Abstract

PT. XYZ is a vehicle-body manufacturer producing four cargo-body variants and experiencing a persistent mismatch between growing demand and material readiness. This study develops an integrated material-planning framework that combines trend forecasting, production planning, a Master Production Schedule (MPS), Material Requirements Planning (MRP), and a cost comparison between Lot for Lot (LFL) and the Wagner–Whitin algorithm. The dataset covers monthly sales and production from January 2024 to December 2025, bills of material, beginning inventory, supplier lead times, and company-provided inventory-cost parameters. Holt’s double exponential smoothing with α=0.3 and β=0.2 achieved a mean absolute percentage error of 2.50%, a mean absolute deviation of 0.737 units, and a mean squared error of 0.902. The 2026 demand forecast totaled 680 units and was consistently disaggregated among the four product variants. MRP generated time-phased gross requirements, net requirements, planned-order receipts, and planned-order releases. Under the company’s cost-accounting scope, Wagner–Whitin reduced total cost from IDR 330.40 million to IDR 271.29 million, saving IDR 59.11 million (17.89%). When the invariant finished-product component was excluded, the lot-sizing-sensitive cost decreased by 67.53%. The results indicate that combining MRP with dynamic lot sizing can improve material-timing reliability and inventory-cost efficiency, particularly for stable-demand items with relatively high ordering costs.

Abstract

Objective: Visitor safety is an essential aspect of swimming pool management because various hazards may lead to accidents. This study aims to identify potential hazards, assess visitor safety risks, and propose mitigation measures at Cihanjawar Swimming Pool, Purwakarta, using the Hazard Identification and Risk Assessment (HIRA) and Hazard and Operability Study (HAZOP) methods. Data were collected through observation, interviews, and documentation. HIRA was applied to identify hazards and assess risks based on likelihood and severity, while HAZOP analyzed causes, consequences, existing controls, and improvement recommendations. The results identified 10 major hazards, including six high-risk and four medium-risk categories. The highest risk was inadequate supervision by lifeguards and parents, with a risk score of 16, while the drowning risk scored 15. HAZOP analysis revealed that human factors, facility conditions, and the surrounding environment were the main causes of these risks. Recommended mitigation measures include increasing lifeguards, installing clearer safety signs, providing pool depth information, applying anti-slip flooring, repairing damaged facilities, and improving visitor safety education. The novelty of this study lies in integrating HIRA and HAZOP to provide a more comprehensive risk assessment and mitigation strategy.

Abstract

Occupational safety and health (OSH) is essential to prevent workplace accidents in industrial activities. CV. Dea Ayu Lestari, a solid waste collection and sales company, involves intensive manual material handling that poses significant accident risks. This study aims to identify potential hazards and analyze risk levels in waste processing activities using Job Safety Analysis (JSA) and Failure Mode and Effects Analysis (FMEA). JSA was applied to qualitatively break down work steps and identify hazards, while FMEA quantified the risks using Severity (S), Occurrence (O), and Detection (D) parameters to determine the Risk Priority Number (RPN). The results revealed that high-priority risks are concentrated in handling wooden and iron sheet pallets, with the highest risk occurring during the transportation of wooden pallets to collectors due to the hazard of being struck by collapsing materials, yielding a peak RPN of 126. Other critical risks include sharp metal cuts from rusted iron sheets (RPN 112). Recommended control measures include the operationalization of four newly designed Standard Operating Procedures (SOPs), the introduction of mechanical handling aids, and mandatory compliance with standardized personal protective equipment (PPE), specifically cut-resistant gloves and safety shoes.

Abstract

This study aims to identify workplace hazards, assess risk levels, and formulate occupational health and safety (OHS) risk control recommendations for production activities at PT Asian Isuzu Casting Center (AICC), a foundry manufacturer of automotive components in Karawang, West Java, whose melting, casting, machining, and finishing processes involve high heat exposure, molten-metal splashes, machine noise, metal dust, and suboptimal use of personal protective equipment (PPE). Methods: A descriptive method combining field observation, interviews, and documentation study was applied using the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) approach, with risk scoring based on Likelihood and Severity referring to the company's internal procedure USY-06-SY-002 (Rev. 4, 2025). Results: Twenty hazard activities were identified across four production areas; ten (50%) were categorized as high risk and ten (50%) as medium risk, with the Melting area showing the highest risk profile due to direct exposure to molten metal and ladle-handling activities. Novelty: The study provides an integrated, area-based HIRARC risk mapping specific to a foundry component manufacturer and translates the findings into control recommendations structured according to the hierarchy of controls (engineering, administrative, and PPE), offering a practical reference for similar metal-casting industries.

Abstract

Product quality is an important factor in maintaining customer satisfaction and business competitiveness. This study aims to improve the quality control of milk sticky rice products by applying Statistical Quality Control (SQC) and the Plan-Do-Check-Act (PDCA) method. Production and defect data collected from December 2025 to May 2026 were analyzed using check sheets, control charts, Pareto diagrams, and cause-and-effect diagrams to identify dominant defects and their root causes. The PDCA cycle was then implemented to develop and evaluate improvement actions. The results indicate that the dominant defects were hard sticky rice, soft sticky rice, poor product presentation, and inconsistent taste. The main causes originated from human factors, work methods, raw materials, and production processes. The implementation of improvement actions through the PDCA approach enhanced process control and reduced the defect rate, resulting in more consistent product quality. This study provides practical recommendations for implementing continuous quality improvement in food production businesses.

Abstract

This study aims to identify occupational hazards, assess occupational safety and health (OSH) risks, and propose appropriate risk control measures in the Joint Plate production process at PT. Caraka Anugrah Santoso using the Hazard Identification, Risk Assessment and Determining Control (HIRADC) and Job Safety Analysis (JSA) methods. A descriptive approach was employed through observation, interviews, documentation, and literature review. HIRADC was applied for macro-risk assessment, while JSA was utilized for systematic task analysis. Dominant hazards were identified in the Furmace, Rolling Mill, Hole, Bumping, Channel, and Skirtboard processes, including being pinched by machinery, injuries from sharp and hot materials, metal fragment exposure, slipping, and tripping. The Rollling Mill process presented the highest risk level with a Risk Rating of 16 (High Risk), while other processes scored 12 (High Risk). Proposed controls include engineering controls, administrative controls, SOP development, OHS training, and PPE compliance. The integration of HIRADC and JSA methods provides a comprehensive risk analysis and hierarchy of control based recommendations tailored to the characteristics of Joint Plate production. Occupational Safety and Health, HIRDC, Job Safety Analysis, Risk Assessment, Joint Plate.
