Abstract

Background: Spontaneous intracerebral hemorrhage or hemorrhagic stroke is one of the leading causes of mortality and disability in Indonesia, but until now there is no specific therapy for this disease. The intracerebral hemorrhage (ICH) score is a widely used predictive tool for the prognosis of death 30 days after spontaneous intracerebral hemorrhage, but the intracerebral hemorrhage-grading scale (ICH-GS) score has a more specific interval to assess the prediction of the prognosis after intracerebral hemorrhage. Rumah Sakit Islam (RSI) Jemursari Surabaya has not carried out data collection related to the ICH-GS score with the outcome (death) of patients with spontaneous intracerebral hemorrhage, especially while still receiving hospital treatment. Objective: To identify the number of ICH-GS scores in patients with spontaneous intracerebral hemorrhage as a predictor of prognosis at RSI Jemursari Surabaya. Method: The type of this research is retrospective research. The population comprised of all patients with spontaneous intracerebral hemorrhage hospitalized at RSI Jemursari Surabaya in 2017-2019, with affordable population of all patients with spontaneous intracerebral hemorrhage diagnosed by a neurologist. Results: The data showed that 5.5% (6 people) had an ICH-GS score of 5; 38.2% (42 people) had an ICG-GS score of 6; 21.8% (24 people) had an ICH-GS score of 7; 20 % (22 people) had an ICH-GS score of 8; 5.5% (6 people) had an ICH-GS score of 9; 4.5% (5 people) had an ICH-GS score of 10; 3.6% (4 people) had an ICH-GS score of 11; and 0.9% (1 patient) had an ICH-GS score of 12. Conclusions: The results of the ICH-GS score can be used to facilitate communication both with fellow health workers and with the patient's family. Trend of the data showed that ICH-GS score is not consistent in showing the prognosis of spontaneous intracerebral hemorrhage (the smaller the ICH-GS score should have better prognosis than the higher score), while the information based only on volume and location can predict the prognosis more consistently`.

Abstract

ABSTRACT Early detection of stroke is an effort to prevent more severe disability in stroke patients. Delays in bringing stroke patients to the hospital are one of the reasons why patients miss the golden period of stroke treatment. Health cadres play an important role in identifying health problems in the community, but most cadres do not understand how to carry out early stroke detection. To increase the knowledge and ability of cadres in detecting strokes so that treatment does not exceed the golden period of strokes. The method used in this community service activity is a participative educational approach, including providing health education and demonstrations regarding early stroke detection. This activity was conducted in April 2023 and was attended by 35 health cadres. Knowledge and ability to detect stroke are evaluated used knowledge questionnaire instruments and observation sheets. The results of community service show that the knowledge and ability to perform stroke detection among cadres has increased after training. Cadres' knowledge and ability to detect stroke in the health community can be improved through early stroke detection training. It is hoped that trained health cadres can provide health education to other residents to increase residents' knowledge about stroke and when is the right time to take patients to hospital. Keywords: Early Detection, Health Cadres, Non-Communicable Disease, Stroke

Abstract

Family as an informal caregiver has a very important role in the care of stroke patients. Families do not have sufficient knowledge and skills in caring for stroke patients because they are not trained. Families actively involved in care will feel more prepared to face new responsibilities in caring for stroke patients. This community service activity aims to improve knowledge and behavior to prevent recurrent stroke. The method used in this community service activity is a participatory education approach, including educational methods using the stroke self-management education module. This activity was conducted in May 2024 at RW 07, Banyu Urip Village, Surabaya. It was attended by 20 families of stroke patients. Evaluation of knowledge using the recurrent stroke prevention knowledge questionnaire and behavior using the stroke patient care behavior questionnaire. The community service results showed that family caregivers' knowledge and behavior increased after the family empowerment program. Family empowerment can improve knowledge and behavior in family caregivers of stroke patients in preventing recurrent stroke. Family empowerment programs can be provided periodically by community nurses to improve the ability of family caregivers to improve the quality of life of stroke patients.
