Abstract

Epilepsy and malnutrition in children are a vicious circle. Epilepsy with non-treated recurrent seizures could lead to malnutrition. Conversely, malnutrition which results in macronutrient and micronutrient deficiencies also potential to trigger seizures. We reported an adolescent, male, 14 years 6 months, with epileptic state accompanied by malnutrition status. In this case, a repeated seizures cause the unconscious level of patient and at risk of respiratory failure. Multidisciplinary therapy was arranged by anesthesiologist, pediatrician, and clinical nutritionist. The treatments began with procedure of a ventilator installation, anticonvulsant drugs, and administration of a nasogastric tube for accessing nutrition intake. Nutrition therapy focused on managing diet in critical ill children and attention to undernourished conditions that require long-term treatment. The target of nutrition calorie was achieved on day 6-8 of treatment through enteral to oral diet. Finally, nutrition education for catch-up growth was given to the families to be applied during the recovery and rehabilitation period at home.

Abstract

Background: The incidence of diabetes in Bandung City was 43,761 cases. The high number of cases can be caused by an unhealthy lifestyle, one of which is a bad diet.Purpose: To determine the effect of sugar consumption and fiber consumption on the incidence of diabetes mellitus.Method: A case-control research design with the population of cases was people with diabetes mellitus, while the control population was people with other non-communicable diseases. The number of samples in this study was 50 case groups and 50 control groups. The sampling technique used is systematic random sampling. The data collection instrument used is a questionnaire. Data analysis using chi-square test.Results: The study respondents were excessive in consuming sugar at 58%, and more than half of respondents were consuming enough fiber at 53%. There is an effect of sugar consumption on the incidence of diabetes mellitus (p = 0.008). And there is an effect of fiber consumption on the incidence of diabetes mellitus (p = 0.005).Conclusion: The incidence of diabetes mellitus can be influenced by excessive sugar consumption and low fiber consumption.Suggestion: It is recommended to health workers to further increase education about the importance of a healthy diet.Keywords: Diabetes Mellitus; Diet; Sugar Consumption; Fiber Consumption.Pendahuluan: Kejadian diabetes di Kota Bandung sebanyak 43.761 kasus. Tingginya jumlah kasus dapat disebabkan oleh gaya hidup yang tidak sehat, salah satunya adalah pola makan yang tidak baik.Tujuan: Untuk mengetahui kebiasaan konsumsi gula pasir berlebih dan konsumsi serat terhadap kejadian diabetes melitus.Metode: Penelitian case control. Populasi kasus adalah penderita diabetes melitus, sedangkan populasi kontrolnya adalah penderita penyakit tidak menular lainnya. Jumlah sampel dalam penelitian ini adalah 50 kelompok kasus dan 50 kelompok kontrol. Teknik sampling yang digunakan adalah systematic random sampling. Instrumen pengumpulan data yang digunakan adalah kuesioner. Analisis data menggunakan uji chi square.Hasil: Sebagian besar responden penelitian berlebih dalam mengkonsumsi gula yaitu 58%, dan lebih dari setengah responden yang cukup konsumsi serat yaitu 53%. Kebiasaan konsumsi gula berlebih dengan kejadian diabetes melitus (p=0,008). Dan ada konsumsi serat dengan kejadian diabetes melitus (p=0,005).Simpulan: Kejadian diabetes melitus dapat dipengaruhi oleh konsumsi gula yang berlebih dan konsumsi rendah serat.Saran: Kepada tenaga kesehatan untuk lebih meningkatkan edukasi tentang pentingnya pola makan sehat.

Abstract

Introduction and importance: Enterocutaneus fistula (ECF) is an uncommon connection between the gastrointestinal tract and the skin. Loss of intestinal fluid in ECF patients results in an imbalance of electrolytes. The objectives of nutritional therapy were to fulfill nutrient needs, maintain fluid balance, & promote ECF closure.  Presentation of case:    A 22-year-old male, was diagnosed with post-adhesiolysis and end colostomy surgery. His oral intake was reduced for one month, and he experienced weight loss. We detected a loss of subcutaneous fat and fistula in the right abdomen region and wasted time on physical examination. Abnormal laboratory findings were leucocytosis, thrombocytosis, imbalance electrolyte, and hypoalbuminemia.  Discussion: Nutritional assessment was based on mid-upper arm circumference. Nutritional treatment was provided with a calorie target of 2300 kcal and protein 2g/kg ideal body weight (IBW)/day. On the seventh day of treatment, the patient went through relaparotomy due to high output ECF (2000 ml/d) and ileum adhesion grade 3-4. Following surgery, the patient was on parenteral nutrition for eighteen days. Due to high ECF output, protein intake was increased by 2g/kg IBW/day, and fluid intake was adjusted based on fistula output. The patient received a combination of polymeric formula and whey protein by oral intake and micronutrient supplementation at twice the usual dosage. By the 27th day of treatment, ECF output had decreased to 0 ml/d, and laboratory results showed improvement. Conclusions:   Adequate nutrition, fluid, and electrolyte balance through optimal nutritional therapy can improve laboratory values and reduce ECF output in ECF patients

Abstract

ABSTRAK Fungsi kognitif dan kadar asam urat darah lansia tidak termasuk dalam pemeriksaan wajib dalam program Prolanis. Fungsi kognitif perlu dinilai untuk memastikan bahwa peserta Prolanis tidak mengalami kemunduran fungsi kognitif. Tujuan kegiatan ini adalah untuk melakukan skrining tingkat fungsi kognitif dan kadar asam urat darah peserta Prolanis di Klinik Dokter Keluarga Fakultas Kedokteran Universitas Muhammadiyah Palembang. Kegiatan dilaksanakan dalam satu hari selama 2 jam 30 menit. Pemeriksaan fungsi kognitif dilakukan menggunakan kuesioner Mini Mental State Examination (MMSE) berbahasa Indonesia. Pemeriksaan kadar asam urat darah dilakukan menggunakan darah dari ujung jari dengan alat pemeriksa asam urat dan strip khusus. Hasil skrining menunjukkan bahwa 80,4% peserta Prolanis memiliki fungsi kognitif yang normal dan 66,7% memiliki kadar asam urat darah yang normal.   ABSTRACT Cognitive function and blood uric acid levels in the elderly are not included in the mandatory screening in the Prolanis program. Cognitive function needs to be assessed to ensure that Prolanis participants do not experience cognitive decline. The purpose of this activity was to screen the cognitive function and blood uric acid levels of Prolanis participants at the Family Physician Clinic, Faculty of Medicine, Muhammadiyah University of Palembang. The activity was carried out in one day for 2 hours and 30 minutes. Cognitive function testing was conducted using the Indonesian language Mini Mental State Examination (MMSE) questionnaire. Blood uric acid testing was performed using fingertip blood with a uric acid tester and special strips. The screening results showed that 80.4% of Prolanis participants had normal cognitive function and 66.7% had normal blood uric acid levels.
