Abstract

Background: Young permanent teeth are one of the challenges in performing endodontic treatment. Revascularization is a good yet simple treatment option for pulp tissue regeneration to stimulate periapical lesion healing, continue root growth and restore pulp tissue function. Purpose: This case report describes management of pulp tissue regeneration in teeth with infection. Case: Female, 16 years old, came complaining of pain in the left lower back tooth accompanied by recurrent swelling. Radiograph showed an open apex with apical lesion on tooth 35. The diagnosis was pulpal necrosis accompanied by a chronic apical abscess. Case management: The treatment plan was revascularization and direct composite restoration. After accessing the cavity, the teeth were prepared, irrigated, and medicated with TAP, which was repeated for 2 visits. On the next visit, after the root canal was cleaned, over instrumentation was performed for bleeding and Collaplug was placed over the blood clot, covered with MTA, and composite resin for coronal sealing. Follow up was carried out for up to 9 months and was asymptomatic. Radiographs show healing of the lesion accompanied by closure of the root canal. Conclusion: Regenerative endodontics has been shown the successful in stimulating root canal closure to treat immature teeth

Abstract

Background: Root canal treatment can fail due to several causes. One of them is root canal calcifications. Purpose: To explain root canal retreatment technique in premolar with calcification. Case(s): A 65 years old female patient came to universitas airlangga dental hospital with a chief complaint of pain on tooth number 34 and 35. Tooth number 34 and 35 had an old restoration with secondary caries. Both teeth did not respond to vitality test. Both teeth were tender on percussion and bite test. Surrounding soft tissue was normal. Radiograph image reveals inadequate root canal obturations on both teeth and a periapical lesion on tooth 35. Case Management: Old restoration and root canal filling material was removed, then root canal retreatment was performed. Subsequently the tooth was restored with fiber post and zirconia crown. Discussion: old restoration has to be removed gently in order to prevent further damage to tooth structure. Root canal filling material can be removed with heated instruments, solvent, and rotary instruments. After root canal filling material removal, the remaining root canals need to be thoroughly shaped, cleaned, and sealed. Conclusion: root canal retreatment on premolar with an impediment can be successfully performed with appropriate instruments, materials, and technique.

Abstract

Background: Trauma to immature teeth can cause disrupt of the blood vessel supply in the pulp which leads to pulp necrosis. This can then, indirectly disrupts of root formation and incomplete apex closure.4,5 Young permanent teeth with wide open apex to be a challenge for root canal treatment, especially during obturation. Apexification is a treatment option for open apex with necrotic pulp. The purpose is to create an apical stop at the apical third of the apex so that obturation can be achieved. Case: A 27-year-old man came to RSKGMP UNAIR with chief complaints that his upper front teeth were fracture and discolored and the pain had disappeared since march 2023. From the clinical examination, tooth 21 showed Ellis fracture class III and there was tooth discoloration. Pulp vitality examination with cavity test and unresponsive k-file test, percussion (+) and bite test (+). Radiographic examination show an exposed pulp and exposed apex of tooth 21 accompanied by an unclearly defined radiolucent image at the apex of tooth 21. The diagnosis of this tooth was pulp necrosis with symptomatic apical periodontitis. Treatment: Apexification use bioceramic root repair material as apical plug at the apex. After that, obturation is achieved using a thermoplastic technique. Final restoration with individual customized post and lithium disilicate crown. Conclusion: Apexification is an option in cases of pulp necrosis with an open apex using bioceramic root repair material. Bioceramic root repair material is a bioactive material, that induces osteogenesis, is easier to apply, has faster setting time, biocompatible, hydrophilic, high pH and has anti-microbial properties.

Abstract

Background: One of the important virulence factors of S. mutans is the biofilm formation. Some bacteria in biofilms show resistance to antimicrobials. Therefore, the addition of nano brown anchovy (S. insularis) as antibiofilm agents can potentiate the caries and infection prevention efficacy of endodontic treatment. Purpose: To describe the effect of the combination of nano brown anchovy (S. insularis) 12% and Ca(OH)2 with a ratio of 1:1 on inhibition of S. mutans biofilm formation. Methods: Laboratory experimental research on S. mutans bacteria was carried out in vitro using the crystal violet assay method. Brown anchovy was made into nanoparticles, dissolved to a concentration of 12%, and combined with Ca(OH)2. Biofilm samples were given treatment and observed for inhibition of biofilm growth. Biofilm growth inhibition was seen from the Optical Density (OD)540nm absorbance value measured using a spectrophotometer. Results: In the group that was tested with Ca(OH)2, the average percentage in S. mutans biofilm reduction was 55%, while in the group that was tested with a combination of nano brown anchovy (S. insularis) and Ca(OH)2, the average percentage in S. mutans biofilm reduction was 61%. Conclusion: The combination of nano brown anchovy (S. insularis) 12% and Ca(OH)2 with a ratio of 1:1 can inhibit the formation of S. mutans biofilm.

Abstract

Background: Lactobacillus acidophilus and Aggregatibacter actinomycetemcomitans are gram bacteria that can cause various problems in the oral cavity so that materials that have antibacterial potency are needed. Calcium hydroxide, chlorhexidine, and sodium hypochlorite as antibacterial ingredients have disadvantages such as forming tunnel defects, causing dysgeusia, and damaging periapical tissue, so it is hoped that there will be natural materials as alternatives. The nano brown anchovy has fluor as the active compound which has the potential as an antibacterial agent. Purpose: To analyze the effect of antibacterial potency of nano brown anchovy (Stolephorus insularis) on Lactobacillus acidophilus and Aggregatibacter actinomycetemcomitans. Methods: This research is a laboratory experimental in vitro with the post-test only control group design. Brown anchovy is made into nano and diluted by dilution method into several concentrations. The direct contact method was used between some concentrations and the two bacteria. The values of MIC (Minimum Inhibitory Concentration) and MBC (Minimum Kill Concentration) were obtained by counting the number of bacterial colonies on Mueller Hinton Agar media. Bacterial colony growth was calculated manually in Colony Forming Units (CFU). Results: Antibacterial potency of nano brown anchovy on Lactobacillus acidophilus showed MIC results at 1.56% concentration and MBC at 3.125% concentration. Antibacterial potency of nano brown anchovy on Aggregatibacter actinomycetemcomitans showed MIC results at 3.125% concentration and MBC at 6.25% concentration. Conclusion: Nano brown anchovy has antibacterial activity on Lactobacillus acidophilus and Aggregatibacter actinomycetemcomitans.

Abstract

Background: Nanohybrid composite resins have many advantages, but one disadvantage is their absorbent nature which can lead to discoloration. Phenols and flavonoids in nano propolis cause diffusion and affect the matrix and pigments of nanohybrid composite resins. The accumulation of reduction-oxidation reactions causes new compounds of nanohybrid composite resins to occur so that they have different colors. Objective: The study was conducted to determine the changes in surface color of nanohybrid composite resin after immersion in nano propolis at 5% concentration. Methods: Research experimental laboratory method with pre and post test design was applied to 25 nanohybrid composite resin fruit samples of size (5x5x1.5) mm embedded in bovine incisive, then immersed in 5% nano propolis and 25 ml of distilled water at 37oC for five days. The color change (ΔE) was measured with a DSLR camera and processed with Adobe Lightroom Classic software in CIE L*a*b* format. Results: The anova test on ΔE in the data showed the p-value of sphericity assumed was smaller than 0.05. There is a difference in the average measurement results on the first day to the fifth day in the treatment group. Independent t-test of control and treatment data showed a significant difference in measurement results. The results of the post hoc test with the Tukey method showed that the treatment groups at 4x24 hours and 5x24 hours did not have significant differences. P-value at 4x24 hours and 5x24 hours was 0.957. Conclusion: There was a change in the surface color of the nanohybrid composite resin after being immersed in a nano-solution of propolis with a concentration of 5%.

Abstract

Background: Asymmetrical smile design due to uneven gingival margin heights of the anterior upper teeth can lead to aesthetic issues and inadequate crown restorations, necessitating crown lengthening treatment. Purpose: The objective of this case report is to demonstrate the outcomes of crown lengthening to achieve improved aesthetics and symmetric gingival margin heights. Case: A 16-year-old female patient sought treatment for decayed left maxillary anterior teeth. Clinical examination revealed a non-vital tooth (tooth 21) with decay and excessively low gingival margins, along with asymmetrical gingival heights between the incisors and canines. Radiographs indicated radiolucent mass in the pulp chamber. Case Management: Crown lengthening was performed on teeth 12, 11, 21, and 22, and apexification was conducted on tooth 21 using a customized fiber post and lithium disilicate crown. Follow-up evaluations showed no complaints, well-maintained crowns, and healthy gingiva. Conclusion: Crown lengthening is feasible for anterior teeth with low gingival margins, enhancing clinical crown length to support aesthetic and final restorations in maxillary anterior teeth.

Abstract

Background: Traumatic dental injuries often occur in the community reaching 85% of patients and in preschool children the proportion reaches 17%. Among the injuries are tooth fracture, tooth luxation, tooth avulsion and open apex which causes pulp death or necrosis, causing root growth to stop. Regenerative endodontic treatment (RET) with a biological approach to treat necrotic immature permanent teeth can promote pulpal growth, increase the thickness of the dentin wall and form apical foramens. Enterococcus faecalis bacteria can be resistant to some drugs and root canal irrigation substances and may persist in the root canal system. Recently, sodium diclofenac has been reported to retain a new potential to eliminate Enterococcus faecalis biofilm. This shows that sodium diclofenac is potential as an anti-biofilm material. Purpose: To evaluate the potential use of sodium diclofenac in RET. Review: This review was investigated in PubMed and Google Scholar containing keywords: Sodium Diclofenac, Enterococcus faecalis and Regenerative Endodontic Treatment. Sodium diclofenac is a potential medicament to eliminate Enterococcus faecalis biofilm therefore has a potency to be used in Regenerative Endodontic Treatment. Conclusion: The success rate of RET depends on the elimination of microorganisms in the root canal systems. The use of sodium diclofenac as a medicament to eliminate Enterococcus faecalis biofilm is potential to be used in RET. Further researches are encouraged to explore the benefits of sodium diclofenac as a medicament in RET.

Abstract

Background: Dental aesthetic issues may affect individual’s aesthetic, function, and psychological well-being. The functions of the stomatognathic system are supporting mastication, speaking, breathing and swallowing functions, so that structural integrity and an ideal and harmonious smile can be achieved as well. Aesthetic issues due to caries of maxillary anterior teeth requires deep analysis, consideration and appropriate treatment planning to aim a good result both functionally and aesthetically. Digital smile design (DSD) plays an important role in aesthetic dentistry where problems can easily be rectified. Purpose: This case report describes a step-by-step aesthetic treatment of a male patient with multiple caries of anterior teeth. Case: This study reports a 21-year-old man with multiple caries on his maxillary anterior teeth. Tooth number 12 with class IV caries lesion. Teeth number 11 and 22 with class IV and VI caries lesion and were endodontically treated, tooth number 21 with class III mesiodistal caries lesion. The patient’s chief complaint is that he felt less confidence with his appearance. Case Management: tooth number 21 was managed with endodontic treatment and followed by insertion of fiber post. Teeth number 11 and 22 that endodontically treated previously, were managed with prefabricated fiber post. Tooth number 12 was managed with crown. Teeth number 11, 21, 22 11, 21, 22 were managed with gingivectomy for aesthetic crown lengthening. Lithium disilicate was chosen for the crown materials Conclusion: Anterior teeth with multiple caries can be improved through endodontic-periodontal-restoration treatments to improve the function and aesthetic performance.

Abstract

Background: Lactobacillus acidophilus is involved in secondary caries that can accelerate demineralization of the tooth surface. Bacteria forms biofilm as part of a defense mechanism. One of the caries treatments is direct pulp capping with calcium hydroxide (Ca(OH)2) as the gold standard. Due to its high pH, calcium hydroxide can induce necrosis of pulp tissue when it comes into direct contact. Propolis is a natural material produced by honey bees. Propolis has many benefits including antibacterial effects. Nanoparticles are microscopic particles measuring 1-100 nm, reducing particle size can increase the rate of dissolution and absorption. Purpose: This study aims to prove the difference in the biofilm inhibitory ability of a micro and nano-sized combination of propolis and Ca(OH)2 against L. acidophilus biofilms. Methods: This study used true experimental laboratories research with treatment groups that included a control, combination of micro-sized propolis and Ca(OH)2 and combination of nano-sized propolis and Ca(OH)2. Results: The results showed that there was a significant difference between the micro and nano-sized combination of propolis and Ca(OH)2 in inhibiting biofilm growth, with the combination of nano-sized propolis and Ca(OH)2 showing higher effectiveness compared to the micro-sized one. The combination of nano-sized propolis and Ca(OH)2 produced an inhibition value of 84.35%, while the micro-sized one produced an inhibition value of 77.18%. Conclusion: The combination of nano-sized propolis and Ca(OH)2 had a greater biofilm inhibition compared to the combination of micro-sized propolis and Ca(OH)2.
