Abstract

BACKGROUND: Given the urgent need to address the significant morbidity and mortality associated with Rheumatic Heart Disease (RHD) in Indonesia, there is a growing interest in exploring cost-effective screening approaches, such as handheld echocardiography.OBJECTIVES: The purpose of this study was to ascertain the prevalence of RHD in North Sumatra, Indonesia.METHODS: This descriptive observational study was conducted within the population of North Sumatra from 2022 to 2023. Junior high school students aged 12 to 15 years were included from randomly selected schools in Langkat, Tebing Tinggi, and Labuhan Batu. Data collection encompassed various parameters, including social demographic information, parental characteristics, environmental factors, household details, anthropometric measurements, physical assessments, auscultation findings, and echocardiographic data. The data were analyzed descriptively.RESULTS: In our study, a total of 692 children were examined, with an average age of 12.9 years and a standard deviation of 1.1 years, among whom 42.5% were male. Utilizing echocardiographic evaluations, we identified RHD in four children, yielding a prevalence rate of 0.6%. Further examination of these cases revealed that the majority, accounting for three individuals (75%), exhibited borderline RHD, while one child (25%) presented with definite RHD.CONCLUSION: In our study population, the prevalence of RHD was 0.6%. A broader echocardiographic screening program is necessary to determine the overall prevalence of RHD, assess the disease burden, and identify individuals earlier to prevent adverse outcomes.

Abstract

Objective: To assess whether there is right ventricular-pulmonary artery (RV-PA coupling by tricuspid annular plane systolic excursion/pulmonary artery systolic pressure TAPSE/PASP) changes in determining the clinical results of decongestion in patients with acute heart failure (AHF) with non-preserved ejection fraction (non-HFpEF) before and after decongestive at Adam Malik Hospital Medan. Methods: This study is an observational study with a prospective approach and consecutively collected, in patients AHF with non-HFpEF at Adam Malik Hospital Medan in the period from September 2023 until November 2024d. The sample is an eligible population that meets the inclusion and exclusion criterias. Results: This study consisted total 44 subjects with majority of 37 men (75%), an average age of 61 years old. A total of 28 subjects (63.6%) had hypertension as comorbid. There were 33 people (75%) who had sinus rhythm. The most acute heart failure phenotype commonly found was acute decompensated heart failure (ADHF) with 21 subjects. The longest length of stay AHF was cardiogenic shock with a median of 9 days. A significant decrease in median heart (p 0.0001), tricpuspid valve gradient (TVG) examination showed a significant decrease in (p 0.0001), PASP examination had a significant decrease in the average (p 0.0001), RV-PA coupling showed a significant increment (p 0.0001). A significant relationship was found between RV-PA coupling changes and decongestion in all acute heart failure groups (p 0.005). No significant relationship between RV-PA coupling ratio with length of stay was found. Conclusion: There were differences found in RV-PA coupling as increment with significant value in complete and partial decongestion groups, with a RV-PA coupling cut-off value of 0.65 and sensitivity and specificity of 68.8% and 67.9%, respectively.

Abstract

BACKGROUND Cardiovascular disease (CVD) is projected to affect more than 23.3 million people by 2030. Therefore, CVD prevention strategies were established to decrease morbidity and mortality while enhancing overall well-being. The Joint European Societies (JES) guidelines on CVD prevention were developed to enhance preventive cardiology practices. This study aimed to evaluate the adherence to JES guidelines for cardiovascular prevention in routine clinical practice for secondary prevention. METHODS This multicenter cross-sectional study was conducted in 7 centers between August 2020 and June 2021. Patients under 80 years old who had undergone percutaneous transluminal coronary angioplasty, coronary artery bypass graft, percutaneous coronary intervention, or experienced acute coronary syndrome were identified from medical records and interviewed a year later. Descriptive statistics were used to calculate the occurrence of risk variables, medication use, and index events associated with low-density lipoprotein cholesterol (LDL-C), hemoglobin A1c, and blood pressure (BP). RESULTS A total of 402 participants (13.9% female) were interviewed, and their medical records were reviewed. Among the study population, 74.4% had a smoking history, 35.4% had dyslipidemia, 33.1% did not meet the BP target, and only 28.4% achieved the LDL-C target. Additionally, less than half (43.8%) participated in physical activity for >150 min/week. Only 15.6% of the patients among the centers who had scored >8 for the guideline-based target score. CONCLUSIONS Most patients did not meet the guidelines for secondary prevention, primarily due to the high prevalence of dyslipidemia and physical inactivity, although some achieved the LDL-C target.

Abstract

Background: This study aims to comprehensively describe the incidence, types, and associated risk factors of bleeding complications in patients with ST-elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PPCI), addressing a critical gap in the literature given the global burden of cardiovascular disease and the inherent bleeding risks of contemporary antithrombotic therapies. Methode: This retrospective cross-sectional study will investigate the incidence and types of bleeding complications, along with associated risk factors, in ST-elevation myocardial infarction (STEMI) patients undergoing primary percutaneous coronary intervention (PPCI) at Adam Malik Hospital Medan, analyzing data from May 2022 to December 2024 through ethical review and statistical analysis using SPSS version 23. Result: Of 245 STEMI patients undergoing primary PCI, 42.9% experienced bleeding, predominantly minor (BARC 1 and 2, 94.2% combined), with significant associations observed between bleeding and lower hemoglobin, higher leukocyte and creatinine levels, higher TIMI score, Killip class 3 and 4, diabetes, use of maintenance heparin, and increased mortality (84.6% of all deaths occurred in bleeding patients), while hematuria and puncture site hematoma were the most common bleeding sources. Conclusion: This study found that 42.9% of 245 STEMI patients undergoing primary PCI experienced bleeding complications, predominantly minor (94.2%), with an average age of 55.22 years and a male majority. Abstract

Background: Coronary Heart Disease (CHD), the leading global cause of death, stems from artery constriction due to fat deposition and vessel stiffness, leading to insufficient blood flow to the heart muscles. Despite numerous studies on CHD characteristics, none reflect the latest data specific to North Sumatra, contributing to 32% of global deaths. Objective: To describe the characteristics of CHD patient that underwent coronary artery bypass graft (CABG) in 2022 at RSUP Haji Adam Malik Medan. Methods: This descriptive research with cross-sectional design was done to CHD patients that underwent CABG operation with total sampling technique and used medical record data that was taken and grouped to characteristics before, during, and after the surgery. Results: Out of 48 complete medical records, majority of patients are in the age range of 48-65 years old (73%), male gender (85.4%), hypertension and/or type 2 diabetes patient (70.9%), abnormal lipid profile (93.8%), operation duration 240309 minutes (66.7%), normal ejection fraction (56.3%), in use of cardiopulmonary bypass machine (on pump) during operation (54.2%), CPB time 118-159 minutes (53.8%), Aox time 59-88 minutes (50%), in use of 3 grafts (56.3%), normal kidney function after the operation (83.3%), abnormal routine blood profile (53.88%), and without complications (66.7%). Conclusion: In this study, the average age of patients was 56, mostly male, with abnormal lipid profiles. The prevalent comorbidities were hypertension and/or type 2 diabetes. Operations averaged 284.75 minutes, using 3 grafts, with 43.7% having lowered ejection fraction. Arrhythmia was the most common postoperative complication. Keyword: CABG, Characteristics, CHD Latar Belakang: Penyakit Jantung Koroner (PJK), penyebab utama kematian global, disebabkan oleh penumpukan lemak dan/atau kekakuan pembuluh darah, mengurangi perfusi otot jantung dan menyumbang 32% angka kematian dunia. Banyak penelitian karakteristik pasien PJK, tetapi kurangnya data terbaru karakteristik PJK di Sumatera Utara menjadi perhatian. Tujuan: Untuk mendeskripsikan karakteristik pasien PJK yang menjalani bedah pintas arteri koroner pada tahun 2022 di RSUP Haji Adam Malik Medan. Metode: Penelitian deskriptif dengan desain cross-sectional ini dilakukan pada pasien PJK yang menjalani operasi bedah pintas arteri koroner (BPAK) dengan teknik total sampling serta menggunakan data rekam medis yang diambil dan dikelompokkan menjadi karakteristik sebelum, selama, dan setelah operasi. Hasil: Dari 48 rekam medis lengkap, pasien paling banyak pada kelompok usia 48-65 tahun (73%), jenis kelamin laki-laki (85,4%), penderita hipertensi dan/atau DM tipe 2 (70,9%), profil lipid abnormal (93,8%), durasi operasi 240-309 menit (66,7%), fraksi ejeksi normal (56,3%), menggunakan mesin jantung paru (on pump) selama operasi (54,2%), CPB time 118-159 menit (53,8%), Aox time 5988 menit (50%), menggunakan 3 graft (56,3%), fungsi ginjal normal paska bedah (83,3%), profil darah rutin abnormal (58,33%), dan tidak mengalami komplikasi (66,7%). Kesimpulan: Pasien penelitian ini rata-rata berusia 56 tahun, didominasi oleh laki- laki, mayoritas memiliki profil lipid abnormal, dengan komorbid terbanyak adalah hipertensi dan/atau DM tipe 2. Mayoritas pasien tidak mengalami komplikasi paska pembedahan. Komplikasi tersering adalah aritmia. Kata Kunci: BPAK, Karakteristik, PJK
