Abstract

Background and purpose: Tuberculosis continues to be a concern at the global level. Indonesia is a high-burden country that keeps efforts through TB control programs by implementing a private-public mix (PPM) strategy. This study aims to analyze the implementation of PPM in a private hospital using the Generic Implementation Framework approach. Methods: This study conducted a mixed method approach including quantitative and qualitative analysis of data with explanatory sequential design. The analyzed variables were the assessment elements used in determining hospital accreditation, variables to get an overview of PPM, and achievement of indicators. Quantitative data were analyzed descriptively, while the qualitative with thematic analysis. Results: The quality of TB control at the hospital was under the accreditation standards. There was an increase in the number of patients treated but uptrend in the number of loss to follow-up patients in the last three years. The success rate of treatment in 2021 has not yet achieved as it was influenced by patients who were lost to follow-up and treatment failure. Supporting factors for the PPM implementation were the script of cooperation, commitment, cooperation, communication, mutual trust, maintained service quality, inter-dependence, and mutual benefit as inhibiting factors related to the management of treatment adherence. Conclusion: The achievement of program indicators has yet to meet the target. Therefore, it is necessary to evaluate regulations, standard operating procedures, and cooperation scripts to reach the standard.

Abstract

Badan Penyelenggara Jaminan Sosial (BPJS) mulai beroperasi pada 1 Januari 2014 dan bekerja sama dengan rumah sakit sebagai Fasilitas Kesehatan Rujukan Tingkat Lanjut (FKRTL) menggunakan sistem pembayaran prospektif berdasarkan tarif INA-CBG’s. Sistem ini dinilai efisien dalam pengendalian mutu dan biaya layanan kesehatan. Namun, di beberapa wilayah, selisih negatif antara total tarif rumah sakit dan tarif INA-CBG’s mencapai 13%, yang berisiko merugikan rumah sakit. Hingga kini, belum banyak penelitian yang membahas tarif pasca diterapkannya Permenkes No. 3 Tahun 2023. Penelitian ini bertujuan menganalisis perbedaan tarif rumah sakit dan tarif INA-CBG’s rawat inap di RS Petrokimia Gresik Driyorejo setelah kebijakan tersebut berlaku, serta mengidentifikasi faktor yang memengaruhi selisih tarif. Penelitian ini menggunakan metode kuantitatif dengan pendekatan cross sectional dan data retrospektif dari klaim pasien rawat inap periode 1 Januari–31 Maret 2024. Variabel independennya meliputi kelompok usia, lama rawat inap (LOS), bidang spesialistik, kelas perawatan, dan tingkat keparahan penyakit. Variabel dependen adalah tarif rumah sakit dan tarif INA-CBG’s. Hasil analisis dengan regresi logistik menunjukkan bahwa perbedaan tarif disebabkan oleh metode pembayaran INA-CBG’s. Kelas perawatan dan LOS merupakan faktor utama yang memengaruhi keuntungan atau kerugian rumah sakit. Kelas rawat yang lebih tinggi cenderung memberikan keuntungan lebih besar, sedangkan LOS yang lebih lama justru menurunkan keuntungan karena margin tarif semakin kecil.

Abstract

Waktu layanan dan kepuasan pelanggan menjadi dua indikator utama dalam layanan farmasi di raumah sakit. Untuk mencapai target diperlukan sistim inovasi layanan bertransfomasi yaitu sistim Electronic Presciption Check-in Information Monitor Evaluation Delivery (ECIMED), suatu sistim layanan famasi yang terintegrasi dengan pelayanan rawat jalan serta pihak asuransi kesehatan sehingga terjadi percepatan waktu tunggu dan memberikan peningkatan kepuasan kepada pelanggan. Tujuan penelitian ini mengetahui implementasi dari sistim ECIMED beserta mengukur hasil capaian dan mengetahui faktor-faktor pengaruhnya. Penelitian ini adalah penelitian implementasi, Generic Implementation Framework (GIF) dengan menggunakan design mix-methode. Hasil didapatkan dengan jumlah resep pada triwulan III 2024 meningkat 11,4% dibandingkan dengan triwulan yang sama di tahun 2023, namun waktu tunggu rata-rata terjadi percepatan dari 1 jam 17 detik menjadi 40 menit 12 detik untuk obat non racikan dan untuk obat racikan dari 1 jam 39 menit 9 detik menjadi 42 menit 19 detik. Capaian indeks kepuasan pada bulan September 2024 sebesar 91,13, angka tersebut menunjukkan terdapat peningkatan 5,6% dibandingkan tahun 2023 di bulan yang sama. Simpulan, meskipun terjadi peningkatan jumlah resep dari tahun ke tahun, namun waktu tunggu semakin cepat. sebagai faktor pendukung keberadaan tim pelayanan, aplikasi elektronik sesuai kebutuhan, informasi proses dan antar obat, monitoring evaluasi dan sebagai faktor penghambat, gangguan teknis dan karakter pasien. Sistim ECIMED sebagai inovasi proses sangat bisa digunakan dalam rangka upaya peningkatan kualitas layanan farmasi.

Abstract

Employee job satisfaction is a crucial aspect in the health sector, which directly impacts the quality of patient care. In hospitals, job satisfaction is often affected by key factors such as leadership style and workload. According to data from the Central Statistics Agency of 2021, job satisfaction in the health sector is lower than in other sectors, with only 65% employees expressing satisfaction with their work. This study aims to analyze the effect of participative leadership style and workload on employee job satisfaction in the outpatient unit of Gresik Petrokimia Hospital. This current research employed a quantitative method with a cross-sectional design. Data were collected using a questionnaire distributed to 41 respondents, and then analyzed using multivariate multiple linear regression with SPSS version 25. The findings revealed that the participative leadership style had no significant effect on job satisfaction. However, workload partially had a significant effect. When assessed together, both participative leadership style and workload demonstrate a statistically significant influence on job satisfaction.

Abstract

Background: The research explores the advancements and implications of e-prescribing systems within modern healthcare.Objective: The primary objective is to assess the growing body of literature, examining global trends, effectiveness, and challenges associated with e-prescribing implementation.Methods: Using a comprehensive literature review methodology, data were sourced from the Scopus database, revealing many publications from 1980 to 2024.Results: A notable increase in scholarly interest was observed, particularly from 2015 to 2024, highlighting e-prescribing as a mature research subject. The results indicate a high concentration of research activity in advanced healthcare systems, contrasting with the limited contribution from developing contexts such as Indonesia.Conclusion: The study identifies that while e-prescribing improves medication safety and efficiency, gaps in understanding its long-term impacts on patient outcomes persist. Additionally, disparities in technology adoption among demographic groups emerge as significant barriers. This research contributes to existing knowledge by underscoring the need for localized studies and international collaboration to enhance e-prescribing practices. Future research directions should focus on evaluating long-term outcomes, improving digital literacy, and assessing training impacts on healthcare professionals.
