Abstract

The COVID-19 pandemic poses the challenge ofining a healthy and varied diet and regular physical activity. This is because people prefer to buy processed foods, such as snacks, fast food, and ready-to-eat cereals, which tend to be high in fat, sugar, and salt. In addition, psychological and emotional responses to the outbreak of COVID-19 can increase the risk of developing dysfunctional eating behavior. The choice of foods and how often they are eaten can affect the health of the body, teeth, and gums. Excessive consumption of sugar-containing soda, fruit drinks with added sugar, or low-nutritional snacks can increase the risk of teeth hole. Therefore, a community service program (PkM) was designed with the aim of providing education and training to the community to gain knowledge about diet and improve health. The development and training of a healthy diet to the Lisiuex Apartment Meditarenia 1 West Jakarta community was conducted online, illustrating the improvement of the participants' knowledge and yielding good results. Sustainable education and training programmes are needed as a means of improving the development of health that starts with the family.

Abstract

Background: The high level of public awareness regarding aesthetic values influences the evolution and selection of restoration materials. Nanofiller composite resin is an option in aesthetic restoration because it has good aesthetic qualities, mechanical properties and high wear resistance. Color changes that occur in nanofiller composite resin are undesirable because they disturb the aesthetic value and appearance. Color changes in restoration materials are influenced by the acidic pH contained in artificial saliva. Objective: This study aims to determine the effect of artificial saliva with a pH of 5 on the color change of nanofiller composite resin restoration materials. Methods: The research object is a nanofiller composite resin with a diameter of 10 mm, and a thickness of 2mm. The research was carried out by soaking nanofiller composite resin in artificial saliva and distilled water solution. The study was divided into 2 groups, namely the treatment group and the control group. Which is placed in an incubator at a temperature of 37˚ C for 24 hours. In the control group, samples were soaked in distilled water for 24 hours, and repeated for 7 days. In the treatment group, samples were soaked in artificial saliva for 4 hours, repeated for 7 days, followed by immersion in distilled water. The color parameters of the research subjects were measured using a Vita Easyshade V spectrophotometer before and after immersion in the control and treatment groups on the 7th day. The measurement results data after carrying out the normality test, T-test (dependent), and Wilcoxon test. Results: The results of this study showed a significant color change (p<0.05). Conclusion: There is an effect of artificial saliva with a pH of 5 on the color change of the nanofiller composite resin material.

Abstract

ABSTRACTIntroduction: Dental hard tissue problems were relatively high in Indonesia. If these problems were ignored, they would cause pulpal necrosis. The main treatment for this case was Root Canal Treatment (RCT). The selection of post-RCT treatment and suitable materials was important. One of the examples was short fiber reinforced composite (SFRC) which can be an option for intracanal retention treatment. This study aims to analyze the resistance of the intracanal retention thickness with flowable fiber reinforced composite materials toward fracture toughness. Methods: This research was using a correlative laboratory experimental method with a post-test group-only design. This study's samples were SFRC material inserted in 27 extracted mandibular first premolars with a length of 21 mm ± 2 mm and root canal walls width of 2 mm. All samples were given intracanal retention at a depth of 2 mm below the orifice and divided into 3 groups: intracanal retention with thickness of (A) 6 mm, (B) 5 mm, and (C) 4 mm. RCT was performed using the crown-down technique. The bonding agent was applied, and then light cured for 20 seconds. SFRC was applied according to the sample group and light cured for 20 seconds per 2 mm, followed by filling with composite resin. The sample was implanted in the dental stone 2 mm below the CEJ. The fracture toughness test was performed using UTM on the occlusal surface with a 0o tilt and crosshead of 1 mm/min. Results: The fracture toughness results of group A, B, and C are 735.44, 756.78, and 829 respectively. Statistical tests with one-way ANOVA showed no significant difference with p value = 0.412 (p < 0.05). Conclusion: There is no difference in resistance of the intracanal retention thickness with flowable fiber reinforced composite materials towards fracture toughness.Keywords: post-RCT treatments, intracanal retentions, SFRC, fracture toughness

Abstract

Objective: This study aims to investigate the effectiveness of ultrasonic tip with and without solvent in removing gutta-perca and bioceramic sealer from the middle and apical third of the root canals. Methods: This is a true experimental study with randomized pretest-posttest control group design. Eighteen freshly extracted straight single-rooted mandibular first premolars were confirmed by CBCT and selected. The teeth were decoronated until a length of 16 ± 0.5mm. The teeth were prepared and obturated using gutta-percha and bioceramic sealer with a hydraulic condensation technique. Pre-instrumentation CBCT imaging of all teeth was carried out to obtain the initial volume of the obturation material. After incubating for 30 days, the treated teeth were randomly divided into three groups (n = 6): ultrasonic and xylene, ultrasonic and orange oil, and ultrasonic only. Post-instrumentation CBCT was carried out and the amount of obturation material removed was calculated using CT Analyser. The two-way ANOVA was used to determine the solvent and location interaction. Results: The results showed that there was no significant difference among the groups based on the treatment and location (p > 0.05). In addition, no significant interaction was found in relation to the cleanliness of the root canal (p > 0.05). Conclusion: There was no difference in the effectiveness of ultrasonic obturation material cleaning with and without endodontic solvents. Based on the results, the obturation material with the most residue is a sealer.

Abstract

Introduction: Composite resin is highly valued for its aesthetic appeal, as its colour closely resembles that of natural teeth. Nanofilled composite resins, in particular, offer impressive colour stability. However, restorative materials can undergo colour changes due to intrinsic and extrinsic factors. One such extrinsic factor is fruit pigments, like those found in pomegranates. This study aims to analyze the colour stability of nanofilled composite resin after immersion in pomegranate juice. Methods: This laboratory experiment utilized a pre-test and post-test control group design. A total sample of forty nanofilled composite resins (Filtek Z350 XT), each with a diameter of 10 mm and a thickness of 2 mm, was divided into four groups. Each sample in the two treatment groups was immersed in 10 mL of commercial pomegranate juice for one day and seven days, respectively. Meanwhile, each sample in the two control groups was immersed in 10 mL of distilled water for the same duration. Colour measurements were carried out before and after immersion using a spectrophotometer. The data were statistically analyzed using the Shapiro-Wilk normality test to assess distribution. For normally distributed data, a one-way ANOVA was conducted, followed by the Tukey HSD post-hoc test for significant differences. For non-normally distributed data, the Kruskal-Wallis test was applied, followed by the Mann-Whitney post-hoc test if significant differences were observed. Results: The one-way ANOVA test for the L parameter yielded a p-value of 0.067 (p>0.05), indicating no significant changes after immersion in both pomegranate juice and distilled water. The Kruskal-Wallis test yielded p-values of 0.000 for E, 0.001 for C, and 0.000 for H (p<0.05), demonstrating substantial differences after immersion in both pomegranate juice and distilled water. Conclusion: The nanofilled composite resin exhibited minimal discolouration after immersion in pomegranate juice, as evidenced by changes in E, C, and H. No significant change was observed in L.

Abstract

Dental caries is a significant health issue in Indonesia, particularly among preschool children, due to a lack of knowledge and habits in maintaining dental hygiene. Schools serve as strategic venues to promote dental health through activities such as tooth-brushing programs. Teachers play a crucial role in setting examples and guiding children in practicing routine oral hygiene. A health education program at Santa Maria Juanda Kindergarten aims to enhance awareness and understanding of oral health among teachers and students. Based on the pretest and posttest results, this results demonstrate an increase in knowledge by 12.69%, and participants are expected to be able to use the knowledge they have acquired in their everyday lives.

Abstract

Introduction: Dental caries is often associated with bacterial pathogens such as Aggregatibacter actinomycetemcomitans, which lower the oral pH and contribute to tooth damage. Calcium hydroxide (Ca(OH)2) is commonly used in vital pulp therapy due to its antibacterial properties; however, it may cause adverse effects, including pulp necrosis. As a natural alternative, tobacco leaf extract (Nicotiana tabacum) extract has shown potential due to its content of antibacterial compounds such as flavonoids and terpenoids. This study aims to analyse the antibacterial effect of Nicotiana tabacum leaf as an antibacterial agent against A. actinomycetemomitans. Methods: An in-vitro experimental laboratory research with a post-test-only control group design. The antibacterial activity was tested using the plate count method on samples from seven different groups. The test was conducted using the microdilution method in 96-well plates, followed by the plate count method for bacterial enumeration. The test samples included Nicotiana tabacum leaf extract at concentrations of 3.125%, 6.25%, 12,5%, 25%, and 50%, with calcium hydroxide serving as the positive control and distilled water as the negative control. The data were analysed using one-way ANOVA followed by Tukey’ HSD post hoc test. Results: The minimum inhibitory concentration (MIC) of the extract against Aggregatibacter actinomycetemcomitans of 12.5% while a concentration of 25% represented the minimum bactericidal concentration (MBC). The 25% concentration exhibited antibacterial activity comparable to that of calcium hydroxide. The Tukey HSD post hoc test revealed significant differences between the negative control and the Nicotiana tabacum leaf extract at concentrations of 6.25% (p=0.025), 12.5% (p=0.001), 25% (p=0.001), and 50% (p=0.001). This study aims to analyse the antibacterial effect of Nicotiana tabacum leaf as an antibacterial agent against A. actinomycetemomitans. Conclusion: Nicotiana tabacum leaf extract exhibits antibacterial activity against Aggregatibacter actinomycetemcomitans, with an MIC of 12.5% and an MBC of 25%.

Abstract

Perawatan saluran akar J-shaped pada premolar pertama mandibula: laporan kasus ABSTRAKPendahuluan: Klinisi harus waspada dengan adanya kelainan pada anatomi internal saluran akar. Pemilihan instrumentasi yang tepat diperlukan untuk preparasi saluran akar melengkung. Kesalahan iatrogenik pada saluran akar melengkung dapat terjadi seperti ledge, separasi instrumen, penyumbatan saluran akar, perforasi, dan transportasi apikal tear-drop. Laporan kasus ini bertujuan untuk menyajikan perawatan endodontik dengan bentuk anatomi saluran akar berbentuk J-shaped. Laporan kasus: Pasien perempuan berusia 52 tahun memiliki keluhan tidak nyaman saat makan. Gigi #34 didiagnosis sebagai nekrosis pulpa. Radiografi awal mengungkapkan lengkungan akar (J-shaped). Preparasi saluran akar dimulai dengan file #8 yang telah dilengkungkan sebagai file awal dan dilanjutkan dengan file rotari gold NiTi (M3-Pro Gold, UDG, Cina) hingga #25/.06 mengikuti kelengkungan saluran akar. Saluran akar J-Shaped dalam klasifikasi Schneider menunjukkan severly curve (29°). Manajemennya dimulai dengan akses lurus ke saluran akar, preparasi dengan k-file kecil dan precurved #08, #10, #15 dengan gerakan push and pull, dilanjutkan dengan file rotary berurutan, bahan kelasi (Ethylenediaminetetraacetic acid/EDTA) digunakan, dan irigasi berulang dengan Natrium hipoklorit (NaOCl). Apikal patensi dilakukan selama cleaning dan shaping untuk menjaga panjang kerja dan mencegah penyumbatan saluran akar. Simpulan: Variasi anatomi saluran akar, teknik instrumentasi, dan obturasi saluran akar penting dipahami guna mencapai keberhasilan perawatan saluran akar pada saluran akar J-Shaped. Kata kunciJ-shaped canal, perawatan saluran akar, glide path, gold nickel-titanium Root canal treatment of a J-shaped canal mandibular first premolar: a case report ABSTRACTIntroduction: Clinicians should be aware of internal anatomical variations. Appropriate selection of instrumentation is essential for preparing curved canals. Iatrogenic errors in curved canals may occur, such as ledge formation, instrument separation, canal blockage, perforation, and tear-shaped apical transportation. The aim of this case report is to present the challenging endodontic management of a mandibular first premolar with a J-shaped canal. Case report: A 52 year-old female patient presented with discomfort while eating. Tooth #34 was diagnosed with pulp necrosis. A pre-operative radiograph revealed root curvature consistent with a J-shaped configuration. Root canal preparation was initiated with precurved file #8 (Dentsply, Switzerland) as initial file, followed by gold NiTi rotary instrumentation (M3-Pro Gold, UDG, China) up to #25/.06, respecting the canal curvature. According to Schneider’s classification, the J-shaped canal showed a severe curvature (29°). Management began with straight access into the canal then prepared with small and precurved k-file #08, #10, #15 with push and pull motion. This was followed by rotary file sequencing. A chelating agent (EDTA) and copious irrigation with sodium hypochlorite were employed. Apical patency was maintained throughout cleaning and shaping to preserve the working length and prevent canal blockage. Conclusion: Knowledge of root canal anatomy, together with appropriate instrumentation materials and techniques, is crucial to achieving successful root canal treatment.  KeywordsJ-shaped canal, endodontic treatment, glide path, gold nickel-titanium

Abstract

Vital pulp therapy (VPT) is a crucial dental intervention designed to maintain the vitality of the dental pulp, particularly in carious lesions or trauma. The evolution of biomaterials in VPT has significantly affected dental practices by providing less invasive alternatives that safeguard tooth vitality and functionality. Continued exploration of the properties and effects of these materials is crucial for enhancing treatment strategies and optimizing dental care outcomes. Bibliometric analysis allowed the assessment of trends in the biomaterials used for VPT. Data mining was performed using the Scopus database and Boolean expressions. Data extraction and analysis were conducted using VOSviewer version 1.6.20., VOSviewer thesaurus, and Microsoft Excel. A total of 856 documents were identified, and the United States had the largest number of documents and citations (110 documents, 6,938 citations). The top three sources were the Journal of Endodontics, International Journal of Endodontics, and Dental Materials Journal. Mineral trioxide aggregate (MTA) is still the dominant biomaterial used in VPT to this day. MTA can accelerate dentinogenesis in VPT. The prominence of “MTA” and “VPT” as keywords highlights research on prevalence, causes, prevention, and evaluation, emphasizing the need to investigate biomaterial-driven healing, regeneration, stem cell activity, and gene expression.

Abstract

ABSTRAKPendahuluan: Lesi periapikal yang besar, terlepas dari apakah itu granuloma, abses atau kista, utamanya disebabkan oleh infeksi saluran akar, dan oleh karena itu protokol perawatannya adalah eliminasi faktor etiologi dalam sistem saluran akar. Tujuan laporan kasus ini menjelaskan perawatan  endodontik non-bedah pada cyst-like periapical lesion pasca trauma pada gigi insisivus rahang atas. Laporan kasus: Pasien laki-laki berusia 39 tahun datang dengan keluhan utama diskolorasi gigi 11 dan nyeri perkusi pada gigi 12, 11, 21, dan 22. Pemeriksaan radiografis menunjukkan lesi periapikal mirip kista yang besar pada regio anterior rahang atas. Perawatan endodontik dilakukan dalam empat kali kunjungan dengan menggunakan kalsium hidroksida sebagai medikamen intrakanal antar kunjungan. Pengisian saluran akar menggunakan sealer berbahan resin dan kemudian dilakukan restorasi sementara resin komposit. Penyembuhan lesi diamati pada follow-up 9 bulan dengan pemeriksaan radiograf. Cyst-like periapical lesion yang besar telah berhasil mengecil setelah terapi endodontik non-bedah, namun pilihan perawatan ini memerlukan beberapa kali kunjungan untuk aplikasi medikamen dalam saluran akar sebelum pengisian saluran akar. Kalsium hidroksida memainkan peran penting karena memiliki sifat anti-bakteri yang sangat efektif dan kemampuan khusus memperbaiki jaringan dengan pembentukan jaringan keras. Simpulan: Perawatan endodontik non-bedah menggunakan medikamen kalsium hidroksida dalam laporan kasus ini terbukti berhasil dalam penyembuhan cyst-like periapical lesion pasca trauma pada gigi rahang atas.Kata kunci: kista periapikal, cyst-like periapical lesion, endodontik, perawatan saluran akar, kalsium hidroksida Non-surgical endodontic approach for periapical cyst-like lesion post-trauma in maxillary incisors: a case reportABSTRACTIntroduction: Large periapical lesions, whether they are granulomas, abscesses, or cysts, are primarily caused by root canal infections, and thus, the treatment protocol should focus on removing etiological factors from the root canal system. This case report describes the non-surgical management of a large periapical cyst-like lesion in a patient with a history of a traumatic accident in the maxillary anterior area. Case report: A 39-year-old male patient presented with the chief complaint of a discolored tooth (#11) and pain on percussion in teeth 12, 11, 21, and 22. Radiographic examinations revealed a large periapical cyst-like in the maxillary anterior region. Endodontic treatment was performed in four visits using interappointment calcium hydroxide intracanal medicament. The canal was obturated using resin-based sealer and a definitive restoration was then placed. Radiographic evidence of lesion healing was observed at the 9-month follow-up. The large cyst-like apical periodontitis lesions have been demonstrated to regress to smaller sizes after non-surgical endodontic therapy. Nevertheless, this treatment option requires multiple visits for intracanal medicament applications before permanent root canal filling. Calcium hydroxide is crucial in such procedures because it has highly effective anti-bacterial properties as well as the unique ability of tissue repair by hard tissue formation. Conclusion: In this case report, non-surgical root canal treatment along with calcium hydroxide intracanal medicament proved successful in promoting the healing of a large periapical cyst-like lesion.Keyword:periapical cyst, cyst-like periapical lesion, endodontic, root canal therapy, calcium hydroxide
