Abstract

Hypertension is a major cause of morbidity and mortality worldwide, and its prevalence increases with age. Hypertension is a chronic condition characterised by increased arterial blood pressure. In Indonesia, the prevalence of hypertension ranges from 45.9% in people aged 55-64 years to 63.8% in people aged 75 years and over. Hypertension is known as the "silent killer" because it often appears without symptoms and requires early detection to prevent complications such as heart disease, stroke and kidney failure. This activity uses the PDCA (Plan-Do-Check-Act) method for continuous improvement in the health examination process, including blood pressure measurement. 61 participants (14 men and 47 women) participated in this activity at the Hana Nursing Home in South Tangerang. A physical examination revealed that 11 people (18.03%) had hypertension. Regular monitoring is essential to identify high-risk individuals early, allowing them to take preventive measures and manage their blood pressure effectively. This approach can significantly reduce the risk of serious complications and improve the quality of life for seniors.

Abstract

Sarcopenia is a medical condition characterized by a progressive decrease in skeletal muscle mass as well as muscle strength. Sarcopenia in the elderly causes an increased risk of falls, fractures and mortality. The main risk factors include advanced age, lack of physical activity, malnutrition, and chronic disease. Deficiency of albumin, a protein important for maintaining osmotic pressure and nutritional status, is often associated with malnutrition and sarkopenia. This community service activity was attended by 93 elderly people with an average age of 74 years. The results showed that the majority of participants had normal albumin levels, but there was a significant proportion who experienced hypoalbuminemia, more commonly found in men than women. Education and screening are carried out to increase awareness regarding the importance of albumin intake in preventing sarkopenia. A holistic and integrated nutritional approach is needed in the management of sarcopenia. Appropriate nutritional interventions, including increasing protein intake from foods such as snakehead fish, can increase serum albumin levels and prevent muscle mass loss. Collaboration between nutritionists, physicians, and other health professionals is essential to designing and implementing a comprehensive treatment plan. It is hoped that with appropriate monitoring and treatment, the quality of life of elderly people with sarkopenia can be improved. Keywords: albumin, education, hypoalbuminemia, elderly, sarcopenia

Abstract

Ultraviolet (UV) radiation, especially UVB and UVA, plays an important role in the pathogenesis of skin damage. Excessive sun exposure causes various skin problems, such as burning, dehydration, and premature aging, and increases the risk of skin cancer. Precautions such as using sunscreen, protective clothing, and avoiding peak UV exposure times are essential. Using the PDCA (Plan-Do-Check-Act) cycle, we carry out facial skin health checks, including education about sun protection, identification of skin problems, and follow-up interventions. 33 participants (11 men and 22 women) participated in this activity. It was found that the average wrinkle, roughness, moist, and UV damage were 17%, 20%, 46%, and 22.18% respectively. Early counseling and screening is essential to increase awareness, change behavior, and prevent skin problems, ultimately improving skin health and improving quality of life.

Abstract

Insomnia affects 30-50% of the adult population in the United States, with 5-15% experiencing chronic symptoms. Insomnia is characterized by dissatisfaction with the quality or quantity of sleep, insomnia presents challenges such as difficulty initiating, maintaining, and waking early, and is common in older adults. The Hana Nursing Home hosted this activity, with 61 participants in attendance. In this activity, there were 9 elderly people with mild and moderate insomnia respectively (14.75%) and 5 people (8.2%). Therefore, comprehensive clinical evaluation in the form of education and early detection is essential for effective clinical management of insomnia. Keywords: Insomnia, Education, Early detection

Abstract

Nutritional status is an important determinant of overall health and well-being, having a major influence on growth, development, and overall health outcomes. The prevalence of stunting, wasting, and underweight among children in low- and middle-income countries around the world decreased by 29.1%, 6.3%, and 13.7% respectively between 2006 and 2018. For this investigation, PDCA (Plan-Do-Check-Action Method) is used. A total of 165 people participated in a community outreach initiative conducted in March 2024 at an elementary school located in West Java, Indonesia. Early detection of nutritional status is carried out by taking anthropometric measurements. Nutrition during childhood and adolescence is critical for optimal development, growth, and long-term health. Rapid assessment of nutritional status will facilitate timely interventions aimed at preventing disease and reducing premature mortality

Abstract

Latar Belakang: Kekuatan genggam tangan (Handgrip Strength/HGS) merupakan indikator kesehatan fisik secara umum pada lansia. HGS mencerminkan keseimbangan antara massa otot dan lemak serta membantu dalam diagnosis sarkopenia. Memahami hubungan antara ukuran antropometri, komposisi tubuh, dan HGS penting untuk mengatasi penurunan kesehatan otot akibat penuaan. Tujuan: Studi ini mengevaluasi korelasi antara parameter antropometri sederhana, komposisi tubuh, dan HGS pada lansia, guna mendukung deteksi dan intervensi dini sarkopenia. Metode: Studi potong lintang dilakukan pada 31 lansia (≥60 tahun) di Panti Wreda Santa Anna tahun 2024. Pengukuran antropometri mencakup lingkar pinggang, pinggul, betis, leher, dan lengan atas. Komposisi tubuh seperti distribusi lemak dan otot rangka dianalisis menggunakan Omron Karada Scan HBF 375. HGS diukur dengan dinamometer terkalibrasi. Uji korelasi Spearman’s Rho digunakan dengan signifikansi p<0,05. Hasil: Terdapat korelasi signifikan antara HGS dan beberapa parameter, terutama berat badan, tinggi badan, lingkar betis, lemak viseral, dan indeks otot rangka. Otot rangka tungkai menunjukkan korelasi kuat dengan HGS (r=0,653; p<0,001). Hasil ini menegaskan keterkaitan antara kesehatan otot, komposisi tubuh, dan perubahan terkait usia. Kesimpulan: HGS, yang dipengaruhi oleh parameter antropometri dan komposisi tubuh, merupakan indikator yang andal untuk sarkopenia pada lansia. Intervensi yang menargetkan faktor-faktor ini dapat meningkatkan fungsi otot dan kualitas hidup lansia.

Abstract

ABSTRACT Urinary tract infection (UTI) poses a serious concern in the elderly population, with approximately 25% of total infection cases in the geriatric group being UTIs. Diabetes mellitus stands as a leading cause of global morbidity and is associated with severe complications, including an increased risk of infections impacting the quality of life. Patients with diabetes face a twofold higher risk of mortality due to infections. Advanced age is also a risk factor for UTIs, influenced by factors such as uncontrolled diabetes. Measuring parameters such as fasting insulin and HbA1c levels is crucial for understanding correlation between diabetes and UTIs. HbA1c can reflect long-term glycemic control in diabetic patients. This cross-sectional study aims to investigate relationship between HbA1c and fasting insulin levels concerning the occurrence of UTIs in the elderly population, conducted at the Santa Anna Nursing Home in 2023. The study included 33 respondents with an average age of 72.88 years, predominantly comprising 24 female respondents (72.7%). The mean fasting insulin level was 12.46 mIU/mL, and HbA1c level was 5.97%. UTIs were diagnosed in 11 respondents (33.3%). The correlation analysis between these variables indicated no significant difference in the mean HbA1c levels between groups with or without UTIs (p = 0.955). However, a significant difference was observed in the mean fasting insulin levels between the two groups (p < 0.001). This finding is noteworthy as fasting insulin levels have a direct correlation with circulating blood glucose levels and formation of red blood cells, potentially influencing HbA1c levels. Further clarification is needed on how these three variables interact. Keywords: Elderly, Fasting Insulin, HbA1c, Urinary Tract Infection  ABSTRAK Infeksi saluran kemih (ISK) adalah masalah serius pada populasi lanjut usia, sekitar 25% dari total kasus infeksi pada kelompok geriatri adalah ISK. Diabetes melitus adalah penyebab utama morbiditas global dan berhubungan dengan komplikasi serius, termasuk risiko infeksi yang berdampak pada kualitas hidup. Pasien diabetes memiliki risiko dua kali lipat lebih tinggi untuk kematian akibat infeksi. Peningkatan usia juga merupakan faktor risiko untuk ISK, yang dipengaruhi oleh faktor-faktor seperti diabetes yang tidak terkontrol. Pengukuran parameter seperti insulin puasa dan HbA1c adalah kunci dalam memahami korelasi antara diabetes dan ISK. HbA1c dapat mencerminkan kontrol glikemik jangka panjang pada pasien diabetes. Penelitian potong lintang ini bertujuan untuk mengetahui hubungan antara kadar HbA1c dan insulin puasa terhadap kejadian ISK pada kelompok lanjut usia yang dilakukan di Panti Lansia Santa Anna pada tahun 2023. Penelitian mengikutsertakan 33 responden dengan rata-rata usia 72,88 tahun dan didominasi oleh jenis kelamin perempuan  sebanyak 24 responden (72,7%). Rerata kadar insulin puasa sebesar 12,46 mIU/mL dan kadar HbA1c sebanyak 5,97%. Sebanyak 11 responden (33,3%) terdiagnosis ISK. Hasil korelasi antara kedua variabel tersebut menunjukkan bahwa tidak ada perbedaan signifikan dalam rerata kadar HbA1c antara kelompok dengan atau tanpa ISK (p = 0,955). Meskipun demikian, terdapat perbedaan signifikan dalam rerata kadar insulin puasa antara kedua kelompok (p < 0,001). Hal ini menjadi sebuah pertimbangan karena kadar insulin puasa memiliki korelasi langsung dengan jumlah gula yang beredar bebas dalam darah dan pembentukan sel darah merah yang dapat meningkatkan tingkat HbA1c. Abstract

ABSTRACT The research on vitamin D is still developing until its role in the immune system, the absorption of phosphate from the intestine, and preventing its excretion in the kidneys are known. The prevalence of low levels of vitamin D is high, and recently, it was discovered that there was a relationship between vitamin D deficiency and the incidence of sarcopenia in the elderly. Determining vitamin D levels and the incidence of sarcopenia in the elderly at the Santa Anna Nursing Home. This study was an analytical study with a cross-sectional design conducted on elderly residents of the Santa Anna Nursing Home in July 2023 to assess vitamin D levels and calf circumference to assess sarcopenia. The data obtained were analyzed using Mann-Whitney. Result Of the 32 respondents, the average age was 72.66 years, and 22 (68.7%) respondents were women. It was found that 30 (93.8%) respondents experienced vitamin D deficiency, and 15 (46.9%) respondents experienced sarcopenia. The results of statistical tests showed that there was no difference in the mean levels of vitamin D between the sarcopenic and non-sarcopenic groups. However, the group of patients with sarcopenia had vitamin D levels that tended to be lower compared to the group without sarcopenia (18 vs. 21 ng/mL; MR 15.50 vs. 17.38). There was no statistically significant difference in mean vitamin D levels between the groups with and without sarcopenia. However, there is a tendency for groups of patients with sarcopenia to have vitamin D levels that tend to be lower than those in groups without sarcopenia. Keywords: Vitamin D, Sarcopenia, Elderly  ABSTRAK Hingga saat ini, penelitian terhadap vitamin D terus berkembang hingga diketahui perannya dalam sistem imun, absorpsi fosfat dari usus dan mencegah terekskresinya pada ginjal. Tingginya prevalensi kurangnya kadar vitamin D dan baru-baru ini, diketahui ada hubungan defisiensi vitamin D dengan kejadian sarkopenia pada lansia. Mengetahui kadar vitamin D dengan kejadian sarkopenia pada lansia di Panti Werda Santa Anna. Penelitian ini adalah analitik dengan desain potong lintang yang dilakukan pada lansia penghuni Panti Werdha Santa Anna pada Juli 2023 untuk menilai kadar vitamin D dan lingkar betis untuk menilai sarkopenia. Data yang didapat kemunian dianalisis menggunakan Mann-Whitney. Didapat 32 responden dengan rerata usia 72,66 tahun dan sebanyak 22 (68,7%) responden adalah perempuan. Didapat 30 (93,8%) responden mengalami defisiensi vitamin D dan 15 (46,9%) responden mengalami sarkopenia. Hasil uji statistic didapat hasil tidak terdapat perbedaan rerata kadar vitamin D antara kelompok sarkopenia dan tidak. Namun dapat terlihat bahwa kelompok pasien dengan sarkopenia memiliki kadar vitamin D yang cenderung lebih rendah dibandingan dengan kelompok tanpa sarkopenia (18 vs 21 ng/mL; MR 15,50 vs 17,38). Tidak terdapat perbedaan rerata kadar vitamin D yang bermakna secara statistik antara kelompok dengan atau tanpa sarkopenia. Namun terdapat kecenderungan kelompok pasien dengan sarkopenia memiliki kadar vitamin D yang cenderung lebih rendah dibandingan dengan kelompok tanpa sarkopenia. Kata Kunci: Vitamin D, Sarkopenia, Lanjut Usia

Abstract

Malnutrition significantly disrupts metabolic processes like fasting blood glucose (FBG) regulation, insulin secretion, and insulin resistance (HOMA-IR), especially among the elderly. Aging amplifies these effects through declining β-cell function, reduced insulin sensitivity, and chronic inflammation, increasing the risk of type 2 diabetes and cardiovascular diseases. Globally, malnutrition affects 22% of the elderly, with prevalence reaching 28% in Indonesia due to food insecurity, limited healthcare access, and poor nutrition education. Malnutrition exacerbates glucose-insulin imbalances by causing muscle loss, increasing FBG levels, and inducing insulin resistance through chronic inflammation. Despite its impact, critical knowledge gaps remain, particularly in low- and middle-income countries like Indonesia. This study investigates how malnutrition affects FBG, insulin levels, and HOMA-IR in elderly individuals, focusing on mechanisms like inflammation and muscle loss to inform targeted interventions and improve metabolic health in resource-limited settings. This cross-sectional study evaluated fasting blood glucose, insulin, and HOMA-IR among 31 elderly participants at Panti Werdha Santa Anna. The study found no significant differences in fasting blood glucose, fasting insulin, HOMA-IR, or HbA1c levels across Mini Nutritional Assessment categories (normal, at risk, malnourished; p > 0.05). These results suggest no strong association between nutritional status and metabolic parameters, highlighting the need for further research with larger samples. This study finds no significant association between nutritional status and glycemic control in the elderly, suggesting malnutrition's indirect impact via mechanisms like inflammation and muscle loss. Comprehensive nutritional assessments remain essential to understanding its effects on metabolic health.
