Abstract

The neuregulin 1 (NRG1) gene which influences the development of white matterconnectivity has been associated with schizophrenia. It influences neuronal migration, synaptogenesis,gliogenesis, neuron-glia communication, myelination, and neurotransmission in the brain and others.NRG1 is located in 8p13, and it is frequently replicated in schizphrenia. SNP8NRG433E1006 geneNRG1 is one of core at risk haplotype of schizphrenia. This study looked forward differencesSNP8NRG433E1006 neuregulin 1 between Bataks ethnic with schizophrenia paranoid and Bataksethnic healthy control. Methods: Batak ethnic with schizophrenia paranoid were recruited andinterviewed with semi-structured MINI ICD-X to establish the diagnosis. All the eligible subjectswere requested their permission for blood sampling. Healthy Batak ethnic were also recruited bymathcing the age and gender. The blood samples went through DNA isolation, Nested PCR, and DNAsequencing. Results: Ninety three subjects were recruited, but only 74 blood samples weresuccesfully sequenced. We found three types of polymorphisms, i.e. G/A allele at base pair (bp) 76,G/T allele at bp 112, and deletion at bp 110 in Batak ethnic with schizophrenia. There were two kindsequences at bp 113-116 in Batak ethnics, and Batak ethnics with ATCG were at higher risk forhaving schizophrenia. This study support that NRG1 is a schizophrenia-susceptibility gene.

Abstract

Salah satu penyakit yang masih menjadi permasalahan kesehatan di dunia termasuk Indonesia adalah tuberkulosis (TB) yang disebabkan oleh kuman Mycobacterium tuberculosis (M. tuberculosis). Keberhasilan pengobatan TB memerlukan kepatuhan penderita dalam minum obat anti tuberkulosis (OAT). Dukungan keluarga seperti pengawasan dan memberi motivasi kepada penderita TB merupakan faktor yang berperan dalam kepatuhan penderita. Penelitian ini dilakukan untuk mengetahui hubungan dukungan keluarga dengan kepatuhan minum obat pada penderita TB paru di Kabupaten Tapanuli Utara. Desain penelitian ini adalah cross sectional dengan responden 60 penderita TB paru kategori 1 dan 2 yang mendapat pengobatan minimal 1 bulan serta tinggal bersama keluarga. Penelitian dilaksanakan di Puskesmas Pangaribuan, Puskesmas Situmeang Habinsaran, dan Puskesmas Hutabaginda di Kabupaten Tapanuli Utara pada bulan Juni-September 2017. Responden diwawancarai dengan menggunakan pertanyaan kuesioner untuk mengumpulkan data. Hasil penelitian ini menunjukkan terdapat hubungan antara dukungan keluarga dengan kepatuhan minum obat pada penderita TB paru di Puskesmas Tapanuli Utara (p=0,002). Pada penelitian ini, dukungan keluarga yang baik meningkatkan kepatuhan penderita TB untuk minum obat anti tuberkulosis (OAT).

Abstract

Gangguan ansietas menyeluruh merupakan bagian penting dari beban global penyakit, dan diproyeksikan sebagai penyebab kedua paling umum kecacatan pada tahun 2020. Gangguan ansietas menyeluruh menyebabkan peningkatan  distres dan terganggunya fungsi kehidupan. Penelitian ini merupakan penelitian potong lintang dengan consecutive sampling pada 100 pasien klinik psikiatri Rumah Sakit dr. Pirngadi Medan selama periode 1 Juli hingga 31 Juli 2012. Pengumpulan data dilakukan melalui wawancara menggunakan kuesioner, Mini International Neuropsychiatric Interview for International Classification of Disease-10 (MINI ICD-10), dan dilanjutkan dengan kriteria diagnostik Pedoman Penggolongan dan Diagnosis Gangguan Jiwa di Indonesia III (PPDGJ III) yang digunakan di Indonesia untuk konfirmasi diagnosis. Hasil penelitian ini mengungkapkan, jumlah pasien dengan gangguan ansietas menyeluruh adalah 23%. Jumlah gangguan ansietas menyeluruh diidentifikasi lebih besar dalam kelompok subjek perempuan (14/23), menikah (13/23), dan bekerja (12/23). Simpulan, tidak ada hubungan jenis kelamin, status pernikahan, dan status pekerjaan dengan gangguan ansietas menyeluruh.Kata kunci: Gangguan ansietas menyeluruh, klinik psikiatri, MINI ICD-10The Relationship between Gender, Marital Status, Employment Status and Generalized Anxiety Disorder in Psychiatry Clinic of dr. Pirngadi Hospital MedanAbstractGeneralized anxiety disorder constitute a substantial proportion of the global burden of disease, and are projected to form the second most common cause of disability by 2020. Generalized anxiety disorder led to increased distress and impaired life functioning. This study was a cross-sectional study with consecutive sampling to 100 patients of psychiatric clinic of dr. Pirngadi hospital Medan during the period 1st July until 31th July 2012. Data was collected through interviews using questionnaire, Mini International Neuropsychiatric Interview for International Classification of Disease-10 (MINI ICD-10) form, and continued with the diagnostic criteria of Pedoman Penggolongan dan Diagnosis Gangguan Jiwa di Indonesia III (PPDGJ III) that have been used in Indonesia to confirm the diagnosis. Results of this research revealed, the number of patients with generalized anxiety disorder was 23%. The number of generalized anxiety disorder was larger identified in the groups of female’s subject, (14/23), married (13/23) and employed (12/23). Abstract

Chronic kidney disease (CKD) patients, especially those with hemodialysis, frequently struggle with mental health issues like anxiety and depression. Psychotherapy has been known to treat psychological problems, but its effectiveness in managing CKD patients is still rarely scientifically proven. The aim of this study was to analyze the role of psychological treatments in improving the mental health of CKD patients with hemodialysis. We comprehensively reviewed the related studies published in PubMed, Google Scholar, ScienceDirect, and Clinical Key over the last ten years, up to June 7, 2023. A keyword combination was used in the search engine strategies, and all articles about CKD patients receiving hemodialysis and psychotherapy were included. Based on the eligibility criteria, 716 patients were included in 13 out of 18,830 studies in the final analysis. Psychological problem was complained by 399 CKD patients. The psychotherapy included cognitive behavioral therapy (reported in four studies, n=4), diaphragmatic breathing relaxation (n=1), meditation (n=1), hypnotherapy (n=1), Kidney Optimal Health Program (KOHP) (n=1), psychological intervention (n=1), murottal Al-Qur'an therapy (n=3), and spiritual therapy (n=1). These interventions were performed once to four times a week, for ten minutes to five hours during hemodialysis for two to ten weeks. Meditation and KOHP showed no significant improvement in anxiety and depression. The remaining psychotherapies significantly improved the quality of life by reducing anxiety and depression in hemodialysis patients and enhancing sleep quality, self-esteem, hopefulness, medication adherence, and physical condition. In conclusion, psychotherapy should be considered in an interdisciplinary team to treat CKD patients comprehensively. Further studies are still necessary to determine the efficacy of each psychological intervention in CKD patients with psychiatric problems.

Abstract

Background:During the Coronavirus Disease 2019 (COVID-19) pandemic, nurses and doctors served as the frontline healthcare workers. Consequently, mental health issues among healthcare workers can develop into significant health problems and impose a substantial social burden, particularly on nurses and doctors working in hospital settings. Objective:To analyze the prevalence and factors influencing anxiety levels among healthcare workers, specifically nurses and doctors, at Adam Malik General Hospital Medan during the COVID-19 pandemic. Methods:This study employed a cross-sectional design with bivariate analysis. Data were collected using the DASS-42 questionnaire, which was distributed to 114 participants comprising nurses and doctors working in both the COVID-19 emergency isolation and ward isolation units. Results:The findings revealed that 29 participants (25.4%) exhibited symptoms of anxiety, with the majority experiencing mild anxiety (13.2%). The nursing profession demonstrated the highest prevalence of anxiety disorders, with 33.3% of nurses reporting symptoms. Being a nurse was significantly associated with higher anxiety levels compared to being a doctor, and working in the emergency isolation room was significantly associated with higher anxiety levels compared to working in the isolation ward or both units (p<0.05). Conclusion:Factors such as gender, marital status, parental status, post-COVID-19 status, family history of COVID-19, vaccination history, and comorbidities did not significantly affect anxiety levels among healthcare workers during the COVID-19 pandemic at Adam Malik General Hospital Medan. However, anxiety was prevalent among healthcare workers, with nurses and those working in the emergency isolation room being particularly affected. Abstract

Background: The Early Warning Score (EWS) is a clinical monitoring system based on vital signs, designed to support early detection of patient deterioration. However, its implementation in hospitals is often hindered by limited knowledge and skills among healthcare professionals. Objective: To evaluate the impact of EWS training on improving doctors' and nurses' knowledge regarding the application of the system at Laras Hospital, PT. Prima Medika Nusantara. Methods: A quasi-experimental pre-post study without a control group was conducted involving 155 participants. The EWS training, delivered through lectures and simulations, was followed by pre- and post-intervention knowledge assessments using a structured questionnaire. Statistical analysis was performed using the Wilcoxon test. Results: The findings demonstrated a significant improvement in knowledge after training (p = 0.00), indicating the effectiveness of the intervention in enhancing clinical readiness among medical staff. Conclusion: EWS training plays a vital role in strengthening the clinical competencies of doctors and nurses. Its routine implementation can serve as a practical strategy to enhance early detection systems and improve patient safety and care quality in hospital settings.

Abstract

Background: Loneliness is a psychological problem that often arises in adolescents. One of the causes is unfulfilled emotional and social needs. The intensity of loneliness can vary, there can be immediately through a feeling of loneliness, but there are also those who constantly feel lonely. It can affect adolescents mentally, so it is often associated with psychological problems and somatic health. Objectives: This study aimed to know the relationship of loneliness with psychological problems and symptoms of somatic disorders in adolescents. Methods: This study is an analytical study with a cross-sectional design, data that used is primary data from questionnaires. The sample was selected by using the stratified random sampling method from all questionnaire data. Results: In the Spearman correlation analysis, the p-value was 0.001 (p<0,005) which showed that there was a relationship between loneliness and psychological problems and symptoms of somatic disorders in adolescents. Bivariate analysis between loneliness and depression obtained correlation coefficient r=0.548, loneliness and anxiety r=0.515, loneliness and stress r=0.472 and loneliness and symptoms of somatic disorders r=0.528. Conclusion: There was a significant relationship between loneliness and psychological problems and symptoms of somatic disorders in adolescents. Keywords: loneliness, psychological problems, symptoms of somatic disorders     Latar Belakang: Kesepian menjadi salah satu masalah psikologis yang sering muncul pada remaja. Salah satu  penyebab timbulnya kesepian pada remaja adalah tidak terpenuhinya kebutuhan emosi dan sosial. Intensitas kesepian bisa berbeda-beda, ada yang dapat segera melalui perasaan kesepian, namun ada juga yang terus-menerus merasakan kesepian. Hal ini dapat mempengaruhi remaja secara mental, sehingga sering dikaitkan dengan masalah psikologis dan kesehatan somatis. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan kesepian dengan masalah psikologis dan gejala gangguan somatis pada remaja. Metode: Penelitian ini merupakan studi penelitian analitik dengan desain penelitian cross sectional menggunakan data primer yang berasal dari kuisioner. Sampel penelitian dipilih dengan metode stratified random sampling dari seluruh data kuisioner yang memenuhi kriteria penelitian. Hasil: Pada analisis korelasi Spearman didapatkan nilai p sebesar 0,001 (p<0,05) yang menunjukkan terdapat hubungan antara kesepian dengan masalah psikologis dan gejala gangguan somatis pada remaja. Analisis bivariat kesepian dengan depresi didapatkan nilai koefisien korelasi r=0,548, kesepian dengan kecemasan r=0,515, kesepian dengan stres r=0,472 dan kesepian dengan gejala gangguan somatis r=0,528. Kesimpulan: Terdapat hubungan yang bermakna antara kesepian dengan masalah psikologis dan gejala gangguan somatis pada remaja. Kata kunci: gejala gangguan somatis, kesepian, masalah psikologis

Abstract

Background: Digital technology addiction includes internet addiction, social media addiction, smartphone addiction, online gaming addiction, and online gambling addiction. Someone who spends a lot of time using digital technology and internet-related activities will cause dependence on technology for social interaction, entertainment, and information resources. This will lead to increased mental disorders, emotional instability, and sleep disturbances. Since the COVID-19 pandemic began, many prevention and control strategies have been implemented by the Indonesian government to suppress the spread of this virus. One of the best ways to suppress the spread of this virus is to stay at home. This affects the daily activities of people around the world which leads them to use digital technology more often, such as the internet, social media, and smartphones Methods: This study aims to evaluate association between gender, age, and residence with smartphone addiction among students of the 2018 Faculty of Medicine, North Sumatra University during COVID-19 pandemic. The type of research used in this study is a analytic observational study with a cross-sectional design. The research sample was obtained using the large-sample method based on proportions with the research sample population being all students of the Medical Education Study Program, Faculty of Medicine, University of North Sumatra, 2018. The data was obtained online using a questionnaire Conclusion: Among 80 respondents there are 59 people experiencing smartphone addiction and there is no significant association between gender, age, and residence with smartphone addiction.
