Abstract

Introduction: Type 2 Diabetes Mellitus (DM) is a significant global health concern. In Type 2 DM, hyperglycemia results from inadequate insulin production and the body’s reduced ability to respond to insulin, a condition known as insulin resistance. Diabetes Mellitus is often associated with cognitive decline, as observed in many patients with Type 2 DM. This study aims to analyze factors influencing cognitive status in Type 2 Diabetes Mellitus patients at Royal Prima General Hospital Medan. Methods: This descriptive quantitative study used a cross-sectional design. The study population consisted of 754 patients with Type 2 DM, with a sample of 150 selected. A mini-mental state exam was used to check the cognitive status. Data were analyzed using univariate, bivariate, and multivariate methods. Results: The findings indicate that gender, education level, blood glucose status, and duration of DM significantly influenced cognitive status in Type 2 DM patients at Royal Prima General Hospital Medan, with a p-value < 0.05 for each variable. In contrast, occupational status did not show a significant influence (p-value > 0.05). Collectively, gender, education level, occupational status, blood glucose status, and DM duration impacted cognitive status in these patients (p-value < 0.05). Conclusion: Among the variables studied, the duration of Type 2 DM was the most influential factor affecting cognitive status in patients at Royal Prima General Hospital Medan.

Abstract

In conditions of tight competition, the main thing that hospitals need to prioritize is a strategy to improve the quality of service to their patients. The quality of nursing services indicates the quality of health services. Nurses determine the image of health service institutions in society. This research aims to analyze indicators of nursing service quality strategies on the perception of satisfaction of inpatients at RSU Royal Prima Medan. This type of research is quantitative descriptive. The population in this study was inpatients at RSU Royal Prima from January 2024 to February 2024, totaling 300 patients. The sampling technique in this research used an accidental sampling technique with a sample size of 300 patients. Data analysis used SEM and SPSS, including univariate, bivariate, and multivariate analysis. The research results showed that the majority of respondents in this study were 31-40 years old, 137 (45.7%); the majority were female, 215 (71.7%); and had a PT (higher education) education, 157 (52.3%). There is an influence of the reliability service quality strategy indicator on the perception of satisfaction of inpatients at RSU Royal Prima Medan, there is an influence of the responsiveness service quality strategy indicator on the perception of satisfaction of inpatients at RSU Royal Prima Medan, there is an influence of the assurance service quality strategy indicator on the perception of satisfaction of inpatients inpatients at RSU Royal Prima Medan, there is an influence of the empathy service quality strategy indicator on the perception of satisfaction of inpatients at RSU Royal Prima Medan, there is an influence of the tangible service quality strategy indicator on the perception of satisfaction of inpatients at RSU Royal Prima Medan, Service quality strategy indicator variable The most influential influence on the perception of satisfaction of inpatients at RSU Royal Prima Medan is the reliability service quality strategy indicator variable.

Abstract

Currently, the issue of patient safety is getting serious attention from the government. Hospitals must fulfill the patient's right to security and safety while in hospital treatment. Patient safety is a serious global public health problem. Medical errors can be caused by both system and human factors. This research was conducted at the Royal Prima Hospital Medan with the issue of patient safety at the Central Surgical Installation (CSI). The formulation of the problem in this study is about the implementation of patient safety programs by nurses in the operating room based on the rules of the Royal Prima Hospital Medan in 2021. The type of research used is descriptive using a phenomenological approach which is a type of qualitative research. The population in this study were all nurses who worked in the Central Surgical Installation ward (CSI) of the Royal Prima Hospital Medan. The total sampling was 23 nurses with inclusion criteria, willing to be respondents and not on vacation/leave the service facilities at the hospital. The results showed that the implementation of the patient safety program in the operating room by nurses based on the rules of the Royal Prima Hospital Medan had been implemented and had a program prepared policies and patient safety guidelines as a reference in improving the quality of health services.

Abstract

Covid-19 is an infectious disease that will become a global pandemic in 2020. To reduce the transmission rate, WHO has issued various recommendations for the community, such as the use of masks, regular hand washing, coughing and sneezing etiquette, keeping a distance between people, avoiding crowded places, and others. The existence of public restrictions creates new problems for patients because of difficulties in seeking treatment. Telemedicine is a form of telehealth, which is a health service that is carried out remotely. This technology makes it easier for patients to interact with doctors without having to meet face to face. The purpose of this study was to determine the level of satisfaction of users of telemedicine services at RSIA Stella Maris Medan. The type of research used is non-experimental quantitative with a quantitative analytical study design. The population and samples used were 186 users of telemedicine services at RSIA Stella Maris. The results showed that users considered the features of the availability of options to be able to choose the treating doctor, the confidentiality of medical records, the stability of the internet network, the friendly and ready to help the attitude of the nurses, and the long waiting time for the drug delivery process as the most important features of each service dimension. To improve the quality of RSIA Stella Maris telemedicine services, many features are a priority to be improved, as well as features are maintained.

Abstract

The medical check-up service process has several challenges such as the quality of medical personnel, the complexity of the work, long waiting times, uncomfortable patient waiting areas, the quality of medical equipment, and decreased visits. Seeing the medical check-up business opportunity, hospitals must continue to evaluate the MCU service business process so that the use of resources for these services becomes more efficient. The purpose of this study is to evaluate the development of medical check-up services. This study uses a quantitative analytical survey technique with a cross-sectional approach. The research was conducted at Putri Hijau Hospital Level II Medan. The population and research sample are 7 key persons. Sampling by purposive sampling. Data analysis was carried out using the Internal Factor Analysis Strategic (IFAS) matrix, External Factor Analysis Strategic (EFAS) matrix, and SWOT analysis. The results showed that internal factors that have the potential as strengths in the development of medical check-up services at the Putri Hijau Hospital Level II Medan are the quality of administrative staff, quality of functional staff, number of collaborating companies, ease of payment, ability to build good relations, waiting room for medical services. check-up, medical and non-medical equipment in the medical check-up room. Internal factors of weakness are marketing installation, marketing organization, marketing funds, marketing managerial ability, and condition of the size of the examination room. Opportunity external factors are the labor law, the acceptance of military recruits, and the many companies that cooperate. External threats are the quality of medical check-ups at competing hospitals and the Covid-19 pandemic.

Abstract

The performance of nurses in hospitals is closely related to reliable, skilled, and professional human resources. Many factors are thought to influence the performance of nurses, namely age, education, length of work, training, motivation, incentives, work environment, and supervision. The purpose of this study was to analyze the factors that influence the performance of nurses. This research is an analytical study with a cross-sectional approach. The research was conducted at Raden Mattaher Hospital, Jambi Province. The study population was 406 people and a sample of 261 people. Sampling by purposive sampling. Data analysis was univariate, bivariate with a chi-square test, and multivariate with multiple logistic regression at a 95% confidence level (=0.05). The results showed that the factors that influenced the performance of nurses at the Raden Mattaher Regional General Hospital Jambi Province were education and training (p=0.000), motivation (p=0.000), incentives (p=0.020), and supervision (p=0.002 ). Variables that had no effect were age (p=0.584), education (p=0.786), length of work (p=0.421), and work environment (p=0.166). The most dominant variable affecting the performance of nurses is motivation with a value of Exp(B)/OR = 5.626, meaning that nurses who have high motivation have a 5.6 times higher chance of good performance than nurses with low motivation. This study concludes that education and training, motivation, incentives, and supervision affect the performance of nurses. The leadership of the Raden Mattaher Hospital in Jambi Province needs to hold regular education and training, provide incentives according to the nurse's responsibilities, the head of the room supervises properly so that it can improve the performance of implementing nurses.

Abstract

Insulin resistance is a condition with damage to target organs that generally respond to the activity of the hormone insulin. About 10% of the population in big cities in Indonesia suffer from insulin resistance. At the cellular level, insulin resistance can result from damage to the pre-receptor, receptor, or post-receptor. The most common cause of insulin resistance is post-receptor damage, namely disruption and damage to insulin signaling pathways. To find out that the methanol extract of Sunkist orange peel can reduce blood sugar levels in Wistar rats. This research uses the experimental laboratory method in vivo with the chosen research design form, namely True Experimental Pre Test and Post Test Control. The Kruskal Wallis test showed that the Sunkist Orange Peel Methanol Extract resulted in a significant decrease in blood sugar levels of Wistar rats with a p-value <0.05, and also the Anova test showed that the Sunkist Orange Peel Methanol Extract resulted in a decrease in blood sugar levels of Wistar rats, which is significant with p-value <0.05. All doses tested affect lowering blood sugar levels, namely, 300 mg/kg BW, 450 mg/kg BW, and 600 mg/kg BW.

Abstract

The procurement of drugs is an important early stage in determining the success of the next stage because the planning stage is useful for adjusting the procurement needs with available funds to support health services in hospitals. The purpose of the study was to analyze the Management of Procurement of Insurance Medicines and Medicines for BPJS Health in the Pharmacy Unit of Bakti Timah Hospital, Pangkal Pinang, Bangka Belitung Islands Province in 2022. The research design used a qualitative descriptive method with a phenomenological approach, namely collecting, compiling, and analyzing so that a social phenomenon/fact can provide information. solving the problems encountered. This research was conducted at the Pharmacy Unit of Bakti Timah Hospital, Pangkal Pinang, Bangka Belitung Islands Province in April-May 2022. The sampling technique in this study used the Total Sampling technique, the number of samples was 5 pharmacists. The results of the research on the drug planning process at the Bakti Timah Pinang Hospital Pharmacy Installation, the drug budgeting process at the Bakti Timah Pinang Hospital Pharmacy Installation, the drug procurement process at the Bakti Timah Pinang Hospital Pharmacy Installation, the drug storage process at the Bakti Timah Pinang Hospital Pharmacy Installation, The process of storing drugs at the Pharmacy Installation of the Bakti Timah Pinang Hospital. Suggestions for hospital management to find out what strategies should be applied to increase revenue and services by reducing the percentage of prescriptions that are not served.

Abstract

The Covid-19 pandemic has caused a decrease in the number of patient visits and changes in service procedures for patients being treated to prevent transmission of Covid-19. This has an impact on decreasing patient satisfaction with service quality which includes physical evidence, reliability, responsiveness, assurance, and empathy. The purpose of this study was to analyze the quality of nursing services on the satisfaction of internal medicine inpatients during the Covid-19 pandemic. This research is a quantitative analytic study with a crosssectional approach. The research was conducted at the Royal Prima Hospital, Medan. Theresearch population was 285 people, the sample obtained was 166 people. Sampling by purposive sampling. Data analysis was univariate, bivariate with a chi-square test, and multivariate with multiple logistic regression at a 95% confidence level (α = 0.05). The results showed that the quality of nursing services affected the satisfaction of inpatients for internal medicine at the Royal Prima Hospital Medan during the Covid pandemic including physical evidence (p = 0.000), and reliability (p = 0.008), responsiveness (p = 0.001), and assurance. (p=0.000). While the factor that has no effect is empathy (p = 0.959). The physical evidencevariable (tangibles) is the most dominant variable affecting the satisfaction of internal medicine inpatients during the Covid-19 pandemic with a value of Exp(B)/OR = 13,773. Patients who stated good physical evidence had a 13.7 times higher chance of being satisfied than patients who stated poor physical evidence

Abstract

Clinical pathways are an important part of documents and tools in realizing good clinical governance in hospitals. In Indonesia, this document is also one of the requirements that must be met in the KARS version of the Hospital Accreditation Standard. However, the process of implementing this Clinical Pathway itself also has various problems. Things that might hinder the implementation of this clinical pathway could come from the staff and hospital members themselves because they have not adapted to this Clinical Pathway. This study aims to analyze the implementation of the Clinical Pathway at the Royal Prima Hospital in Medan. This study uses a quantitative design with a cross sectional approach. The research sample was all doctors, nurses, nutritionists, pharmacists at the Royal Prima Hospital Medan who filled the Clinical Pathway as many as 83 people. The results of the Fisher's Exact test showed that there was a significant effect between regulation (p=0.000), compliance (p=0.004), supporting factors (p=0.025), and inhibiting factors (p=0.002). By using the t-test at a significance level of 0.05, the results obtained that regulation is the variable that has the main influence on the implementation of the Clinical Pathway at Royal Prima Hospital Medan (Sig.0.001). Suggestions to hospitals are expected to strengthen policies regarding the implementation of Clinical Pathway to improve health services in hospitals. Caregivers are expected to maintain compliance with the implementation of the Clinical Pathway to improve health services in hospitals.
