Abstract

Rheumatoid arthritis (RA) is an autoimmune and prominent inflammatory disorder that can affect wide of variety body systems, mainly joints. In Indonesia, the prevalence of RA is about 7.3% and mostly are women. The majority of women with RA are at childbearing and can be worsening throughout pregnancy and lactation. Conventional synthetic disease-modifying antirheumatic drugs (csDMARDs) are the most common used medicine in RA for pregnancy and lactation. This scoping review was conducted using publications obtained from PubMed, Embase, The Cochrane Library, POPLINE, and Google Scholar concerning the safety of csDMARD in rheumatoid arthritis during pregnancy and lactation from 2011 to 2021. Among csDMARDs reviewed, sulfasalazine, hydroxychloroquine, chloroquine, and cyclosporine are relatively safe for pregnant and lactating women. However, they should be used in caution by considering the risk and benefit as well as under clinical supervision.

Abstract

Breast milk production should be prepared from pregnancy, especially in the third trimester. Many pregnant women in their third trimester have not yet produced colostrum. This impacts breast milk production in the first week postpartum, thus triggering the use of formula milk. One effort to stimulate colostrum production is by consuming carrot juice and honey. This study aims to determine the effect of giving carrot juice and honey to pregnant women in their third trimester on the timing of colostrum production as an effort to achieve successful breastfeeding at Susiani Independent Midwife Practice Banyuwangi in 2025. This research method uses a quasi-experimental two-design. The study used a post-test only with a control design. The population of this study was 60 respondents, 30 of whom were in the intervention group and 30 in the control group, with consecutive sampling as the sampling technique. The intervention was administered and consumed for 7 days, twice a day. The control group was given basic IEC (Information, Education, and Communication)  care for the Third Trimester. Normality and homogeneity tests were carried out, and the Levene test obtained normal values , so data analysis used the Independent Samples T-Test. The results showed that the average time for colostrum release in the intervention group was 4.87 days, with the fastest time on day 2 and the longest on day 7. In contrast, in the control group, it was 8.10 days, with the fastest time for colostrum release on day 4 and the longest on day 12. The results of the Independent Samples T-Test with a p value = 0.000 (<0.05), which means that there is an effect of giving carrot juice and honey to pregnant women in the THIRD TRIMESTER on the time for colostrum release as an effort to achieve successful breastfeeding. The results of this study will enable midwives to provide IEC on carrot juice and honey and to implement it as support for successful breastfeeding.

Abstract

This study aims to analyze the impact of bureaucratic structure and legal audit on the optimization of Local Government Property management in the Aceh Besar District Government. Using a quantitative approach, data were collected via questionnaires from officials across regional apparatus organizations. The findings indicate that, collectively, bureaucratic structure and legal audit exert a significant positive influence on the optimization of asset management. Furthermore, when examined individually, each independent variable also demonstrates a positive effect. A well-defined bureaucratic structure contributes to more organized and accountable management practices. Similarly, the implementation of legal audits strengthens the legal certainty of asset ownership and helps prevent disputes, thereby supporting more optimal asset utilization. The study concludes that both a sound bureaucratic framework and regular legal audits are crucial factors in enhancing the effectiveness and efficiency of local government property management

Abstract

Suboptimal breast milk production during postpartum is a reason for formula feeding, thus preventing exclusive breastfeeding targets. According to the WHO, the global coverage of exclusive breastfeeding should reach 68% by 2023. Non-pharmacological efforts can be implemented by administering moringa leaf tea, which contains flavonoids, phytosterols, alkaloids, saponins, and tannins, which function as lactogogues to stimulate the hormones prolactin and oxytocin. The purpose of this study was to determine the effect of administering moringa leaf tea to breastfeeding mothers at the Fitriatul Laili Rejoagung Banyuwangi TPMB in 2025 on their ability to increase breast milk production. This study involved 30 postpartum mothers: 15 in the intervention group and 15 in the control group. The study used a two-group quasi-experimental pretest-posttest design. Five grams of moringa leaf tea, 200 milliliters daily for seven days, was administered to the intervention group. The control group received normal postpartum care. The sampling method used was sequential sampling. Ten breast milk production indices were used as instruments. A t-test was used for data analysis. This study has been ethically reviewed. According to the findings, the average breast milk production in the intervention group increased from 5.87 to 8.87, while in the control group it increased from 6.00 to 6.60. The intervention group's averages differed from the control group by 3.00 and 0.6, respectively. The p-value was 0.000, indicating that tea made from moringa leaves can increase breast milk production. Therefore, this tea is recommended as a natural substitute for breast milk production.

Abstract

AbstractBreast milk is the primary source of nutrition for newborns and plays a crucial role in infants' growth and immune system. However, smooth breast milk production is often a challenge for some breastfeeding mothers. One non-pharmacological effort that can support smooth breast milk production is consuming Papaya. Papaya contains lactogagum, which can secrete oxytocin and prolactin, hormones that stimulate breast milk production. This study aimed to determine the effect of papaya consumption on smooth breast milk flow in breastfeeding mothers in the Songgon Community Health Center working area in 2025. This study implemented a quasi-experimental design through a pre-test and post-test approach. 30 respondents were obtained through accidental sampling, which was divided into 2 groups, 15 intervention group respondents and 15 control group respondents, thus meeting the minimum number of samples suitable for the study. The intervention group was given papaya fruit 3 times a day for 7 days with a dose of 250 grams at each meal. The control group was not given any treatment. The sampling technique used was accidental sampling. The instruments used were a questionnaire indicator of smooth breast milk flow and an observation sheet. Data analysis was conducted using the Wilcoxon and Mann-Whitney tests using SPSS 25 for Windows. The study was conducted on January 31–February 25, 2025, with KEPK No. 071/01/KEPK-STIKESBWI/I/2024-2025. The results showed that the average smoothness of breast milk in the intervention group increased from 1.87 to 7.40, while the control group increased from 2.20 to 4.00. The average difference between the intervention group was 5.53, and the control group was 1.8. The p-value = 0.000 indicates an effect. Papaya can be recommended as a natural alternative to increase breast milk flow. Keywords: Breastfeeding Mothers, Breast Milk, Papaya, Smoothness of Breast Milk
