Abstract

AbstrakOsteoporosis merupakan suatu kelainan metabolik tulang yang sering timbul, kondisi ini ketika tulang secara perlahan kehilangan kepadatannya disertai transformasi mikroarsitektur jaringan tulang yang dapat mengakibatkan penurunan kekuatan tulang sehingga kecenderungan untuk mengalami fraktur. Salah satu penyebab yang berpengaruh terhadap terjadinya penurunnya massa tulang ialah Indeks Massa Tubuh (IMT). IMT merupakan cara yang diterapkan untuk penilaian status gizi yang cukup praktis. Dimana IMT ini dapat digunakan untuk menentukan status berat badan sesorang dengan kategori berupa nilai IMT < 18,5 kg/ = berat badan kurang, nilai IMT 18,5 - 22,9 kg/ = normal, nilai IMT 23-24,9 kg/ = obesitas ringan, nilai IMT 25,0 - 29,9 kg/ = obesitas sedang, nilai IMT ≥ 30,0 kg/ = obesitas berat. Indeks massa tubuh (IMT) dan osteoporosis berhubungan kuat karena IMT dipercaya sebagai indikator yang dipakai dalam pengukuran densitas mineral tulang (DMT). Tujuan dari penelitllian adalah untuk menentukan hubungan indeks massa tubuh dengan kepadatan mineral tulang collum femur dan tulang vertebra lumbal. Penelitian ini merupakan penelitian potong lintang pada 233 pasien yang menjalani pemeriksaan densitometri di Rumah Sakit Royal Prima Medan tahun 2020 dengan menggunakan alat Radiofrequency Echographic Multi-Spectrometry (REMS). Hasil penelitian ini menunjukkan bahwa adanya hubungan yang signifikan antara Indeks Massa Tubuh (IMT) dengan nilai T-score vertebra lumbal dan T-score collum femur (P <0,000). Dan uji korelasi antara nilai IMT dengan nilai T-score leher femur (r=-.195;p<0.05) dan T-score vertebra lumbal (r= -.179;p<0.05) menunjukkan hubungan yang sangat lemah dan berpola negatif yang artinya semakin rendah IMT maka semakin rendah T-score yang berarti semakin rentan terhadap resiko osteoporosis di leher femur dan tulang belakang.   Kata Kunci: Indeks Massa Tubuh (IMT), Osteoporosis, Densitas Mineral Tulang (DMT)

Abstract

Keselamatan dan kesehatan kerja merupakan suatu pemikiran dan upaya untuk menjamin keutuhan dan kesempurnaan baik jasmani maupun rohani pekerja dan dengan keselamatan dan kesehatan kerja maka para pihak diharapkan dapat melakukan pekerjaan dengan aman dan nyaman. APD adalah seperangkat alat yang digunakan oleh tenaga kerja untuk melindungi seluruh atau sebagian tubuhnya terhadap kemungkinan adanya potensi bahaya di tempat kerja atau kecelakaan kerja. Adapun tujuan dari penelitian ini adalah untuk mengetahui Hubungan Perilaku Penggunaan APD dengan Kecelakaan Kerja pada Pekerja Pabrik Kelapa Sawit PT. Padasa Enam Utama Kalianta Satu, Kec. Kabun, Riau. Jenis penelitian ini adalah kuantitatif dengan desain deskriptif. Populasi penelitian ini adalah seluruh pekerja yang berkerja di Pabrik Kelapa Sawit dengan jumlah 140 orang dan sampel sebanyak 50 orang. Pengumpulan data dilakukan dengan menggunakan kuesioner tentang pengetahuan dan sikap para perkerja saat di tempat kerja. Analisis data yang digunakan dalam penelitian ini adalah analisis univariate dan bivariate menggunakan uji Chi-Square. Hasil penelitian didapatkan bahwa terdapat 2 faktor yang memiliki hubungan yang bermakna dengan Kecelakaan Kerja pada Pekerja Pabrik Kelapa Sawit PT. Padasa Enam Utama Kalianta Satu, Kec. Kabun, Riau yaitu pengetahuan (p=0.023) dan sikap (p=0.019). Diharapkan perusahaan menyediakan training (pelatihan) kepada pekerja untuk meningkatkan pengetahuan para pekerja dan juga diharapkan perusahaan membentuk tim untuk memonitor kedisplinan pekerja dalam kepatuhan penggunaan APD sehingga dapat mengurangi resiko terjadinya kecelakaan kerja.

Abstract

Green betel leaf (Piper betle L.) is widely used in the healing of cut wounds. The aim of this study was to investigate the efficacy of green betel leaf extract cream in the healing of cut wounds on the skin of white Wistar rats. The design employed was a pre-test post-test with a control group design and a sample size of 20 white rats divided into four groups. Betel leaf extract was prepared using the maceration method, while a cream preparation of betel leaf extract was prepared using the oil-in-water emulsion type (M/A). Macroscopic histopathological evaluation was performed by examining fibroblast growth. A one-way analysis of variance (ANOVA) test (p < 0.05) was used to determine the difference between groups, and a post hoc least significant difference (LSD) test was used to analyze which treatment group had the greatest efficacy. Technical terms were explained upon first use, and the language used was clear, objective, and value-neutral to improve academic quality. The text was free from grammatical errors, spelling mistakes, and unnecessary jargon. A one-way analysis of variance (ANOVA) test (p < 0.05) was used to determine the difference between groups, and a post hoc least significant difference (LSD) test was used to analyze which treatment group had the greatest efficacy. The study revealed a substantial difference in the average wound healing rates between the control group (P0) and treatment groups P1, P2, and P3. This is due to the lack of active substances in the control group (P0), which aids in accelerating the wound healing process. Notably, the 15% green betel leaf extract cream demonstrated superior efficacy in healing wounds in white rats compared to the 10% and 25% green betel leaf extract creams. The results indicated no significant difference between the application of 15% and 25% green betel leaf extract cream. This is due to the fact that secondary metabolite compounds present in the extract produce a healing effect on wounds at these particular levels. However, lower concentrations only act as microorganism inhibitors, rendering them less effective for promoting wound healing.

Abstract

Logistics management is the science and/or art of planning and determining needs, procurement, storage, distribution, maintenance, disposal and control of medical devices. Provision of logistics at the Puskesmas is a matter that needs to be considered for its provision through a good and orderly management system. This management system can be carried out in several ways, namely through supervision, supervision, monitoring and evaluation related to the implementation of logistics provision. The purpose of this study was to analyze the evaluation process, supervision work, policies, supervision in health and health monitoring of the Head of the Puskesmas regarding the provision of logistics at the UPTD Puskesmas Lahusa. This type of research is descriptive analytic, quantitative approach with a cross sectional design. The sample in this study were all staff involved in implementing logistics provision at the Lahusa Health Center. The sampling technique is the total population, that is, the entire population is used as a research sample totaling 112 people. The results showed that all variables had a significant effect between evaluation, supervision work, policy, supervision in health and health monitoring of the Head of the Puskesmas on the provision of logistics at the UPTD Puskesmas Lahusa. The results of this study are expected to be input for the puskesmas management services to pay more attention to the implementation of logistics provision.

Abstract

Measuring patient perceptions of healthcare service quality as an important element in assessing service quality has attracted much post-covid-19 attention in recent years. Therefore, this research was conducted to determine the quality of clinical services at accredited hospitals in Medan and Binjai. This cross-sectional study was conducted from May to August 2022, and 190 patients were randomly selected from two hospitals. Data was collected using a questionnaire whose validity and reliability had been confirmed in previous studies. T-test, ANOVA, and Pearson correlation coefficients were calculated using SPSS to analyze the data. The bivariate test results showed a relationship between service quality dimensions and patient satisfaction at RSUD Dr. Pringadi and Dr. R.M. Djoelham. Multivariate testsfound that the dominant factors influencing patient satisfaction at RSUD Dr. Pirngadi Medan are l evidence, reliability, responsiveness, assurance, and empathy. And for Dr. General Hospital. RM Djoelham Binjai is a tangible factor, reliability, responsiveness, and empathy. So that it can be concluded that to increase patient satisfaction, speed support is needed in services such as providing a fast response to every patient complaint, as well as providing short and responsive service to patient needs, bearing in mind that patient needs are urgent needs that must be met immediately.

Abstract

Lead significantly affects health because of its sensitivity to the central nervous system and rapid absorption rate. The phytochemicals included in celery, such as flavonoids, may mitigate lead-induced kidney damage. This research aimed to determine how administering celery extract affected the expression of the Caspase-3 gene and the kidney TNF-α gene in Rattus Norvegicus Wistar white rats exposed to lead. The research method true experiment examined the effects of 80 mg/day celery extract on Caspase-3 gene expression and renal TNF-α gene expression in rats exposed to lead. Each of the three groups will include six rats; there will be eighteen male Wistar rats in the study. The study used SPSS 26.0 and the Saphiro Wills test to analyze data on TNF-α and Caspase 3 expression in rats exposed to lead acetate (Pb). The results showed a normal distribution for the negative control group (K-) and the positive control group (K+) and a normal distribution for the treatment group (P). However, the paired t-test showed significant differences in all treatment groups (p <0.05), indicating a normal distribution in the data. The study concludes the impact of celery extract on kidney function in lead-exposed Wistar rats, revealing various active compounds like flavonoids. Despite insignificant caspase 3 reduction, flavonoids showed potential in mitigating lead-induced kidney damage.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a respiratory disease characterized by a reduction in lung capacity and is the third leading cause of death worldwide. COPD is a complex disease involving various environmental and genetic factors. This study aimed to determine the prevalence of COPD among TB patients at Royal Prima Hospital in Medan. This was a retrospective descriptive study with a cross-sectional design. The population of this study was all outpatient TB medical records at Royal Prima Hospital in Medan. Purposive sampling was used, resulting in a sample size of 68 individuals. Data was analyzed univariately using descriptive statistics presented in frequency distribution tables and percentages. The results showed that most COPD patients at Royal Prima Hospital were elderly (73.5%), male (85.3%), employed (64.7%), had a junior high school education (47.1%), had suffered from COPD for less than 10 years (70.3%), and were active smokers (63.2%). Most COPD patients were categorized as having moderate disease (39.7%). It is hoped that Royal Prima Hospital will continue to develop prevention programs for Chronic Obstructive Pulmonary Disease to reduce the number of cases, and that COPD patients who frequently visit healthcare facilities will receive instructions on self-care management for their long-term condition to reduce disease severity.

Abstract

Diabetes mellitus (DM) is a significant global health issue, with its prevalence continuing to rise in Indonesia. Self-care is a key component in managing DM, yet many patients face various obstacles in its implementation. This study aims to deeply explore the challenges faced by individuals with type 2 diabetes in managing their condition independently. Using a descriptive qualitative approach, the study analyzes the experiences of seven diabetes patients receiving care at Royal Prima Hospital. The social ecological model serves as the analytical framework to identify factors influencing self-care behavior, including individual internal factors, interpersonal relationships, healthcare systems, and the social environment. Data was collected through in-depth interviews and analyzed using thematic content analysis. The findings reveal several barriers to effective self-care in diabetes management. Firstly, at the individual level, a lack of knowledge, limited resources, non-adherence to medication, and financial constraints were major obstacles. Secondly, insufficient social support and misinformation about alternative treatments also influenced patient behavior. Thirdly, at the healthcare service level, long waiting times and inadequate social support were identified as challenges. Lastly, limited accessibility due to transportation issues and community resource constraints further hindered self-care. Patients require comprehensive education, easy access to medical tools, and psychological support to manage their condition effectively. Families and communities must be equipped with accurate knowledge about diabetes and trained to support patients. Healthcare providers should improve service quality by adopting a holistic approach and collaborating with communities to ensure optimal care. With collective efforts, it is hoped that the quality of life for diabetes patients can improve, and complications resulting from the disease can be prevented.

Abstract

Latar Belakang: Salah satu jenis diabetes yang paling umum, diabetes melitus tipe 2 yang memengaruhi 90% populasi dunia sebagai akibat dari pilihan gaya hidup yang tidak sehat. Kebaruan penelitian tentang gaja hidup pasien penderita diabetes melitus type 2 di rumah sakit royal prima medan. Penelitian bertujuan mengetahui hubungan gaya hidup dengan kejadian DM tipe 2 pasien di RS Royal Prima Medan dan pemahaman responden tentang gaya hidup sebagai pemicu.Metode: Penelitian kuantitatif cross-sectional digunakan untuk meneliti suatu fenomena. Penelitian melibatkan pasien Rumah Sakit Royal Prima Medan dari April sampai Juni 2024. Data dari 50 pasien rawat jalan dan dianalisis menggunakan SPSS pada analisis bivariat dan univariat.Hasil: Penelitian menemukan bahwa terdapat hubungan yang signifikan antara gaya hidup dengan kejadian Diabetes Mellitus diantaranya pola makan (p = 0,02), aktivitas fisik (p = 0,02), kebiasaan merokok (p = 0,04), manajemen stres (p = 0,01), pola tidur (p = 0,04). Variabel yang tidak memiliki hubungan signifikan antara gaya hidup dengan kejadian Diabetes Mellitus adalah konsumsi alkohol (0,27).Kesimpulan: Konsumsi alkohol merupakan faktor gaya hidup paling krusial dalam meningkatkan risiko timbulnya gejala diabetes melitus. Hasil tidak signifikan kebiasaan merokok dan pola tidur tidak dapat diabaikan sebagai faktor pengaruh peningkatan resiko timbulnya gejala diabetes melitus.

Abstract

This study aimed to investigate the relationship between anemia and quality of life in patients with chronic renal failure undergoing hemodialysis at Royal Prima Hospital. An analytical observational design using a cross-sectional approach was used. Data were collected from pre-existing medical records, and quality of life was assessed using a modified WHOQOL-BREF questionnaire that evaluates various aspects of life using a 5-point Likert scale. The chi-square test was used to determine the significance of the relationship between anemia status and quality of life among patients. A study of 51 hemodialysis patients at Royal Prima Hospital showed a high prevalence of anemia, with 86.3% of patients showing hemoglobin levels below the normal range. Although most patients (92.2%) were prescribed Sangobion, an iron supplement, many patients remained anemic, indicating a potential problem in terms of treatment efficacy or compliance. The duration of hemodialysis varied among the participants, with most undergoing treatment for more than 24 months, while the others were on hemodialysis for less than 24 months. About 92.2% reported a moderate quality of life, while only 7.8% experienced a poor quality of life. Statistical analysis showed no significant difference in the quality of life between patients with and without anemia (p=0.495), indicating that anemia may not have a direct impact on the overall quality of life for this patient population. In conclusion, although anemia is prevalent in hemodialysis patients at Royal Prima Hospital, its association with quality of life appears to be limited based on the findings of this study. Future research should focus on understanding the causes of anemia in this population and developing targeted strategies to improve the hematological status and overall quality of life of patients undergoing hemodialysis.
