Abstract

PT. ABC faces a quality control issue with a defect rate of 5.5%, resulting in customer complaints. This study aims to identify the types of defects in sheet film products, analyze their root causes, and evaluate quality control measures to reduce defects. The research uses Statistical Process Control (SPC) to monitor the production process and Failure Mode and Effect Analysis (FMEA) to analyze potential failures in the production system. The SPC analysis revealed that the most frequent defects were ILK Defect (501 rolls), followed by Thickness Defect (188 rolls) and Scratch Defect (100 rolls). The root causes of defects were identified using a fishbone diagram, which pointed to Method, Material, Machine, and Man as the main factors. FMEA results indicated that the primary cause of defects was an issue with the hopper on the machine, which caused material transfer delays, leading to V-ILK and Thickness Defects. This study recommends improving machine maintenance, revising operational procedures, and enhancing operator training to reduce defects and improve product quality

Abstract

In the rapidly accelerating era of globalization, soft skills have become crucial elements required by university graduates, alongside hard skills. Soft skills such as leadership, time management, self-management, and group management are essential in both the professional world and organizational settings. In the Industrial Engineering Student Association (HMTI) at the Institut Teknologi Sumatera (ITERA), many students still face challenges in developing these essential soft skills. Therefore, a structured and continuous soft skills development training program is necessary. This community service activity was conducted by the lecturers of Industrial Engineering at ITERA using coaching and simulation games to equip students with relevant soft skills for managing the organization. The coaching method was used to provide direct guidance, while simulation games were applied to train communication skills, decision-making, and problem-solving in real-life scenarios. The training proved to be effective in enhancing students' leadership, time management, and self-management abilities, as well as helping them face challenges in organizational and professional settings. The PDCA (Plan, Do, Check, Action) cycle was also implemented to evaluate and improve the organizational processes. The conclusion of this activity is that the development of soft skills plays a significant role in the success of student organizations and future careers. Several recommendations are provided to enhance the effectiveness of future training, including aligning the training materials with the specific needs of organizations and collaborating with professionals from various industries

Abstract

The implementation of Occupational Health and Safety (OHS) management reflects a business owner's commitment to protecting workers' health and safety. One key step in implementing OHS is identifying and mapping potential workplace hazards. In small and medium enterprises (SMEs) producing tofu, such hazards often arise during the production process. The HIRA (Hazard Identification and Risk Assessment) method is used for hazard mapping because it systematically identifies hazard locations and critically analyzes risk levels. This study was conducted at a tofu SME in Krajan, Surakarta, and identified several potential hazards categorized into four groups: hazards from the work environment, production materials, unsafe equipment, and production waste. The analysis revealed five high-risk hazards and four moderate-risk hazards. Through hazard mapping, business owners are expected to take preventive measures to minimize or eliminate exposure to these risks. This ensures worker health and safety while maintaining a smooth and uninterrupted production process.

Abstract

PT ABC adalah perusahaan petrokimia yang memproduksi biji plastik dan memiliki tiga gudang, salah satunya adalah gudang bahan baku pendukung. Gudang ini mengalami masalah penempatan yang tidak teratur, dimana barang tidak dikelompokkan berdasarkan jenisnya, sehingga menyulitkan staf dalam pencarian dan memperpanjang jarak material handling. Penelitian ini bertujuan untuk mengevaluasi tata letak dan jarak tempuh material handling pada gudang tersebut setelah usulan perbaikan tata letak diimplementasikan. Penelitian juga membandingkan tata letak gudang saat ini dengan tata letak usulan yang dioptimalkan menggunakan metode Dedicated Storage, yaitu metode pengaturan lokasi barang secara tetap sesuai jenisnya, serta menghitung jarak tempuh menggunakan metode rectilinear distance. Berdasarkan hasil analisis, total jarak tempuh material handling pada kondisi awal adalah 40.670,5 meter, yang diakibatkan oleh tata letak yang tidak optimal. Setelah perbaikan tata letak diusulkan, jarak tempuh material handling berkurang signifikan menjadi 9.545,25 meter, dengan pengurangan jarak sebesar 31.125 meter atau setara dengan penurunan sebesar 76,60%.

Abstract

One of the largest steel industries in Indonesia produces cold-rolled sheet steel, with production handled by the Cold Rolling Mill Factory. This study focuses on the Continuous Pickling Line and Continuous Tandem Cold Mill units, which have the highest downtime of 230 hours, resulting in a delay cost of 6 billion Rupiah. The objective is to identify the functions, failures, causes, and impacts on critical components, and to propose appropriate maintenance activities and intervals. Failure information is gathered using failure mode and effect analysis (FMEA), while the appropriate maintenance activities are determined using a reliability-centered maintenance II decision worksheet. The study identifies four critical components in the CPL unit and five in the CTCM unit. Five failure modes are addressed by on-condition scheduling, seven by restoration scheduling, and seven by failure interval. Proposed maintenance intervals for on-condition scheduling include 9,413 minutes for stand #5, 8,371 minutes for stand #4, 12,383 minutes for stand #3, 12,996 minutes for stand #2, and 7,123 minutes for stand #1. Restoration scheduling intervals are 34,000 minutes for the saddle breaker, 21,000 minutes for the wiper, and between 12,000–23,000 minutes for other components. Failure interval maintenance for the welder, pickling tank, and stands ranges from 2,409 to 6,540 minutes.

Abstract

PT XYZ is a shipping company that operates in the ad hoc and short-term contract (spot charter) market. The company faces challenges in selecting the most profitable cargo offers and in managing fuel consumption in order to maximise profits and minimise operational costs. This study aims to develop a mathematical model that optimises revenue through optimal cargo selection, while simultaneously minimising operational costs, including fuel consumption for the main and auxiliary engines. The model is based on the actual conditions of the company's fleet and focuses on tramp general cargo services operating under less-than-shipload (LTS) conditions. In addition to addressing fuel management, the study covers fleet routing, vessel allocation and port docking considerations. The proposed model uses a mixed integer nonlinear programming (MINLP) approach to optimise variables such as sailing speed, ship displacement and the sequence of port visits. Feasibility analysis using real fleet data and hypothetical cargo offers indicates that the model consistently produces viable solutions. Sensitivity analysis of key parameters within a variation range of -50% to +50% from their nominal values confirms that the model's outputs remain within acceptable bounds.
