-5 scale (Increased to Improved), which included Protective Behavior, Pain Complaints, and Restlessness, as well as the Numeric Pain Scale (0-10). The implementation of SI-VIGUR consistently showed significant improvement across all indicators of chronic pain in both subjects. For Ny. S, the pain scale decreased from 6 at the beginning of the intervention to 2 on the third day, accompanied by improvements in Protective Behavior, Pain Complaints, and Restlessness, which showed progress from 'Initial Assessment' values to 'Improved' at the end of the intervention. Ny. A also showed a similar pattern of improvement, with the pain scale decreasing from 6 to 3 on the third day, although it was stagnant on the second day, and SLKI indicators also showed improvement. Both subjects reported increased relaxation and improved sleep quality. The application of Virtual Reality Guided Imagery (SI-VIGUR) is an effective, easily applicable, and highly potent non-pharmacological nursing intervention for reducing chronic pain in older adult patients with Rheumatoid Arthritis. The success of this intervention highlights the role of technology in supporting complementary therapies to improve the quality of life of the older adults. Keywords: Chronic Pain, Older Adults, Rheumatoid Arthritis, Virtual Reality Guided Imagery (SI-VIGUR). Abstract

Gout arthritis is an inflammatory joint disease often causing severe pain due to the accumulation of uric acid crystals in the joints. Typically, it results in swelling, redness, and pain in one or multiple joints, particularly in the big toe. This condition is associated with high levels of uric acid in the body, triggering harrowing gout attacks. Thus, physical activities and health education are initial steps in addressing pain complaints and elevations in uric acid levels among older adults and individuals with gout arthritis. This involves providing ergonomic exercise training through lectures, simulations, and practice sessions. The results show a reduction in pain intensity among older adults before and after the activity conducted over a month with four meetings. Furthermore, there is an expectation for the government and stakeholders to introduce scheduled and community-based structured activities as an initial step in addressing health issues, particularly among older adults.

Abstract

Sleep disturbances are common among older adults, especially those residing in nursing homes, and can significantly affect their quality of life and overall well-being. Non-pharmacological interventions, such as guided imagery and virtual reality (VR), have shown promise in improving sleep quality by promoting relaxation and reducing cognitive arousal. The SI-VIGUR (Virtual Reality Guided Imagery) app combines VR technology with guided imagery to offer a therapeutic solution for improving sleep in older adults. This study aimed to evaluate the effectiveness of the SI-VIGUR VR app in improving sleep quality among older adults in nursing homes. This study aimed to evaluate the effectiveness of the Virtual Reality Guided Imagery (SI-VIGUR) application in improving sleep quality among older adults in nursing homes in Palembang City, South Sumatra, Indonesia, in 2024. Using a quasi-experimental design with pre- and post-tests, 80 participants aged 60 or older with sleep disturbances were selected. They received 15-minute SI-VIGUR sessions three times a week for 1 months. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI). Participants in the experimental group showed a significant improvement in sleep quality, as evidenced by a decrease in their PSQI scores (from 14.20 ± 1.86 to 7.31 ± 2.81). The intervention group demonstrated a marked reduction in poor sleep quality, with a higher percentage of individuals reporting fair or good sleep quality after the intervention compared to before. These findings were statistically significant (p = 0.0001), confirming the effectiveness of the SI-VIGUR app. The SI-VIGUR VR app proved to be an effective non-pharmacological intervention for improving sleep quality among older adults in nursing homes. The results suggest that VR-guided imagery can significantly reduce sleep disturbances, offering a promising solution for enhancing the well-being of older adults in institutional settings.

Abstract

Background: Gout arthritis is a chronic inflammatory joint disorder in older adults that causes persistent pain and limits daily functioning. Despite growing interest in non-pharmacological pain management, most available evidence comes from clinical or hospital-based settings, while evidence from community contexts in LMICs, including Indonesia, remains limited. This study aimed to evaluate the effectiveness of a warm cinnamon powder compress in reducing pain intensity among community-dwelling older adults with gout arthritis. Methods: This quasi-experimental study with a non-equivalent control group pretest–posttest design was conducted in accordance with the TREND reporting guideline at Pagar Agung Health Center, Lahat Regency. A total of 70 older adults with gout arthritis who met the inclusion criteria were recruited and allocated to an intervention group (n = 35) and a control group (n = 35). The independent variable was warm cinnamon powder compress therapy administered once daily for 15 minutes over seven consecutive days, while the dependent variable was pain intensity measured using the Numeric Rating Scale (NRS). The control group received routine care. Data were analyzed using the Wilcoxon signed-rank test for within-group comparisons and the Mann–Whitney U test for between-group differences. Results: The intervention group demonstrated a marked reduction in mean pain scores from 5.11 ± 0.82 at pretest to 2.14 ± 0.93 at posttest, representing a very large within-group effect (Cohen’s d = 3.38), while the control group showed only a minimal change from 5.03 ± 0.79 to 4.89 ± 0.88 (Cohen’s d = 0.17). Between-group comparison indicated a significantly greater reduction in pain in the intervention group than in the control group, with a large effect size (Cohen’s d = 1.54). Clinically, the intervention group predominantly shifted from moderate–severe pain to mild pain categories, indicating a meaningful improvement in pain experience. Conclusion: Warm cinnamon powder compress therapy is effective in reducing pain intensity among older adults with gout arthritis and can serve as a safe and practical complementary intervention in community healthcare settings.

Abstract

Background: Major surgery is frequently associated with high levels of preoperative anxiety, which can negatively affect physiological stability and postoperative recovery. Despite evidence supporting the effectiveness of nature-based virtual reality (VR) therapy in reducing anxiety in high-income settings, its applicability in low- and middle-income countries (LMICs), particularly district hospitals, remains unclear. This study aimed to evaluate the effectiveness of nature-based VR therapy in reducing preoperative anxiety among first-time major surgery patients. Methods: This study employed a quasi-experimental pre-test–post-test control group design, reported in accordance with the TREND guideline. A total of 104 adult patients scheduled for first-time elective major surgery were recruited using consecutive sampling and equally allocated to an intervention group (n=52) and a control group (n=52). The independent variable was nature-based VR therapy, and the dependent variable was preoperative anxiety. Anxiety was measured using the State Anxiety subscale of the State–Trait Anxiety Inventory (STAI), a validated instrument with good reliability (Cronbach’s α = 0.83). The intervention group received a single 5-minute nature-based VR session 20–30 minutes before surgery, while the control group received standard preoperative care. Data were analyzed using paired and independent t-tests, and effect size was calculated using Cohen’s d. Results: Participants were predominantly aged 46–60 years (39.4%), equally male and female, mostly classified as ASA II (69.2%), and commonly scheduled for abdominal surgery (40.4%). The intervention group showed a significant anxiety reduction (mean difference = −16.10; p < 0.001; Cohen’s d = 1.98), whereas the control group showed no significant change (−2.38; p = 0.062). Post-intervention anxiety scores were significantly lower in the VR group than in the control group (t = 9.074; p < 0.001; Cohen’s d = 1.78). Conclusion: Nature-based VR therapy is an effective, nurse-led, non-pharmacological intervention for reducing preoperative anxiety in first-time major surgery patients. Integration of VR into routine preoperative nursing care may enhance patient-centered care and support anxiety management in resource-limited hospital settings.
